임상실습 경험 내러티브에 나타난 간호학생의 자리매김 (Analysis of Positioning in the Nursing Students' Narrative of the Experiences of Clinical Practice)

  • Purpose: The purpose of this study was to understand the positioning which was perceived by nursing students from their clinical practice experiences. Methods: The data were 80 narratives of practice experience written by 20 nursing students. Using qualitative contents analysis, nursing students' reflective narratives were analyzed in the aspects of the positioning which including personal characteristics, designated social role, and ethical order. Results: In personal characteristics, nursing students positioned themselves as they had vague fear and cold feet about the physical and personal environment of the clinical setting. In the aspects of the designated social role, they positioned themselves to learn more knowledge and skills of the nursing, to put effort in maintaining good relationship with their patients and other nurses, and to evaluate the nurses on their job performances and professional attitudes. In ethical order, nursing students positioned themselves as a person who worries about their work as a future professional nurse. Conclusion: The result of this study can be used to better understand nursing students and application of the nursing students' narratives in the clinical practice education, and to facilitate positive outcomes and transitions from nursing students to nurses.

일개 의과대학 본과 4학년의 롤 모델링 경험과 성찰수준 분석: 임상실습을 중심으로 (Analysis of Role Modeling Experiences and Reflection Level of Fourth-Year Medical Students at a Single Medical College: A Focus on the Clinical Clerkship)

  • This study aimed to analyze the subjects, situations, and reflection levels related to role modeling experienced by medical students during their clinical clerkship and their own reflections. This study intends to suggest ways of improving how residents and clinical faculty should treat and teach medical students. Written interviews were conducted regarding senior medical students' role modeling experiences during their clinical clerkships in 2018 and 2019. Content analysis was conducted for a total of 224 cases from 196 students. Content analysis revealed three types of role modeling content: subjects (faculty, residents, nurses, peer students), situations (clinical competence, personal qualities, teaching skills), and the level of reflection (critical reflection, reflection, thoughtful action, and habitual action) in each case. As role model subjects, faculty were found to be the paramount role model (n=142, 62.83%). Role modeling was the most frequently performed for clinical competence (n=103, 45.98%). Clinical competence was frequently shown in communication and empathic listening during rounds and outpatient relationships between the patient and doctor. Regarding the level of reflection for role modeling, the number of critical reflections was 86 (38.39%) and that of reflections was 80 (35.71%). In particular, negative role modeling showed a high level of critical reflection in relation to faculty (64.44%) and nurses (8.89%). In conclusion, role modeling of medical students participating in clinical clerkships occurs in situations that the role models are not aware of, with positive or negative effects on the formation of professional identity among medical students.

신규간호사의 실무적응지원 교육 만족도 및 역할 이행 경험에 관한 조사 (New Graduate Nurses' Satisfaction with Transition Programs and Experiences in Role Transition)

  • Purpose: The aim of this study was to investigate the satisfaction of newly graduated nurses with educational programs and their experiences in role transition. Methods: Data were collected from November 1 to December 15, 2018 and 483 new graduate nurses working at 15 tertiary hospitals and 10 general hospitals participated. For data collection, self-report questionnaires including the Casey-Fink Graduate Nurse Experience Survey tool and satisfaction with education were used. Data were analyzed using descriptive statistics, t-test, and one-way analysis of variance. Results: Satisfaction with education ranged from 3.09 to 3.27, and satisfaction with preceptors was 3.45(maximum 4). The skill that new nurses ranked as most difficult during the first 3 months was charting/documentation, and throughout a whole year, the top 4 difficult skills were cardiopulmonary resuscitation/emergency response, ventilator care, end-of-life care, and prioritization/time management. In comfort/confidence, new graduates felt most comfortable with support and least comfortable with patient safety. More than 50 percent of new graduates experienced stress during role transition, and the most frequently experienced stressors were related to job performance and personal life. Levels of satisfaction with education and comfort/confidence differed according to the hospital type and number of preceptors for new nurses. Conclusion: In order to facilitate the transition of new graduate nurses to professional nurse, an extended period of education, systematic and standardized transition programs, and continuous support during the first year of practice are required.

간호대학생의 진로탐색과정: 예비간호사의 진짜 간호사 찾아가기 (Career Exploration Process of Nursing Student in Clinical Practice: A Voyage to Find the Authentic Nurse of Pre-registration Nurses)

  • Purpose: The purpose of the study is to understand the experiences of career exploration among nursing students during their clinical practice and to develop a theoretical framework for the career exploration process. Methods: From February 2 to 28, 2015, data were collected through focus group interviews from 24 participating fourth-year nursing students. The data were analyzed using grounded theory. Results: The central phenomenon of 'confusion between the ideal and the reality of nursing' occurred in contexts for various reasons of choosing one's major, nurses in the ideal, a vague fear of the field, arduous nursing assignments in the field, and severe adversity in the nursing field. Nursing students tried to deal with their confusion by using interaction strategies such as pursuing the intrinsic value of nursing, finding role models, trying to project oneself, and preparing for adapting to reality. The career exploration process in clinical practice was a voyage to discover the authentic nurse of the pre-registration nurse. Conclusion: This result shows that nursing students were to gain confidence in their career choices by finding the value of nursing and interacting with patients. Therefore, opportunities for learning experiences about interaction with patients and various positive experiences in clinical practice should be provided for nursing students.

음악치료사의 목소리 사용 경험에 대한 현상학적 연구 (A Phenomenological Study of Music Therapist's Experiences of Using Voice)

  • 이 연구는 음악치료사들의 목소리 사용 경험을 심층적으로 탐구하는 데 목적이 있다. 연구자는 이와 관련된 경험에 대하여 자세히 진술해줄 수 있는 음악치료사 7명을 대상으로 심층면담을 진행하였으며, Giorgi의 현상학적 연구방법으로 분석하였다. 분석결과 '임상에서의 목소리 사용으로 인한 다양한 감정촉진', '치료사의 개인적 성향에 따른 목소리 사용', '치료적 목적을 위한 목소리 사용', '목소리 사용으로 인한 내담자와의 긍정적인 음악적 경험', '음악치료 도구로서의 목소리 사용의 어려움', '만족스럽지 않은 목소리의 변화 시도'로 총 6개의 구성요소가 도출되었다. 본 연구를 통해 음악치료사들은 음악치료 안에서 노래 부르기 혹은 즉흥적 노래를 통해 내담자와의 긍정적인 관계를 형성할 수 있다고 보고하였다. 그러나 개인적인 이유로 목소리 사용에 있어서 어려움을 경험하기도 하였다고 설명하였으며 다양한 자기작업을 통해서 목소리 사용을 확장하고 더 나아가서 자신의 변화와 성장을 실현하고자 하였다고 진술하였다.

의학 고전의 개별 경험 진술과 일반 원칙 서술 (Individual experience reports and statements on general rules in medical classics)

  • Objectives : In studying East Asia's medical classics, what must first be done before evaluating the validities of clinical grounds is the discernment of the techniques that are based on universal principles from the statements that arise from individual experience. The purpose of this paper is to discover a method of discerning the statements of individual experiences and the descriptions of universal principles. Ingredients and Methods : The paper investigates how each individual clinical experience was introduced in ancient medical classics such as Huangdineijing, Nanjing, Shanghanlun, Jinkuiyaolue, and Maijing, in the books that belong to Yian such as Canggongliechuan, Linzhengzhinanyian, Gujinyianan, and Xumingyileian, and in Korean ancient medical texts such as Eonhae-gugeubbang and Gugeubganibang-eonhae. Results : Books of precriptions and scriptures of medicine were found to include individual experiences. Furthermore, this paper's effort has revealed that the evaluation of the quality of a testimony in an ancient book must be preceded by discerning whether the said testimony is a mere personal testimony or a testimony based on the universal law of causality. Conclusions : Before conducting a clinical research of a precription contained in an East Asian medical classic, the text should first be evaluated to determine whether the prescription in question derives from clinical tests or not.

치과진료실에서 수행되는 치과진료 협조업무에 대한 질적연구 - 간호조무사를 중심으로 - (A qualitative study on dental clinical assistance work performed in dental clinics - Focusing on nursing assistant -)

  • Objectives: The purpose of this study is to provide basic data for elucidating the clinical experiences of nursing assistants working at dental clinics and for establishing the bases necessary for building a safe working environment for the staff at dental clinics. Methods: From March 28 to August 20, 2022, the study conducted in-depth interviews with nursing assistants working with dental hygienists in dental clinics. The collected data were analyzed using the phenomenological method. Results: The study analyzed the meanings associated with the dental clinical assistance work experiences of nursing assistants in dental clinics. A total of 98 semantic words and 9 sub-categories were extracted, which were ultimately categorized into 4 central meanings: 'imperfect reality', 'work without boundaries', 'putting oneself in someone else's shoes', and 'symbiosis'. Conclusions: The nursing assistants were exposed to an environment where they needed to perform delegated treatment procedures even when they lacked dental work competency. They recognized their gaps in learning and the necessity for designated roles as clinical assistants. Thus, it is necessary to review the allocation and expansion of the work roles of dental practitioners based on the practical and rational realities in dental practice.

Family Caregivers of Korean Patients on Ventilators at Home: A Penomenological Study

  • Purpose. This study aimed to explore family caregivers' experiences with ventilator-dependent patients at home. Methods. The number of patients using mechanical ventilators at home is expected to increase and family caregivers must be able to care for them. However, few studies focus on the experiences of family caregivers. We conducted in-depth interviews with 11 family caregivers who cared for a ventilator-dependent patient at home in South Korea. Data were analyzed using phenomenological method of enquiry. Results. Statements generated from the interviews on the meaning of the family caregiver's experiences were organized into 27 themes, 5 theme clusters, and 2 categories. The theme clusters included endurance under the burden situation, role strain as a caregiver, separation from others, trying to find coping methods, and oriental ethnical customs. Family caregivers of patients using domestic mechanical ventilators need systematic education and emotional support to cope with the challenges of managing ventilator equipment and learning new ways of communicating with patients on the ventilators. Conclusion. These findings may contribute to family caregivers' knowledge and competence, thereby allowing them to better support their ventilator-dependent family members.
