Investigation of the Prevalence of Cholelithiasis in Liver Cirrhosis Cases and Controls on Upper Abdominal Ultrasound Images (상복부 초음파 영상에서 간경변증 환자군과 대조군의 담석증 유병률 연구)

  • This study aimed to understand the correlations of prevalence and relevant variables of cholelithiasis with a group of cirrhosis patients and a control group targeting the subjects who received the abdomen ultrasonography from K university hospital in Daejeon Metropolitan City from January 1st 2019 to December 31st. And the results are as follows. First, the group of cirrhosis patients showed relatively higher prevalence of cholelithiasis than the control group as ordinary people, which showed statistically significant differences. Second, in the control group, there were statistically significant differences in the occurrence of cholelithiasis with respect to age. Conversely, in the cirrhosis patient, there was no statistically significant association observed with age; nonetheless, age itself exhibited statistical significance. Third, according to sex, the prevalence was not statistically significant in both group of cirrhosis patients and control group. Fourth, in each degree and cause of subdivided cirrhosis, the correlation was only shown in each degree. In the results of this study, the cirrhosis patients showed high correlation with the incidence of cholelithiasis, and the control group showed the high correlation with the incidence of cholelithiasis according to age.

Saenggangeonbi-tang's Effect on Alcoholic Liver Cirrhosis - 1 Case (생간건비탕가감을 투여한 알콜성 간경변증 환자 치험 1례)

  • We treated a 53 year-old male patient. He had alcoholic liver cirrhosis and diabetes. The main symptoms were fatigue, ascites, edema, anorexia, indigestion, constipation, and abdominal pain. We diagnosed him as 'the retention of pathogenic damp-heat in the liver and gall bladder’ and adminstered Saenggangeonbi-tang three times a day. We observed clinical symptoms, liver function test(AST, ALT, ${\gamma}-GT$, ALP, total bilirubin), nutritional state(total protein, albumin) and capillary blood glucose. After treatment, all the main symptoms disappeared and the patient's nutritional state has improved. Child's grade using the improvement index in liver cirrhosis has changed from B to A. Although the very mechanism of Saenggangeonbi-tang was not proven, we could suggest that Saenggangeonbi-tang is not toxic to the liver and has a good effect on alcoholic liver cirrhosis with diabetes mellitus.

Family Support, Alcohol Consumption and Drinking Motives in Patients with Liver Cirrhosis (간경변증 환자의 가족지지, 음주정도 및 음주동기)

  • Purpose: The purpose of this study were to investigate the level of family support, alcohol consumption and drinking motives in patients with liver cirrhosis and to examine the relationships among those variables. Methods: The subjects consisted of 60 patients with liver cirrhosis. Family support scale, Q-F methods and Drinking Motives Questionnaire were used to measure the level of family support, alcohol consumption and drinking motives. Results: The level of family support was 43.62. Prevalence of drinking was 90% and 43.3% were currently heavy drinkers. Alcohol consumption was related to sex and education. There were positive low relationships between the level of enhancement motive, coping motive, social motive, and Q-F Index. Conclusion: Alcohol drinking is a serious health problem in patients with liver cirrhosis. It is necessary to have an educational approach for controlling drinking and family support.

Non-traumatic Spontaneous Gastrocnemius Muscle Blood Stasis Associated with Alcoholic Liver Cirrhosis (알콜성 간경변증 환자의 비외상성 자발성 비복근 어혈에 관한 증례 보고)

  • We report a rare case of non-traumatic spontaneous intramuscular mass of blood stasis, especially gastrocnemius muscle, associated with alcoholic liver cirrhosis. A 53-year-old man with alcoholic liver cirrhosis was admitted to our hospital for ICH. It was improved by Korean Medicine treatments, however, he was complaint of severe pain in left calf. Ultrasound revealed intramuscular hypoechoic mass at the left gastrocnemius muscle and we diagnose it as mass of blood stasis, which is similarly called 'hematoma' in Western medicine. Herbal extracts, acupuncture and indirect moxibustion reduced it, but the effects could not be compared as there were no reports of these kinds. It seems important to consider intramuscular mass of blood stasis as a complication of alcoholic liver cirrhosis and Ultrasound seems to be very useful in diagnosis and observation of intramuscular mass of blood stasis.

  • Objectives: The purpose of this study was to report the improvement of dyspepsia in a patient with liver cirrhosis and pancytopenia following traditional Korean medical treatment. Methods: A patient who suffered from dyspepsia with liver cirrhosis and pancytopenia was treated with herbal medicine, acupuncture, cupping, and moxibustion. The effect of treatment was measured with a visual analogue scale (VAS). Results: After treatment, the symptoms of dyspepsia improved and VAS score decreased. Conclusions: Traditional Korean medicine (TKM) could be effective for treatment of dyspepsia in patients with liver cirrhosis.

  • This survey was performed to report rare outbreak of liver cirrhosis in Korean native goat (KNG) which was died of Yangpyeong's goat farm on Feb. 1992. The examination for the KNG was carried out by clinical signs, necropsy and various lab-oratory test including parasitic, bacterial and histological test. The KNG looked jaundice, ascite, hemorrhage of lumen, abomasum and intestine, and brownish smooth cirrhotic liver at necropsy. Histological examination for liver revealed considerable proliferation of connective tissue and piecemeal necrosis which was caused by chronic active inflammation in interlobules and intralobules. There were atrophic micro and macro nodules which were sur-rounded by connective tissue. The lobular structure lack almost all central vein. The portal areas appearred proliferation of bile ducts, blood vessels and connective tissues. These connective tissue infiltrated heavily with plasma cells, Iymphocytes and histocytes. Histological examination for brain proved to be hepatic encephalopathy by virture of congestion and edema in cerebral medullary. From these results were demonstrated miked nodular, active, postnecrotic liver cirrhosis.

Clinical Case of Liver Cirrhosis, with Herbal Medicine, Acupuncture, and Moxibustion Treatment (간경변 환자의 한방치험례)

  • Object : Liver cirrhosis is a disease of the liver in which normal cells are replaced by scar tissue. The purpose of this case is to report the improvement of liver cirrhosis after herb-med, acupuncture & moxibustion complex therapy. Methods : We provided herb-med, acupuncture & moxibustion complex therapy to a patient who suffered from ascites and fatigue. We examined LFT & Child-Pugh class to evaluate the effectiveness of oriental treatment. Result and Conclusion : We observed that herb-mod. acupuncture & moxibustion complex therapy decreased symptoms of liver cirrhosis and improved general condition of a patient who suffered from ascites and fatigue. In the LFT result, Child-Pugh class was improved.

Pallidal Signal Intensities on T1-weighted MRI are Highly Observed in Advanced Liver Cirrhosis

  • Background: Manganese (Mn) has been found to increase the signal intensity of the globus pallidus (GP) on T1-weighted magnetic resonance images (MRI). We performed this study in order to determine the features of liver disease that correlate with pallidal signal intensities. Methods: We assessed blood Mn levels and pallidal signals in T1-weighted MRI in 49 patients with liver cirrhosis and 23 healthy controls. Results: Increased signal intensity in the GP was observed in 30 of 49 (61.2%) patients with liver cirrhosis, with the pallidal index (PI) in patients with Child-Pugh classes B and C differing significantly from the PI in controls. Multiple linear regression analysis showed that blood Mn concentrations and Child-Pugh scores in cirrhotics were significantly associated with increased PI after controlling for other confounders (p<0.05 each). Conclusions: Pallidal signals on T1-weighted MRI are mainly observed in advanced liver cirrhosis. The present study suggests that advanced liver cirrhosis may be a human model for manganism.

The Relations among Self-care, Symptom Experiences, and Disease Status of the Patient with Liver Cirrhosis (간경변증 환자의 자가간호수행과 증상경험 및 질병상태와의 관계)

  • Purpose: The purpose of this study was to examine the degree of self-care, symptom experience and disease state among the patients with liver cirrhosis and to identify its correlation. Methods: The subjects were 220 patients with liver cirrhosis who were treated in D-university hospital in Busan. The instruments used for study were the self-care inventory, the symptom experience inventory and modified childpugh classification scale. Results: The average points of the patient's self-care performance, symptom experience, and disease status were 3.67, 29.10 and 6.12 respectively. Their self care was negatively correlated to symptom experience, and symptom experience was positively correlated to disease state. Conclusion: This research findings suggested that active self-care by patients with liver cirrhosis might contribute to reduce various symptoms they can undergo and to prevent aggravation of disease state. Therefore, patients with liver cirrhosis have to carry out self care actively in daily life and discreet nursing intervention should be offered to facilitate their self-care.

Antifibrotic Effects of Oriental Herbs Extraction on Liver Cirrhosis (한약재(자하거, 백출, 차전자) 추출물의 간경화 억제효능에 관한 연구)

  • Hepatic cirrhosis is an important feature of chronic liver disease. Liver cirrhosis is characterized by hyperaccumulation of fibrous tissue components and is commonly observed in latter or terminal states of chronic hepatic disease. The antifibrotic effects on liver cirrhosis by oriental herbs extraction material were examined in bile duct ligated rats. Oriental herbs extraction (0.99 mg/kg rat weight/day) was administrated to cirrohotic rats for 4 weeks. Liver collagen content of bile duct ligated rats was significantly increased. And liver histology showed collagen fiber deposition was increased as well as the normal architecture was lost with large zone of necrosis being observed. Herbs extraction administrated rats showed significantly decreased liver collagen content, accumulation of collagen fiber in histological analysis, and biochemical markers of hepatic diseases. Those results demonstrate the usefulness of herbs extraction materials as an antifibrotic agent for liver cirrhosis.
