  • Background: The new drug HP228 is a cytokine restraining agent with a broad spectrum of anti-inflammatory, analgesic, and antipyretic activity. Six healthy, adult, male volunteers were studied to determine the independent and interactive effects of HP228 and morphine on pain perception. Methods: Two groups of stimuli were applied to each volunteers before drug administration as control, 20 min after morphine and HP228 administration, and 20 min after combined administration of these two drugs. Two adhesive electrically-conducting pads were applied on opposite sides of the arm approximately 8 cm apart. The electrode were connected to an electrical impulse generator and 50 Hz 1 msec pulses of incrementally increasing intensity were delivered at 1 sec intervals. The analgesic endpoints were the current intensity (mA) at which the subject first detected the stimulus (THRESH), the intensity at which the stimulus was first idenfied as being painful (PAIN), and the intensity at which the subject requested that the stimulus be terminated due to discomfort (LIMIT). A second series of stimuli were applied immediately thereafter using 1-sec duration 50 Hz tetanus pulses with increasing intensities at 2~5 sec intervals. Results: There were significant differences between drug treatments (Morphine, HP228, HP228/Morphine) and control (No drugs) in any of the measurements (PAIN, LIMIT) except THRESH with the twitch and tetanus test. Conclusions: The data suggests that HP228 is an analgesic, but it does not appear to interact with morphine in an additive manner.

이부프로펜 리지네이트의 직장흡수에 미치는 좌제기제 및 첨가제의 영향 (Effects of Suppository Bases and Additives on Rectal Absorption of Ibuprofen Lysinate)

  • Ibuprofen is an effective non-steroidal anti-inflammatory drug (NSAID), but it has several limitations in clinical application because of low solubility in water and gastrointestinal irritation. A water-soluble salt of ibuprofen, ibuprofen Iysinate, has been synthesized to overcome these shortcomings, and it was formulated as suppository for rectal administration. Witepsol and polyethylene glycols were employed as suppository bases for either ibuprofen or ibuprofen Iysinate, in order to compare the bioavailability in rabbits. The plasma concentrations of ibuprofen were assayed by HPLC after a rectal administration of ibuprofen and ibuprofen Iysinate, respectively. In addition to the comparison of two suppository bases, the other factors which affect on rectal absorption were also evaluated, especially in the point of not only particle size and shape of ibuprofen Iysinate but also effects of additives such as stearic acid, cetyl alcohol and capric acid. And pharmacokinetic parameters such as AUC, $C_{max}$, and $T_{max}$ were also compared. In conclusion, spray-dried ibuprofen Iysinate which was polyporous and spherical shape gave an increased absorption from the rectal formulations with Witepsol Hl5 and stearic acid.

The Efficacy of Additional Intravenous Patient-controlled Analgesia to the Interscalene Block in Arthroscopic Shoulder Surgery: A Prospective Randomized Controlled Study

  • Background: The purpose is to determine the efficacy of additional intravenous patient-controlled analgesia (IV-PCA) by comparing the analgesic effects between interscalene block (ISB) combined with IV-PCA and single ISB after arthroscopic shoulder surgery. Methods: A total of 213 patients who underwent arthroscopic shoulder surgery were divided into two groups based on the type of perioperative anesthesia. The single ISB group included 100 patients, while the IV-PCA group included 113 patients. The visual analogue scale for pain (VAS pain) scores were assessed at 12, 24, and 48 hours postoperatively in accordance with shoulder pathology. Postoperative narcotics-related complications and consumption of additional non-steroidal anti-inflammatory drugs between the two groups were compared. Results: VAS pain showed no significant difference between the two groups at most points of the postoperative timeline, regardless of shoulder pathology, except in patients with rotator cuff repair at postoperative 24 hours. Although the IV-PCA group showed a statistically lower VAS pain score than the ISB group at postoperative 24 hours (p=0.04), the difference in the VAS pain score was only 9.0 mm in patients with rotator cuff repair. Narcotics-related complications were observed more frequently in the IV-PCA group than in the ISB group for patients with rotator cuff repair. Conclusions: Additional IV-PCA demonstrated no booster effect for immediate pain control in patients undergoing arthroscopic shoulder surgery with preoperative single ISB. Furthermore, patients with IV-PCA experienced greater narcotics-related complications.

A Behavioral Study of Promethazine Interaction with Analgesic Effect of Diclofenac: Pain Combination Therapy

  • Objectives: Pain is considered as a cause of sickness and the most prevalent symptom which makes people visit a physician. Nowadays, combination therapy is becoming useful to relieve chronic and postsurgical pain. The aim of this study was to study the promethazine (as an antihistamine) interactions with antinociceptive effect of diclofenac (as a non-steroidal anti-inflammatory drugs). Methods: In initial part of the study, we investigate the analgesic effect of diclofenac. Using writhing test, we demonstrate that diclofenac significantly reduces writhe response induced by acetic acid in a dose-dependent manner. In this study, we evaluate the combination effect of promethazine on diclofenac analgesic effect. Results: We observed that diclofenac inhibited pain in the dose dependent manner which means that by increasing dose of diclofenac a significant decrease in pain was observed. This experimental setup allowed calculation of the dose that caused 50% antinociception (ED50) for diclofenac. The ED50 for diclofenac in this study was determined to be 9.1 mg/kg according our previous study. Additionally, promethazine was showed a dose-dependent inhibition of writhes. The combination of different doses of promethazine (2, 4, 6 mg / kg) with diclofenac ED50 (9.1 mg / kg) was injected to mice. Promethazine 4 and 6 mg / kg in combination with diclofenac had significantly led to increase analgesic effect of diclofenac. Conclusion: In conclusion, these results add important information to the existing knowledge on combination of diclofenac and antihistamine in pain therapies to be used in clinical practice and maybe helpful in designing the future guidelines.

리그난의 화학적 특성, 인체 건강에 미치는 영향 및 리그난의 우유 기능 강화에 관한 연구 고찰 (Chemical Properties of Lignans, Their Effects on Human Health, and the Enhancement of Milk Function of Lignans)

  • This review discusses the characteristics of major lignans and related studies and provides a basis for future studies. Lignans are present in various food products consumed daily, such as flaxseed and other seeds, vegetables, fruits, and beverages including coffee, tea, and wine. Lignans are natural phytoestrogens with a structure similar to that of secoisolariciresinol (Seco), mataireinol (Mat), pinoresinol (Pin), medioresinol (Med), lariciresinol (Lari), and syringaresinol, which is then converted to enterodiol (END) and enterolactone (ENL), which are mammalian lignans and the primary biologically active enterolignans, by the intestinal microflora. The associations between lignans and a decreased risk of cardiovascular disease are promising; however, they are not yet well-established, probably owing to low lignan intake in habitual Western diets. Nonetheless, these associations were more prominent at the higher doses in interventional studies. Many studies on humans and animals have reported the benefits of lignan consumption in protecting against CVD and metabolic syndrome by reducing lipid and glucose concentrations. END and ENL reportedly exert protective effects including phytoestrogenic, antioxidant, anti-inflammatory, and anticancer effects through various mechanisms. Moreover, lignans reportedly exert beneficial effects in breast, colon, and prostate cancer and osteoporosis have reported that. However, future studies are required to confirm the association between lignan and disease.

Anthocyanin Profiling and Radical Scavenging Activity of Selected Pigmented Rice Varieties

  • BACKGROUND: Anthocyanins have been recognized as health-enhancing substances due to their antioxidant activity, anti-inflammatory, anticancer, and hypoglycemic effects. The objective was to identify anthocyanins-rich rice grains for the development of functional foods and/or functional food colorants METHODS AND RESULTS: Rice grains of one black and three red-hulled rice varieties were extracted with acidified 80% aqueous methanol. The antioxidant activity of the methanolic extracts was screened on TLC plates and in an in vitro assay using DPPH (1, 1-diphenyl-2-picrylhydrazyl) as a free radical source. Red-hulled rice varieties exhibited higher antioxidant activity (88%, 1 mg/mL) than black rice (67%, 1 mg/mL). Among the red-hulled varieties tested, rice variety SSALBYEO54 (901452) was the most active (72%, 0.5 mg/mL). Rice extracted anthocyanin compounds were analyzed by HPLC-DAD-FLD and LC-MS/MS. Red-hulled varieties comprised cyanidin-3-glucoside in addition to ferulic acid esters, apigenin and kaempferol glycosides. CONCLUSION(s): Anthocyanins identified in the black rice variety were cyanidin-7-O-galactoside, cyanidin-3-Oglucoside, cyanidin-3'-O-glucoside, cyanidin-5-O-glucoside, cyanidin-3, 7-O-diglucoside, cyanidin-3, 5-O-diglucoside and peonidin-4'-O-glucoside. The results of this study show that the black rice (IT212512) and red-hulled rice variety SSALBYEO54 (901452) contain notable antioxidant activity for potential use in nutraceutical or functional food formulations.

Immunomodulatory Effects of a Methanol Extract from Opuntia ficus indica on Murine Splenocytes

  • Multiple beneficial properties of Opuntia ficus indica (OPF) are well established. In the present study, we have investigated the immunological role of OPF extract (OPFE) on murine splenocytes. OPFE dose- and time-dependently enhanced the proliferation of splenocytes without cytotoxicity. Our results also showed that the number of $CD4^+$ helper T cells and CD45R/$B220^+$ pan B cells increased markedly, but not $CD8^+$ cytotoxic T cells or $CD11b^+$ granulocytes/macrophages. In addition, OPFE significantly decreased the production levels of T helper (Th) 1 type cytokines, interferon (IFN)-$\gamma$, and tumor necrosis factor (TNF)-$\alpha$, although had no significantly differences in those of interleukin (IL)-4, a Th2 type cytokine in concanavalin A (Con A)-stimulated blastogenic cells. Furthermore, OPFE alone strongly increased IL-4 production and decreased TNF-$\alpha$ production even in the absence of Con A. On the basis of these results, this study suggests that OPFE enhances immunity by regulating the pro- and anti-inflammatory response, indicating that this extract exerts a marked immunomodulatory effect, confirming its usefulness as therapy for immune-related diseases.

아산 지역특산물과 설화를 활용한 Food Tourism 제품 개발 (Study on the Development of Food Tourism Products Based on the Local Food and Folktale)

  • This study aimed to develop unique, local "food tourism" products by finding specialized items that combine tourist attractions, such as folklore or hot springs. Traditional ingredients were analyzed with ancient texts for the methods of research. A brand image was made possible utilizing hot spring lore and other regional stories. The tofu products were produced using local specialty beans. Products, such as tofu residue cake and willow bean tea, were made with the tofu residue. After the products were completed, the sensory test began at the local tourist attraction. Asan City's food tourism product willow tofu was made with beans that were given as compensation for building the Onyang temporary palace according to the Annals of the Joseon Dynasty and the willow tree that appears in Sunshin Lee's anecdotes. After the preference test between normal tofu and willow-extract tofu was conducted to measure the product potential of willow tofu, among the sample extracts, 0.04% of the willow extract showed a significant preference. The hot spring tofu-residue cake was baked using tofu residue and vegetable olive oil to substitute for animal oil, such as butter, or margarine. After the sensory test targeting the adults was conducted, both products displayed significant product potential with average scores above 5.0. Willow tree bark, which has antioxidation and anti-inflammatory effects without a bitter taste or strong smell, was proven to bean appropriate ingredient for leached tea. The nutty flavor of leached tea was enhanced by roasted green kernel black beans and willow tree bark. The sensory test showed that the leached tea and tofu received a high preference rating on both color and flavor.

LPS로 유도된 흰쥐의 신장염에 대한 음곡(KI10) 저령약침의 효과 (Effects of Polyporus Herbal-acupuncture at KI10 on LPS-induced nephritis in rats)

  • Objectives: The purpose of this study was to evaluate the effects of Polyporus Herbal-acupuncture(PO-HA) at KI10 (Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods: Rats were allocated into normal, control, and 2 experimental groups. The rats in the control group were intra-peritoneally injected with LPS for nephritis induction. The rats in the groups of experiment 1 and experiment 2 were treated with Saline injection, and PO-HA, respectively at KI10 three times for a week and then intra-peritoneally injected with LPS. To evaluate the effects of PO-HA at KI10, WBC count in blood, creatine, tumor necrosis factor-alpha (TNF-${\alpha}$), cytokine-induced neutrophil chemoattractant 1(CINC-1) in serum, urinary volume, creatinine, total protein in urine, myeloperoxidase(MPO) in kidney were measured. Results: PO-HA at KI10 significantly suppressed the increase of WBC in blood, TNF-${\alpha}$, CINC-1 in serum, MPO in kidney of LPS-stimulated rats. PO-HA at KI10 significantly suppressed the increase creatinine, total protein in urine of LPS-stimulated rats. Conclusions: According to these results, it is postulated that PO-HA at KI10 has an anti-inflammatory and renal-protective effects on LPS-induced nephritis in rats. Therefore, it is suggested that PO-HA at KI10 may be an useful therapeutics for nephritis in clinical field after further researches.

황련(黃連)과 구성 생화합물의 약리작용에 대한 고찰 (Review of Pharmacological Effects of Coptidis Rhizoma and its Bioactive Compounds)

  • Objectives: Coptidis Rhizoma is one of the most popular and multi-purpose herbs in traditional medicine. The purpose of this study was to contribute to research and applications of Coptidis Rhizoma in clinic, by analyzing and reviewing international studies on its chemical compositions and pharmacological effects. Methods: This study analyzed 344 articles published from 2000 to 2010 in PubMed, Refworks, Riss, and KTKP. The search keywords were "Coptis chinensis", "Coptis japonica", "Coptidis Rhizoma", "huanglian" and "huanglian in Chinese". From them, we selected 114 articles which met our inclusion criteria. Results: This study reviewed 114 articles on Coptidis Rhizoma and its active components in terms of 'Active components', 'Experimental studies', 'Clinical studies', 'Industrial use' and 'Side Effects/Toxicity'. Conclusions: The active components of Coptidis Rhizoma are berberine, coptisine, epiberberine, palmatine, jateorrhizine, magnoflorine, worenine, etc. It is reported that Coptidis Rhizoma and its active components have anti-inflammatory, antibacterial, antitumor, and antioxidant activity, and cardiovascular, hepatoprotective, antidiabetic, neuroprotective, gastrointestinal, pain relieving, discharge phlegm and metrocyte proliferation effects. Moreover, we found that Coptidis Rhizoma can be used for bath preparation, cosmetic products and as a natural antimicrobial substance.
