  • BacJung-pil(百中丸) using in stagnation-syndrome of gi and blood(痺證) is a expience prescription, A female patient diagnosed as wind syndrome of head(頭風), and stagnation-syndrome of gi and blood(痺證) showed severe pruritus, rash and eruption of skin. After the patient took BackJun-Pill, two hours later, We examined her change and witnessed macular papule and severe pruritus. Those symptoms disappeared in 7 days since we administer anti-histamine, steroid and GamDu-TangGamiBang(甘豆湯加味方).

A Case Report of Severe Keratinized Prurigo Nodularis (중증 각화성 결절성 양진 한방치료 1례)

  • Objectives : The purpose of this study is to report the effect of korean medical treatments on prurigo nodularis. Methods : We treated a 37-year-old female prurigo nodularis patient who has failed to respond to conventional therapies such as steroids and anti-histamines for 8 years. The patient was admitted to Daejeon University korean medical hospital for 21 days to get korean medical treatments including acupuncture, herbal-medicine, vapor treatment, venesection, pharmacopuncture and herbal-med aerosol therapy. The degree of improvement in prurigo nodularis was determined by the patient's subjective opinions on pruritus, febrile sensation, pain, and sleep disorders. Results : The results of this patient showed distinct improvements on pruritus, febrile sensation, pain, and sleep disorders. Pruritus decreased from 10 to 1, febrile sensation from 10 to 0, pain from 10 to 2, and sleeping hours increased 2 hours to 8 hours according to the patient's subjective opinions. Conclusions : This study shows the possibility that such korean medical treatments can improve prurigo nodularis.

  • Objectives : The present study was undertaken to determine whether the ethanol extact of Citri Pericarpium (CP, Pericarp of Citrus unshiu Markovich, Rutaceae) is effective against atopic dermatitis according to it's storage period. Methods : To evaluate antiallergic effect of CP and OCP (Old Citri Pericarpium) evaluated in vivo their inhibitory effects against passive cutaenous anaphylaxis (PCA) reaction induced by IgE-antigen complex and scratching behaviors by compound 48/80. The Anti-atopic effects were measured by contact dermatitis, prurient animal model and PCA reaction. Contact dermatitis in mice as a model of the Type IV reaction caused by Oxazolone. Results : The results showed that anti-pruritus effects, analgesic effects of CP was depends on its hesperetin contents. And It also showed that keep longer in storage appeared to be higher in hesperetin contents. Both CP and OCP(Old CP) have a dose-dependent analgesic action in acetic acid induced writhing syndrome. OCP Potently inhibited PCA reaction in mice, although OCP weakly inhibited in long term contact dermatitis model in mice. Conclusions : These results suggest that the Proportional to the storage period, Citri Pericarpium possesses analgesic effects and anti-allergic effects.

Anti-inflammatory Effects of Extracts of Duchesnea chrysantha in Human Monocytic THP-1 Cells and Human Eosinophilic EoL-1 Cells

  • Atopic dermatitis is a recurrent or chronic eczematous skin disease with severe pruritus and has annually increased in Korea. In this study, we investigated whether Duchesnea chrysantha (Dc) extracts have an anti-inflammatory effect in human monocytic THP-1 cells and human eosinophilic EoL-1 cells. The dried and powdered whole plants of Dc were extracted with 80% EtOH (Dc-1). The residue was diluted with water, and then successively partitioned with n-hexane, EtOAc, and BuOH to produce the n-hexane (Dc-2), EtOAc (Dc-3), BuOH (Dc-4), and the water-soluble fractions (Dc-5), respectively. The mite extract and LPS increased the production of IL-6, IL-8 and MCP-1 in THP-1 cells and the increase was strongly suppressed by Dc-3 extract, as compare with other extracts. Dc-3 also inhibited the release of IL-6 increased by mite extract and LPS in EoL-1 cells. However, Dc-3 extract increased IL-8 production induced by the mite extract and LPS in EoL-1 cells. These results suggest that Dc extract may be used as anti-inflammatory agents in treating allergic disorders such as asthma and atopic dermatitis.

Synergic Anti-Pruritic and Anti-Inflammatory Effects of Scutellariae Radix plus Flos Loncerae Extracts in Rat Peritoneal Mast Cell and Chemical Antigen-Induced Mice (렛트 복강 비만세포와 화학항원 유도 알레르기 마우스에서 황금과 금은화 추출물의 항가려움 및 항염증 상승효과)

  • Pruritus is a unpleasant symptom in the skin that provokes the act of or desire to scratch. Mast cells are important effector cells in allergic reactions such as pruritus and inflammation. The purpose of this study was undertaken to investigate the synergic anti-pruritic and anti-inflammatory effects of Scutellariae Radix (SB) plus Flos Loncerae (FL) extracts in rat peritoneal mast cells (RPMCs), pruritogen-induced scratching mice and 2,4-dinitrofluorobenzene (DNFB)-induced allergic mice. We investigated the effect of SB, FL and SB plus FL extracts on the production of tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$, and histamine in RPMCs, on the scratching behavior in ICR mice, and skin clinical serverity and inflammatory mediators in DNFB-induced allergic hairless mice. RPMCs stimulated with PMA plus A23187 or compound 48/80 significantly increased TNF-${\alpha}$, IL-$1{\beta}$ or histamine production compared with media control. However, TNF-${\alpha}$ IL-$1{\beta}$ or histamine levels increased by PMA plus A23187 or compound 48/80 treatment were significantly inhibited by SB, FL in a dose-dependent manner. Especially, SB plus FL pretreatment had a synergic inhibitory effects on stimulator-induced cytokines (TNF-${\alpha}$ and IL-$1{\beta}$) and histamine production. Moreover, SB plus FL administration had a synergic inhibitory effects on the scratching behavior induced by pruritogen (compound 48/80, histamine, serotonin, substance P) in ICR mice. Furthermore, SB plus FL administration had a synergic inhibitory effects on skin damage, inflammatory mediators, leukocyte and mast cell infiltration induced by DNFB in hairless mice. These results suggest that SB plus FL administration has a potential use as a medicinal plant for treatment against itching and inflammation-related skin disease.

Anti-oxidant Activities and Anti-atopic Dermatitis Effect of Combined Extract of Phellinus linteus and Glycyrrhiza uralensis (상황버섯, 감초 복합추출물의 항산화 및 인체적용 아토피 개선 효능 연구)

  • Objectives : This study investigated the anti-oxidant activities and improving effect of Phellinus linteus and Glycyrrhiza uralensis Extract (PGE) on Atopic Dermatitis. Methods : 2,2′-diphenyl-1-picrylhydrazyl (DPPH) radical, 2,2′-azino-bis 3-ethylbenzothiazoline-6-sulphonic acid (ABTS) radical, Hydrogen peroxides scavenging activities and Superoxide dismutase (SOD)-like activities were used for the measurement of anti-oxidant ability. Cytotoxicity of PGE in Raw 264.7 cell was evaluated by MTT assay. To evaluate the anti-atopic dermatitis effect of PGE, a total of 33 patients with atopic dermatitis were observed trans epidermal water loss, skin moisture content, modified SCORAD index of atopic dermatitis and pruritic degree after applying the PGE for 4 weeks. Results : PGE scavenged DPPH ($IC_{50}=25ppm$) effectively, ABTS and Hydrogenperoxides scavenged similar to BHA. As for the SOD-like activity, it had lower effect than ascorbic acid, but it comparable activities in 500ppm. There was no cytotoxicity at PGE at concentrations of 10,000ppm. In clinical research about PGE on patients with atopic dermatitis, skin condition was improved. After 4 weeks, the application of PGE increased skin moisture content from 19.43 to 31.22. Moreover, it reduced the skin temperature (from 32.5 to 31.9), skin pH (from 5.39 to 5.22), trans epidermal water loss (from 39.03 to 24.46) and pruritus score (from 6.07 to 3.87). In addition, the Modified SCORAD index decreased from 31.28 to 20.3. Conclusions : In conclusion, PGE possesses anti-oxidant and anti-atopic dermatitis activities, thus it could be potentially valuable as anti-atopic dermatitis material.

An Experimental Study on the Anti-inflammatory Effects of Cheonggisan Extract in Allergic Late Inflammation (알레르기 후기 반응 염증 억제효과에 관한 청기산(淸肌散)의 실험적 연구)

  • Objectives : Allergic diseases have a various symptoms of hyperresponsiveness and recently hyperresponsive reaction in the chronic phase is reported as the important mechanisms. Cheonggisan(CGS) is used in oriental clinics for curing various skin diseases due to effect of controlling of pruritus. There have been studies on the anti-allergic effect and anti-inflammatory effect of CGS, but there had no study of anti-allergic effects in allergic late inflammation of CGS, so we aimed to find out the effects of CGS in allergic late inflammation in our study.Methods : To investigate the anti-allergy effect and anti-inflammatory effect of CGS, RAW 264.7 macrophage cells and CSG water-extracts were used. Cytotoxic effect of CSG was examined by MTT assay, an oxidative product of NO was measured in the culture medium by the Griess reagent assay. The level of prostaglandin E2(PGE2) was measured by competitive enzyme-linked immunoassay. Cytokine(PGE2, IL-1β, IL-6, TNF-α) was measured by Bio-Plex suspension assay system and quantitative multiplexed cytokine/chemokine assay.Results : We investigated that there was no cytotoxic effect of CGS water-extract at any levels of concentration on RAW 264.7 macrophage cells by MTT assay. CGS water-extracts significantly suppressed the levels of the inflammatory mediators such as NO and PGE2, cytokine of IL-1β, TNF-α at the level of 400 ㎍/㎖ CGS concentration. But there was no significant effect on IL-6 production suppression.Conclusions : These results suggest that CSG water-extract has and anti-inflammatory effects in allergic reaction. These properties may contribute to the allergic diseases and inflammatory related disease care.

Metabolism of Wogonoside by Human Fecal Microflora and Its Anti-pruritic Effect

  • To understand the relationship between the metabolism of wogonoside from the rhizome of Scutellaria baicalensis, and its anti-pruritic effect, we anaerobically incubated it with human fecal microflora, identified its metabolite identified, and investigated its anti-pruritic effect in compound 48/80 or histamineinduced pruritic mice. Wogonoside was metabolized to wogonin, with metabolic activity of $6.9{\pm}5.1\;nmol/h/mg$ wet weight of fecal microflora. Orally administered wogonoside had more potent anti-scratching behavioral effect in compound 48/80 or histamine-treated mice than intraperitoneally treated one, apart from orally administered its metabolite, wogonin, which was more potent than the orally administered one. Wogonoside showed more potent anti-pruritic effects when administered at 5 h prior to the pruritic agent treatment than when administered at 1 h before. However, wogonin orally administered 1 h before the treatment with pruritic agents showed a more potent anti-pruritic effect than when treated at 5 h before. Orally administered wogonoside may be metabolized to wogonin in the intestine and its anti-scratching behavioral effect may be dependent on its metabolism by intestinal microflora.

The Effect of Anti-microbial and the Inhibitory Effect of Biofilm Formation and Inflammatory Factors Production of Perillae semen Supercritical Fluid Extracts (초임계 자소자추출물의 항균효과와 바이오필름, 염증매개인자 생성 억제 효능)

  • In this study, we assessed anti-oxidant activity, anti-microbial, inhibition of biofilm formation and inflammatory factors(nitric oxide, interleukin-6, interleukin-8) inhibitory effect of Perillae semen hydrothermal extract(PSW) and three kinds of Perillae semen supercritical fluid extract(PSSs) extracted by controlling temperature with no variation of pressure. Compared with PSW, PSSs had significantly lower minimal inhibitory concentrations(MICs) against Staphylococcus aureus(S. aureus) and the ability of PSSs to inhibit formation of biofilm was also superior. PSSs reduce the production of inflammatory mediator and inflammatory cytokines significantly compared to PSW. We suggest, therefore, Perillae semen supercritical fluid 45℃ extract which showed the best anti-microbial, inhibition of biofilm formation, and inhibition of inflammatory factors production among the supercritical fluid extracts could be used for protecting patients with atopic dermatitis from pruritus and transepidermal water loss as a functional ingredient from nature.

Effects of Cicadae Periostracum (CP) in Allergic Contact Dermatitis (ACD) Induced by DNCB in Mice (DNCB로 유발된 생쥐의 알레르기성 접촉피부염에 선태(蟬蛻)가 미치는 영향)

  • Objectives In the theory of Korean Medicine, Cicadae Periostracum (CP) has been used to treat skin diseases such as inflammatory dermatitis, tetanus and pruritus. CP can reduce heat and disperse wind. In prior studies, anti-allergic effect and anti-inflammatory effect of CP were reported. However, there has been no report regarding the correlation of CP and allergic contact dermatitis (ACD). This study was performed to show the effects of CP in ACD induced by 2,4-dinitrochlorobenzene (DNCB) in mice. Methods In this experiment, the effects of CP on biological changes were measured, such as changes in ear and spleen weight, ear and dorsum skin thickness, clinical aspect on the dorsum skin and histological changes. The effect of proliferation rate of splenocytes was also investigated in vitro and vivo study. Results In results, CP application (CPA) group and CP application and administration (CPAA) group were significantly restrained in ear weight gain and increase in ear and dorsum skin thickness compared to the control group. In addition, CPA and CPAA group showed diminished erythema, desquamation, bloodstain, and marks. Also, the histological assessment showed that CP treatement diminished thickness of epidermis, hyperkeratosis, pigmentation and parakeratosis. Conclusions In conclusion, these data suggest that CP can decrease symptoms of ACD.
