고유수용성 신경근 촉진법이 편마비 환자의 보행에 미치는 영향 (Effects of Proprioceptive Neuromuscular Facilitation Techniques on the Gait for Hemiplegic Patients)

  • The purpose of this study was to investigate the effects of pelvic and lower extremity exercise on the gait of 25 patients with hemiplegia. 11 men and 14 women were studied, with a mean age of 61.2 years and a means duration post-onset of 8.7 months. Each participant recieved a total of 15 sessions of PNF(five times per week), and each procedure lasting for 40 minutes with affected side pelvic anterior elevation and posterior depression pattern, lower extremity flexion adduction with knee flexion pattern had 20 minutes respectively. Results showed significant improvement in gait speed and cadence in after 1st, 2nd, 3rd week(5th, 10th, 15th session, respectively) compared with the before treatment(p<0.05). And improvement in gait speed and cadence accompanied with longer stride length.

  • 본 논문에서는 Push-Pull 패스 트랜지스터 구조로 인하여 향상된 Load Transient 특성을 향상시킨 LDO(Low Drop-Out)를 제안하였다. LDO 레귤레이터 내부의 오차증폭기의 출력단과 패스 트랜지스터의 게이트단 사이에 제안된 Push-Pull 회로와 출력단에 Push-Pull 회로를 추가하여 전압 라인에 들어오는 Overshoot, Undershoot를 개선시켜 기존의 LDO 레귤레이터보다 개선된 Load Transient 특성의 델타 피크 전압 값을 갖는다. 제안하는 회로는 Cadence의 Virtuoso, Spectre 시뮬레이션을 이용하여 삼성 0.13um 공정에서 특성을 분석하였다.

  • 본 논문에서는 센트럴 모우먼트 알고리즘을 디지틀 통신 채널에 적용함으로써 기존의 시뮬레이션인 기법인 Conventional Importance Samping와 Improved Importance Sampling 으로 해결할 수 없었던 문제점들의 해결 뿐 아니라 수행시간의 획기적인 개선도 가능하게 하였다. 즉 디지털 통신 채널의 수신단에서 잡음이 혼합된 수신신호의 센트랄 모우멘트를 측정하여 수신 신호의 확률적인 특성인 누적확률분포를 구함으로써 채널의 성능을 평가한다. 제안 알고리즘을 검증하기 위하여 Cadence사의 시뮬레이션 프로그램인 SPW를 이용하여 QPSK 위성통신 채널을 구현한 후 시뮬레이션 수행시간의 개선 효과를 확인하였다.

노인의 균형 및 보행과 족관절 근력과의 상관관계 (Correlations between Muscle Strength of the Ankle and Balance and Walking in the Elderly)

  • Purpose: This study was designed to investigate the correlations between muscle strength of the ankle and balance, walking in the elderly. Methods: Thirty-nine subjects were selected from a population of female volunteers. Measurement of balance ability included evaluation of timed "up and go", functional reach, and a one leg standing test. Measurement of walking analysis included evaluation of cadence, stride length, step length, and walking speed. Maximal voluntary isometric contraction (MVIC) of the ankle muscle strength was measured by use of a dynamometer. Results: For balance, there were significant negative correlations between timed "up and go" and the MVIC of the ankle dorsiflexor. There were significant positive correlations between one leg standing with the eyes closed and the MVIC of the ankle dorsiflexor. For walking, there were significant positive correlations between cadence, walking speed and the MVIC of the ankle dorsiflexor. Conclusion: This study showed that there were close relationships between muscle strength of the ankle dorsiflexor and walking and balance in the elderly.

Effects of Knowledge of Result Versus Knowledge of Performance on Treadmill Training on gait ability in Stroke Patients: A Randomized Clinical Trial

  • Purpose: This study compared the walking ability of chronic stroke patients following either treadmill training with knowledge of the result (KR group) or treadmill training with knowledge of the performance (KP group). Methods: Nineteen patients with chronic stroke were recruited from a rehabilitation hospital. The patients were divided into two groups: a KR group (10 patients) and a KP group (9 patients). They received 30 minutes of neuro-developmental therapy and treadmill training 30 minutes, five times a week for three weeks. The gait parameters were measured before and after training using the Optogait system. Results: After the training periods, the KR group showed significant improvement in gait speed, cadence, step length of the unaffected limb, stance time of the affected limb, and functional gait assessment compared to the KP group (p<0.05). Conclusion: The results showed that treadmill training with KR was more effective in improving the gait speed and cadence, step length of the unaffected limb, stance time of the affected limb, and functional gait ability than the treadmill training with KP. Therefore, to improve the walking ability of stroke patients, it is necessary to consider treadmill training with KR. If it can be combined with conventional neurological physiotherapy, it would be an effective rehabilitation for stroke patients.

Effects of Motor Imagery Practice in Conjunction with Repetitive Transcranial Magnetic Stimulation on Stroke Patients

  • The aim of the present study was to examine whether motor imagery (MI) practice in conjunction with repetitive transcranial magnetic stimulation (rTMS) applied to stroke patients could improve theirgait ability. This study was conducted with 29 subjects diagnosed with hemiparesis due to stroke.The experimental group consisted of 15 members who were performed MI practice in conjunction with repetitive transcranial magnetic stimulation, while the control group consisted of 14 members who were performed MI practice and sham therapy. Both groups received traditional physical therapy for 30 minutes a day, 5 days a week, for 6 weeks; additionally, they received mental practice for 15 minutes. The experimental group was instructed to perform rTMS and the control group was instructed to apply sham stimulation for 15 minutes. Gait analysis was performed using a three-dimensional motion capture system, which is a real-time tracking device that delivers data via infrared reflective markers using six cameras. Results showed that the velocity, step length, and cadence of both groups were significantly improved after the practice (p<0.05). Significant differences were found between the groups in velocity and cadence (p<0.05) as well as with respect to the change rate (p<0.05) after practice. The results showed that MI practice in conjunction with rTMS is more effective in improving gait ability than MI practice alone.

FES를 이용한 편마비 환자의 보행증진에 대한 연구 (Study for gait improvement of hemiplegic patients using Functional Electrical Stimulation)

  • The purpose of this study was to determine the effects of FES on the clinical test patients who had disturbance gait due to cereberal vascular accident. The subjects for study were 16 hemiplegic patients (9 Male and 7 Female) who can gait with or without device. Their average age was 55 and they received average of 20.19 month of treatment collected data analysis was completed by using one-way analysis variable(ANOVA), Pearson ($-1{\leq}r{\geq}1$). The results were as follows : 1) There was difference in four variable (stride length,gait speed,gait cadence) between at the biginning and at the end of the treatment of FES (p<0.01). 2) There was relationship in capacity of activity between MAS and stride length (r = 0.751), gait speed (r = 0.689) but no relationship gait cadence (r = 0.236). 3) Age revealed relationship of stride length (r = -0.727), gait speed (r = -0.725), gait cadenc (r = -0.362). 4) There was no relationship in months post-CVA with MAS (r = 0.171), stride length (r = -0.110), gait speed (r = -0.096), gait cacedce (r = -0.154).

장애물 높이에 따른 보행의 운동형상학적 변화에 대한 연구 (The Kinematic Patterns of Walking according to Obstacle's Height)

  • Background : The Purposes of this study were to understand difference between free walking and obstacle over walking through the naked eye and motion analysis device, and to review merits of obstacle walking training as item of functional assessment in clinical situations. Methods : All participants were male and performed 3 types of walking methods: free walking, obstacle over walking with low block(height=10cm, width=8cm), and obstacle over walking with high block(height=20cm, width=8cm). All walking were performed 3 trials respectively. Results : In the naked eye, initial contact with toes occurred more than heel strike in obstacle over walking, and the flexion angle of hip and knee were increased in obstacle over walking. On interpretations though motion analysis device, cadence, gait speed and weight accept were significant statistically(p<.05). Cadence and gait speed were decreased, and weight accept duration was increased in obstacle over walking. Rotation among three pelvic motions was significant statistically(p<.05), flexion among three hip motions was significant statistically(p<.05) and flexion among three ankle motions was significant statistically(p<.05). Rotation and flexion among three ankle motions was significant statistically(p<.05). Conclusion : Both the naked eye and interpretations of the device presented many difference between free walking and obstacle over walking. In overcrossing obstacles, many participants appeared walking strategy by perform initial contact with toes. Knee flexion was most significant statistically(p<.05) in obstacle over walking with 20cm block.

Spatiotemporal Gait Parameters That Predict the Tinetti Performance-Oriented Mobility Assessment in People With Stroke

  • The purpose of this study was to find which spatiotemporal gait parameters gained from stroke patients could be predictive factors for the gait part of Tinetti Performance-Oriented Mobility Assessment (POMA-G). Two hundred forty-six stroke patients were recruited for this study. They participated in two assessments, the POMA-G and computerized spatiotemporal gait analysis. To analyze the relationship between the POMA-G and spatiotemporal parameters, we used Pearson's correlation coefficients. In addition, multiple linear regression analyses (stepwise method) were used to predict the spatiotemporal gait parameters that correlated most with the POMA-G. The results show that the gait velocity (r=.67, p<.01), cadence (r=.66, p<.01), step length of the affected side (r=.49, p<.01), step length of the non-affected side (r=.53, p<.01), swing percentage of the non-affected side (r=.47, p<.01), and single support percentage of the affected side (r=.53, p<.01) as well as the double support percentage of the non-affected side (r=-.42, p<.01) and the step-length asymmetry (r=-.64, p<.01) correlated with POMA-G. The gait velocity, step-length asymmetry, cadence, and single support percentage of the affected side explained 67%, 2%, 2%, and 1% of the variance in the POMA-G, respectively. In conclusion, gait velocity would be the most predictive factor for the POMA-G.

Effects of Diagonal Pattern Self-Exercise on Trunk Control, Balance, and Gait Ability in Chronic Stroke Patients

  • Background: Weakness of the trunk muscles decreases the trunk control ability of stroke patients, which is significantly related to balance and gait. Objectives: To compare the impact of diagonal pattern self-exercise on an unstable surface and a stable surface for trunk rehabilitation on trunk control, balance, and gait ability in stroke patients. Design: Nonequivalent control group design. Methods: Twenty four participants were randomized into the experimental group (diagonal pattern self-exercise while sitting on an unstable surface, n=12) and the control group (diagonal pattern self-exercise while sitting on a stable surface, n=12). All interventions were conducted for 30 minutes, three times a week for four weeks, and the trunk impairment scale (TIS), berg balance scale (BBS), functional gait assessment (FGA), and G-walk were measured. Results: All groups indicated significant increases in all variables (TIS, BBS, FGA, cadence, speed, stride length) after four weeks. The TIS, BBS, FGA, cadence, gait speed, and stride length group-by-time were significantly different between the two groups. Conclusion: We found that, in stroke patients, diagonal pattern self-exercise on an unstable surface is a more effective method for improving trunk control, balance, and gait ability than diagonal pattern self-exercise on a stable surface.
