The Effects of Red Ginseng on Blood Pressure and the Quality of Life in Essential Hypertensives (본태성 고혈압 환자의 혈압과 생활상에 미치는 홍삼의 영향)

  • We studied the effects of red ginseng on blood pressure (BP) and the quality of life (QOL) in 19 hypertensive patients treated with antihypertensive agents. Red ginseng was administered at a dosage of three grams a day for three months. Systolic blood pressure was significantly lowered during the administration period of red ginseng. while diastolic blood pressure and hear1 rate remained unchanged. QOL was significantly improved in $89\%$ of the patients. Among the QOL variables. sleep disorder. thirst. fatigue. mood. sexual life and general well-being were improved. Furthermore. an improved coefficient of variation of R-R intervals $(CV_{R-R})$ was observed during the period. A month after the cessation of red ginseng systolic blood pressure returned to the level before the administration and QOL variables detariorated to previous pre-treatment states. These results suggest that red ginsing may lower systolic blood pressure and may improve QOL in patients treated with antihypertensive agents.

Study on Cold-Heat Pattern Identification in Hypertensive Patients with Antihypertensive Agents (항고혈압제제를 복용 중인 성인 고혈압 환자의 한열 특성에 관한 연구)

  • Objectives : The aim of this study was to investigate the correlation between Cold-Heat pattern tendency and demographic traits, physical character, and vital signs in hypertensive patients taking anti-hypertensive agents. Methods : 28 hypertensive patients 30 to 69 years old were recruited. We assessed their general characteristics, physical characteristics (height, weight, waist circumference, etc.), vital signs without respiratory rate (blood pressure, pulse rate, temperature) and administered a Cold-Heat pattern questionnaire. After that, we analyzed statistical data on separate groups according to Cold-Heat characteristics or other criteria. Results : 1. Heat group patients had statistically higher scores in waist, body mass index (BMI), waist circumference and pulse rate. 2. Over weighted group patients had statistically lower scores in cold Pattern Identification. 3. Higher BMI group (above 23/25) patients had statistically significantly higher Heat scores and lower Cold scores, higher waist circumference group patients had lower Cold scores. Conclusions : From the above result in hypertensive patients taking hypertensive agents, Heat group had a obesity tendency and the inverse is also valid. Later, progressed study based on more samples and varied data will contribute to diagnosis the Cold-heat Pattern identification in hypertensive patients.

Experimental Intervention to Reverse Inhibition of Nitric Oxide Production by Cyclosporin A in Rat Aortic Smooth Muscle Cells (혈관평활근세포에서 Cyclosporin A에 의한 Nitric Oxide 생성억제를 길항하는 실험적 중재법)

  • The inhibitory effect of cyclosporin A (CsA) on nitric oxide production is not related to the immunosuppressive action of the drug, but to the renal toxicity and arterial hyper-tension. In this study the experimental interventions to reverse the inhibition of nitric oxide production by cyclosporin A in rat aortic smooth muscle cells were examined. CsA inhibited the accumulation of nitrite, the stable end product of nitric oxide, in culture media in a concentration $(0.1{\sim}100{\mu}g/ml)-dependent$ manner. The inhibitory effect of CsA on nitrite accumulation were not antagonized by arginine (10 mM), a substrate of nitric oxide synthase, nor by calcium ionophore A23187 $(7{\mu}M)$. Forskolin, an activator of adenylate cyclase, which enhanced iNOS induction at transcriptional level, completely reversed the inhibitory action of CsA on nitrite accumulation. However, PMA (2 nM) and PDB (50 nM), PKC activators, increased the inhibitory action of CsA on nitrite accumulalion. From these results, it is suggested that cyclic AMP-elevating agents may be candidates of therapeutic agents in prevention and treatment of renal toxicity and arterial hypertension induced by CsA. Among conventional antihypertensive drugs, calcium channel blockers and ${\alpha}-blockers$ are preferred to ${\beta}-blockers$.

  • This manuscript reports the synthesis of two series of Mannich Bases 3-12 and 21-40 obtained respectively by the reaction of either 2- ethoxycarbonylindoles 1-2 or 5H-pyridazino [4,5-b]indoles 17-20 as a substrate with formalin and the appropriate $2^{\circ}C$amines under the suitable Mannich conditions. fourteen of the synthesized Mannich basese were screened as antihypetensive agents in normotensive anesthetized rats. The effect of compound 4 in normotensive anesthetized dogs was also studied.

  • Attempts were made to investigate the pharmacological actions of Euonymus japonicae Cortex which is used as a substitute of Eucommiae Cortex. Practically the influence of the alcohol extract of the bark on the blood pressure response of rabbit, was examined, and the results are as follows. 1) The intravenous injections of the alcohol extracts of the bark decreased the blood pressure of rabbits to 4.6 11.1 and 34.2 mmHg respectively. 2) The intravenous injection of 10 mg/kg of the alcohol extract after intravenous injection of atropine 4 mg/kg or that of propranolol 2 mg/kg did not give any notable change to the blood pressure. 3) The intravenous injection of 10 mg/kg of the alcohol extract slightly decreased the raised blood pressure caused by the administration of acetylcholine $10^{-8}g/kg\;and\;10^{-7}g/kg$.

  • Morphological characteristics of the Eucommia barks produced in Korea and China and of the barks of Euonymus japonicus $T_{HUMBERGI}$ in Korea and Euonymus pellucidifolia $H_{AGATA}$ in Taiwan were compared. The similarity in morphological features between Eucommiae Cortex produced in Korea and that of China suggests that both plants are of same botanical origin. However, the size of tissues and cells indicates that there is only one difference due to warm climatic environment. The similarity and difference of the barks of Euonymus japonicus produced in Korea and Euonymus pellucidifolia produced in Taiwan comfirmed that the two plants belong to the same genus and that they are different species.

  • Influence of Eucommiae Cortex in Korea on the blood pressure of the rabbit was examined, 1) The intravenous injection of 3 mg/kg, 10 mg/kg and 30 mg/kg of the water extract to the rabbit decreased its blood pressure to $9.7\;{\pm}\;1.9,\;26.4\;{\pm}\;1.7\;and \;35.5\;{\pm}\;2.7\;mmHg$. respectively. 2) The intravenous injection of 10 mg/kg of the water extract did not show any remarkable influence to the blood pressure caused by the intravenous injection of acetylcholine $10^{-8}\;g/kg\; and \;10^{-7}\;g/kg$. 3) The intravenous injection of the water extract after the intravenous injection atropine 2mg/kg did not show any remarkable change to the blood pressure 4) The intravenous injection of the water extract after the intravenous injection of propranolol 2mg/kg slightly decreased the blood pressure.

The Risk of Rosacea According to Chronic Diseases and Medications: A 5-Year Retrospective, Multi-Institutional Case-Control Study

  • Background: Rosacea is associated with chronic systemic disease. However, research is lacking in Asian countries. Objective: To evaluate the association between rosacea and cardiovascular diseases (CVDs) related systemic comorbidities, and the use of antihypertensive and antihyperlipidemic drugs in Korea. Methods: A five-year retrospective study, using hospital database, was conducted in five medical centers for five years. Totally 1,399,528 patients were evaluated. Results: The overall frequency for diagnosed rosacea was 0.18% over five years (2,536 rosacea patients). Patients with diabetes and patients with dyslipidemia were more likely to have rosacea (odd ratio [OR] 2.724, 95% confidence interval [CI] 1.295~5.730, p=0.016; OR 1.788, 95% CI 1.445~2.212, p<0.001). Patients with CVD were less likely to have rosacea (OR 0.431, 95% CI 0.244~0.760, p=0.003). Patients with ${\alpha}$-blocker prescriptions and patients with ${\beta}$-blocker prescriptions showed a tendency diagnosed with rosacea frequently (OR 1.963, 95% CI 1.200~3.212, p=0.006; OR 3.939, 95% CI 3.512~4.419, p<0.001). Patients with [beta]-hydroxy-[beta]-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitor, and those with fibrate, were prone to have rosacea (OR 1.599, 95% CI 1.390~1.839, p<0.001; OR 1.660, 95% CI 1.056~2.609, p=0.026). As adjusted results, among the patients who took HMG-CoA reductase inhibitor without dyslipidemia, rosacea was less likely to be diagnosed (OR 0.780, 95% CI 0.620~0.982, p=0.034). Conclusion: Rosacea is associated with chronic diseases and drugs.

Intensive care unit management of uncomplicated type B aortic dissection in relation to treatment period: a retrospective observational study

  • Background: Medical therapy is the standard treatment for uncomplicated acute type B aortic dissection (ATBAD), but there is little evidence of the need for intensive care unit (ICU) management. Therefore, we aimed to investigate the effects of ICU treatment on uncomplicated ATBAD. Methods: We retrospectively studied patients with uncomplicated ATBAD who were medically treated between January 2010 and July 2020. Patients were divided into short-term ICU stay (SIS) and long-term ICU stay (LIS) groups, according to a 48-hour cutoff of ICU stay duration. The incidence of pneumonia and delirium, rate of aortic events, hospital mortality, and survival rate were compared. Results: Fifty-five patients were treated for uncomplicated ATBAD (n=29 for SIS and n=26 for LIS). The incidence of pneumonia (3.6% vs. 7.7%) and delirium (14.3% vs. 34.6%) was higher in the LIS group than in the SIS group, but the differences were not statistically significant. The survival rates at 1, 3, and 5 years were not different between the two groups (SIS: 96.4%, 92.2%, and 75.5% vs. LIS: 96.2%, 88.0%, and 54.2%, respectively; p=0.102). Multivariate Cox regression analysis for aortic events showed that using a calcium channel blocker lowered the risk of aortic events. Conclusion: Long-term ICU treatment is unlikely to be necessary for the treatment of uncomplicated ATBAD. Active use of antihypertensive agents, such as calcium channel blockers, may be needed during the follow-up period.

Factors Influencing Sleep Disorders in Patients on Hemodialysis (혈액투석 환자의 수면장애 영향요인)

  • Purpose: This study was conducted to identify the factors contributing to sleep disorders in patients on hemodialysis. Methods: A descriptive correlational study design was used. The participants were 135 patients on hemodialysis in hemodialysis clinics. Data were collected from March to May 2007 using structured questionnaires and hematologic sample. Stepwise multiple regression was used to identify factors influencing sleep disorders among the demographic-clinical factors, depression and fatigue. Results: About 69% of the patients had a sleep disorders. There were significant differences in sleep disorders according to marital status, caregiver, religion, economic level, insurance, erythropoietin, somnifacient, and antihypertensive agents. But there were no differences according to age, gender, dialysis period, or antidepressants. Their sleep disorders had significant correlations with depression, and fatigue, and a significant negative correlation with Protein, Albumin, Phosphate and BUN. But there were no correlations with hematocrit, hemoglobin, creatinine, sodium, potassium, or calcium. Depression and fatigue were factors influencing sleep disorders. They accounted for 43.8% of the variance in sleep disorders in these patients. Conclusion: Findings provide an understanding of sleep disorders and the factors that are an influence in patients on hemodialysis. To promote sleep in these patients, nursing interventions to manage depression and fatigue are needed.
