Contrast Enhanced Cerebral MR Venography: Comparison between Arterial and Venous Triggering Methods (조영 증강 자기공명정맥 촬영술에서의 동맥과 정맥 triggering 방법의 비교)

  • Purpose : To compare the arterial and venous detection sites of triggering methods in contrast-enhanced-MR-venography (CE-MRV) for the evaluation of intracranial venous system. Materials and Methods: 41 healthy patients underwent CE-MRV with autotriggering at either the cavernous segment of internal carotid artery with an inserted time-delay of 6 seconds (n = 20) or the superior sagittal sinus without any timedelay (n = 21). 0.1 mmol/kg gadolinium-based contrast material ($Magnevist^{(R)}$, Schering, Germany) was intravenously injected by hand injection. A sagittal fast-spoiled-gradient-echo-sequence ranging from one ear to the other was performed (TR/TE5.2/1.5, Matrix $310{\times}310$, 124 sections in the 15-cm-thick volume). 17 predefined venous structures were evaluated on all venograms by two neuroradiologists and defined as completely visible, partially visible, or none visible. Results: The rate of completely visible structures were 272 out of 323 (84%) in the arterial triggering CE-MRV and 310 out of 340 (91%) in the venous triggering CE-MRV. The venous triggering CE-MRV demonstrated an overall superior visualization of the cerebral veins than the arterial triggering CE-MRV (Fisher exact test, p < 0.006). Conclusion: CE-MRV using venous autotriggering method provides higher-quality images of the intracranial venous structures compared to that of arterial.

Effects of Covering Newborn's Head after Bath on Body Temperature, Heart Rate and Arterial Oxygen Saturation

  • Purpose: In this study changes were observed in body temperature, heart rate and arterial oxygen saturation (SaO2) of newborns after bathing and to determine the effects of covering their heads with cotton hats after bathing. Methods: Participants were 58 newborn infants, 31 in the experimental group had their heads covered with cotton hats after their bath while 27 in the control group did not. Body temperature, arterial oxygen saturation and heart rate were measured at 8 consecutive times after bathing. Data were analyzed using t-test and repeated measures ANOVA. Results: Body temperature declined shortly after bathing. The experimental group showed faster recovery (p<.001). Heart rate increased after bathing in both groups. Heart rate in the experimental group decreased for 120 minutes and gradually increased to baseline (p<.001). In the control group, heart rate decreased for 180 minutes and then increased but did not reach the baseline (p<.001). Arterial oxygen saturation decreased shortly after bathing and recovery to the baseline was more rapid in the experimental group (30 minutes vs. 60 minutes) (p<.001). Conclusion: With significant changes observed in newborns' body temperature, arterial oxygen saturation and heart rate, covering the head right after bathing is effective in stabilizing infants' physiological system.

Classification of Urban Arterial Roads Based on Traffic Characteristics (교통특성에 따른 도시간선도로 위계분류법)

  • Studies on classification of national roads have been continued, but there is little research on the classification of urban arterial roads. Due to the increase of traffic volume, urban arterial roads do not perform well as main roads. In this paper, the function of urban arterial road was established by using cluster analysis using traffic characteristics. Traffic characteristics such as traffic volume, weekend coefficient and speed coefficient were used to establish the functions of 55 main arterial roads in Seoul. The results of this paper are compared with those of the method using AADT. The method using AADT classifies the characteristics according to the traffic volume of the whole lane. In this paper, however, the results are derived using the traffic volume per lane reflecting the actual traffic volume. In addition, the functional classification of the arterial roads in Seoul was compared with the results of this paper to verify that the traffic characteristics were reflected. As a result, the method presented in this paper is more effective in showing traffic characteristics than the current highway functional classification method, and the functional classification system will be helpful for road extension and planning design.

A Study of the Cardiovascular Aging Effect on the Pulse Shape (심혈관 노화가 맥상(脈象)에 미치는 영향)

  • Background and purpose: Cardiovascular disease will undoubtedly rise along with the aging of the 'baby-boom' generation. The purpose of this study is to find the new index of the cardiovascular aging. Methods: The effects of aging on the heart and the arterial system are surveyed in the point of structure and function. Results: Arterial stiffening is due to the fatiguing effects of periodic stress on the arterial wall and is the main reason for increasing pulse wave velocity. The systolic hypertension is caused by the early return of wave reflection. The increased after-load by the arterial change leads to the development of left ventricular hypertrophy. The reduction in left ventricular compliance cause the impairments of the diastolic function. In contrast to the lower limb, aging effect in the upper limb are almost due to the ascending aortic pressure wave and the reflected wave from the lower limb. Conclusion: We have the following points. (1) The change of physiological pulse pattern by age can be explained by the early returning of reflected wave. (2) The atrial pulse in old age are generated by the left ventricular hypertrophy.

Comparison of Arterial Oxygen Saturation Measured by Pulse Oximetry at Different Sensor Sites in Neurocritical Patients (신경계 중환자의 측정부위별 맥박 산소포화도의 비교)

  • Purpose : This study aimed to compare peripheral pulse oxygen saturation (SpO2) values, measured at different monitoring sites, and arterial oxygen saturation (SaO2) of neurocritical patients. Methods : The study included 110 patients admitted to the neurosurgical intensive care unit of a university hospital. The patients' SpO2 values were measured in their index fingers, both second toes, both earlobes, and foreheads, using the patient monitoring system. These values were compared with the standard value of SaO2 measured using a blood gas analyzer. Data were analyzed using descriptive values, Pearson's correlation coefficients, Lin's concordance correlation coefficients (CCC), and Bland-Altman plots. Result : Regardless of the measuring site, SpO2 was correlated with the paired measurements of SaO2 (r=.40~.60, p<.001, CCC range=.40~.58). No significant bias in paired measurements of SpO2 and SaO2 was observed at all sites (-0.06~0.19%, p>.05). SpO2 values at the left finger and right earlobe had the narrowest range, with a 95% limits of agreement (LOA) (left finger -3.04~2.93% and right earlobe -3.18~2.79%). SpO2 at the index finger, on the side without an arterial catheter, had a narrower range of 95% LOA than that of the opposing finger (-3.00~2.97% vs. -3.73~3.26%). Conclusion : SpO2 at the finger without an arterial catheter had the highest level of precision. This study suggests using the index finger, on the side without an arterial catheter, for pulse oximetry in neurocritical patients.

Design of Noninvasive Arterial Blood Pressure Measurement System by Using Double-Cuffs Oscillometric Method (이중 커프 오실로메트릭법을 이용한 비침습 동맥압 측정시스템의 설계)

  • In this study, a new blood pressure measuring system was proposed and implemented. An additional small-cuff was placed on the center of a inner cuff to measure morphological signals and new oscillometric ratio. The proposed BP-measuring system is composed of an external cuff, an inner cuff and a small-cuff. Oscillation signal from small-cuff is interpolated with 7th-order fitting polynomials and SBP, DBP ratio were 22.2% and 87.7%. Experimental data were gathered from 20 volunteers ($25{\pm}4$ years) and arterial blood pressure values were compared with auscultation, sphygmomanometers, small-cuff and inner-cuff. As a result, the difference in systolic BP between auscultation and the small-cuff was 1.93(${\pm}1.28$) mmHg, and the inner-cuff was 4.53(${\pm}4.39$) mmHg, and sphygmomanometer was 6.68(${\pm}3.99$) mmHg, and the corresponding difference in diastolic BP was 2.50(${\pm}2.04$) mmHg, 3.50(${\pm}3.19$) mmHg, 7.35(${\pm}5.62$), respectively.

  • For developing a wireless implantable device to monitoring the artery variation in real-time. The concept of a special vessel variation measurement capacitive sensor is presented in this paper. The sensor consists of two part; main sensor to measuring the arterial variation, and reference sensor is used to improve the accuracy of the capacitance value variation. Before sensor manufacture, a model of the sensor attached on the artery was designed in 3D to conduct in the FEA simulation to validate the validity and feasibility of the idea. The artery model was designed as layered structures and made of collagenous soft tissues with intima inside, followed by the media and the adventitia. Also, a grease layer was designed in the inner of the arterial wall to imitate the clogged arteries. The simulation was divided into two parts; sensor performance test by changing the diameter of the grease layer, and arterial wall tension test by changing the blood pressure. As the simulation results, the capacitance value measured by the proposed sensor is decreased follow the diameter of the grease increased. Also, large elastic deformation of the arterial wall since changing the blood pressure has been observed.

Effects of DaeSiHo-Tang extract on Hypertension and Arterial Contraction (대시호탕(大柴胡湯)이 고혈압과 수축혈관에 미치는 영향)

  • This study was undertaken to define the effect of DaeSiHo-Tang extract on the hypertension in spontaneous hypertensive rat and norepinephrine-induced arterial contraction in rabbit. Systolic blood pressure and blood velocity were significantly attenuated by administration of DaeSiHo-Tang extract. but blood flow and renin-angiotensin-aldosterone system unaffected by DaeSiHo-Tang extract. The relaxation effect of DaeSiHo-Tang extract was dependent on the presence of endothelium, showing that DaeSiHo-Tang extract-induced relaxation was not observed in the strips without endothelium. The endothelium-dependent relaxation induced by DaeSiHo-Tang extract was decreased by the pretreatment of $N{\omega}$-nitro-L-arginine or methylene blue, but it was not observed in the strips pretreated with indomethacin or tetraethylammonium chloride. When $Ca^{2+}$ was applied, the strips which were contracted by norepinephrine in a $Ca^{2+}$-free solution, arterial contraction was increased. But pre-treatment of DaeSiHo-Tang extract inhibited contractile response to $Ca^{2+}$. These results indicate that antihypertensive effect of DaeSiHo-Tang extract is due to descend arterial resistance by the arterial relaxation through the formation of nitric oxide in the vascular endothelial cells.

Clinical effects of veno-venous extra-corporeal membrane oxygenation for acute myocardial infarction (급성심근경색 환자에게 적용된 정맥-정맥 체외막산화기의 임상 효과)

  • Extra-corporeal membrane oxygenation (ECMO) has the potential to rescue patients in cardiac arrest or respiratory failure. ECMO has two systems such as veno-arterial and veno-venous circulation. In cardiac arrest resulting from acute myocardial infarction, veno-arterial ECMO is mandatory for systemic circulation and oxygenation. A 75-year old female patient underwent primary coronary intervention for acute myocardial infarction. Despite successful revascularization, recurrent ventricular tachycardia and heart failure were progressing. We performed a veno-arterial ECMO through the femoral artery and vein, then the patient seemed to be stable clinically. However, laboratory studies, echocardiography, and vital signs indicated multi-organ failure and decreasing cardiac function. We found out an error that we performed veno-venous ECMO instead of veno-arterial ECMO. We added a femoral artery cannula and exchange the circuit system to veno-arterial ECMO. While the systemic circulation seemed to be recovered, the left ventricular function was decreased persistently. A hypovolemia resulting from pulmonary hemorrhage was occurred, which lead to ECMO failure. The patient died of cardiac arrest and multi-organ failure 23 hours after ECMO. Because the color of arterial and venous circuits represent the position and efficacy of ECMO, if unexpected or abnormal circuit colors are detected, prompt and aggressive evaluation for ECMO function is mandatory.

Development of pulse diagnosis possible simulator using the stepper motor pumps (스텝 모터 펌프를 이용한 맥진 가능한 시뮬레이터의 개발)

  • Virtual testing devices are required due to rapid changes in the health care industry and the increase of the medical or nursing workforce. The importance of devices such as the simulator, blood vessels, and lab equipment for modeling blood flow to the heart is increasing too. In this study, we made heart pump by using a step motor and developed device which simulates arterial, venous blood pressure, and blood flow. We finally evaluated the function of proposed device. The proposed system is composed of the pump for simulating, the valve device to describe the resistance of the artery and vein, and a reducing device showing the characteristics of the venous system. We used BOXER pump for heart simulator and silicon tube for arterial and venous vessels, and designed a reducing device. We also used the pressure sensor to measure arterial blood pressure. For the evaluation of the proposed system, we selected a range of 50~100mmHg of the blood circuit 60 per minute and then compared the blood pressure of a person and the measured blood pressure.
