Embolization of Multiple Systemic Artery to Pulmonary Artery Fistula with Recurrent Hemoptysis

  • Herein, we report a case of multiple systemic arteries to pulmonary artery fistulas without any underlying causes, presenting recurrent hemoptysis. Transcatheter embolization was successfully performed several times on multiple systemic feeding arteries. Multiple systemic arteries to pulmonary fistulas can be a source of uncontrolled bleeding, and embolization may be a reasonable therapeutic option to control the bleeding.

  • The most significant factor in pathogenesis of vascular headaches like migraine and cluster headache is dynamic changes of diameters of the cerebral arteries. TCD is a valuable noninvasive tool to assess the cerebral hemodynamic status by measuring the flow velocities of the intracranial cerebral arteries around the circle of Willis. TCD can evaluate flow velocities and vasoreactivity of the patients with a vascular headache during the ictal phase as well as during intericatal phase. Distribution of the changes recorded differ between types of headaches and also between the major ictal symptoms. The changes suggest the presence of prolonged vasospasm interictally and more marked relaxation of the cerebral arteries. TCD can be used to monitor the long-term clinical course of patients with vascular headache by correlation the symptomatic improvement and TCD data before and after long-term pharmacological prophylactic treatments. During the ictal phases large intervention. The results may be used in selecting and evaluating the agents for abortive therapy for acute attacks. In conclusion TCD can quantitatively evaluate vascular headaches when making diagnosis and classification and can provide guidelines to choose more individualized therapeutic options for both acute and long-term treatment.

  • By employing the vinylite and neoprene latex corrosion technique, the ramification and arrangement of A. renalis were observed in fifty adult dogs (100 kidneys). Obtained results were as follows: 1. In the dog a single renal artery arosed on each side from abdominal aorta in 99% and double renal arteries in 1%, a single bifurcated into the ramus dorsalis and ramps ventralis from which 2-6 segmental arteries originated. 2. The dorsal ventral planes of the kidney were divided separately into 3-6 vascular segments by the segmental artery into the parenchyma. Of them four segments were mostly frequent (in 73% of right kidney and 72% of left). 3. The ramifying method of the segmental artery from the ramus dorsalis et ventralis was divided 3 types and 12 kinds. 4. In 30 of 100 kidneys, either dorsal or ventral plane of a kidney was supplied from interlobar or segmental arteries belonging to the opposite ramps. In 29 of them, the dorsal plane was supplied from the ventral ramus. 5. Six right and 11 left kidneys had a common segment in the Extremitas cranialis of the kidney.

Pseudoaneurysms of Peripheral Arteries - A Report of 6 Cases- (말초동맥에 발생한 가성동맥류의 외과적 치료 -치험 6례-)

  • We experienced six cases of pseudoaneurysm of the peripheral artery which occurred after stab wound or after diagnostic and operative procedures. Among 6 cases, 4 cases of pseudoaneurysm were developed in the femoral arteries, and others were the subclavian and the axillary artery. Two of 6 cases were combined with previous arterio enous fistula. Doppler imaging and angiogram were performed for the dignosis and an operation. Operative procedures were resection of the aneurysm, ligation of the involved arteries and reconstruction of the artery with the autogenous saphenous vein or the cephalic vein graft. Each operations were successfully performed without any disability.

  • Spontaneous bilateral cerebellar infarction in the territory of the superior cerebellar arteries is extremely rare. Occasionally there have been reports of bilateral cerebellar infarction due to vertebrobasilar atherosclerotic occlusion or stenosis, whereas no report of bilateral cerebellar infarction due to complicated hemodynamic changes. In this report, we present a patient with bilateral cerebral infarctions related to stenoses of bilateral internal carotid arteries, in whom vertebrobasilar system was supplied by multiple collaterals from both posterior communicating arteries and right external carotid artery. We performed stent-angioplasty of bilateral internal cerebral arterial stenosis, and then acute infarction developed on bilateral superior cerebellar artery territories. The authors assumed that the infarction occurred due to hemodynamic change between internal carotid artery and external carotid artery after stent-angioplasty for stenosis of right internal carotid artery.

  • A 66-year-old woman presented with intermittent paraparesis and generalized tonic-clonic seizure. Cerebral angiography demonstrated dural arteriovenous fistula (AVF) involving superior sagittal sinus (SSS), which was associated with SSS occlusion on the posterior one third. The dural AVF was fed by bilateral middle meningeal arteries (MMAs), superficial temporal arteries (STAs) and occipital arteries with marked retrograde cortical venous reflux. Transfemoral arterial Onyx embolization was performed through right MMA and STA, but it was not successful, which resulted in partial obliteration of dural AVF because of tortuous MMA preventing the microcatheter from reaching the fistula closely enough. Second procedure was performed through left MMA accessed by direct MMA puncture following small decortications of cranium overlying the MMA using diamond drill one week later. Microcatheter could be located far distally to the fistula through 5 F sheath placed into the MMA and complete obliteration of dural AVF was achieved using 3.9 cc of Onyx.

  • It is well established that arteries are subjected to residual stress. Due to the effect of residual stress, the arteries open to a horse-shoe shape when a longitudinal cut is made on an excised arterial segment. Previously, the residual stress has been quantified by the opening angle of the horse-shoe shape. We have employed a finite element analysis of the open arterial segment to restore the same to the original cylindrical shape and computed the circumferential strain as well as the stress distribution in the wall. In this study, the stress distribution in the femoral arteries of miniswine was computed with and without the residual stress for a range of transmural pressures. Our analysis showed that the residual stress has the effect of redistribution of the circumferential stresses between the intima and the adventitia under physiological loading. The redistribution of the stress with the inclusion of residual stress may be important in the studies on effect of wall stresses on the endothelial and vascular smooth muscle cells.

Double-Outlet of Left Ventricle in Corrected Transposition of Great Arteries -One case report- (좌심실 이중유출로를 동반한 교정형 대혈관전위증 -1예보고-)

  • This is one case report of the extremely rare congenital cardiac malformation, Double-outlet of left ventricle in corrected transposition of great arteries. 11-year-old boy complained acrocyanosis and exertional dyspnea, the parents noticed cyanosis since birth. Physical examination revealed acrocyanosis, clubbed fingers and toes, G-III pansystolic murmur on 2nd and 3rd ICS, LSB. Right heart catheterization revealed significant $O_2$ jump in ventricular level. Right and left ventriculography showed the both catheters arriving in the same ventricle i.e. anterior chamber, morphological left ventricle was in right and anterior position, simultaneous visualization of aorta and pulmonary artery and aorta locating anterior and right side of pulmonary artery. Echo cardiogram surely disclosed interventricular septum. Conclusively it was clarified that the patient has Double-outlet of left ventricle and corrected transposition of great arteries [S.L.D.]. Operation was performed to correct the anomalies under extracorporeal circulation with intermittent moderate hypothermia. Right-sided ventriculotomy disclosed the following findings. 1. Right-sided ventricle was morphological left ventricle. 2. Left-sided ventricle was morphological right ventricle. 3. Right side atrioventricular valve was bicuspid. 4. Left side atrioventricular valve was tricuspid. 5. Aortic valve was superior, anterior and right side of pulmonary valve. 6. Subpulmonary membranous stenosis. 7. Non-committed ventricular septal defect. We made a tunnel between VSD and aorta with Teflon patch so that arterial blood comes through VSD and the tunnel into aorta. After correction the patient needed assisted circulation for 135 min. to have adequate blood pressure. Postoperatively by any means, adequate blood pressure could not be maintained and expired in the evening of operation day.

Thromboembolectomy in Acute Arterial Occlusion (급성 동맥폐색증에서 혈전색전 제거술 -48례 경험-)

  • Background : Even though there were developments in various treatment techniques for acute arterial occlusion this disease still has high rate of mortalities and limb amputations. We investigated the combined diseases symptoms location of occlusion type of treatment complication and prognosis in our patients. Meterial and methods: This study recruited 48 patients(42 men, 6 women, mean age 57.7 years) who received the operation from January 1995 toDecember 1998. We investigated the post-operation course via medical record review or telephone interview with patients or their family members. Result: The most common combined diseases were atherosclerosis in 30 patients. other diseases were 17 diabetes mellitus 16 hypertension and 12 atrial firillation. Pain and clod sensation were noticed in all patients paresthesia in 5 patients fibrillation. Pain and cold sensation were noticed in all patients paresthesia in 5 patients and lower extremity paralysis in 11 patients. In 29 patients the time interval from the onset of symptom to admission was over 72 hours and 15 patients were admitted within 24 hours. The distribution of arterial occlusion location was at 28 femoral arteries 14 popliteal arteries and 6 iliac arteries. All the patients were received embolectomy and 5 patients were received additional bypass grafting. Postoperative complications were 12 reocclusions. 6 compartment syndromes 6 skin necrosis and 2 acute renal failure. The mortality rate was 16.7%(8/48) and the amputation rate was 25% Conclusion : This study revealed 25% reocclusion 25% limb amputation and 16.7% mortaliyt. To improve the prognosis of acute lower extrements arterial occlusion early diagnosis and understand the underlying diseases prompt treatment and operation additional operation including interventional radiologic examination and thorough postoperative care would be appreciated.
