Implementation of Service Model to Exchange of Biosignal Information based on HL7 Fast Health Interoperability Resources for the hypertensive management (고혈압 관리를 위한 헬스레벨 7 FHIR 기반 생체정보 교환 서비스 모델 구현)

  • Hypertension is one of the major causes of death in the world as it is related with cardiovascular or cerebrovascular disease, so it is needed to provide continuos management for blood pressure. This study selected Health Level 7 Fast Health Interoperability Resources (HL7 FHIR) as a bio-signal data exchange service model that can provide constant blood pressure management in the rapidly growing mobile health care environment. The HL7 FHIR framework developed communicates with the IEEE 11073-10407 Personal Health Device (PHD) protocol through the bluetooth Health Device Profile (HDP) between the manager (smart phone) and the agent (hemomanometer) and acquires information about blood pressure. According to the test results, it performed its tasks successfully including hypertension patients' blood pressure monitoring, management on measured records, generation of document, or transmission of measured information. Because in the actual, clinical environment, it is possible to transmit measured information through the TCP/IP protocol, it will be needed to conduct constant research on it and vitalize it in the field of mobile health care afterwards.

  • In this paper, we proposed multipurpose signal detection methods for ECG (electrocardiogram) based u-healthcare systems. For ECG based u-healthcare system, QRS signal extraction for cardiovascular disease diagnosis is essential. Also, for security and convenience reasons, it is desirable if u-healthcare system support biometric identification directly from user's bio-signal such as ECG for this case. For this, from Lead II signal, we developed QRS signal detection method and also, we developed signal extraction method for biometric identification using Lead II signal which is relatively robust from signal alteration by aging and diseases. For QRS signal detection capability from Lead II signal, ECG signals from MIT-BIH database are used and it showed 99.36% of accuracy and 99.68% of sensitivity. Also, to show the performance of signal extraction capability for biometric diagnosis purpose, Lead III signals are measured after drinking, smoking, or exercise to consider various monitoring conditions and it showed 99.92% of accuracy and 99.97% of sensitivity.

Immunocell Therapy for Lung Cancer: Dendritic Cell Based Adjuvant Therapy in Mouse Lung Cancer Model (폐암의 면역세포 치료: 동물 모델에서 수지상 세포를 이용한 Adjuvant Therapy 가능성 연구)

  • Background: The anti-tumor therapeutic effect of autologous tumor cell lysate pulseddendritic cells (DCs) was studied for non-immunogenic and immune suppressive lung cancer model. To test the possibility as an adjuvant therapy, minimal residual disease model was considered in mouse in vivo experiments. Methods: Syngeneic 3LL lung cancer cells were inoculated intravenously into the C57BL/6 mouse. Autologous tumor cell (3LL) or allogeneic leukemia cell (WEHI-3) lysate pulsed-DCs were injected twice in two weeks. Intraperitoneal DC injection was started one day (MRD model) after tumor cell inoculation. Two weeks after the final DC injection, tumor formation in the lung and the tumor-specific systemic immunity were observed. Tumor-specific lymphocyte proliferation and the IFN-${\gamma}$ secretion were analyzed for the immune monitoring. Therapeutic DCs were cultured from the bone marrow myeloid lineage cells with GM-CSF and IL-4 for 7 days and pulsed with tumor cell lysate for 18 hrs. Results: Compared to the saline treated group, tumor formation was suppressed in 3LL tumor cell lysate pulsed-DC treated group, while 3LL-specific immune stimulation was minimum. WEHI-3-specific immune stimulation occurred in WEHI-3 lysate-pulsed DC treated group, which had no correlation with tumor regression. Conclusion: The data suggest the possible anti-tumor effect of cultured DCs as an adjuvant therapy for minimal residual disease state of lung cancer. The significance of immune modulation in DC therapy including the possible involvement of NK cell as well as antigen-specific cytotoxic T cell activity induction was discussed.

  • This paper demonstrates empirically that individuals who monitor indicators of their current cardiovascular health status by undergoing medical examinations are more likely to invest in their own health than those who do not observe such monitoring protocols. Using data from the 2001 National Health and Nutrition Examination Survey of Korea in a structural econometric model, this paper attempts to control the endogeneity problem inherent to the individual decision as to whether to undergo medical examinations, and provides estimation results showing that increased individual health awareness via medical examinations exerts a statistically significant positive effect on health investments. From the policy perspective, the estimation results of this paper may provide a rationale supporting the health policy of free provision of health examination services to the insured via National Health Insurance.

  • High blood pressure causes various cardiovascular diseases and is associated with mortality. Periodic self-monitoring and recording of blood pressure is very helpful in preventing the occurrence of secondary diseases caused by hypertension. However, existing cuff-type blood pressure monitors have many limitations. As an alternative of that, a method of estimating the blood pressure by measuring the velocity change of the blood flow using the photo plethysmography is widely known. However, photo plethysmography have a low correlation with blood flow. So, we will propose an algorithm for estimating blood pressure using the relationship between velocity change of blood flow measured by magnetic field instead of photo plethysmography and electrocardiogram. For this purpose, First, we analyzed the correlation between photo plethysmography and magneto plethysmography. the correlation between MPG and PPG was r = 0.9449. Second, we compared estimated blood pressure and measured blood pressure. In the four experiment each result was r = 0.5737, r = 0.7863, r = 0.5669, and r = 0.7445.

  • Blood pressure (BP), one of the most important vital signs, is used to identify an emergency state and reflects the blood flow characteristics of the cardiovascular system. The conventional noninvasive method of measuring BP is inconvenient because patients must wear a cuff on their arm and the measurement process takes time. This paper proposes an algorithm for estimating the BP using the pulse transit time (PTT) of the photoplethysmography (PPG) and pressure pulse from finger at the same time as a more convenient way to measure the BP. After recording the electrocardiogram (ECG), measuring the pressure pulse, and performing PPG, we calculated the PTT from the acquired signals. Then, we used a multiple regression analysis to measure the systolic and diastolic BP indirectly. Comparing the BP measured indirectly using the proposed algorithm and the real BP measured with a sphygmomanometer, the systolic pressure had a mean error of ${\pm}3.240$ mmHg and a standard deviation of 2.530 mmHg, while the diastolic pressure had a satisfactory result, i.e., a mean error of ${\pm}1.807$ mmHg and a standard deviation of 1.396 mmHg. These results are more superior than existing method estimating blood pressure using the one PTT and satisfy the ANSI/AAMI regulations for certifying a sphygmomanometer i.e., the measurement error should be within a mean error of ${\pm}5$ mmHg and a standard deviation of 8 mmHg. These results suggest the possibility of applying our method to a portable, long-term BP monitoring system.

A prospective study to assess the efficacy and safety of oral propranolol as first-line treatment for infantile superficial hemangioma

  • Purpose: To determine the efficacy and safety of oral propranolol as a first-line treatment for superficially located infantile hemangioma (IH) and propose an assessment tool to measure treatment response. Methods: Patients with superficial IH under 1 year of age were prospectively recruited between May 2012 and December 2013 at the Department of Pediatrics of Chungbuk National University Hospital. Propranolol was administered to 12 infants (median age, 3.8 months) while monitoring cardiovascular and adverse metabolic effects. If a patient showed no adverse events, the dosage was gradually increased up to 3 mg/kg/day and maintained for 1 year. We used our own scoring system to assess treatment response using parameters like change in color, and longest diameter, and thickness of the IH. Results: Eleven out of 12 patients completed the protocol with consistent improvement of hemangiomas during therapy. Patients on propranolol showed a more than 50% involution in the first 3 months, with additional steady involution until 1 year. Patients with the highest scores at 1 month maintained their score and showed better responses until treatment termination. The patient with the lowest score at 1 month did not show any further regression and stopped propranolol treatment 4 months after initiation. In two children with recurrences after successful therapeutic regression, propranolol was effective after being reintroduced. Propranolol treatment was not interrupted in any patient due to adverse events. Conclusion: Oral propranolol at 3 mg/kg/day showed a consistent, rapid, and therapeutic effect on superficial IHs without significant adverse events.

Effects of Antihistamines on the Cardiovascular System in Telemetered Conscious Dogs (원격무선측정장치를 이용한 항히스타민제의 심혈관계 영향평가)

  • Two Antihistamines, terfenadine and astemizole have been withdrawn from major markets for the reason that these durgs have been reported to induce QT interval prolongation associated with the onset of Torsades do Pointes (TdP), resulting in a life-threatening ventricular arrhythmia. In this study, we investigated effects of diphenhydramine on electrocardiograms and hemodynamic parameters in conscious telemetered dogs. We validated and defined the sensitivity of the test system by monitoring basal parameters and using positive control substance, terfenadine. Single administration effects were tested during 24 hours for each test drug at dose 1 mg/kg, 10 mg/kg, 30 mg/kg, 100 mg/kg. We monitored QT, QTc, heart rate, blood pressure and body temperature after administering test drugs. In conscious telemetered dogs, diphenhydramine significantly prolonged $QT\;(6.8\%\;of\;basal)\;and\;QTc\;(7.8\%\;of\;basal)$ at 100 mg/kg. Other parameters were not affected significantly. These findings suggest that antihistamines could induce important clinical relevance for patients taking excessive dosages of conventional antihistamines and those at risk of developing cardiac arrhythmias. Future studies that include other antihistamines and other classes will be necessary to predict the torsadogenic risk of drugs in humans.

Cardiac Toxicities Associated with Herbal Remedies (초본 섭취 후 발생한 심장독성)

  • Purpose: Herbal preparations have long been used for medical purposes by traditional cultures, and their use is increasing in modern societies. However, many herbal agents produce specific cardiovascular toxicities in humans. We performed this study in order to investigate the clinical characteristics of the cardiac toxicities associated with herbal remedies. Methods: We conducted a retrospective study of 45 patients (mean age $57{\pm}10$ years) who presented with cardiotoxicity between January 2007 and May 2011 due to ingestion of herbal remedy substances. Patients were identified as suffering cardiotoxicity if they presented with chest pain, EKG abnormality, and elevation of cardiac enzyme. Results: Of the 45 total cases, 17 included hemodynamic instability (37.8%), 7 with increasing cardiac enzyme (15.6%), 2 with cardiac arrest (4.4%) and one case of mortality (2.2%). The cardiotoxic herb group that demonstrated the worst clinical course was Ranunculaceae. Conclusions: In our study results, 57.6% of the herbal intoxication patients demonstrated the effects of cardiotoxicity. Thus, we recommend careful monitoring of herbal intoxication patients.

  • There have recently been an increasing number of reports claiming that the periodontal disease, the most typical oral disease, is closely associated with various systemic diseases such as cardiovascular disease. Therefore, it is imperative to explore the methods that can ensure effective and cost-effective monitoring of periodontal disease in specific population groups, as well as individuals in the electronic medical record environment. However, traditional periodontal examination methods, such as periodontal probe, are considerably demanding and time-consuming for dental specialists, and furthermore, cause the risk of bacteremia as they are invasive procedures. Thus, the objective of this study was to validate the capability of new method for screening the periodontal disease using the self-reported questionnaire for periodontitis which is the non-invasive procedure to replace radiography.
