임상실습경험에 따른 간호대학생의 죽음에 대한 태도와 자아존중감 및 삶의 만족도 비교 (Comparison of Attitudes of Nursing Students toward Death, Self-esteem and Life Satisfaction according to Clinical Experience)

  • 목적: 본 연구는 간호대학생의 임상실습 경험유무에 따른 죽음에 대한 태도, 자아존중감 및 삶의 만족도의 차이를 파악하기 위한 서술적 조사연구이다. 방법: 양산시에 위치한 2개 대학에 재학 중인 간호대학생 1,030명으로 대상으로 하였다. 자료수집은 2010년 10월 1일~10월 30일까지 강의시간 이외의 시간에 연구의 목적과 방법을 설명하고 설문에 대한 응답은 익명으로 처리됨을 알려 준 뒤 연구에 참여하기로 자발적으로 동의한 대상자에게 서면동의서와 설문지를 배부하고 개별적으로 설문내용을 작성하도록 한 후 현지에서 직접 회수하였고, 수집된 자료는 PASW statistics data editor 통계 프로그램을 이용하여 실수, 백분율, 평균, 표준편차, 카이제곱 검정, t 검정, 분산분석, Scheffe 검정과 Pearson correlation coefficients로 분석하였다. 결과: 죽음에 대한 태도 점수는 임상실습 경험이 있는 간호대학생의 경우 2.85점, 실습경험이 없는 간호대학생의 경우 2.79점으로 중간수준이었으며, 임상실습 경험이 있는 간호대학생이 임상실습 경험이 없는 간호대학생에 비해 죽음에 대해 보다 긍정적인 태도를 가지고 있었고 자아존중감과 삶의 만족도가 높았다. 임상실습 경험이 있는 간호대학생의 죽음에 대한 태도는 자신이 지각하는 경제수준과 본인이 지각하는 자신의 건강상태에 따라 죽음에 대한 태도의 차이가 있었으며, 자아존중감과 만족도와 낮은 상관관계가 있었다. 결론: 간호제공자의 죽음에 대한 태도는 임종간호의 가장 중요한 요소로 간호의 질을 결정함을 감안할 때 미래의 간호사가 될 간호대학생의 죽음에 대한 태도를 보다 긍정적으로 변화시킬 필요가 있다. 특히 임상실습 경험이 없는 간호대학생이 죽음에 대해 보다 부정적인 태도를 가지고 있었고 자아존중감과 삶의 만족도가 낮았으므로 이들에게 보다 적극적인 관심이 필요할 것이다. 한편 임상실습 경험 유무에 따른 간호대학생의 죽음에 대한 태도, 자아존중감 및 삶의 만족도에 대한 선행연구가 부족하여 논의하는데 제한이 있으므로 반복연구가 필요할 것을 제언한다.

간호학 교수들의 간호전문직에 대한 태도 조사연구 (Attitudes of Nursing Educators Toward Percussional Nursing)

  • This study of nurse educator's attitudes which found aspects of professional nursing was carried out from September 10 to 30, 1977. Subjects were 205 respondents from a total population of 314 nurse educators from the level of instructors to professors in all the diploma (post high school 3 year), professional junior college (3 year level) and university (4 year) nursing schools in Korea. Specific objectives were to determine their attitudes about the 1. social position of nursing as a profession, 2. nurses attitudes towards their word, 3. factors influencing the development of nursing, 4. the future of nursing, 5. nursing educational problems, 6. their own educational position's intrinsic job satisfaction and 7. their salary and benefits. The instrument used was a questionnaire developed from consultations with nursing educators and sociologists, and based on earlier. worts by Burke (1976), Mason (1974) and Hong 1969. Data were analysis by computer using one-way analysis. Statistically significant findings included the following; 1. Responses were positive toward all seven aspects of professional nursing. 2. Compared to diploma nursing school staff whose responses were positive, professional and university school faculty response toward the social position of nursing were neutral. 3. Faculty with clinical experience of 10 to 19 years were most positive in their responses about nurses' attitudes towards their work. In all three types of schools, in comparison to professors, associate professors' responses were more positive about nurses' attitudes toward their work. Faculty with longer clinical experience expressed high agreement with statements about factors which influence the development of nursing. Without any differences between school, all faculty had positive attitudes towards the future of nursing, the younger the staff member, the more positive were the responses. 4. faculty in diploma schools and those with 10 to 19 years clinical experience expressed high agreement with statement about nursing educational problems, 6. their own educational position's intrinsic job satisfaction and 7. their salary and benefits. 간호관리학실습 교과내용에 대한 중요도와 실습 경험에 대한 간호대학생과 임상현장지도자의 인식 비교 연구 (A Comparative Study on the Perceptions of Nursing Students and Clinical Instructors on the Importance of Learning Contents of Nursing Management and Practical Experience)

  • 본 연구의 목적은 간호관리학실습 교과내용의 중요도와 실습 참여 경험에 대한 학생과 임상현장지도자의 인식을 비교·분석해 간호관리학실습 개선의 기초자료를 제공하고자 하였다. 3개 대학 4학년 학생과 임상현장지도자를 대상으로 Mann-Whitney test와 교차분석 등으로 최종 343부 분석하였다. 연구 결과 간호대학생과 임상현장지도자 모두 간호관리학실습의 평가항목 중 '의사소통', '대인관계', '책임', '신뢰', '도덕성'을 중요 요소로 인식하였고 간호관리학실습 교과내용 중 가장 중요하게 인식한 상위 10개 항목 중 6개 항목('감염관리', '간호기록관리', '환자간호관리', '약품관리', '환자안전', '간호과오')은 두 그룹이 일치했다. 또한 임상현장지도자는 대인관계 및 관계윤리를 중요하다고 인식한 반면 간호대학생은 간호사의 '법적책임'을 중요하다고 인식했다. 두 그룹간의 실습경험 분석 결과, 간호대학생은 모두 간접 참여 경험이 높았으나 현장지도자는 직접 지도 경험이 높았다. 이와 같은 인식 차이가 확인된 교과내용은 학교와 실습기관의 유기적 파트너쉽 관계를 구축해 차이를 개선하고 간호대학생의 직접 실습 경험을 증대시키는 방안 마련이 필요하다.

우리나라 소아 물리치료사의 임상적 추론 (Clinical Reasoning by Pediatric Physical Therapists in South Korea)

  • Purpose: The purpose of this qualitative study was to understand the application of clinical reasoning throughout clinical and therapeutic knowledge of clinical pediatric physical therapists under physician prescriptions. Methods: Purposive sampling techniques were used in selection of nine clinical pediatric physical therapists in South Korea. Open and semi structured interviews were conducted, and were thoroughly examined and reviewed, followed by analysis of the clinical reasoning portion. Results: Pediatric physical therapists have been influenced by information gathering, physical function and evaluation of their patients, hypothesis-orientation, and re-evaluation and self-monitoring. Novice physical therapists were more dependent on physician prescriptions and parent's hope than the result of their own evaluation and critical pathway. Middle experienced pediatric physical therapists were more dependent on rapport with children and their parents. Highly experienced pediatric physical therapists were more reliant on hypothesis-orientation and self-monitoring. As reports on clinical experience of pediatric physical therapists have accumulated, clinical reasoning of pediatric physical therapists has been influenced by their experience, such as education, clinical pathology conference, their own clinical experience and rapport with patients and their parents than physicians' prescriptions. Conclusion: The findings of the current study are generally consistent with existing research on clinical reasoning. The results of the current study may be used by educators for enhancement of clinical reasoning abilities and knowledge of students or novices as well as development of a guide for use by suitable novices or students, and could provide important information for use in physical therapy practice and research.

의학 고전의 개별 경험 진술과 일반 원칙 서술 (Individual experience reports and statements on general rules in medical classics)

  • Objectives : In studying East Asia's medical classics, what must first be done before evaluating the validities of clinical grounds is the discernment of the techniques that are based on universal principles from the statements that arise from individual experience. The purpose of this paper is to discover a method of discerning the statements of individual experiences and the descriptions of universal principles. Ingredients and Methods : The paper investigates how each individual clinical experience was introduced in ancient medical classics such as Huangdineijing, Nanjing, Shanghanlun, Jinkuiyaolue, and Maijing, in the books that belong to Yian such as Canggongliechuan, Linzhengzhinanyian, Gujinyianan, and Xumingyileian, and in Korean ancient medical texts such as Eonhae-gugeubbang and Gugeubganibang-eonhae. Results : Books of precriptions and scriptures of medicine were found to include individual experiences. Furthermore, this paper's effort has revealed that the evaluation of the quality of a testimony in an ancient book must be preceded by discerning whether the said testimony is a mere personal testimony or a testimony based on the universal law of causality. Conclusions : Before conducting a clinical research of a precription contained in an East Asian medical classic, the text should first be evaluated to determine whether the prescription in question derives from clinical tests or not.

임상간호사의 도덕적 고뇌 구조모형 구축 (A Structural Model on the Moral Distress in Clinical Nurses)

  • Purpose: This study was to build a path model geared toward explaining the relationships of influential variables for the moral distress of clinical nurses by analyzing literatures of earlier studies. Methods: Data were collected from four hospitals with 300 beds or more. The participants were 257 nurses with more than 1 year of clinical experience. The data collection period was from June 14 to October 24, 2017. The questionnaire included general and ethical education-related characteristics, personality type, moral dilemma experience, moral sensitivity, moral climate and moral distress. Results: The direct influencing factors of moral distress were the individual's experience of moral dilemma, moral sensitivity, and the moral climate of the organization. Factors that indirectly affected via moral sensitivity were personality type, experience in ethical education, and moral climate. The explanatory power was 40.3%. Conclusion: It is necessary to develop an intervention program that can reduce moral distress by considering the factors influencing the moral distress of clinical nurses. In addition, it is necessary to identify additional influencing factors of moral distress.

임상간호사의 돌봄대상자로부터 언어폭력 경험 (Clinical Nurses's Experience of Verbal Violence by Care Recipients)

  • Purpose: The purpose of this study are to understand the meaning and essential structure of clinical nurses' experiences of verbal violence and find nursing intervention for them by applying phenomenological research method. Methods: The participants in this study were seven clinical nurses with experience of verbal violence. Data collection were conducted from August 2016 through January 2017 until data saturation. Data analysis was performed through a cyclical process of data collection and analysis by applying Giorgi's phenomenological method. Results: Four categories were identified: A weapon of the mind: the shock of verbal abuse; negative emotions that shake my heart; skepticism about the value and meaning of a job; an unforgettable wound. Conclusions: The results of this study provide a deeper understanding of the pain caused by verbal violence experienced by nurses from their care recipients. These results suggest that nursing interventions for nurses with experience of verbal violence will contribute to resolving the problem.

임상실습 현장에서 간호대학생이 경험하는 도덕적 고뇌 (The Experience of Nursing Students' Moral Distress in Clinical Practice)

  • Purpose: This study explores the moral distress that nursing students experience during their clinical practice in Korea. Methods: Data were collected using focus group interviews, and analyzed using qualitative content analysis. Participants were recruited from three nursing schools in three different cities; each focus group interview lasted between one to two hours. Results: Twenty-two nursing students with more than one year of clinical practice experience participated. Three categories and ten themes were extracted. The following situational categories: "unprotected patients' right and dignity," "clinical settings in which standards of care are not upheld," "disrespectful hospital culture," and "inconsistent and unsystematic clinical education" caused moral distress. Types of responses to moral distress included: "shock and confusion over the gap between reality and moral standards," "powerlessness when cannot advocate patients," "fear and doubts about nursing career," and "moral desensitization and disappointment in oneself." "Expressions of moral distress and the need for advice" and "a search for meaning and hope" were identified as coping strategies. Conclusion: These results demonstrate the need for systematic clinical practicum and education programs to minimize moral distress. These programs may offer opportunities for students to turn moral distress into opportunities for learning and growth in the future.

임상간호사의 직무만족 경험: 초점집단 연구방법 적용 (Experience of Job Satisfaction in Clinical Nurses : Application of Focus Group Methodology)

  • Purpose: The purpose of this study was to describe the experiences of job satisfaction in clinical nurses. Methods: Data was collected from three focus groups composed of 17 hospital nurses. Each focus group had an interview for an average of two and a half hours with the guidance of researchers. The main question was 'how do you describe your lived experience of job satisfaction as a clinical nurse?' Qualitative data from the field notes and transcribed notes were analyzed using a grounded theory methodology developed by Strauss and Corbin. Results: The core category of experience of job satisfaction in clinical nurses was identified as 'Finding success.' Supportive interpersonal relationships and environment affected this category. In the process of attaining job satisfaction through finding success, the participants were using four interactional strategies such as giving meaning, finding self-esteem, extending the horizon of life, and strengthening self-capability. The dimensions of job satisfaction in clinical nurses were the sense of achievement, stability, and pride. Conclusion: The results of this study yields very useful information for nursing mangers to design a program which enhances job satisfaction of clinical nurses based on interactional strategies.

임상간호사의 연구관련 활동 실태와 관련요인 (Research Related Activities and Its Related Factors in Clinical Nurses)

  • Purpose: This study was don(B to describe research-related activities and attitudes toward research, barriers to and support needs for undertaking research in clinical nurses. Method: Data were collected by a questionnaire from 238 clinical nurses with over one year clinical experience working at 2 university hospitals. Results: Research related activities included courses or lecture about Nursing Research 85.7%, journal reading at least once every 2 to 3 months 30.0%, memberships in academic societies 29.4%, participation in academic conferences 45.0%, conducting research 45.4%, research utilization 24.6%. The score for attitudes toward research was 3.08(range 1-5). The score of barriers to undertaking research was 3.37(1-5) and the score for support needs for undertaking research 4.14(1-5). Attitudes toward research significantly correlated with barriers to undertaking research(r=.- 36, p=.000). Barriers to undertaking research significantly correlated with support needs for conducting research(r=.23, p=.000). Nurses with experience in conducting research had more negative attitudes toward research(t=-2.130, p=0.034) and more barriers to undertaking research than those without experience in conducting research (t=2.194, p=0.029). Conclusion: These results suggest that it is necessary to increase positive attitudes toward research in clinical nurses and nursing organizations need to provide strong supports for nurses conducting research.
