Food allergen sensitization in young children with typical signs and symptoms of immediate-type food allergies: a comparison between monosensitized and polysensitized children

  • Purpose: The clinical interpretation of children sensitized to allergens is challenging, particularly in children with food allergies. We aimed to examine clinical differences between children with monosensitization and those with polysensitization to common food allergens and to determine risk factors for polysensitization in young children <10 years of age with immediate-type food allergies. Methods: The study included children <10 years of age with signs and symptoms indicative of immediate-type food allergies. Serum total IgE level was measured, and ImmunoCAP analysis for food allergens was performed. Results: The mean age of the study subjects was $1.6{\pm}1.6$ years (75 boys and 51 girls). Thirty-eight children (30.2%) were monosensitized and 88 children (69.8%) were polysensitized. Multivariate logistic regression analysis showed that the development of polysensitization to common food allergens was positively associated with a parental history of allergic rhinitis (adjusted odds ratio [aOR], 6.28; 95% confidence interval [CI], 1.78-22.13; P=0.004), season of birth (summer/fall) (aOR, 3.10; 95% CI, 1.10-8.79; P=0.033), and exclusive breastfeeding in the first 6 months of age (aOR, 3.51; 95% CI, 1.20-10.25; P=0.022). Conclusion: We found significant clinical differences between children with monosensitization and those with polysensitization to common food allergens and identified risk factors for the development of polysensitization in young children with immediate-type food allergies. Clinicians should consider these clinical risk factors when evaluating, counseling, treating, and monitoring young children with food allergies.

  • Objectives : To propose the referential data to evaluate the health impacts of Vietnam veterans' children whose father were exposed to herbicides in Vietnam War. Methods : Vietnam veterans who visited to Pusan Veteran Hospital for medical care were recruited from April to October, 1998. They were 71 and asked about their own combat history, symptoms and illness, and health status of their 182 children. The informations were collected by direct and phone interview. Exposure estimation was also performed as exposure score depending on year and unit of participation, and personal episodes related to exposure to herbicide in the war. It classified into three groups; lower(<18.0), moderate(18-53), high$(\geq53)$ exposure group. Results : The mean age and the period into the combat of the veterans were 52.8 years and 15.0 months. The mean exposure score was $18.1{\pm}9.9$, and mainly distributed in lower (46.5%) and moderate(52.1%) exposure group. Most(90.1%) of them were diagnosed as sequelae(21 cases) and suspected sequelae(43 cases) of the herbicides by Korean veteran's hospital diagnostic criteria. The major sequelae was peripheral neuropathy 13 cases, chloracne 5 cases, and the major suspected sequelae was hypertension 20 cases, diabetes mellitus 18 cases, liver disease 12 cases, central neuropathy 11 cases, etc. About birth, 42.2% and 16.9% experienced spontaneous abortion and stillbirth, respectively. The mean exposure score was higher in stillbirth experience group(p<0.05). About half of the children(90 cases, 49.5%) hold the abnormal health status: those were skin pigmentation 38 cases, rash 23 cases, congenital anomaly 15 cases, general weakness 12 cases, purpura 8 cases, visual disturbance 8 cases, etc. These health problems had no association with father's exposure level(p>0.05). Conclusions : These results were depend on their own answers, and expectation for compensation did not excluded, therefore, this study may have limitations: inaccuracy of informations due to recall bias and response bias. Nevertheless, through this study, we could image the fundamental aspect for health impacts of Vietnam veterans' children for preparing the national control program and policy. A large scale epidemiologic study with valid exposure assessment on the health impacts of Vietnam veterans' children is recommneded.

  • The number one female cancer in Korea is breast cancer, and the incidence rate continues to increase. There are many opinions that this is due to the impact of increased sedentary life along with economic growth. In this study, the international standard exercise status questionnaire was administered to patients and normal group who visited to the specialized cancer hospital in Korea, and to standardized scores of the exercise status (MET score) obtained from questionnaire were used to compare the impaction on breast cancer risk and difference of the MET score between the patients and control group. The ages of the study subjects were selected as subjects without statistical differences between the patients group and the normal group. also done with survey the family history, BMI, the menarche, age at birth of first child, the degree of education to exclude impaction on breast cancer risk depending on physical activity. In general, the breast cancer patients showed lower MET scores than the normal group, and the effect of activity on breast cancer was greater in post-menopausal women than in pre-menopausal women. Also BMI was similar to breast cancer risk. Maintaining proper weight through exercise can reduce the accumulation of carcinogens in breast cancer in the body, thereby reducing the incidence of breast cancer.

A Comparison of Growth Development and Nutrient Intakes between Double Income Families' and Full-Time Housewives' Preschool Children (맞벌이 가정과 전업주부 가정 미취학 자녀의 신체발달과 영양섭취상태 비교연구)

  • The purpose of this study was to examine the comparison of growth development, bone mineral density and nutrient intakes between double income families' preschool children(DIFPC) and full-time housewives' preschool children(FHPC). Subjects were 111 preschool children. Anthropometric characteristics and bone mineral density in right forearm were measured. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the DIFPC(n=60) was 53.02 months and that of the FHPC(n=51) was 54.80 months. The birth height and weight of the subjects were 50.47㎝ and 3.27㎏ for DIFPC and 50.85㎝ and 3.36㎏ for FHPC, respectively. The average height, weight, % body fat, and obesity index were 108.50㎝, 18.35㎏, 15.35%, 96.71% in DIFPC and 111.46㎝, 19.64㎏, 16.80%, 97.31% in FHPC, respectively. The bone mineral density in forearm of two groups were 0.24g/㎠ in all. The infant feeding method was significantly different between DIFPC and FHPC; 58.9% of DIFPC was fed formula, while 44.4% of FHPC was fed breast milk(p<0.05). Proportions of children for their regular meal were 59.4%, 89.6%, and 61.0% for breakfast, lunch, and dinner, respectively. The major reasons for irregular meal were lack of time and poor appetite for breakfast and snacks for lunch and dinner. Most of the children answered they have snack over once a day, and 60.0% have unbalanced diet. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B1, niacin, and vitamin C did not meet the Korean RDAs. The intakes of K and vitamin A for DIFPC were significantly higher than those of FHPC(p<0.05, p<0.05). In conclusion, double income families' preschool children more have a low frequency of breast feeding and low intakes of micro nutrients, such as K and vitamin A than full-time housewives' ones.

Descriptive Study on the Development of Educational Program for Mothers with Children who have had Open Heart Surgery in Congenital Heart Disease (개심술을 받은 심기형 환아모의 교육프로그램 개발을 위한 조사연구)

  • The aims of this study were to identify the mothers educational needs when they have children who have had open heart surgery in congenital heart disease, to identify the children's characteristics with this problem, and also to provide the basic information on the development of the educational program for the mothers. The subjects of the study were 101 mothers of children and their children hospitalized with congenital heart disease in pediatric wards of G. University Hospital in Inchon and S. Hospital in Puch on from June to November 2000. The study used a Likert-type questionnaire with 39 questions which was based on a previous questionnaire developed by Lee Mi-ryun(1989) for adult open heart surgery patients. The reliability of the questionnaire was Cronbach' ${\alpha}$=.9375. The data were collected directly from mothers of the patients in wards. The t-test, ANOVA was adopted for the data analysis. The key results of the study are as follows : 1) The average age of the children was 32.90 months. The majority of the children are male and weighted 3.0-3.5Kg at birth. Most of the children had VSD(29.7%), and 10.9% of children had a family history of heart disease. 2) An average score of 4.62 out of a maximum of 5.00 was recorded for the educational needs of the children's mother. The highest score of 4.69 was for home care after discharge followed by a score of 4.67 for diagnosis and prognosis on congenital heart disease. And pre-post operation care scored 4.51. The mean scores of single-question items of educational need were, in order, 4.81 for immunization after operation, 4.80 for recovery process, and 4.77 for prognosis of the disease. The score of 4.18 for measurement of intake and output was the lowest. 3) The analysis on the children's mothers educational needs by social-demographic characteristics such as sex, age of patient and mother, economical status, educational status, number of child, hospitalization times, and hospitalization cause showed no significant difference statistically, but there was a significant difference between mothers with jobs and with out jobs.

  • 'Headline Database of Newspaper Articles' is a database which contains about 141,500 newspaper articles from 1949 to March, 2009. They are collected from two perspectives; 'language' and 'language life' by KOKKEN. There were 3312 newspaper articles (about 2.34%) which included the word ONNA at 'Headline Database of Newspaper Articles'. The number of newspaper articles related to ONNA started to increase in 1975 but they decreased afterwards. They increased rapidly in 1980 and maintained the condition. However, they started to decrease rapidly in 1990 and maintained the decreased condition. They increased rapidly again in 2004 and 2007. The main causes of rapid increase were the commercial message of instant noodles "I am the one who is making. I am the one who is eating." in 1975, newspaper articles related to "Starting of full-scale studies on female language" in 1980, comments of "active women" and "men's crime" related to a murder case of an elementary school student in Sasebo City and mixed attendance books in 2004, a comment of "Women are machines which give birth to babies" in 2007. Those six causes of rapid increase suggested that the perception of gender such as 'Men need to work outside and Women need to do housework and take care of child' which was fixed until then was changing and becoming a stereotype of virtual reality rather than reality. The vocabulary related to ONNA appeared 3411 times among 3312 newspaper articles which included ONNA. Typical forms of the vocabulary related to ONNA were and . They appeared 2390 times and occupied 70% of the whole data. (3411 times) The form of ONNANOKO among the vocabulary related to ONNA appeared 113 times and occupied a high rate. ONNANOKO(113) and other words such as SHOJO(115), JOJI(28), YOJO(9) (152 in total) implied that appearing of young women at newspaper articles were increasing. Also, the vocabulary related to 'female language' such as ONNAKOTOBA(28) ONNANOKOTOBA(10) and a woman's heart such as ONNAGOKORO(35) and ONNANOKIMOCHI(34) appeared frequently. The vocabulary related to JOSEI were divided into <$JOSEI^{**}$> and <$^{**}JOSEI$>. <$JOSEI^{**}$> were mainly related to an occupation. <$^{**}JOSEI$> were mainly used to express women by regional groups such as or combined with modifiers to express women such as . In case with modifiers, WAKAIJOSEI appeared 35 times and showed the highest frequency. It had negative evaluations in many cases. The vocabulary related to JOSI appeared on the form of <$JOSI^{**}$> and mainly associated with 'a girl's school' and 'a female student'.

  • Plato said that our cosmos was created from the union of nous and $anank{\bar{e}}$ in Timaeus. In addition to this, 'the third kind', namely $ch{\bar{o}}ra$, exists already with genesis before the birth of the cosmos. Plato explains this $ch{\bar{o}}ra$ with several metaphors. That is, 'receptacle', 'mother', 'gold', 'space', 'place' etc. From Aristotle to contemporary scholars, generally three types of interpretations are presented. First, $ch{\bar{o}}ra$ is a kind of a 'void.' Secondly, $ch{\bar{o}}ra$ is Aristotle's $prot{\bar{e}}$ $hyl{\bar{e}}$. Thirdly, $ch{\bar{o}}ra$ has two aspects of space and matter. I will accept the third opinion. Plato's $ch{\bar{o}}ra$ has both a spatial aspect and material aspect. $Ch{\bar{o}}ra$ is a place that accommodates all sensible things in itself. On the other hand, $ch{\bar{o}}ra$ seems to be a mother who nourishes sensible things like a child. In this, Plato lacks a logical consistency. The research of $ch{\bar{o}}ra$ can not deviate from the limits of $eik{\bar{o}}s$ logos.

  • Congenital panhypopituitarism is an uncommon condition, present from birth, characterized by the decreased secretion of most of the hormones produced by the pituitary. The purpose of this case report is to present a case about caries treatment of a 26-month-old female patient with congenital panhypopituitarism under general anesthesia. A 26-month-old girl with congenital panhypopituitarism visited Seoul National University Dental Hospital for caries treatment of anterior primary teeth. Because of the child's age and underlying systemic disease, dental treatment under general anesthesia was considered. Prior to the dental procedure, 30mg of cortisol was administered intravenously in order to prevent possible adrenal crisis by stressful events. The dental procedure was successfully performed under general anesthesia. This case report suggests that general anesthesia may be useful for the dental treatment with congenital panhypopituitarism. Hormone deficiency should be assessed prior to dental procedure and, if necessary, stress hormone replacement therapy should be considered.

A Study on Joseon Royal Cuisine through Sachanbalgi of the Jangseogak Archives - Focusing on Royal Birthday, Child birth, Weddings and Funerals- (장서각 소장 사찬발기를 통한 조선왕실의 사찬음식 연구 - 탄일, 출산, 가례, 상례를 중심으로 -)

  • This study investigated the Sachanbalgi, which record the royal feasts given by the royal family of the Joseon Dynasty of Korea. These records are contained within the Gungjung Balgi, which recorded the types and quantity of items used in royal court ceremonies. The Eumsikbalgi is the general name for the records of food found within this document. Using these Eumsikbalgi, and in particular the Sachanbalgi, this study investigated the food eaten and bestowed by the Joseon royal family. The Sachanbalgi describes four categories or occasions of feasts: royal birthdays, childbirth, royal weddings, and funerals. These records allow us to reconstruct who the attendees were and what the table settings and food were for instances not directly indicated in oral records, books, or other documents. The food at these Sachan (feasts) was diverse, being related to the specific event, and its contents varied based on the position of the person who was receiving the food. Usually, Bab (rice) was not found at a Sachanbalgi, and only on two occasions were meals with Bab observed. Specifically, it was served with Gwaktang (seaweed soup) at a childbirth feast. There were seven kinds of soups and stews that appeared in the Sachanbalgi: Gwaktang, Yeonpo (octopus soup), Japtang (mixed food stew), Chogyetang (chilled chicken soup), Sinseonro (royal hot pot), and Yukjang (beef and soybean paste). Nureumjeok (grilled brochette) and Saengchijeok (pheasant), and Ganjeonyueo (pan-fried cow liver fillet) and Saengseonjeonyueo (pan-fried fish fillet) were eaten. Yangjeonyueo, Haejeon, Tigakjeon (pan-fried kelp) and other dishes, known and unknown, were also recorded. Boiled meat slices appeared at high frequency (40 times) in the records; likewise, 22 kinds of rice cake and traditional sweets were frequently served at feasts. Five kinds of non-alcoholic beverages were provided. Seasonal fruits and nuts, such as fresh pear or fresh chestnut, are thought to have been served following the event. In addition, a variety of dishes including salted dry fish, boiled dish, kimchi, fruit preserved in honey, seasoned vegetables, mustard seeds, fish, porridge, fillet, steamed dishes, stir-fried dishes, vegetable wraps, fruit preserved in sugar, and jellied foods were given to guests, and noodles appear 16 times in the records. Courtiers were given Banhap, Tanghap, Myeonhap, wooden bowls, or lunchboxes. The types of food provided at royal events tracked the season. In addition, considering that for feasts food of the royal household was set out for receptions of guests, cooking instructions for the food in the lunchbox-type feasts followed the cooking instructions used in the royal kitchen at the given time. Previous studies on royal cuisine have dealt mostly with the Jineosang presented to the king, but in the Sachanbalgi, the food given by the royal family to its relatives, retainers, and attendants is recorded. The study of this document is important because it extends the knowledge regarding the food of the royal families of the Joseon Dynasty. The analysis of Sachanbalgi and the results of empirical research conducted to reconstruct the precise nature of that food will improve modern knowledge of royal cuisine.

The Relationship between Basic Psychological Needs and Postpartum Depression: Focused on the Mediating Effects of Spousal Support and Marital Satisfaction (기본심리욕구와 산후우울의 관계: 배우자지지, 결혼만족도의 매개 효과)

  • Based on the Self-Determination Theory and previous studies, this study confirmed that basic psychological needs among Self-Determination Theory have an indirect influence on the postpartum depression through mediation of spousal support and marital satisfaction. A sample of 313 mothers who raises her child within a year of giving birth completed a questionnaire survey, and a structural equation model was utilized to analyze the data. Results indicated that both the measurement model, and the hypothesized structural model fit to the data. Meanwhile, when compared with the hypothesized model in which the path between all variables is established, the competitive model excluding the path from spousal support to postpartum depression has no difference in fitness, and was relatively simple and adopted as the final model. In the final model, the link between basic psychological needs and postpartum depression was partially mediated by spousal support and marital satisfaction. In addition, marital satisfaction was found to mediate completely the relationship between spousal support and postpartum depression. Based on the results, it would be helpful to identify the status of basic psychological needs, spouse support, and marriage satisfaction in counseling related to postpartum depression.
