Endobronchial Leiomyoma - A case report - (기관지내 평활근종 -1례 보고-)

  • Endobronchial leiomyoma is extremely rare and accounts for less than 2% of benign tumors of the lower respiratory tract. Leiomyomas are predominantly found in the young and the middle aged : of the average age being 35 years for bronchial and lung parenchymal lesions and 40.6 years for tracheal lesions. The symptom depends on the location of the tumor, its size, and changes in the lung distal to the lesion. A 37-year-old woman was admitted to our hospital complaining of coughing. Bronchoscopy revealed complete obstruction of the right main bonchus at the carina by an oval-shaped, nonulcerative, smooth, and pinkish-tan tumor with a broad margin and extended to the left main bronchus. A biopsy was performed and showed a benign spindle cell tumor. A right pneumonectomy was performed because of chronic infection, and the lung could not expanded during aeration. The histological diagnosis of the resected specimen was leiomyoma. The postoperative course was uneventful.

The Effect of Artemisia Capillaris Crude Juice Extract on $CCl_4$ Induced Liver Damage in Dogs (인진쑥이 사염화탄소 투여로 유발된 개의 간 손상 회복에 미치는 영향)

  • Artemisia is a major edible vegetable in Korea and it has traditionally been used as a herbal medicine for the treatment of coughing, abdominal pain, indigestion, bleeding, jaundice, chronic liver disease and diabetes. However the biological and pharmacological actions of the herb have not been studied well. Recently it is known to possess antibacterial, antihelmintic and antifertility activities. But the effect of Artemisia capillaris extract on carbon tetrachloride($CCl_4$) induced liver damage in dogs have not been reported yet. This study was designed to investigate the effect .of Artemisia capillaris crude juice extract on $CCl_4$ induced liver damage in dogs. 30 clinically healthy dogs were divided into 2 groups: crude Artemisia capillaris juice treated group(CEC group) and carbon tetrachloride($CCl_4$) administerd group. The results are as follows: I. The degree of increase in AST activity and ALT activity in CEC group was lower than that in $CCl_4$ group and the recovery in CEC group was faster than that in $CCl_4$ group. 2. Changes of ALP concentration in CEC group were significant(P < 0.05) but changes of Total-bilirubin concentration were not significant(P < 0.05) in both groups. 3. The recovery of GGT concentration in CEC group was faster than that in $CCl_4$ group. 4. Hematological changes other than MCHC were significant(P < 0.05) in CEC group only and changes of GSH and Met-Hb concentration were significant(P < 0.05) in $CCl_4$ group.

Antimicrobial Effects of Artemisia capillaris Extracts on the Pathogenic Bacteria In Vitro (이진쑥 추출물의 병원성 세균에 대한 시험관내 항균효과)

  • Artemisia capillaris THUNB is a perennial herb that belongs to the family Compositae spp and the most common plant among the various herbal folk remedies used in treatment of abdominal pain, hepatitis, chronic liver disease, jaundice and coughing in Korea. In this study, antimicrobial effects of Artemisia capillaris extracts on the food poisoning bacteria were investigated for further clinical application, which is an alternative for the use of antibiotics and their unexpected resistance. Artemisia capillaris extract using ethyl acetate showed the highest antimicrobial effects on S. enteritidis, E. coli O157 : H7, L. monocytogenes and S. aureus. The chloroform extract showed strong effects on all kinds of bacteria; whereas ethanol and methanol extracts showed weaker effects. Finally, ether and water extracts showed the weakest effects under the same conditions. The minimum inhibitory concentration (MIC) of ethyl acetate extract was 1 mg/mL for E. coli O157 : H7 and L. monocytogenes, and 2 mg/mL for S. enteritidis and S. aureus. The inhibitory effects on all the bacteria continued for 12 hours after incubation using 20 mg/mL and 30 mg/mL of ethyl acetate extract. The inhibitory effects continued maximally for 72 hours. The results of these studies indicate Artemisia capillaris extract exhibited excellent antimicrobial and inhibitory effects on the food poisoning pathogenic bacteria; S. enteritidis, E. coli O157 : H7, L. monocytogenes and S. aureus.

Cytotoxic Effect of Artemisia capillaris Extracts on the Cancer Cells on in vitro (인진쑥 추출물의 세포독성 효과)

This experiment was conducted to investigate cytotoxic effects of Artemisia capillaris extracts on the Hepa-1c1c7 and Sarcoma 180 cancer cells on in vitro experimental tests. On in vitro tests using MTT assay and SRB assay, the extracts showed prominent cytotoxic effects on the two kinds of cancer cell lines, respectively. Antihumor effects appeared in the concentration of over $250{\mu}g/mL$ of both ethanol and ethyl acetate extract, over $500{\mu}g/mL$ of methanol extract, over $5000{\mu}g/mL$ in water extract and over 50% cytotoxicity on the Hepa-1c1c7 and Sarcoma 180. The results suggest that Artemisia capillaris extracts have prominent cyotoxic effects on the cancer cell lines Hepa-1c1c7 and Sarcoma 180.

Clinical Analysis of Suspected Airway Foreign Bodies in Infants and Children (의증 유소아 기도이물의 임상적 고찰)

  • The bronchoscopy is usually performed in the patients who present symptoms of wheezing, hemoptysis, signs of airway obstruction, unexplained long-term chronic cough with or without radiologic features of pneumonia, emphysema or atelectasis. The symptoms and signs of cases of suspected airway foreign body are of often cofused with those of asthma pneumonia and other respiratory tract disease. The ventilating bronchoscopy will be helpful for removal of certain foreign bodies as well as alleviating medically-unresponsive bronchopulmonary conditions via removing abnormal secretions or merely improving ventilation. The authors have performed clinical analysis of 59 patients who were suspected of airway foreign bodies and treated by ventilating bronchoscopy from September 1985 to February 1995. The results were as follows: 1) Among the 59 cases of bronchoscopy, 51 cases(97%) were under the age of 3. The ratio of male and female was 2.69:1 2) Most common presenting symptom was coughing(84.7%) followed by dyspnea, fever, wheezing and anterior chest pain. 3) Among 39 cases of identified foreign bodies, abnormal findings in the chest X-ray films were found in 33 cases(84.6%) and most common abnormal radiologic feature was pneumonia in 10 cases(25.6%) followed by combination of pneumonia and emphysema, or emphysema alone. All the 20 cases of absent foreign body presented abnormal radiologic features and common findings were pneumonia and atelectasis. 4) Vegetable foreign bodies were the most common form of removed airway foreign bodies(69.2%) followed by metallic foreign bodies(12.8%) and plastic foreign bodies(7.7%). 5) By distribution of the location of foreign bodies, 29 cases(74.4%) were located in the main bronchus( 16 cases of right side and 13 cases of left side), 8 cases were in the trachea and 2 cases were found at the cordal level. 6) By duration of foreign body lodgement in the airway, 9 cases(23.1%) were removed within 7 to 30 days, 10 cases(5.6%) within 3 days and 5 cases(12.8%) were enlodged over 30 days. 7) In the cases of absent foreign bodies, common bronchoscopic findings were mucopurulent or thick mucoid discharge in the main bronchi, erythema or edema of bronchial mucosa and all the cases were improved after ventilating bronchoscopy with concomitant medical management.

Study on Perceived Symptoms and Preventive Behaviors during Asian Dust storms among Children, Adults and Elders (황사 기간 동안에 어린이, 성인, 노인에서의 자각증상과 예방행동에 대한 조사연구)

  • Purpose: Recently there are scientific evidences for the health effects of Asian dust-storms. Particularly in 2002, the daily average of $PM_10$ exceeded 600 and 700 $\mu{g}/m^3$ in March and April respectively. We examined the effects of Asian dust‐storms on perceived symptoms and preventive behaviors among children, adults, and elders. Method: The subjects of this study were 425 children, 444 adults and 60 elders. A questionnaire survey was carried out on the children and the adults by teachers and on the elders by interviewers to obtain information on demographic variables, perceived symptoms and preventive behaviors, previous respiratory disease, environmental tobacco smoke, and proximity between the house and the road during Asian dust-storms. We analyzed using descriptive statistics, $X^2$ test and multiple logistic regression models. After adjustment for potential confounders, we estimated the odds ratios and 95% confidence intervals of the children and the elders for perceived symptoms and preventive behaviors. Result: The elders had a significantly higher prevalence of respiratory symptoms than the other groups. The children had a significantly higher prevalence than the adults in getting URI (1.51), coughing (1.68), rhinorrhea (1.46), fever (2.39) and medication for allergy or asthma (1.90). The children had better behaviors than the elders in closing windows (3.57), taking less outdoor recreational activity (2.59) and staying home (2.60). Conclusion: This study suggested that children and elders are susceptible to the effects of Asian dust on health. Health educators should inform these populations about the influence of Asian dust and provide health education for preventive behaviors.

The Clinical Feature of Reflux Esophagitis Patients Visiting a Single Korean Medicine Hospital: A Retrospective Study

  • Objectives: This study investigated the clinical characteristics and symptoms of patients with reflux esophagitis (RE) visiting a Korean medicine hospital. Methods: We retrospectively reviewed the medical records of patients with an RE diagnosis who visited a Korean medicine hospital from June 15, 2020, to April 15, 2021. We analyzed the clinical characteristics and the results of a symptoms questionnaire. Results: Our sample included 1,151 patients (711 females). The median age was 58 years; the most common age bracket was 60-69 years. A total of 837 patients (72.7%) presented with symptoms lasting more than one year, and 1,035 (89.9%) had been prescribed medications for more than three months before visiting a Korean medicine hospital. Belching (77.6%) and acid regurgitation (70.9%) were the most frequent symptoms, followed by epigastric pain (61.6%) and throat globus (58.5%). Throat globus was the most bothersome symptom, and 72.4% of the patients experienced the symptom on more than five days in the preceding two weeks. Patients with a longer symptom duration were older (p<0.01) and more likely to be overweight (p=0.03). Patients experiencing symptoms for more than five years were more likely to report throat globus (p=0.02), hoarseness (p<0.01), and coughing (p<0.01). Conclusions: Most RE patients visiting a Korean medicine hospital were females in their sixties with chronic and refractory RE. Atypical symptoms (belching, epigastric pain, and throat globus) were common. Throat globus was highly prevalent in severe cases, suggesting that it warrants attention when treating RE patients.

Pectolinarigenin ameliorated airway inflammation and airway remodeling to exhibit antitussive effect

  • Cough is a common symptom of several respiratory diseases. However, frequent coughing from acute to chronic often causes great pain to patients. It may turn into cough variant asthma, which seriously affects people's quality of life. For cough treatment, it is dominated by over-the-counter antitussive drugs, such as asmeton, but most currently available antitussive drugs have serious side effects. Thus, there is a great need for the development of new drugs with potent cough suppressant. BALB/c mice were used to construct mice model with cough to investigate the pharmacological effects of pectolinarigenin (PEC). Hematoxylin-eosin and Masson staining were used to assess lung injury and airway remodeling, and ELISA was used to assess the level of inflammatory factor release. In addition, inflammatory cell counts were measured to assess airway inflammation. Airway hyperresponsiveness assay was used to assess respiratory resistance in mice. Finally, we used Western blotting to explore the potential mechanisms of PEC. We found that PEC could alleviate lung tissue injury and reduce the release of inflammatory factors, inhibit of cough frequency and airway wall collagen deposition in mice model with cough. Meanwhile, PEC inhibited the Ras/ERK/c-Fos pathway to exhibit antitussive effect. Therefore, PEC may be a potential drug for cough suppression.

A case of recurrent respiratory infection resulting from a congenital anomaly of the bronchial tree tracheal bronchus (반복적인 호흡기 감염을 가진 환아에서 진단된선천성 기도 기형, 기관기관지 1예)

  • The term tracheal bronchus refers to an abnormal bronchus that comes directly off of the lateral wall of the trachea (above the carina) and supplies ventilation to the upper lobe. Tracheal bronchi occur almost exclusively on the right trachea and are associated with other congenital anomalies. In addition, tracheai bronchus may be related to other inflammatory conditions with persistent wheezing, such as recurrent pneumonia, chronic bronchitis and bronchiectasis, which is a result of the relatively poor local drainage of the involved bronchi. An infant with recurrent wheezing is likely to be a challenge for a clinician in the evaluation of the etiology of airway obstruction and in the differential diagnosis of wheezy breathing. The authors report a case of an 8-month-old female infant with a ventricular septal defect, who presented with stridor and recurrent respiratory infection and finally was finally diagnosed with a tracheal bronchus using computed tomography and a bronchoscopy. Therefore, tracheal bronchus should be included in the differential diagnosis of any child who presents with chronic or recurrent respiratory tract symptoms such as coughing, wheezing, stridor and recurrent respiratory infection, particularly in children with other congenital deformities.

A Case of Coccidioidomycosis Manifested as Solitary Pulmonary Nodule (고립성 폐결절상을 보인 콕시디오이데스진균증 1예)

  • Coccidioidomycosis is a fungal infection acquired by inhalation of the arthrospore of Coccidioides immitis, and endemic disease in specific geographic areas, such as south central California, south Arizona. Nevada, and New Mexico. Approximately 60 percent of infected people is asymptomatic and the remainders mostly exhibit respiratory complaints, from flu-like coughing to overt pneumonia. Usually the infection due to Coccidioides immitis is self-limited. Symptoms resolve within several weeks but radiographic abnormalities could be resolved more slowly. If the radiographic abnormalities persist more than 8 weeks, the term' chronic pulmonary coccidioidomycosis' is designated. They take forms of nodules, cavities or progressive pneumonia. When manifested as nodule(s), lung malignancy is suspected as a possible diagnosis and histologic confirmation is needed. Here, we report a case of chronic pulmonary coccidioidomycosis manifested as solitary pulmonary nodule in a Korean woman who has traveled in Arizona, which is diagnosed finally by lobectomy and histologic examination.
