Urinary Tract Infections in Catheterized Patients Receiving Home Care Nursing (도뇨관 관리를 위해 가정간호에 의뢰된 환자의 요로감염실태)

  • Purpose: In this study, we aimed to explore the risk factors for catheter-associated urinary tract infections (CAUTI) in patients receiving home care nursing. Methods: A total of 117 participants registered for home care nursing provided by the University of C hospital in Seoul were included in the study. Data were based on a survey and urine examination results from June to July 2018. Results: Asymptomatic bacteriuria was identified in 96 (82%) patients. Age, sex, activity, diabetes mellitus, mental status, presence of other diseases, catheter material type, catheter size and fixation, hand washing (care giver), gloving (care giver), perineal care, and bladder irrigation were not recognized as risk factors for CAUTI. Age and catheterization duration were associated with CAUTI. Conclusions: Old age was found to be a risk factor for CAUTI (p=0.048). CAUTI incidence decreased as catheter use exceeded 70 months (p=0.028).

Reproductive Ecology of the Bladder Moon, Glossaulax didyma (Gastropoda: Naticidae) in Western Korea (한국 서해산 큰구슬우렁이, Glossaulax didyma (복족강: 구슬우렁이과) 의 번식생태)

  • The reproductive cycle, egg capsules in the egg-mass, first sexual maturity, and sex ratio of the bladder moon, Glossaulax didyma ($R\ddot{o}ding$) were investigated. The gastropods collected from the intertidal zone of Biin Bay, Seocheon, Korea were studied by using histological analysis and morphometric data. The gonadosomatic index (GSI) of females and males began to increase in March and reached maximum in May. Then their values sharply decreased from late in May to August due to spawning. The condition index (CI) began to increase in February and reached maximum in May, then gradually declined in the spawning period. The CI calculated for determination of the spawning period was coincided with changes in the GSI and gonadal phases. Spawning occurred between late in May to August in females and early in May to August in males. Spawning peak was observed between July and August when the seawater temperature rose to 19 $^{\circ}C$. Reproductive cycle with the gonadal development phases of this species can be divided into five successive stages in females and four in males: in females, early active stage (December to February), late active stage (February to March), ripe stage (April recovery stage (August to November); in males, active stage (December to March), ripe stage (March to July), copulation stage (early May to August), and recovery stage (August to January). Fully matured oocytes were approximately 250-270 ${\mu}m$ in size. The egg-mass was a hat in shape, and a number of egg capsules were found in an egg-mass. An egg capsule was 0.53-0.57 mm in size. An embryo (veliger larva) hatched from an egg capsule. Percentage of first sexual maturity in females and males were over 50% for individuals that are 40.1-45.0 mm in shell radius, and 100% for those that are over 45.1 mm. The sex ratio of female to male was significantly different from 1:1 $(x^2\;=\;57.22,\;p\;<\;0.05)$.

The Oogenesis of Coreoleuciscus splendidus, Cyprinidae, Teleostei (경골어류 잉어과 쉬리(Coreoleuciscus splendidus)의 난자형성과정)

  • Coreoleuciscus splendidus is a teleost belonging to Gobioninae, Cyprinidae. The oogenesis was investigated by light microscope. The ovary was located between intestine and air bladder, a grayish and ellipsoidal shape with the major axis 20 mm and the minor axis 5 mm. Cytoplasm of oogonia was basophilic and many nucleoli were located at inside of nuclear membrane. In primary oocytes, yolk vesicles were distributed only in the marginal area and egg envelope was not formed on the outside of an egg. In secondary oocytes, the egg envelope was formed and yolk vesicles in the cytoplasm were increased than the earlier stage. The basophilic substance of cytoplasm was changed to acidic. In case of matured egg, thickness of egg envelope and size of egg were increased. The yolk vesicles were changed to yolk mass in accordance with development. In conclusion, the oogenesis of C. splendidus was characterized by the increase in cell size, the formation and accumulation of yolk, and the decrease of basophilic substance in the cytoplasm. The oogenesis of C. splendidus is similar with other Cyprinidae fishes. But further study on ultrastructural study of fertilized egg envelope will be necessary to get the species specificity.

The Spermatogenesis of Chinese minnow, Leuciscinae, Teleostei (경골어류 황어아과 버들치의 정자형성과정)

  • The ultrastructure of spermatogenesis and sperm in Chinese minnow, Rhynchocypris oxycephalus belonging to Leuciscinae was investigated by light and electron microscopes. The whitish testis was located between intestine and air bladder. The size of testis was major axis 2.3 cm, minor axis 6 mm. The testis contained numerous testicular cysts, and spermatogenesis was non-synchronized in these testicular cysts. In the case of spermatogonium, the nucleus was comparatively large ellipsoidal, and mitochondria showed a marked development. The size of primary spermatocyte was smaller than that of spermatogonia, and secondary spermatocyte was smaller than primary spermatocyte. The chromatin of spermatocyte was highly condensed according to their development. The nucleus with electron-dense was round shape. In spermiogenesis, flagella started to be formed and chromatin was more condensed. The mitochondria were rearranged in a middle piece. The sperm was formed by loss of cytoplasm. The head of mature sperm was a spherical shape and have not acrosome. The microtubules of flagella were arranged 9+2 structure. Also, the tail of sperm have not lateral fins.

The Oogenesis of Glow-light Tetra, Characidae, Teleost (경골어류 카라신과 glow-light tetra의 난자형성과정)

  • Glow-light tetra, Hemigrammus erythrozonus is a teleost belonging to Characidae. The natural habitate of this fish is the wild in the Essequibo river, Guyana and South America. The oogenesis of glow-light tetra was investigated by light microscopy to compare with those of other families. A pair of ovary was located between swim bladder and intestines. The ovary was of white color and ellipsoidal shape with the major axis 11 mm and the minor axis 4 mm. Cytoplasm of oogonia was basophilic and many nucleoli were located at inside of nuclear membrane. In primary oocytes, yolk vesicles were distributed only in the marginal area and egg envelope was not formed on the outside of an egg. In secondary oocytes, the egg envelope was formed and yolk vesicles in the cytoplasm were increased than the earlier stage. The amount of basophilic substance was decreased. In case of matured egg, thickness of egg envelope and size of egg were increased, basophilic substance was distributed in only around the egg envelope. The yolk vesicles were changed to yolk mass in accordance with development. In conclusion, the oogenesis of glow-light tetra, Hemigrammus erythrozonus was characterized by the increase in cell size, the formation and accumulation of yolk, and the decrease of basophilia a in the cytoplasm. The oogenesis of glow-light tetra seems to share common patterns in Characidae, teleost and have a similar pattern with other teleost.

  • A Gymnophallid cercaria was found in the gonadal tissue of Tapes philippinarum, and was described as a new species, Cercaria tapes n. sp. The host clams were collected from the intertidal zone of Seo-myon, Seo-chon gun, Korea from September 1980 to August 1981. The size of sporocysts was $380-650{\times}130-160{\mu}m$. The anterior part of the cercaria bears a subterminal opening and pharynx. The sporocyst bears ca. 30 cercariae and germ balls. The cercaria is elliptical in shape. The dimension of body is $160-187{\times}62-77{\mu}m$. the tail stem is $47-52{\times}17{\mu}m$, and the tail furcae is $67-72{\times}12-15{\mu}m$. Excretory bladder is Y-shaped, and the flame cell formula is 2(1)+(1)=4. The infection rate of the cercaria to the host clam was $2.01\%$ (42 infected/2,086 short-necked clams observed). The highest infection rate ($5.74\%$) was found in March, and the lowest one ($0.4\%$) observed in January. No infection was detected in May and June. The infection rates varied with the size of the short-necked clam. It was the clams of host of 2.1-3.0 cm, 3.1-4.0 cm and 4.1-5.0 cm in shell length are $0.38\%,\;15.3\%$ and $0.09\%$ respectively. The cercaria was not observed in the specimens smaller than 2.0 cm in shell length.

The head of matured sperm was a spherical shape and had not acrosome. The microtubules of flagella were arranged 9+2 structure. Also, the tail of sperm had not lateral fins and 7 outer coarse fibers.

Pretreatment Prognostic Factors in Carcinoma of the Uterine Cervix (자궁경부암에 있어서의 치료전 예후인자)

  • To identify pretreatment prognostic factors in carcinoma of the uterine cervix, a retrospective analysis was undertaken of 510 patients treated with curative radiation therapy in Seoul National University Hospital during the 7 year period, from March 1979 through December 1986. According to FIGO classification,35 patients were stage I B,89 were stage IIA, 232 were stage IIB,8 were stage IIIA, 134 were IIIB, and 12 were stage IVA. Five year locoregional control (LRC) rates in stage I B, II A, II B, IIIA, IIIB, and IVA were $79\%,78\%,70\%,58\%,51\%\;and\;27\%,$ respectively. Five year disease free survival (DFS) rates were $76\%,67\%,60\$,57\%,40\%,\;and\;25\%,$ respectively. Overall survival (OS) rates at five years were $82\%,72\%,67\%,67\%,51\%,\;and\;33\%,$ respectively. In univariate analyses, stage, age, initial hemoglobin level, type of histology, tumor size, and several CT findings including pelvic lymph node (LN) status, paraaortic lymph node (PAN) status, extent of parametrial invasion, bladder invasion, and rectal invasion were significant factors in terms of LRC. All these factors and elevation of BUN or creatinine were associated with DFS. In terms of overall survival, stage, initial hemoglobin level, type of histology, tumor size, elevation of BUN or creatinine, and five CT findings associated with LRC were prognostically significant. In multivariate analysis excluding CT findings, stage IV disease, non-squamous histology, and tumor size $\ge$4 cm were associated with poor LRC and DFS. Stage IV disease and tumor size significantly affected OS. in multivariate analysis including CT findings, histology, tumor size, and pelvic LN status on CT were uniformly significant in terms of LRC, DFS, and 05, PAN status on CT affected overall survival only.

Etiological Studies on the Acute Fatal Disease of Angora Rabbits : The So-Called Rabbit Viral Sudden Death (앙고라 토끼의 급성폐사성질병(急性斃死性疾病)의 병인학적(病因學的) 연구(硏究) : 소위(所謂) 토끼의 바이러스성(性) 급사병(急死病))

  • This paper dealt with etiological studies on the acute fatal disease of Angora rabbits occurring as a group in Korea. The disease was confirmed as an acute infectious disease caused by virus. The results obtained were summarized as follows: The disease produced a high morbidity in the rearing Angora rabbits and a high mortality in the infected rabbits, and was acute. The infected rabbits died soon without premonitory signs after inappetence. The body temperature of the affected rabbits rose to $40^{\circ}C$ and nearly all deaths occurred within 48 hours after inoculation. In many cases a bloody foam was visible from the nostrils after death. According to the progress of the disease the nervous signs, such as ataxia, paralysis of the legs, and torticollis could be recognized in the some cases. Rabbits that had recovered from the disease were severe emaciation, and bristly and sparse hairs. In macroscopical findings, there were hemorrhage and edema of the lung, hemorrhage or hyperemia of the tracheal and broncheal mucosae, appearance of blood-tinged effusion in the respiratory tract. The principal lesions were found in the liver. Usually the lobular necrosis of the liver cells was progressed, and focal necrosis and hemorrhagic spots of various sizes were often observed in the liver. Liver was as a whole pale. In chronic cases, however, there was a slight liver cirrhosis with the atrophy of the parenchymal cells. The other lesions encountered grossly consisted of swelling and petechiae of the kidney, hyperemia and hemorrhage of the spleen, catarrh of the small intestine, and hyperemia of the brain. The urinary bladder contained a lot of turbid urine or bloody urine and urinary cast, and was distended with the urine. In microscopical findings, the most striking lesions occurred in the liver and may be classified as viral hepatitis. The hepatic lesions were initially characterized by progression from periportal to peripheral necrosis of the lobules with the infiltration of mononuclear cells. Focal necrosis of various sizes, hemorrhage and hyperemia were often observed in the hepatic lobules. In chronic cases, there were intensive infiltration of lymphocytes, proliferation of fibroblasts, appearance of plasmal cells, and atrophy of parenchymal cells in the hepatic tissue. Perivascular lymphocytic infiltration and meningitis were seen in the brain and spinal cord. In the kidney, there were acute glomerulonephritis, hemorrhage, necrosis of the uriniferous tubules, and retention of eosinophilic substance within the renal tubules. Proliferation of fibroblasts and infiltration of mono-nuclear cells were found in the interstitial stroma of the kidney in chronic case. There were also hemorrhage and edema in the lung, hyperemia and hemorrhage in the trachea and bronchus, perivascular lymphocytic infiltration and focal myocardial necrosis in the heart, hyperemia and hemorrhage in the spleen, vacuolization and desquamation of mucous epithelia in the urinary bladder, catarrhal inflammation of the small intestine, hemorrhage in the adrenal cortex and hyperemia in the other organs. In the electron microscopical findings of the hepatic tissue, crystals of viral particles appeared in the cytoplasm of the hepatocytes and the sinusoidal endothelial cells, and the viral particles, were small in size and polygonal. The authors suppose the virus may belong to picornaviridae family of RNA viruses. Also immature virus-like particles, dilated rough endoplasmic reticulum and destruction of nuclear membrane were seen in the hepatocytes. From these results, it is concluded that the sudden death is an acute viral disease characterized by hepatitis and the affected rabbits may be died of viremia.

Evaluation of the dose of 16-MDCT and 64-MDCT in case of Coronary Artery CT Angiography using Thermoluminescence Dosimeter (열형광선량계를 이용한 16-MDCT와 64-MDCT의 관상동맥 CT 혈관조영술 시 선량평가)

  • Coronary artery CT angiography has short scanning length, the exposure dose is high. Therefore, it is required to study on the organ dose when using MDCT. We compared the differences between the absorbed dose and effective dose in the major organs assessing the absorbed dose in the major organs by 16-MDCT and 64-MDCT in the subjects with coronary artery CT angiography, the same protocol by 16-MDCT and 64-MDCT. As a result, the great orders of absorbed dose when conducting coronary artery CT angiography had been shown as heart, stomach, liver, pancreas, kidney, spleen, large intestine, lung, small intestine, thyroid gland, ovary, bladder, and orbit with the absorbed dose distribution of $0.538{\pm}0.026(Mean{\pm}SD,\;p<0.05)mGy{\sim}71.316{\pm}4.316mGy$ in 16-MDCT, and heart, stomach, pancreas, spleen, liver, kidney, small intestine, large intestine, lung, thyroid gland, ovary, bladder, and orbit with the absorbed dose distribution of $0.87{\pm}0.01mGy{\sim}115.26{\pm}1.59mGy$ in 64-MDCT, demonstrating some different distributions. The exposed doses to the patient per one time scanning with coronary artery CT angiography were $71.316{\pm}4.316mGy$ in 16-MDCT as the absorbed dose based on the heart and $115.26{\pm}1.59mGy$ in 64-MDCT. The effective doses were 7.41 mSv and 12.11 mSv in 16 and 64-MDCT, respectively. Taking into account the results of brain CT with 2.8 mSv that has comparatively large scanning length and size, facial CT 0.8 mSv, chest CT 5.7 mSv, pelvic CT 7.2 mSv, and abdominal and pelvic CT 14.4 mSv, it is very high considering the scanning length of 13 cm limited to the heart for the scanning range.
