허리 안정화 운동과 병행한 엉덩근육 강화 운동이 제왕절개 분만 후 만성허리통증을 가진 여성의 통증, 기능장애지수, 산후우울증에 미치는 영향 (Effects of the Gluteus Muscle Exercise combined with the Lumbar Stabilization Exercise on Pain, ODI, and Postpartum Depression in Women with Chronic Back Pain After C-section Delivery)

  • Background: This study aimed to investigate the effects of combining the lumbar stabilization exercise with the gluteal muscle strength exercise on pain, the dysfunction index, and postnatal depression in women experiencing chronic low back pain after a cesarean section. Methods: The study used a randomized design with two groups: one group (n=15) performed the lumbar stabilization exercise along with the gluteal muscle strength exercise, while the other group (n=15) only did the lumbar stabilization exercise. The intervention consisted of 30 minute exercise sessions, twice a week, for 6 weeks. The pain was assessed using the visual analog scale (VAS), the dysfunction index with the Oswestry disability index (ODI), and postnatal depression using the edinburgh postnatal depression scale-Korean version (EPDS-K). Results: Both groups showed significant improvements in their VAS, ODI, and EPDS-K scores within their respective groups (p<.05). Additionally, the between-group analysis revealed significant differences in post-test results (p<.05). Conclusion: The combination of the lumbar stabilization exercise and the gluteal muscle strength exercise proved to be more effective in reducing pain, improving the dysfunction index, and alleviating postnatal depression in women with chronic low back pain after a cesarean section. Therefore, incorporating these exercises could be beneficial as an intervention program for women experiencing chronic low back pain after a cesarean section.

Isolated tubal torsion in the third trimester of pregnancy managed with simultaneous salpingectomy and cesarean section

  • Isolated tubal torsion is an uncommon cause of acute abdomen in pregnancy. Tubal torsion may occur in the absence of adnexal disease. Diagnosing tubal torsion is especially difficult in pregnancy because no precise preoperative radiological and biochemical investigations have been conducted. Most patients are diagnosed during surgery. Here, I present a case of isolated tubal torsion in a pregnant woman at 35 weeks and 6 days of gestation that was managed with salpingectomy and cesarean section simultaneously.

Effect of Acupressure on Preoperative Cesarean Section Anxiety

  • Anxiety is a common preoperative problem in cesarean section candidates. Nonpharmacologic anxiety control has been demonstrated to be more suitable in pregnant women. The current study was a randomized, single-blind clinical trial which evaluated the effect of acupressure on preoperative C-section anxiety. In this study, 60 patients facing surgery were randomly divided into two groups of 30 patients each. Those patients in the intervention group received simultaneous acupressure at the Yintang and HE-7 acupoints for 5 minutes before surgery, and patients in the control group received intervention at a sham acupoint. The anxiety level of patients was preoperatively assessed twice using the Spielberger's State-Trait Anxiety Inventory. The mean anxiety scores of the two groups were shown to be insignificantly different before the intervention (p = 0.859), whereas a significant difference between the mean anxiety scores of the two groups was observed after the intervention (p = 0.001), suggesting that acupressure reduced the anxiety of patients before surgery.

향요법마사지가 제왕절개술 산모의 기분, 유즙사출반사, 유즙 면역글로불린A에 미치는 효과 (Effect of Aromatherapy Massage on the Mood, the Milk ejection Reflex, and the Immunoglobulin A of the Breast Milk of Mothers with a Cesarean Section Delivery)

  • The purpose of this quasi-experimental study was to explore the effect of aromatherapy massage on the mood, the milk ejection reflex, and the immunoglobulin A of the breast milk of mothers who gave birth through a Cesarean section delivery. Twenty mothers who had Cesarean section were selected as an experimental group, and twenty-two were in the control group. Lavander and Rosemary oil mixed with Jojova carrier oil was used to massage the back, both axillar and breasts. Aromatherapy massage was done once a day for 20 minutes by the researcher. Each session consisted of 4 minutes for warm-up, 14 minutes for massage and 2 minutes for closure. The levels of IgA within the breast milk was analyzed by an immunoturbidimeter assay (Cobas INTEGRA, Roche, Swiss) before and after aromatherapy massage. Mood and milk ejection reflex were measured by self-reports at the same time. The data were analyzed using SPSS 7.5 and the hypotheses were tested by ANCOVA and the Pearson coefficient correlation. The results were as follows : 1) Score of mood increased significantly after the use of aromatherapy massage. 2) Score of milk ejection reflex increased significantly after the use of aromatherapy massage. 3) Level of IgA of breast milk did not change significantly after the use of aromatherapy massage.4) After the use of aromatherapy massage, there wasn't any correlation among mood, milk ejection reflex, and level of IgA of breast milk. In conclusion, the results suggest that aromatherapy massage is an effective nursing intervention to enhance the mood and the milk ejection reflex and to increase the rate of breastfeeding in the breastfeeding mothers under stresses like a Cesarean section.

산후마사지프로그램이 제왕절개술 산모의 스트레스 반응에 미치는 효과 (Effects of Postpartum Massage Program on Stress response in the Cesarean section Mothers)

  • The purpose of this study was to explore the effect of a postpartum massage program on stress response in the Cesarean section mothers. The study focused on evaluating the effect of postpartum massage program on mood, anxiety, skin temperature and concentration of saliva and breast milk immunoglobulin A in the Cesarean section mothers. This study was designed as a nonequivalent control group pretest-posttest quasi-experimental study. Twenty-eight Cesarean section mothers were selected as experimental group, whereas twenty- seven were control group. The postpartum massage program consisted of 20 minutes of warm-up, massage and ending phases and used once a day. During each program, there were 4 minutes of warm-up, 14 minutes of massage on back, axillary and breasts, and 2 minutes of ending. Massage were used for the experimental group by the same investigator 20 times per minute. The massage technique used were efflurage, petrissage, accupressure, kneading and vibration. Skin temperature was monitored with YSI Tele-thermometer(Simpson electric Co., USA) before and after massage program. The concentration of immunoglobulin A in saliva and breast milk was analyzed by immunoturbididimeter assay(Cobas INTEGRA, Swiss) before and after massage program. Also at this time mood and anxiety were measured by self-report. The data were analyzed using SPSS version 7.5 and hypothesis was tested with ANCOVA analysis and Pearson coefficient correlation. The results were as follows : 1) Score of mood increased significantly after use of postpartum massage program. 2) Level of anxiety decreased significantly after use of postpartum massage program. 3) Skin temperature increased significantly after use of postpartum massage program. 4) Concentration of saliva immunoglobulin A increased significantly after use of postpartum massage program. 5) Concentration of breast milk immunoglobulin A did not change significantly after use of postpartum massage program. 6) After use of postpartum massage program, there was significant correlation between psychological stress response and physiological stress response.The results suggest that postpartum massage program can be effective nursing intervention to reduce stress response in the postpartum mothers under stress.

Application of Mini-abdominoplasty after Conservative Excision of Extensive Cesarean Scar Endometriosis

  • Endometriosis is defined as the presence of functioning endometrial tissue outside the uterine cavity, and the standard treatment is extensive surgical excision. Cesarean scar endometriosis is a type of cutaneous endometriosis arising on or near a Cesarean section scar. A 44-year-old woman presented with a $9{\times}6$ cm sized dark-brown, stony-hard, irregular, lower abdominal mass of four years duration. The patient had a history of two Cesarean deliveries, 14 and 16 years ago. Suspecting endometriosis, we excised the tumor conservatively rather than extensively to prevent incisional hernia considering the benign nature of the tumor and the low possibility of recurrence because the patient's age was near menopause, along with simultaneous bilateral salpingo-oophorectomy that was performed in this case. On reconstruction, mini-abdominoplasty was adopted to avoid possible wound complications and cosmetic deformities. The patient was satisfied with the cosmetic results, and neither recurrence nor functional problems occurred during the 1-year follow-up period. Plastic surgeons should keep in mind the possibility of cutaneous endometriosis in an abdominal mass of a female of reproductive age with a previous history of pelvic or intra-abdominal surgery. An optimal result from oncological, functional, and cosmetic standpoints can be achieved with conservative excision followed by mini-abdominoplasty of extensive Cesarean scar endometriosis.

Laparoscopic excision and repair of a cesarean scar pregnancy in a woman with uterine didelphys: a case report

  • Cesarean scar pregnancy (CSP) is a rare complication that occurs in less than 1% of ectopic pregnancies, and uterine didelphys is one of the rarest uterine forms. We report a successful laparoscopic excision and repair of CSP in a woman with uterine didelphys and a double vagina. A 34-year-old gravida one, para one woman with a history of low transverse cesarean section presented to our hospital with a suspected CSP. She was confirmed to have uterine didelphys with a double vagina during an infertility examination 7 years earlier. Magnetic resonance imaging showed a 2.5-cm gestational sac-like cystic lesion in the lower segment of the right uterus at the cesarean scar. We decided to perform a laparoscopic approach after informing the patient of the surgical procedure. The lower segment of the previous cesarean site was excised with monopolar diathermy to minimize bleeding. We identified the gestational sac in the lower segment of the right uterus, which was evacuated using spoon forceps. The myometrium and serosa of the uterus were sutured layer-by-layer using synthetic absorbable sutures. No remnant gestational tissue was visible on follow-up ultrasonography one month after the surgery. This laparoscopic approach to CSP in a woman with uterine didelphys is an effective and safe method of treatment. In women with uterine anomalies, it is important to confirm the exact location of the gestational sac by preoperative imaging for successful surgery.

초산모의 질분만 경험 (The Meaning of Vaginal Delivery to Primiparous Mothers)

  • Recent research reported that postpartum mothers showed different responses according to the type of delivery, and though most mothers preferred vaginal delivery to cesarean section, the numbers of negative comments about the delivery experience were higher in a vaginal delivery group than in a cesarean section group. This study set out to understand the meaning of vaginal delivery as perceived by mother who delivered their babies vaginally and how the mothers felt about their delivery experience. The subjects of this study were 17 primiparous mothers right after vaginal delivery in one university affiliated hospital. Date were collected from March to April, 1992 through interviews lasting 20∼30 minutes using open ended questions about the delivery experience. The data were analyzed by Giorgi's phenomenological analysis method and categorized according to similarity of countent. The meaning of the vaginal delivery was grouped in to four categories and the reason for vaginal in to three. One category of the meaning of vaginal delivery was pain, consisting of the subcategories too painful, want to be rid of the pain, unbearable pain and bearable pain. Another cutegory was a sense of accomplishment containing the subgroups wonderful, good and being finished, The third category was the feeling of becoming a mother The fourth category was that of not having any sense yet of the experience. The reasons for preference for vaginal delivery to a cesarean section were categorized in to the instinctive thinking that vaginal delivery was the natural method, a shortened period of recovery and a lower incidence of complications and a stronger feeling of maternal identity.

복부흉터에 발생한 자궁내막종 (A Case of Endometrioma at the Abdominal Scar)

  • Purpose: Plastic surgeon can easily misdiagnose the mass on the postoperative scar as hypertrophic scar. We present a case of endometrioma at abdominal scar after cesarean section. Methods: A 36 year-old female visited with a protruding, rubbery hard mass on her abdominal scar. The mass grew slowly after cesarean section performed 7 years ago. She has felt intermittent pain, not with menstrual cycle, for 2 months. The ultrasonography showed a solid irregular, ill defined mass with heterogenous echogenicity. The MRI finding is suggestive of malignant soft tissue tumor with deep fascial invasion. Incisional biopsy showed acanthosis, melanophage, lymphoplasmacytic infiltration. So we excised mass elliptically, we could see chocolatelike discharge from the mass, adhering to external oblique muscle fascia. The mass confirmed histologically as an endometrioma. Results: The mass was completely removed and did not recurr. The patient does not complain more about pain or any discomfort. Conclusion: Plastic surgeons should be aware of the possibility of endometrioma when the patient present with mass on her abdominal scar after surgery of the pelvis and abdomen.
