Systemic Review of Hyaluronate for the Treatment of Osteoarthritis (골관절염 치료제 히알우론산 임상연구결과의 Systemic Review)

  • Background: The multitude of the therapeutic usefulness of intra-articular injection of hyaluronate on oateoarthritis of the knee is still in question. The objective of this systemic review was to elucidate both the therapeutic efficacy and the safety of intra-articular administration of hyaluronic acid for degenerative osteoarthritis of the knee joints. Methods: I searched MEDLINE and Korea Medical Database (KMbase) from January 1990 to April 2007 using a combination search terms for knee osteoarthritis and hyaluronic acid and a filter for randomized controlled trials. I extracted data on pain at rest, and during or just after movement, on joint function, and on adverse events. Results: Ten trials that reported usable quantitative information on any of the predefined end points were identified and included in the systemic review. Intra-articular injection of hyaluronic acid can decrease symptoms of osteoarthritis of the knee. The study revealed significant improvements in pain and functional outcomes with few adverse effects. However, there was significant between-study heterogeneity in the estimates of the efficacy of hyaluronic acid. Sub-group analysis showed that lower methodological quality such as a single-blind or single-center design resulted in higher estimates hyaluronic acid efficacy, and that patients older than sixty years of age and those with the most advanced radiographic stage of osteoarthritis were less likely to benefit from intra-articular injection of hyaluronic acid. Conclusion: According to the currently available evidence, intra-articular hyaluronic acid has been proven clinically effective for the patients bearing the knee osteoarthritis with NSAID-induced GI troubles or inapplicable to any surgery, and may be associated with lower risk of adverse events.

A Flipflop with Improved Noise Immunity (노이즈 면역을 향상시킨 플립플롭)

  • As the data path of the processor widens and the depth of the pipeline deepens, the number of required registers increases. Consequently, careful attention must be paid to the design of clocked storage elements like latches and flipflops as they have a significant bearing on the overall performance of a synchronous VLSI circuit. As technology is also scaling down, noise immunity is becoming an important factor. In this paper, we present a new flipflop which has an improved noise immunity when compared to the hybrid latch flipflop and the conditional precharge flipflop. Simulation results in 65nm CMOS technology with 1.2V supply voltage are used to demonstrate the effectiveness of the proposed flipflop structure.

Cell Death-Associated Ribosomal RNA Cleavage in Postmortem Tissues and Its Forensic Applications

  • Estimation of postmortem interval (PMI) is a key issue in the field of forensic pathology. With the availability of quantitative analysis of RNA levels in postmortem tissues, several studies have assessed the postmortem degradation of constitutively expressed RNA species to estimate PMI. However, conventional RNA quantification as well as biochemical and physiological changes employed thus far have limitations related to standardization or normalization. The present study focuses on an interesting feature of the subdomains of certain RNA species, in which they are site-specifically cleaved during apoptotic cell death. We found that the D8 divergent domain of ribosomal RNA (rRNA) bearing cell death-related cleavage sites was rapidly removed during postmortem RNA degradation. In contrast to the fragile domain, the 5' terminal region of 28S rRNA was remarkably stable during the postmortem period. Importantly, the differences in the degradation rates between the two domains in mammalian 28S rRNA were highly proportional to increasing PMI with a significant linear correlation observed in mice as well as human autopsy tissues. In conclusion, we demonstrate that comparison of the degradation rates between domains of a single RNA species provides quantitative information on postmortem degradation states, which can be applied for the estimation of PMI.

The Effects of Repetitive Sit-to-Stand Training with a Paretic-side Asymmetrical Foot Position on the Balance of Chronic Stroke Subjects

  • Purpose: This study aimed to improve the asymmetrical weight-bearing ratio, by applying different repetitive sit-to-stand training methods to the paretic-side foot of hemiplegic patients, as well as to provide the necessary information for applying balance training with hemiplegic patients. Methods: The subjects were divided into two groups: a spontaneous foot group and an asymmetrical foot group. They all performed repetitive sit-to-stand training five times a week for a total of six weeks. The sit-to-standing movement was studied using standardized clinical tests. The Biodex Balance System, Time up and go test (TUG), 5 times sit-to-stand test (5XSST), and functional reach test (FRT) were used to measure the static and dynamic standing balance of the patients. Results: In the balance system measurement, the results for the overall index, ant-post index, med-lat index, fall risk index, 5XSST, and FRT after the training differed significantly between the comparison groups (p<0.05). In the evaluation of dynamic balance, the differences in TUG did not differ significantly between the comparison groups after the training (p>0.05). Conclusion: The study found that the asymmetrical group showed significant increases in static and dynamic balance in comparison to the spontaneous group after repetitive sit-to-stand training. Based on this result, it is clear that training in an asymmetrical position with the paretic foot back can increase the left-right stability limit and the anterior-posterior stability limit, thus improving balance control.

  • In order to obtain the geological information of the shallow subsurface and the characteristics of resistivity curves to the lateral extent of the gold bearing gravel layer underlaid by a thick layer, electrical resistivity surveys had been carried out in the northern area of Takaoi village, Kalimantan, Indonesia from September 30th to October 27th in 1999. The Dipole-Dipole (Dp-Dp) resistivity survey was carried out with a electrode spacing of A=3m, 5m and electrode separation index n=10, and the vertical electrical sounding (VES) of the Schlumberger type was performed at 6 sites. The interpretation of the Dp-Dp data and VES curves by computer modeling indicates 5 classified layers as the horizontal structure. And the characteristics of VES curves shows us KH type.

Lubrication Properties of Various Pattern Shapes on Rough Surfaces Considering Asperity Contact (돌기접촉을 고려한 거친 표면 위 다양한 패턴 형상에 따른 윤활 특성 연구)

  • Two surfaces that have relative motion show different characteristics according to surface roughness or surface patterns in all lubrication areas. For two rough surfaces with mixed lubrication, this paper proposes a new approach that includes the contact characteristics of the surfaces and a probabilistic method for a numerical analysis of lubrication. As the contact area of the two surfaces changes according to the loading conditions, asperity contact is very important. An average flow model developed by Patir-Cheng is central to the study of lubrication for rough surfaces. This average flow model also refers to a multi-asperity contact model for deriving a modified Reynolds equation and calculating the lubricant characteristics of a bearing surface with random roughness during fluid flow. Based on the average flow model, this paper carried out a numerical analysis of lubrication using a contact model by considering a load change made by the actual contact of asperities between two surfaces. Lubrication properties show different characteristics according to the surface patterns. This study modeled various geometric surface patterns and calculated the characteristics of lubrication.

Through Knee Amputation: Technique Modifications and Surgical Outcomes

  • Background Knee disarticulations (KD) are most commonly employed following trauma or tumor resection but represent less than 2% of all lower extremity amputations performed in the United States annually. KDs provide enhanced proprioception, a long lever arm, preservation of adductor muscle insertion, decreased metabolic cost of ambulation, and an end weight-bearing stump. The role for KDs in the setting of arterial insufficiency or overwhelming infection is less clear. The purpose of this study is to describe technique modifications and report surgical outcomes following KDs at a high-volume Limb Salvage Center. Methods A retrospective study of medical records for all patients who underwent a through-knee amputation performed by the senior author (C.E.A.) between 2004 and 2012 was completed. Medical records were reviewed to collect demographic, operative, and postoperative information for each of the patients identified. Results Between 2004 and 2012, 46 through-knee amputations for 41 patients were performed. The mean patient age was 68 and indications for surgery included infection (56%), arterial thrombosis (35%), and trauma (9%). Postoperative complications included superficial cellulitis (13%), soft tissue infection (4%), and flap ischemia (4%) necessitating one case of surgical debridement (4%) and four transfemoral amputations (9%). 9 (22%) patients went on to ambulate. Postoperative ambulation was greatest in the traumatic cohort and for patients less than 50 years of age, P<0.05. Alternatively, diabetes mellitus and infection reduced the likelihood of postoperative ambulation, P<0.01. Conclusions Knee disarticulations are a safe and effective alternative to other lower extremity amputations when clinically feasible. For patient unlikely to ambulate, a through-knee amputation maximizes ease of transfers, promotes mobility by providing a counterbalance, and eliminates the potential for knee flexion contracture with subsequent skin breakdown.

The development of pavement section and thickness design guidelines for rural access roads (부체도로 포장의 단면 설계 지침 개발 연구)

  • PURPOSES : This study proposes standards for rural access road pavement section and thickness design considering existing access road construction conditions; the study also proposes a complementary policy that can be used for design convenience. METHODS : Various literature review and case studies had been performed in terms of rural access road section and thickness design, both domestically and internationally, and this was followed by domestic rural access road field surveys. KPRP and KENLAYER were used to analyze the commonalities and predict the remaining life. Data on real cost is used to select an appropriate construction method through economic analysis. RESULTS : The economic efficiency of concrete pavement ($15{\times}15$) was the highest in terms of economic efficiency of performance life and traffic volume. In the case of asphalt pavement, it is considered that the most economical method is to implement micro-surfacing method four times as a preventive maintenance method (once every 10 years and 4.5 years for asphalt concrete pavement and MS construction method, respectively). Repairable asphalt pavement is advantageous for areas where heavy vehicles are expected to pass. In the case of other general areas, it is considered economical to place concrete ($15{\times}15$) pavement. However, as analytical results on its performance life are unavailable, it is to be considered for study in the future. CONCLUSIONS : This study proposed interim design guidelines based on various domestic and international design guidelines and case studies. However, in order to develop the final design criteria applicable to the field, it is necessary to (a) estimate the bearing capacity of the lower level of the pavement at various sites, (b) estimate the daily traffic volume, (c) implement advanced low-cost pavement technologies, and (d) propose maintenance standards and techniques for long-term performance.

  • Weight-bearing building materials are substantially weakened under high temperatures, and this is evident through the collapse of structures once engulfed by fire. Currently, there is no scientific or technological process of evaluating the real-time structural stability of a building whcih is engulfed by flame. There are many building design specifications which aim to reduce the risk of fire, but little consideration given to fire officer safety while operating in a dangerous building. This paper aims to provide direction within building policy in order to ensure the safe evacuation of fire-fighters in case of an impending building collapse. This paper suggests evaluation criteria for buildings which are damaged due to fire, autilizing current information on fire-resistant building design and a fire-resistant capacity.

  • There are many typical epithermal deposits scattering in the Taran of central Kalimantan. Indonesia. To get the shallow geological information, we carried out the geophysical exploration: spontaneous potential survey, in-suite magnetic susceptibility measurement and relistivity survey method in this area for a few weeks from September 30th to October 27th in 1997. SP survey (Fluke 27 multimeter)/magnetic susceptibility (EK -7 meter) measurement was carried out with a 250m$\times$10m lattice net in N45$^{\circ}$W direction. The dipole-dipole array resistivity survey was conducted with an electrode spacing of ${\alpha}$=30 m and electrode separation index n=7 at the line 5. The result shows that was the gold bearing quartz vein area can be divided into two type lone: low sulfidation type and high sulfidation type zone. The low resistivity value in the survey line 5 indicated a fracture Bone associated with the high sulfidation zone.
