크론씨병으로 진단된 소양인 환자 치험 1 례 (Clinical Study on 1 Case of Soyangyin Patient Diagnosed as Crohn's Disease)

  • Crohn' disease is an auto-immune disease characterized by intermittent chronic diarrhea, high fever, weight loss, abdominal spastic pain or abdominal discomfort which is followed by granulomatous necrosis and cicatrical inflammation. It is also called segmental enteritis or granulomatous enteritis. In western medicine the exact cause is undefined, however it is presumed as an immunological unbalance in alimentary tract commoonly occured in ileum portion of small intestitine or ascending colon and therefore immuno suppressive agents(usually steroids) and anti-inflammatory drugs are prescribed. In case of emergency such as ileus, perforation of intestinal wall surgical methods are considered. In oriental medicine this falls under the category of diarrhea(泄瀉), dysentery(痢疾), splenic diarrhea(脾泄). As to the pathological mechanism the abnormal ascending and descending circulation of stomach and splenic energy(脾不升淸, 胃不下降) the hepatic stagnation(肝鬱氣滯) and dysfunction of small intestine in expelling urine and feces(小陽淸獨不利) all together causes such condition. Main treatments are inducing diuresis(利小便), warming kindey to reinforce yang(溫賢助陽), nourishing the middle energy to invigorate spleen(補中健脾), elimination of the dampness by cooling(淸熱燥濕). In this case the patient was diagnosed as soyangyin(少陽人) constitution and herb medicine soyangyin Hyongbangjihwan-tang(少陽人 荊防地黃湯), Sa-am acupuncture Sojangjeonggyeok(小腸政格) was applied. There was an significant improve in chief complaints and general conditions.

로타바이러스 감염성 설사에 대한 항-로타바이러스 난황항체의 치료 효과 (Therapeutic Effect of Anti-Rotavirus Chicken Egg Yolk Immunoglobulin(IgY) on Diarrhea by Infection of Rotavirus)

  • 목 적 : 로타바이러스는 세계적으로 5세 이하 영유아 설사증의 1/3 이상을 차지하는 주요 원인체이다. 이 로타바이러스에 의한 급성위장관염의 치료는 탈수교정을 위한 경구 및 비경구 수액요법 외엔 특이한 치료법이 없는 상태로서 로타바이러스 감염에 대한 새로운 치료법이 필요한 상태이다. 이에 본 연구에서는 로타바이러스에 대한 난황 항체를 투여하여 환아에서의 로타바이러스 억제 효과 및 임상증상의 치료효과를 조사하였다. 방 법 : 본 연구는 두 부분으로 나누어 첫째 로타바이러스 위장관염 환아의 분변에서 바이러스 입자 감소 정도를 보았으며 둘째 환아에게 난황황체를 환아에게 직접 투여하여 임상증상이 개선되는 지를 보았다. 첫째로, 2001년 12월부터 2002년 9월까지 중앙대학교 용산병원 소아과에 급성 장관염을 주요 증상으로 내원한 환아의 분변을 대상으로 역수동 적혈구 응집반응으로 로타바이러스 항원의 존재 유무를 조사하였고 이중 양성인 13명의 환아의 3일간에 각각 걸친 분변시료를 재료로 정량 PCR을 시행하여 바이러스 양의 감소를 측정하였다. 13명 중 7명에게는 난황항체가 투여되었고 6명에게는 위약이 투여되었다. 둘째로는 같은 기간 로타바이러스 감염으로 진단된 36명을 대상으로 하여 18명에는 항-로타바이러스 난황항체가 함유된 난황 2.5 g을 5 mL의 물에 부유하여 하루 2회(아침, 저녁) 3일 동안 투여하였고 위약군 18명에게는 난황항체가 함유되지 않은 난황을 같은 기간 동안 같은 양을 투여한 이후 양군에서 치료 전후의 발열기간, 설사 횟수, 구토 횟수, 전해질 및 혈청 생화학치, 입원기간, 경구 섭취량, 소변 횟수를 비교 관찰하였다. 결 과 : 첫 번째 실험에서, 정량 PCR을 이용하여 환아 분변에서 바이러스 입자의 감소를 측정한 결과 위약군에서는 지속적으로 증가를 보였으나 투여군에서는 지속적인 바이러스 입자의 감소를 보였다. 두 번째 실험에서는 환아의 임상증상 치료효과 면에서 투여군에서 위약군에 비해 3일째에 설사 증상의 개선을 보였다. 결 론 : 항로타바이러스 난황항체의 로타바이러스 감염에 대한 치료효과를 본 결과 환아의 분변에서 바이러스 입자량의 감소와 함께 설사 증상의 개선의 치료 효과를 보이기 때문에 항로타바이러스 난황항체는 로타바이러스 감염증의 치료제가 될 수 있을 것으로 생각된다.

Anti-inflammatory effect of Geranium thunbergii on lipopolysaccharide-stimulated RAW 264.7 cells

  • Geranium thunbergii is a perennial plant commonly used as an oriental medicine for prevention of diarrhea, constipation, and gastrointestinal disorders. However, its anti-inflammatory effect has not been evaluated thus far. Therefore, the present study aimed to investigate the anti-inflammatory effect of G. thunbergii. In this study, G. thunbergii extracted with methanol; this methanol extract was further partitioned using various solvents, and G. thunbergii ethyl acetate fraction (GTEF) was obtained. To determine the anti-inflammatory activity of G. thunbergii, the effects of GTEF on nitric oxide (NO) production in lipopolysaccharide-stimulated RAW 264.7 cells were evaluated. GTEF suppressed NO production in a dose-dependent manner without any toxic effects. In addition, western blotting was performed to examine the effect of GTEF on expression of inducible nitric oxide synthase and cyclooxygenase-2. These results suggest that GTEF as a phytoextract may be useful for the prevention or treatment of inflammation.

Direct-fed Enterococcus faecium plus bacteriophages as substitutes for pharmacological zinc oxide in weanling pigs: effects on diarrheal score and growth

  • Objective: Effects of direct-fed Enterococcus faecium plus bacteriophages (EF-BP) were investigated as potential substitutes for pharmacological ZnO for weanling pigs. Methods: Dietary treatments were supplementations to a basal diet with none (NC), 3,000-ppm ZnO (PC), 1×1010 colony-forming units of E. faecium plus 1×108 plaque-forming units (PFU) of anti-Salmonella typhimurium bacteriophages (ST) or 1×106 PFU of each of anti-enterotoxigenic Escherichia coli K88 (F4)-, K99 (F5)-, and F18-type bacteriophages (EC) per kg diet. In Exp 1, twenty-eight 21-day-old crossbred weanling pigs were individually fed one of the experimental diets for 14 days and euthanized for histological examination on intestinal mucosal morphology. In Exp 2, 128 crossbred weanling pigs aged 24 days were group-fed the same experimental diets in 16 pens of 8 piglets on a farm with a high incidence of post-weaning diarrhea. Results: None of the diarrheal score or fecal consistency score (FCS), average daily gain (ADG), gain: feed ratio, structural variables of the intestinal villus, and goblet cell density, differed between the EF-BP (ST+EC) and NC groups, between EF-BP and PC, or between ST and EC, with the exception of greater gain: feed for EF-BP than for PC (p<0.05) during days 7 to 14 (Exp 1). In Exp 2, ADG was less for EF-BP vs PC during days 0 to 7 and greater for EF-BP vs NC during days 7 to 14. FCS peaked on day 7 and declined by day 14. Moreover, FCS was less for EF-BP vs NC, did not differ between EF-BP and PC, and tended to be greater for ST vs EC (p = 0.099). Collectively, EF-BP was comparable to or slightly less effective than PC in alleviating diarrhea and growth check of the weanling pigs, with ST almost as effective as PC, when they were group-fed. Conclusion: The E. faecium-bacteriophage recipe, especially E. faecium-anti-S. typhimurium, is promising as a potential substitute for pharmacological ZnO.

오배자의 생리활성 연구 (Studies on the Cytoprotective and Anti-inflammatory Effects of Isolate from Galla Rhois)

  • Galla Rhois is a nest of parasitic bug, has been traditionally used for the treatment of the therapy of diarrhea, peptic ulcer, hemauria, etc., that showed various anti-inflammatory activity, and other biological properties. We studied the effect of Galla Rhois ethanol extract. we investigated whether compounds isolated from the ethanol extract of Galla Rhois, could modulate iNOS and COX-2 expression in RAW 264.7 macrophages stimulated with lipopolysaccharide (LPS). We found compounds that suppressed LPS-induced iNOS and COX-2 expression. Suppression of the expression of iNOS and COX-2 was in parallel with the comparable inhibition of the production of nitric oxide (NO) and prostaglandin E2 (PGE2). Our results suggest that compounds can inhibit NO and PGE2 productions through suppression of LPS-induced iNOS and COX-2 expression. Because COX-2- or iNOS-dependent mechanisms are involved in inflammation and tumor progression, our findings provide a new uncovering mechanism responsible for anti-inflammatory and antitumor effects of Galla Rhois.

  • Potentilla chinensis Ser. (Rosaceae) has long been used for a remedy of diarrhea and inflammation in Korea. In this study, the anti-inflammatory effects of the Potentillae chinensis Herba water extract (PCX) was investigated in proteinase-activated receptor-2 (PAR2)-mediated rat paw edema. Paw edema was induced by injection of trypsin or trans-cinnamoyl-LIGRLO-$NH_2$ (tc-$NH_2$) into the hind paw of rats. PCX (10, 50, 100 and 200 mg/kg) was orally administered 1 h before the induction of inflammation. At doses of 50, 100 and 200 mg/kg, PCX showed significant inhibition on both change in paw volume and vascular permeability. PCX (100 mg/kg) significantly inhibited PAR2 agonists-induced myeloperoxidase (MPO) activity in paw tissue. These results indicate that PCX has an anti-inflammatory action in PAR2-mediated paw edema.

  • The Allium tuberosum is a perennial herb. From the ancient times, it has been used for food or medical purpose. Allium tuberosum is widely distributed in Korea, China, Japan and other countries. As herbal medicine, the Allium tuberosum has medical effects on lumbago, sthenia, diuresis, nocturia, hematemesis, anemia, haemorrhoid, hiccups, diarrhea, eczema, sunstroke, pollution and so on. Recently, the Allium tuberosum has been studied on its components, anti-microorganism action, anti-fungal action, anti-arteriosclerosis action, anti-cancer action. However, there has not been any study on hematological effect of allium tuberosum extract on the blood in large dogs whereas the effects of excess garlic and onion on the blood of cattle, horses, dogs, cats and sheep has been reported by many researchers. Thus, this study was performed to observe the hematological effect of excessive Allium tuberosum extract administrated in large dogs. 1. The Red Blood Cell(R3C) count was significantly decreased(p<0.05), Packed Cell Volume (PCV) count were significantly decreased(P<0.05), and Reticulocyte count was significantly increased(p<0.05) 2. The White Blood Cell(WBC) count was significantly increased(p<0.05).3. The Hemoglobin concentration was significantly decreased(p<0.05) and Mean Corpuscular Hemoglobin Concentration (MCHC) was increased. and Mean Corpuscular Volume(MCV) was significantly increased.(p<0.05). 4. The Reduced glutathione(GSH) and Met-Hemoglobin were significantly increased (p<0.05).

택사(澤瀉)의 국내·외 연구동향과 『본초학』, 한국표준질병사인분류의 상관관계에 대한 연구 (The Literature Study of Research Trend of Alismatis Rhizoma and Relationship Between the Herbology and KCD)

  • Objectives : This study was aimed to analyze the correlation between the Herbology and contemporary research results, KCD and terms of Alismatis Rhizoma. The study will present information that can be used to find the direction of further researches and be applied to the education of Herbology.Methods : Papers were searched in Riss4U and PubMed, and categorized. Medicine or pharmacy articles about Alismatis Rhizoma were matched with the Herbology treatment and KCD. KCD-codes and terms were arranged by the Herbology treatment. The Degree of Herbology research (HDR) was calculated by numbers of papers, study method, citation rates.Results : There were 392 articles about Alismatis Rhizoma. Among these there were 141 medicine and pharmacy articles about Alismatis Rhizoma, and 51 articles were matched to the Herbology treatment. Studies about Phlegm-Fluid Retention and Dizziness, Edema and Distention and Fullness was more active than others. Analysis of other articles showed that studies about anti-cancer, anti-biotic effect, anti-diabetes mellitus, anti-oxidant effect, anti-inflammation, hepatoprotective effect were active, too. In HDR, Phlegm-Fluid Retention and Dizziness was scored by 181, Edema- Distention and Fullness by 173, Hyperlipidemia by 83, Dysuria 64, Heat strangury pain by 18, Diarrhea and decreased urine volum by 0.Conclusions : 141 articles about Alismatis Rhizoma were analyzed and 51 articles were matched to the Herbology treatment. Studies about Phlegm-Fluid Retention and Dizziness, Edema and Distention and Fullness were more active than others. And studies about anti-cancer, anti-biotic effect, anti-oxidant effect, anti-inflammation, Osteoporosis should be more studied and be more discussed among Herbology researchers.

  • Most inflammatory disorders are usually treated using anti-inflammatory drugs including non-steroidal anti-inflammatory drugs (NSAID) and steroidal anti-inflammatory drugs (SAID). Prolonged uses of NSAIDs and SAIDs may frequently cause adverse side-effects such as nausea, vomiting, diarrhea, constipation, decreased appetite, kidney and liver failure, ulcers, and prolonged bleeding after an injury or surgery. Thus, it is necessarily required to develop a new anti-inflammatory drug with little side-effects. Dandelion (Taraxacum officinale) possesses the therapeutic abilities to eliminate body heat and toxins and to remove swelling and inflammation. In order to verify the anti-inflammatory activity of dandelion, TPA(12-O-tetra decanoylphorbol-acetate)-induced or croton oil-induced acute edema was developed in the mouse ears, and dandelion extract dissolved in acetone was applied to both sides of inflamed ears. It was found that dandelion could significantly reduce the ear swelling, compared to that of non-treated control. In the case of $20{\mu}{\ell}$ application of $100mg/m{\ell}$ dandelion solution (DA-100), its anti-inflammatory effect was comparable to that of indomethacin, a non - steroidal anti-anflammatory drug. Taken together, it could be concluded that topically applied dandelion extract exhibited its potentials as a new drug candidate with an effective anti-inflammatory activity.

오리나무 수피 엑스의 소염 및 진통 활성 (Anti-inflammatory and Analgesic Activity of Alnus japonica Cortex Ethanol Extract)

  • An extract of Alnus japonica (Betulaceae) cortex has been traditionally used for purifying blood, and curing feces containing blood, enteritis, diarrhea, alcoholism and cut wounds. Alnus japonica cortex extract significantly inhibited carrageenan-induced paw edema at 1, 2 and 3 hrs after oral administration by 32.8, 24.4, and 46.9%. It also inhibited acetic acid-induced vascular permeability in mice by 15.3 and 28.0% at oral doses of 500 and 1,000 mg/kg, and it significantly reduced the volume of hindpaw of adjuvant-induced arthritis rats at the day 6 and 10 by 22.5 and 18.7% after the sample administration. Alnus japonica cortex extract increased threshold on inflamed paw (Randall-Selitto assay) at 3 hr by 137.5% compared to the control group. It also reduced the number of writhing syndrome dose- dependently.
