Effects of Dongsasub Training for Family Caregivers of Patients with Dementia on their Burden, Depression, and Self-esteem (동사섭 훈련이 치매간병 가족의 부양 스트레스, 우울, 자아존중감에 미치는 효과)

  • Purpose: The purpose of this study was to verify the effectiveness of Dongsasub training for the family caregivers of patients with dementia on their care burden, depression, and self-esteem. Methods: An one-group pretest-posttest quasi-experimental design was used. Six family caregivers recruited from a support center for dementia in Seoul and participated in this study. Once a week, the Dongsasub training was provided for 90 minutes during eight weeks. Results: Depression was significantly lower (z=-2.01, p=.044), and self-esteem was significantly higher (z=-2.21, p=.027) than before Dongsasub training. However, care burden was not significantly different between pre and post program (z=-1.58, p=.115). Conclusion: The results of this study indicate that the Dongsasub training can be used as a nursing intervention in community settings to decrease depression and improve self-esteem for the family caregivers of patients with dementia.

Development and Testing of Day Care Program for Demented Elders (주간보호센터 중심의 재가 치매노인을 위한 프로그램의 개발 및 효과 검증 연구)

  • Purpose: The purpose of the study was to develop the day care program for the demented elders and to determine the effects of multiple activity therapies on the physical, emotional, cognitive, social function and physiological indicators of dementia elders in day care center. Method: A total of 13 elders with mild dementia in the data care center were participated. Manual and guideline of data care program was developed. Data care program was composed of movement/exercise, foot massage, music therapy, horticultural therapy, and art therapy. Data care program was performed by trained intervener for 32 weeks. Instruments were physical functioning such as hand grip strength., vital capacity, chiar stand test, sit and reach test, one leg standing, Berg' Balance scale, 6m Walk Velocity, stair climb, ADL, GDS-K, MMSE-K, Integrative Social Functioning Scale, cortisol, Ig A and Ig G. These were measured at the baseline, 16-week, and 32-week post-intervention by the trained nurses. Data were analyzed as Wilcoxon signed rank test. Results: For Hand grip, balance, and vital capacity, there was a significant difference between baseline and 16-week post-intervention(p<.05). For one leg standing, 6m velocity, there was a significant difference between 16-week post-intervention and 32-week post-intervention(p<.05). For Stair climb, there was a significant difference between baseline and 32-week post-intervention(p<.05). For depression and social functioning, there was no significant difference among baseline, 16-week, and 32-week post-intervention. For cognitive functioning, there was a significant difference between baseline and 32-week post intervention(p<.05). There was a significant difference of cortisol, and Ig G between baseline and 32-week post-intervention(p<.05). Conclusion: It would appear that physical fitness, functional performance, ADL, depression, social and cognitive functioning, and physiological indicator of community dwelling elders with dementia can be improved through day care program using multiple stimuli.

The Effect of the Integrated Cognitive Rehabilitation Program on the Cognitive Function and Depression of Elderly with Mild Dementia (경증치매노인에서 통합적 인지재활 프로그램의 인지기능 및 우울정도 개선효과 연구)

  • Purpose: This study aimed to investigate the effect of the integrated cognitive rehabilitation program in elderly patients with mild dementia. Methods: A total of 20 elderly patients with mild dementia participated in the integrated cognitive rehabilitation program. The program was conducted three times per week for a total of 4 periods (10 wks per period) from February 6, 2018 to December 13, 2018. Each session lasted for 1 hour, and the cognitive function and depression of the subjects were measured before and after they participated in the program. Results: After the program, there was a significant increase and decreases in participants' cognitive function and depression respectively. There was a statistically significant difference in cognitive function score over time and period, except for the 2nd period. The difference in the degree of depression over time and period was statistically significant. Conclusion: The results suggest that an integrated cognitive rehabilitation program could help improve cognitive function and the degree of depression in elderly patients with mild dementia.

Validity, Reliability and Efficiency of Pain Self-report Scale in Elderly with Dementia (치매노인의 통증사정에 대한 자가보고 도구의 타당도, 신뢰도 및 효율성 검증)

  • Purpose: The aim on this study was to establish the validity, reliability and efficiency of a Pain Self-Report Scale for elderly with dementia and compare these results with an observational pain rating scale. Methods: Study subjects were 136 elderly with dementia who were residents in a nursing home, geriatric hospital, or day care center. The subject's pain was measured by five self-report scales and observational scale. DS-DAT (discomfort scale-dementia of the Alzheimer's type) was used for pain behavior observational measure. Cognitive state was assessed using the MMSE (Mini-Mental State Examination). Results: Observational rating correlated moderately with self-report (r=.225~.585, p<.05) and tended to underestimate pain intensity. Test-retest reliability was high for all five self-report scales, and the correlation between these scales was very strong (r=.735~.856, p<.05). Comprehension rate of VDS (verbal descriptor scale) was 88.3%, and NRS (numeric rating scale) 69.9%, FPS (face pain scale) 66.9%, HVAS (horizontal visual analog scale) and VVAS (vertical visual analog scale) 65.4%. Conclusion: Nurses should not apply observational scales routinely in demented patients as many of these are capable of reporting their own pain. Self-report, the highest standard of pain measurement can be reliably performed in a large proportion of demented elderly.

Problematic Behaviors recognized by Caregiver in Demented Older Adults (돌봄제공자가 인지한 치매노인의 문제행동 연구)

  • Behavioral symptoms are frequent and problematic components of dementia. The aim of this study was to detect behavioral problem of the 25 item of dementia problem behaviour(DPB)assesment scale for demented older adults. Seventy-three patients with dementia staying in the day care center, group home, dementia hospital, nursing home were recruited data on problematic behavior obtained through interviews with their caregiver during 2 months from March to May 2004. Results shows that problematic behaviors common occurring in 95% of subjects. The most frequently exhibited problematic behaviors were repetitive movement(1.05), repetitive questions(1.03), restlessness(1.0). indifference(0.97). shouting/screaming(0.92). Problematic Behaviors of the demented older adults were closely associated to the distress for caregiver. Most distressful ones for caregivers were repetitive questions(1.46), repetitive movement(1.42), shouting/screaming (1.42), indifference(1.41). restlessness(1.41). Problematic Behaviors of the demented older adults were divided into six subdivisions, among those restless behavior was positively correlated with aggressive behavior, nervous symptom and psychotic symptom. and then nervous symptom was interrelated to psychotic symptom(P<0.01). This study has limitation that field study data were derived from various primary caregivers.

A qualitative study on the present working conditions of dental hygienists and the oral health awareness of older adults with dementia - Focus group interviews - (치과위생사의 치매노인 구강건강관리 실태 및 인식에 관한 질적 연구 - 포커스 그룹 인터뷰 적용 -)

  • Objectives: This study investigated the working conditions of dental hygienists and their challenges in providing oral care to older adults with dementia using focus group interviews. Methods: We collected data for approximately a month beginning from August 2020 and divided the study subjects into two groups: the health dental hygienist and the clinical dental hygienist groups. A total of 11 subjects participated in this study. Results: The oral health management of older adult patients with dementia has not been efficiently carried out in local communities or dental medical institutions. In addition, dental hygienists encounter difficulties in managing the oral health of these patients and hope to actively learn more about their special cases. Conclusions: Based on the results of this study, it is necessary to develop a manual or program for the professional implementation of oral health interventions for older adults with dementia.

A Case Report on the Health Promotion Programs for Rural Residents in a Rural Area during the COVID-19 Pandemic (코로나19 팬데믹 하에서 일개 농촌 지역 주민맞춤 건강관리 프로그램 사례)

  • Purpose: This study aimed to describe a non-face-to-face dementia prevention and physical activity program in small rural villages during the COVID-19 pandemic. Methods: The study used a case report provided by a primary healthcare post in Gyeonggi-do in 2020. Results: The program was "From head to toe, stay healthy", which was largely divided into dementia prevention ("Dementia Zero Zone") and physical activity ("The less fat, The healthier body"). Five elderly people aged 75 and over participated for preventing dementia, and 13 residents joined the health promoting programs over 80 times in total. This program was designed one-to-one customized and person-centered program, including counseling, education, and health services. The program participants responded that the amount of physical activities was increased even under strict social distancing and they felt less isolated and less depressed. Moreover, the number of screening for dementia was increased with this program. Conclusion: This case has shown the applicability of a new approach to sustain health promotion programs in the context of limited interaction with rural nurses. Under the challenging environment that requires adaptation to information and communication technologies (ICTs), it will be necessary to solve not only technical problems but also digital literacy issues of rural residents.

  • Objectives : The purpose of this study was to obtain some knowledge concerning the scalp acupuncture treatment on patients with dementia for the future practice and research from clinical studies in China. Methods : The literatures were searched using the data base-China Academic Journals(CAJ) (2001-2011). Results : We found out 1 case study, and 17 randomized controlled trials(RCT) which met the inclusion criteria in most studies. There were 3 types of scalp acupuncture-applying method, cho-si scalp acupuncture(焦氏頭鍼法), combination of using ordinary acupoints(正經鍼) and extraordinary acupoints(經外寄穴), standard method of scalp acupuncture(標準頭鍼法). The often used acupoints are Yundongqu(運動區), Ganjuequ(感覺區), Yuntingqu(暈聽區), Zuyunganqu(足運感區), Sishencong (HN23), Baihui(GV20), Shenting(GV24), Fengchi(GB20), Dingnieqianxiexian(MS6), and Dingniehouxiexian(MS7). Conclusions : There were significant improvements on the scalp acupuncture-treated groups, compared to the control groups(western medication group or other acupuncture treatment group). Results of this study could be used for future studies concerning the scalp acupuncture for dementia.

Comparing the Effectiveness of the Frequency and Duration of the Horticultural Therapy Program on Elderly Women with Mild Cognitive Impairment and Mild Dementia

  • The purpose of this study was to investigate the effects of the horticultural therapy program on patients with mild cognitive impairment and mild dementia depending on the frequency and duration of the interventions. We developed the same 15-session program to improve cognitive functions and life satisfaction and alleviate depression of the elderly women with mild cognitive impairment or mild dementia. Subjects in Longer Treatment group participated in the program once a week for 15 weeks and subjects in Shorter Tratmet group participated twice a week for 7½ weeks. This study conducted pretest-posttest verification of both groups using quasi-experimental design involving 21 subjects. Elderly life satisfaction, Geriatric Depression Scale (short form), and the Korean Version of Consortium to Establish a Registry of Alzheimer's Disease (CERAD-K) were used in the evaluation. As a result, both groups showed an increase in life satisfaction, and a decrease in depression. However, there was a significant difference in the changes of the CERAD-K scores between the two groups (p < .05). In Longer Treatment group, life satisfaction increased significantly (p < .001), and depression decreased at a marginally significant level (p = .068), but no statistically significant change was observed in neurocognitive function. In Shorter Treatment group, life satisfaction increased at a marginally significant level (p = .059), and depression and CERAD-K scores decreased significantly (p < .05). However, in the case of Mini-Mental State Examination (MMSE-K), there was no significant change in both groups. According to these results, when planning a horticultural therapy program for persons with mild cognitive impairment or mild dementia, it is effective to organize and execute the program by determining the duration of intervention as 3 to 4 months or longer, even if this reduces the number of interventions per week.

Evaluation on the Quality of Research Field with Traditional Herbal Prescriptions for Dementia Therapy (치매 치료용 한약 처방의 연구성과에 대한 정성평가)

  • Objective : This study aimed to review the performance of traditional herbal prescriptions for treating dementia and present a strategy for research on dementia therapy utilizing herbal medicine. Methods : A definition was made to clarify the technology regarding the development of herbal prescriptions for treating dementia. The queries were compounded based on the initial keywords provided by experts in the field, then applied to the Web of Science database search engines from January 1986 to September 2011 to search related scientific articles. Before performing the analysis, papers were extracted from the initial search reviewed by experts and 80 articles were selected. Then, the selected papers were analyzed in terms of publish year, country, and type of herbal prescriptions. Furthermore, the research performance evaluation for treating dementia by herbal prescriptions was also created in terms of country and organization based on forward citation analysis. In addition to, for the evaluation regarding research quality, we classified and reviewed papers into two types: clinical studies and experimental studies. Results : According to the quantitative information analysis of 80 articles, the number of papers has increased by 21.9% per the yearly mean from 1995, and Japan had the largest portion within this research field. There were 34 kinds of traditional herbal prescriptions, among them Ukgansan had the highest number of studies followed by Jodeungsan, Dangkisoosan and so on. In addition, quality index as calculated by cites per paper is higher than average in Switzerland, Turkey and Japan. In the view of the evaluation on quality there were 12 clinical studies, 8 RCT reported that herbal prescriptions had efficacy at cognition, behavioral & psychological symptoms (BPSD) and activity of daily life (ADL) in various type of dementia. In experimental studies most of the studies were performed using animal models. The studies using Ukgansan were aimed at improving BPSD. The papers studied with Jodeungsan and Dangkisoosan targeted vascular dementia. Conclusions : In this study, research to develop traditional herbal prescriptions for treating dementia has the potential to improve symptoms since herbal medicines work as both multi-function and multi-target in dementia with multiple pathological or neurotoxic pathways. Therefore, the results of the research should be used in order to establish strategies to develop technology for treating dementia with traditional herbal prescriptions in the future.
