The purpose of this research is to suggest some measures to improve policy issues by analyzing policy related to a child care service program in the community. Gilbert and Terrell's social policy analysis framework is used to analyze an after-school care service program for elementary school children. Furthermore, the Elementary Care Class(including the After-school Program), Youth After-school Academy, and Community Child Center, referred to as public care services, are employed to analyze community care service programs for children. The Elementary Care Class, Youth After-school Academy and Community Child Center are very similar in terms of the contents and application of the care service program for children, and mainly serve children in low-income groups. In addition, although user overlapping is an inevitable problem because the operating time of the Elementary Care Class and After-school Program coincides with that of the Youth After-school Academy and Community Child Center, it is structurally very hard to adjust the content of service, operating time, and so on because of differences in the delivery system for each program. Therefore, it is necessary to generate a unified delivery system to manage after-school care service programs for children and create a new special control tower to solve these problems. Furthermore, it is needed to extend the services to children from general double-income family, which is a blind spot of the care service.
The purpose of the study was to analyze the determinants on the absence of after-school care among elementary students. This study is based on the National Children and Youth Panel Survey(2010) data and analyzed through Binary Logit Model and Multinominal Logit Model. The Following results were obtained: First, school grades, the number of siblings, mother's education, family type by parental employment, family structure, family type by parental nativity, and family income, all affected the absence of after-school care. Second, the absence days of after-school care was affected by different factors. 1day-2days a week in absence of after-school was more likely to increase among children with more siblings and an older father. On the other hand, spending over 3-4days a week without after-school care was more likely to increase among mothers with lower education, dual-earner families, multicultural families, lower family incomes, small cities and rural areas. Based on the results of this study, we agree with the generalization and the diversification of after-school care policy for elementary school students.

This research aimed at investigating policy change process of after school contracting out and suggesting future alternative. Also, this research conducted a literature search for the research data related to policy and related regulations. As the result of investigation, the Ministry of Education established management plan for after school, however there have been changes in policy, e.g. contracting out was executed from 2004 to 2008 upon autonomy of unit school through school steering committee deliberation(consultation), while standardized procedure was conducted, which was suggested in 'After school operation guideline', produced in cooperation between the Ministry of Education and municipal ministry of education from 2008 to 2015, while since 2016, contract law should be applied when after school contracting out is adopted. Policy change since 2016 is based on the legal necessity that contract law should be followed as the contract size of after school contracting out has become larger along with necessity of clarity of after school contracting out. Nevertheless, there's a worry that quality of after school education could be degraded due to lowest price bidding. The government suggested an alternative to prevent excessive price competition by paying a regular rate of basic price as personnel expenses, however this research suggested a plan to enact an ordinance in regard of specialty of after school educational activities and cities and provinces as the fundamental solution plan.

Background: The study describes the changes resulted from imposition on tertiary hospital outpatient coinsurance rate rise policy and in tertiary or general hospital drug coverage rise policy on healthcare service utilization. Methods: Accordingly, the hypothesis about outpatient healthcare utilization after rise policy in outpatient coinsurance rate and drug coverage was established, using interrupted time-series analysis and segmented regression analysis to test the hypothesis. 5-year analysis period (2007. 3-2012. 3) from the outset year was designated, the data about most common 10 high-ranking of the main diseases targeting visiting patient from age of 6 to 64 were collected. Results: The summary on the major research is followed. First, the medical expense and duration of treatment tends to be increased in case of imposition about rise policy in outpatient coinsurance rate in the tertiary hospital under the interrupted time-series analysis. It showed temporary increase and slow down on account of influenza A even after the policy enforcement. In segmented regression analysis, duration of visit and medical expense in the tertiary hospital increased temporally right after the policy implementation and the decreased rapidly depends on period. Both rise and fall is statistically significant. The second, In case of tertiary or general hospital outpatient drug coverage rise policy, all of the tertiary hospital healthcare service utilization variables by the interrupted time-series analysis, drug coverage policy in the general hospital deeply declined according to decreasing trend before policy implementation. The third, in case of segmented regression analysis, the visit duration and medical expense statistically declined right after the policy implementation in both the tertiary and general hospital. Meanwhile, administration day was statistically meaningful only for the decrease right after the policy implementation. Otherwise, general hospital changes are not statistically meaningful. And the medicine cost was statistically, meaningfully decreased after the increase in drug coverage. Conclusion: Finally, the result demonstrated according to the analysis is only 1 hypothesis is denied, the other 2 are partially supported. Then, tertiary hospital outpatient coinsurance rate increase policy comparatively makes decrease effect on long-term healthcare utilization, and tertiary or general hospital outpatient drug coverage policy showed partially short-term effect is assured.
Using the National Children and Youth Panel Survey (2010) data, this study examines the self-care after school experienced by elementary school students. It argues the necessity of analysis on self-care days for understanding demanders' characteristics of after-school care policy. Based on the Ordered Logit Model, this study analyzes the determinants of self-care days among school-aged children. The main result of the analysis is that self-care days are also more likely to be increased among children with higher school grade, more sibling, lower mother's education, dual-earner family, two-parent family, multi-cultural family, and lower family income.

This study aims to investigate the awareness and policy demands of after school care classes of mothers with elementary school children. The participants of study were 25 mothers with elementary school children in Seoul, Gyeonggi, Incheon, Gangwon. Data collection was conducted from June 23, 2017 to July 18, 2017 by focus group interview. Through the data analysis, the results of the study were analyzed as 'the reality of the after school care class that the mother sees' and 'the policy demands of the mother's after school care class'. 'The reality of the after school care class that the mother sees' is 'the overall level of after school care class is low', 'the probability of selecting the same as a lottery', 'the quality of education that varies depending on the level of the teacher', 'for the child attending the care class negative awareness'. 'Mother's policy demands for after school care classes' are presented as 'positioning as a substitute for thinking and caring', 'improvement of separation between parents' expectations', 'expansion of care recipients', 'request for resilient management'. As the conclusion of this study, first, after school care classes need to focus on the care function. Second, it is necessary to operate after school care classes by level. Third, the realistic needs of mothers should be reflected so that after school care classes can be positioned as alternatives to private education.
Introduction: According the 2011 Taiwanese Government Statistics, the lower secondary school enrollment number of the new-immigrant-children is about 200,000. As known, most of the new immigrants are from the Southeast Asian countries, such as Vietnam, China, Indonesia, Thailand, the Philippines and Cambodia. In order to satisfy the increasing needs and demands on education of the children of new immigrant (CNI, henceforth), Taiwanese government not only develops, but also puts the after-school learning assistance policy into practice from 2006. Therefore, the main purpose of this study is to explore the mathematics achievement of the CNI after the implementation of the after-school learning assistance policy (AsLA policy, henceforth). Purposes: Firstly, to compare the mathematics achievement of the CNI by countries. Secondly, to compare the mathematics performance among the CNI, the children from high-risk family (CHRF, henceforth) and the children of general families. Samples: The 2,452 samples, selected from two junior high schools located in central Taiwan, include 157 CNI, 522 CHRF. Methods: The main method used in this study is interval fuzzy number (IFN, henceforth) in order to compare the mathematics achievement of the children after the implementation of the AsLA policy from different type of families. Results: To reach the two purposes of this study. We can find the effectiveness of mathematics performance from three group's children of new immigrants, high-risk, general family. Therefore, the results provide one of the ways to review the new immigrant's education policy of after-school learning assistance in Taiwan.

Objective : Radiation is known to induce autophagy in malignant glioma cells whether it is cytocidal or cytoprotective. Dexamethasone is frequently used to reduce tumor-associated brain edema, especially during radiation therapy. The purpose of the study was to determine whether and how dexamethasone affects autophagy in irradiated malignant glioma cells and to identify possible intervening molecular pathways. Methods : We prepared p53 mutant U373 and LN229 glioma cell lines, which varied by phosphatase and tensin homolog (PTEN) mutational status and were used to make U373 stable transfected cells expressing GFP-LC3 protein. After performing cell survival assay after irradiation, the IC50 radiation dose was determined. Dexamethasone dose (10 μM) was determined from the literature and added to the glioma cells 24 hours before the irradiation. The effect of adding dexamethasone was evaluated by cell survival assay or clonogenic assay and cell cycle analysis. Measurement of autophagy was visualized by western blot of LC3-I/LC3-II and quantified by the GFP-LC3 punctuated pattern under fluorescence microscopy and acridine orange staining for acidic vesicle organelles by flow cytometry. Results : Dexamethasone increased cell survival in both U373 and LN229 cells after irradiation. It interfered with autophagy after irradiation differently depending on the PTEN mutational status : the autophagy decreased in U373 (PTEN-mutated) cells but increased in LN229 (PTEN wild-type) cells. Inhibition of protein kinase B (AKT) phosphorylation after irradiation by LY294002 reversed the dexamethasone-induced decrease of autophagy and cell death in U373 cells but provoked no effect on both autophagy and cell survival in LN229 cells. After ATG5 knockdown, radiation-induced autophagy decreased and the effect of dexamethasone also diminished in both cell lines. The diminished autophagy resulted in a partial reversal of dexamethasone protection from cell death after irradiation in U373 cells; however, no significant change was observed in surviving fraction LN229 cells. Conclusion : Dexamethasone increased cell survival in p53 mutated malignant glioma cells and increased autophagy in PTEN-mutant malignant glioma cell but not in PTEN-wildtype cell. The difference of autophagy response could be mediated though the phosphatidylinositol 3-kinase/AKT/mammalian target of rapamycin signaling pathway.
Government has extended the benefit coverage and reduced out-of-pocket (OOP) payment for cancer patients in 2005. This paper intends to examine the impact of the above policy on the equity in health care utilization. This paper analyzed the national health insurance data and compared the health care utilization of cancer patients before and after the policy change for people with 10 different income levels. For the equity in health care utilization, we examined the change in concentration index (CI) for visit days, inpatient days, and health expenditure. In the case of outpatient care, CI of visit days and health expenditure were positive(favoring the rich) in both regional and employee health insurance members and both 'before' and 'after' the policy change. CI values rarely changed after the policy change, and the policy change seems to have little impact on the equity of outpatient care utilization except expenditure of regional subscriber. In the case of inpatient care, CI of inpatient days was negative and CI of health expenditure was positive in both regional and work subscriber and both 'before' and 'after' the policy change. After the policy change, CI of inpatient expenditure in both groups of members decreased. CI of inpatient days changed in the direction favoring the poor in regional insurance members, but it rarely changed in employee insurance members. These results suggest that the policy of reducing OOP payment has a positive impact and reduced the inequity particularly in the utilization of inpatient care of cancer patients.
Purpose: This study aims to analyze the factors affecting the agenda-setting process and the formation process of school-based mental health policies by applying a policy stream model. Methods: For this purpose, Kingdon's policy stream model was used as the analytical framework. Results: First, when establishing a school-based mental health policy, the agenda was set going through unpredictable and nonlinear changes. Second, for the school-based mental health policy to be selected onto the agenda and to be developed and implemented as an actual policy, the role of policy makers was considered most important in the process. Third, the policy window for school-based mental health policy was closed around the year 2013. Finally, an analysis of the school-based mental health policy stream identified two key features. One is that the school-based mental health policy first emerged when school violence prevention policy expanded its scope into relevant neighboring policies. The other is that the school-based mental health policy has taken shape through a linear decision-making process (being put on the government's agenda, searching for an alternative, selection, and implementation) during the policy implementation period after it has been selected as an alternative policy. Conclusion: Conclusions can be summed up as follows. The school-based mental health policy needs continuous development and improvement in case the window for the policy may open in the coming future. The government's support is needed to draw policy makers' interest and participation who play the biggest role in establishing policies.
