Transcutaneous Electrical Nerve Stimulation System for Treating Tinnitus based on the Cortex-M4 Microcontroller

  • Among the methods of treating tinnitus, the transcutaneous clectrical nerve stimulation (TENS) method of treating by electrical stimulation is common. However, there is a problem that surgical operation is required to stimulate the vagus nerve (VN) main trunk near most of the bronchus. Alternatively, we found that the same effect could be achieved by electrically stimulating the vagus nerve VN branch (Arnold's nerve) distributed in the outer ear. The TENS system for stimulation of vagus nerve has been developed, but it has not been able to implement to stimulate as a parameter optimized for the patient by simultaneously playing the sound of eliminating the tinnitus frequency. Therefore, in this paper, it is important to develop a safe and practical TENS device for tinnitus treatment based on a 32-bit microprocessor that simultaneously applies non-invasive and notched sounds and to develop optimal treatment methods for treating tinnitus.

  • Congenital bronchial atresia is a rare anomaly, which results from a congenital focal obliteration of a proximal segmental or subsegmental bronchus, with normal development of the distal structures. The short atretic segment leads to the accumulation of mucus within the distal bronchi, forming a bronchocele and air trapping of the alveoli supplied by these bronchi. The diagnostic CT features include the presence of a branching opacity and the bronchocele, which radiate from the hilum and are surrounded by an area of hyperlucency.

Fiberoptic Bronchoscopy for Removal of Endobronchial Foreign Bodies in Adults (굴곡성 기관지경을 이용한 성인의 기도내 이물 제거)

  • Foreign body aspiration occurs uncommonly in adults. A review of recent reports reveals only a few case reports on this topic. We have experienced 8 cases of endobronchial foreign bodies in adults from June 1988 to February 1991 which were removed successfully with fiberoptic bronchoscope and biopsy forceps. Only one of them had the primary disorder predisposing aspiration. Foreign bodies were located in right lower lobe (3 cases), right intermediate (2 cases), left lower lobe (2 cases) and left upper lobe bronchus (1 cases). Removal of endobronchial foreign boides in adult would be accomplished by fiberoptic bronchoscopy successfully.

Endobronchial Hamartoma - 1 case - (기관지내 발생한 폐과오종 - 1례 보고 -)

  • Pulmonary hamartoma has been considered as rare disease, which consists of lung tumor less than 1 %. Originally described by Albrecht in 1904, hamartoma is tumor like malformation-abnormal mixing of the normal components of organ-and is applied also to tumor found in many organs other than the lung. Lately, the major conclusions are that pulmonary hamartoma is neoplastic rather than developmental error in origin. Because pulmonary hamartoma frequently mimics lung cancer, especially in cancer-risk age groups, its clinical significance is great. Recently, we experienced 1 cases of endobronchial hamartoma which located at the right main stem bronchus. The patient was a 54 year old male who was admitted due to symptoms of fever 4 chilliness and dyspnea. Radiologic studies such as chest x-ray, chest tomogram and chest C-T scan revealed that the nearly total haziness of the right lung was caused by endobronchial tumor. The mass was considered as a benign by bronchoscopic exam, so we removed it surgically by tracheobronchotomy without pulmonary resection. Postoperative chest x-ray revealed satisfactory reexpansion of previous collapsed right lung. The patient discharged uneventfully.

Bronchogenic Cyst - 1 case report - (기관지성 낭종 -1례 보고-)

  • Bronchogenic cysts are uncommon congenital lesions which are derived from primitive foregut. Most of the bronchogenic cysts may occur at the tracheal bifurcation, both main bronchi, the lung parenchyme and the mediastinum. We experienced a case of bronchogenic cyst with a esophageal stalk. The diagnosis was made by simple chest x-ray and confirmed by bronchoscopy and chest CT. On the chest CT findings, 6.8X4.8 cm-sized oval shaped mass was located on the right posteroinferior side of the carina, elevating the right main bronchus and the thin wall of the mass was enhanced with contrast materials. On the operative findings, the esophagus and the cyst were connected with a stalk and the cyst was filled with mucinous materials. And on the histological findings, the mass was lined with pseudostratified ciliated columnar epithelium. Thus we report this case of bronchogenic cyst with review of literatures.

  • 유아에서 흔치 않지만, 선천적 혹은 후천적 원인으로 혈관에 의해 기도가 눌릴 수 있다. 8개월 남자 환아가 승모판 성형술, 동맥관 개존증 결찰 후 좌측 폐의 무기폐가 발생하였다. 흉부 컴퓨터 단층 촬영상 동맥관 개존증 결찰 후 남은 비대해진 하행대동맥에 의해 좌측 폐의 무기폐가 발생하였다. 흉부 컴퓨터 단층 촬영상 동맥관 개존증 결찰 후 남은 비대해진 하행대동맥에 의해 좌측 주기관지가 눌리는 소견이 관찰되었다. 결찰 부위를 완전히 분리하고 하행대동맥을 봉합하여 크기를 줄여 기관지 압박을 해결할 수 있었다. 술 후 환자는 특별한 합병증 없이 퇴원하였으며 외래 추적 관찰 중이다.

  • The intractable pneumothorax with continuous air leakage, emphysematous lung and high operative risk treated by selective bronchial occlusion has been seldomly reported abroad. The bronchus responsible for air leakage was occluded with such materials as fibrin glue, gelatin sponge and oxidized regenerated cellulose(surgicel). We performed selective bronchial occlusion by flexible fiberoptic bronchoscopy with gelfoam in two cases. There was no complication after the procedure; therefore, we report the treatment for intractable pneumo- thorax by bronchoscopy with gelfoam packing.

  • Primary endobronchial leiomyosarcoma is a very uncommon tumor and its diagnosis by spontaneous expectoration of tumor fragment has never been reported in the literature. We report a patient with primary endobronchial leiomyosarcoma that was diagnosed by spontaneous expectoration of partial tumor tissue. The expectorated tissue was found to be pathologically consistent with leiomyosarcoma. Right lower lobectomy and all lymph node dissections were performed during the operation. Pathologic examination showed that the tumor was histologically identical to the patient's previous expectorated tissue and was confined to the bronchus and did not invade the adjacent pulmonary parenchyma and lymph nodes.

  • Bronchopleural fistula (BPF) is relatively rare, but it has high morbidity and mortality rates and it is associated with a prolonged hospital stay and high costs. Surgical treatment is the treatment of choice, but other minimal invasive forms of conservative management, and particularly bronchoscopy, have recently been investigated. We report here on the bronchoscopic treatment of a bronchopleural fistula accompanied necrotizing pneumonia, and we used coils and fibrin glue to treat the fistula.

  • We have recently removed three interesting foreign bodies uneventfully. Case 1: In 20 years old boy, the bullet which penetrated his trachea and esophagus through, lodged on second thoracic vertebra. Case 2: In 1 5/12 year old boy, 52 cm metallic neck lace was hung on the bifurcation of bronchus. Case 3: In 17 years old boy, the pin was stuck in the wall of his esophagus.
