2형 당뇨병환자의 만성 치주염 치은조직에서 골흡수 인자의 발현 양상 비교 (The Gingival Tissue Levels of Bone Resorptive Mediators in Human Chronic Periodontitis with Type 2 Diabetes Mellitus)

  • Purpose: The purposes of this study were to compare and quantify the expression of MMP-3, $PGE_2$ and IL-6 in the gingival tissues of patients with $type_2$ diabetes mellitus and healthy adults with chronic periodontitis. Material and methods: Gingival tissue samples were obtained during periodontal surgery or tooth extraction. According to the patient's systemic condition & clinical criteria of gingiva, each gingival sample was devided into three groups. Group 1(n=8) is clinically healthy gingiva without bleeding and no evidence of bone resorption or periodontal pockets, obtained from systemically healthy 8 patients. Group 2(n=8) is inflamed gingiva from patients with chronic periodontitis. Group 3(n=8) is inflamed gingiva from patients with chronic periodontitis associated with type 2 DM. Tissue samples were prepared and analyzed by Westernblotting. The quantification of MMP-3, $PGE_2$ and IL-6 were performed using a densitometer and statistically analyzed by one-way ANOVA followed by Tukey test. Results: 1. The expression levels of MMP-3 were shown highest in group 3 compared to group 1 and 2, and It showed increasing tendency in group 2 and 3. 2. The expressions of $PGE_2$ and IL-6 were shown increasing tendency in group 2 and 3, and It was highest in group 3. 3. As expressions of MMP-3 were increased, $PGE_2$ and IL-6 expressions showed increasing tendency in group 3 than group1 and 2, although there were no proportional relationship. Conclusion: This study demonstrated that the expression levels of MMP-3, $PGE_2$ and IL-6 will be inflammatory markers of periodonta linflamed tissue and DM. It can be assumed that MMP-3 affect to expressions of $PGE_2$ and IL-6 in progression of periodontal inflammation with alveolar bone resorption to type 2 DM.

골반 부전 골절의 임상 양상과 치료 결과 (Clinical Features and Outcomes of Pelvic Insufficiency Fractures)

  • 목적: 이번 연구의 목적은 고령 인구의 골반 부전 골절에 대한 방사선적, 역학적 특징과 임상 경과를 알아보는 것이다. 대상 및 방법: 2010년 3월부터 2017년 5월까지 골반 부전 골절 환자를 대상으로 후향적 조사를 시행하였다. 환자의 인구학적 정보, 골밀도, 골표지자를 조사하였다. 단순 방사선 촬영과 컴퓨터 단층촬영을 비교하였다. 임상 경과 평가를 위해 이환된 합병증과 골절 6개월 이후 보행 능력을 비교하였다. 결과: 연구 대상 총 42명의 평균 나이는 76.5세였고, 1명을 제외하고는 모두 여성이었다. 초진 병원이 타 병원인 5예 중 골절 진단을 받지 못한 경우가 3예였다. 단순 방사선 사진에서 발견된 골절 외 컴퓨터 단층촬영에서 추가로 골절이 발견된 경우가 81.0%였다. 모두에서 보존적 치료를 시행하였다. 골절 후 보행 시작까지 걸린 시기는 평균 2.8주였으며 42명 중 1예를 제외한 전체 예(97.6%)에서 수상 전 보행 상태로 회복이 가능하였다. 결론: 골반 부전 골절을 진단하는 경우 단순 방사선 사진으로는 제한점이 있는 것을 확인하였다. 골반 부전 골절은 보존적 치료로 좋은 결과를 얻을 수 있었으며, 치명적인 합병증이 비교적 적게 발생하였다.

뼈에 발생한 혈관성 파종 양상을 보이는 다발성 상피상 혈관육종: 증례 보고 (Multicentric Epithelioid Angiosarcoma of Bones Showing Angiotropic Spread: A Case Report)

  • 상피상 혈관육종은 혈관에 생기는 육종 중 드문 변이이다. 이 종양은 대개 연부조직에서 발생하고, 뼈에서 발생하는 경우는 특히 드물다. 다발성으로 발생한 경우 이 종양은 이차성 골전이와 혼동될 수 있으며, 매우 치명적인 임상 진행을 보일 수 있다. 우리는 61세의 남성에서 다발성으로 발생한 상피상 혈관육종의 증례를 보고하고자 한다. 이 종양은 혈관성 파종으로 인해 발생한 것으로 추정된다. 엑스선 단순 촬영, CT 및 MRI에서 양쪽 하지 장골에 여러 개의 골융해성 병변이 관찰되었다. 이 중 일부는 골피질을 파괴하면서 연조직성 병변도 동반되었다. 초기 영상의학적 검사 결과로는 전이성 암종이 의심되어, PET-CT를 촬영하였다. 흥미롭게도, PET-CT에서는 이러한 병변들이 양쪽 하지 뼈의 반골, 장골, 경골을 따라 분포하고 있어, 혈관을 따라 파종할 가능성이 있는 혈관육종의 가능성도 확인되었다. 병리학적 소견에서 또한 처음에는 전이성 암종의 가능성을 고려하여 추가적인 면역 화학 염색 검사를 시행하였다. 그러나, 혈관성 표지자인 CD31, ERG 등의 검사 결과 강한 양성 소견을 보여 최종적으로 이 병변은 상피상 혈관육종으로 진단되었다. 이번 증례를 통해 뼈에 일차성으로 발생한 상피상 혈관육종에 대한 임상적, 영상의학적 특징을 정리하여, 진단적 혼동을 줄이고 더 나은 치료 방법을 제공하는데 도움이 될 것이다.

Heat or radiofrequency plasma glow discharge treatment of a titanium alloy stimulates osteoblast gene expression in the MC3T3 osteoprogenitor cell line

  • Purpose: The purpose of this study was to determine whether increasing the Ti6Al4V surface oxide negative charge through heat ($600^{\circ}C$) or radiofrequency plasma glow discharge (RFGD) pretreatment, with or without a subsequent coating with fibronectin, stimulated osteoblast gene marker expression in the MC3T3 osteoprogenitor cell line. Methods: Quantitative real-time polymerase chain reaction was used to measure changes over time in the mRNA levels for osteoblast gene markers, including alkaline phosphatase, bone sialoprotein, collagen type I (${\alpha}1$), osteocalcin, osteopontin and parathyroid hormone-related peptide (PTH-rP), and the osteoblast precursor genes Runx2 and osterix. Results: Osteoprogenitors began to differentiate earlier on disks that were pretreated with heat or RFGD. The pretreatments increased gene marker expression in the absence of a fibronectin coating. However, pretreatments increased osteoblast gene expression for fibronectin-coated disks more than uncoated disks, suggesting a surface oxide-mediated specific enhancement of fibronectin's bioactivity. Heat pretreatment had greater effects on the mRNA expression of genes for PTH-rP, alkaline phosphatase and osteocalcin while RFGD pretreatment had greater effects on osteopontin and bone sialoprotein gene expression. Conclusions: The results suggest that heat and RFGD pretreatments of the Ti6Al4V surface oxide stimulated osteoblast differentiation through an enhancement of (a) coated fibronectin's bioactivity and (b) the bioactivities of other serum or matrix proteins. The quantitative differences in the effects of the two pretreatments on osteoblast gene marker expression may have arisen from the unique physico-chemical characteristics of each resultant oxide surface. Therefore, engineering the Ti6Al4V surface oxide to become more negatively charged can be used to accelerate osteoblast differentiation through fibronectin-dependent and independent mechanisms.

Relationship Between the Dose of Clodronate and Serum Level of Alkaline Phosphatase, Calcium, and Phosphate During Orthodontic Tooth Movement

  • Objective : To evaluate the relationship between the dose of Clodronate and serum level of alkaline phosphatase (ALP), calcium (Ca), and phosphate (PO4) during orthodontic tooth movement MaterialS and MethodS: A total of 18 sex-matched Wistar rats (weight=180~230g, mean age=8 weeks) were allocated into the 2.5mM Clodronate (2.5C) group, 10mM Clodronate (10C) group, or control group (n=6 for each group). After the application of a nickel-titanium closed coil spring (force of 60g) between the upper central incisors and first molars (UFM), 2.5C, 10C, or saline was injected every third day into the subperiosteum of the alveolar bone adjacent to UFM for the experimental and control groups. The animals were sacrificed 17 days later. Trunk blood was quickly collected into a heparinized tube and centrifuged at 2,000 rpm for 20 min. The plasma was used for the biochemical assays of the serum level of ALP, Ca, and PO4. Kruskall-Wallis test and Mann-Whitney test with Bonferroni correction were performed for the statistical analyses. Results : Dose-dependent increase in the level of ALP (P<0.01) and decrease in the level of Ca (P<0.001) were observed among the control, 2.5C, and 10C groups. Although there was no significant difference in PO4 between the 2.5C and 10C groups, the 10C group showed a significantly higher level of PO4 than the control group (P<0.01). Conclusion : Since Clodronate induced significant dose-dependent change in the serum level of ALP, Ca, and PO4 during orthodontic tooth movement, orthodontists should consider these biochemical markers not only as a diagnostic tool for bone turnover rate but also as a monitoring tool for orthodontic tooth movement.

Genome-wide association study identifies 22 new loci for body dimension and body weight traits in a White Duroc×Erhualian F2 intercross population

  • Objective: Growth-related traits are important economic traits in the swine industry. However, the genetic mechanism of growth-related traits is little known. The aim of this study was to screen the candidate genes and molecular markers associated with body dimension and body weight traits in pigs. Methods: A genome-wide association study (GWAS) on body dimension and body weight traits was performed in a White $Duroc{\times}Erhualian$ $F_2$ intercross by the illumina PorcineSNP60K Beadchip. A mixed linear model was used to assess the association between single nucleotide polymorphisms (SNPs) and the phenotypes. Results: In total, 611 and 79 SNPs were identified significantly associated with body dimension traits and body weight respectively. All SNPs but 62 were located into 23 genomic regions (quantitative trait loci, QTLs) on 14 autosomal and X chromosomes in Sus scrofa Build 10.2 assembly. Out of the 23 QTLs with the suggestive significance level ($5{\times}10^{-4}$), three QTLs exceeded the genome-wide significance threshold ($1.15{\times}10^{-6}$). Except the one on Sus scrofa chromosome (SSC) 7 which was reported previously all the QTLs are novel. In addition, we identified 5 promising candidate genes, including cell division cycle 7 for abdominal circumference, pleiomorphic adenoma gene 1 and neuropeptides B/W receptor 1 for both body weight and cannon bone circumference on SSC4, phosphoenolpyruvate carboxykinase 1, and bone morphogenetic protein 7 for hip circumference on SSC17. Conclusion: The results have not only demonstrated a number of potential genes/loci associated with the growth-related traits in pigs, but also laid a foundation for studying the genes' role and further identifying causative variants underlying these loci.

조골세포의 분화에 산겨릅나무 추출물이 미치는 영향 (Effect of Acer tegmentosum Maxim. extract on differentiation of osteoblastic Primary calvarial osteoblasts cells)

  • Objectives : The present study, to confirm the osteoblast differentiation effects of Acer tegmentosum Maxim. (AT) extract. Methods : In this experiment, cell viability, Alizarin red S assay, and Alkaline phosphatase (ALP) activity with AT extract (50, $100{\mu}g/m{\ell}$). Also, we studied the expression of differentiation regulator with AT extract in primary calvarial osteoblasts cells (pOB). Results : As a result of AT treatment, we determined that AT extract stimulates ALP activity and alizarin red activities in the pOB cells for mineralization for 18 days. Moreover, these factors increasing osteogenic markers such as Runt-related transcription factor2 ($Run{\times}2$), osteocalcin (OC), osteopontin, osterix, smad1, smad5, activating transcription factor4 (ATF4) and collagen type I alpha 1. Conclusions : These results indicate that AT extract have effect on bone through the promotion of osteoblastic differentiation, suggesting that it could be used for the treatment of bone diseases.

Detection of Mendelian and Parent-of-origin Quantitative Trait Loci in a Cross between Korean Native Pig and Landrace I. Growth and Body Composition Traits

  • This study was conducted to detect quantitative trait loci (QTL) affecting growth and body composition in an $F_2$ reference population of Korean native pig and Landrace crossbreds. The three-generation mapping population was generated with 411 progeny from 38 $F_2$ full-sib families, and 133 genetic markers were used to produce a sex-average map of the 18 autosomes. The data set was analyzed using least squares Mendelian and parent-of-origin interval-mapping models. Lack-of-fit tests between the models were used to characterize QTL for mode of expressions. A total of 8 (39) QTL were detected at the 5% genome (chromosome)-wise level for the 17 analyzed traits. Of the 47 QTL detected, 21 QTL were classified as Mendelian expressed, 13 QTL as paternally expressed, 6 QTL as maternally expressed, and 7 QTL as partially expressed. Of the detected QTL at 5% genome-wise level, two QTL had Mendelian mode of inheritance on SSC6 and SSC9 for backfat thickness and bone weight, respectively, two QTL were maternally expressed for leather weight and front leg weight on SSC6 and SSC12, respectively, one QTL was paternally expressed for birth weight on SSC4, and three QTL were partially expressed for hot carcass weight and rear leg weight on SSC6, and bone weight on SSC13. Many of the Mendelian QTL had a dominant (complete or overdominant) mode of gene action, and only a few of the QTL were primarily additive, which reflects that heterosis for growth is appreciable in a cross between Korean native pig and Landrace. Our results indicate that alternate breed alleles of growth and body composition QTL are segregating between the two breeds, which could be utilized for genetic improvement of growth via marker-assisted selection.

일부 폐경 후 여성에서 영양상태 및 골대사 관련 일부 혈액 지표와 항산화효소 활성과의 관련성 분석 (The Relationship between Some Blood Parameters and Antioxidant Enzyme Activity in Korean Postmenopausal Women)

  • To elucidate the relationship between blood parameters related bone metabolism and antioxidant enzyme activity in postmenopausal period 60 women residing in Iksan area were recruited. Food and nutrient intake of each individual subject were estimated by 24-hour recalls of 3 non-consecutive days. The biochemical markers including total protein, albumin, osteocalcin (intact bone gla protein; BOP), calcium, phosphorus and hemoglobin were measured in fasting blood. In addition, parameters of antioxidative capacity including the activities of superoxide dismutase (SOD), glutathione peroxidase (GPx) , catalase (CAT) and total antioxidant capacity (TA) were monitored in blood, also. The mean age, height, weight, and BM! of subjects were 64.8 years, 151.1 em, 59.5 kg $26.0\;kg/m^2$, respectively. The mean SOD, GPx, and CAT activities were 138.5 U/ml, 1,273.8 U/ml and 314.3 kU/l respectively, and TA was 1.16 mmol/l without significant difference among different age groups. BMI was positively correlated with SOD activity (p < 0.01). SOD activity and CAT activity showed positive correlation with serum albumin (p < 0.05) and hemoglobin (p < 0.01). In conclusion, this study revealed that antioxidant enzyme activity holds a significant relationship with the blood parameters like as serum albumin and hemoglobin in postmenopausal women and further systematic research is needed to investigate the their relation mechanism.

조기발병형 치주염의 임상적 및 면역유전학적 연구 (CLINICAL AND IMMUNOGENETIC STUDY ON THE EARLY-ONSET PERIODONTITIS)

  • 542 periodontal patients having early-onset periodontitis(EOP) have been reclassified into a more homogeneous phenotypic subsets by newly revised radiographic criteria. Representative patients of each EOP subform have been examined of serum IgG subclass antibodies against periodontopathic bacteria, Porphyromonas gingivalis(Pg) 381 and of genetic markers for IgG allotypes to clarify the relationship between these parameters and phenotype expression of each subform. The early onset periodontitis could be reclassified by the radiographic parameters combining the mean interproximal alveolar bone loss(BL) and the radiographic ratio(between 1st molars and the adjacent teeth: Ratio) with statistical significance(p<0.001 by MANOVA). Moreover these EOP subforms could clearly be delineated from adult periodontitis. Of subform I and II(localized type EOP) patients with minimal mean bone loss(BL<5.0), patients demonstrating disease activities in localized areas(Ratio.>1.5) showed the elevated responses in all the IgG subclasses against Pg compared with those of patients without disease activity(Ratio <1.5). There were gradual increase in the IgG2 and IgG4 titers against Pg as the disease developed into the generalized forms suggesting the possible role of these antibodies in modulating the phenotype expression. The genetic marker study for IgG allotype revealed that mean IgG2 and IgG4 subclass titers were significantly higher(p<0.01, p<0.05, respectively) in patients who were positive for G2m(n). This indicated that IgG subclass responsiveness against the bacterial antigens are under the immnuogenetic control. The observed frequencies of G2m(n) were significantly higher (p<0.05) in subfrom IV patients who had the characteristic features of classical rapidly progressing periodontitis indicating the possible genetic predisposition in these patients.
