Statement of problem : Little is known about the effect of a counter-torque device and the internal hexagon of abutment on the tightening torque transmitted to the implant. Purpose : The purpose of this study was to examine the effect of a counter-torque device and the internal hexagon of abutment on the tightening torque transmitted to the implant. Material and Methods : In this study, three types of abutment were used, (1) two-piece conical abutment with hexagon, (2) two-piece conical abutment without hexagon, and (3) one-piece conical abutment without hexagon. The experimental groups were divided into five groups according to the type of abutment and the usage of a counter-torque device. Group I : two-piece conical abutment with internal hexagon was tightened without the use of a counter-torque device. Group II : two-piece conical abutment without internal hexagon was tightened without the use of a counter-torque device. Group III : one-piece conical abutment without internal hexagon was tightened without the use of a counter-torque device. Group IV : two-piece conical abutment with internal hexagon was tightened with the use of a counter-torque device Group V : two-piece conical abutment without internal hexagon was tightened with the use of a counter-torque device. Abutments were tightened 20Ncm torque with the use of manual torque wrench and then torque values were measured by torque-gauge. After the measurement of torque values, all groups were loosened with the use of manual torque wrench and then detorque values were measured by torque-gauge. Results : The results were as follows. 1. There were no differences in torque values among three types of abutment. 2. Regardless of the existence of the internal hexagon of abutment, a counter-torque device decreased the tightening torque transmitted to the implant about 92% 3. In group III showed the highest detorque value, however there were no differences among group I, II, IV and V. Conclusion : Within the limitations of this study, it was concluded that the internal hexagon of abutment has no effect on the tightening torque transmitted to the implant and the detorque value of abutment screw. The use of a counter-torque device is essential to prevent microfracture on the implant-bone interface but has no effect on preload.

STATEMENT OF PROBLEM: Tensile strength of metal crown cemented with provisional cement have shown clinically difference between metal abutment of implant and natural abutment. PURPOSE: This study was tested to compare the tensile strength of provisional cement on the natural abutment and metal abutment. MATERIAL AND METHODS: Out of the 20 premolars that were selected for this experiment, each 10 were prepared of abutments by chamfer and rounded shoulder margin and then duplicated to produce 20 metal abutments that were same to natural teeth. Then, crowns were fabricated to fit the total 40 natural & metal abutments to be cemented by cavitec, to be added of regular, repetitive vertical load, and to be measured of tensile strength by using Universal Test Machine. RESULTS: There was statistically significant difference in the tensile strength between the crowns cemented to the natural & metal abutments, but no statistically significant difference was observed between the chamfer and the shoulder gingival margin of the each abutments. CONCLUSIONS: Tensile strength of metal teeth is greater about 2 more times than that of natural teeth when it is cemented with Cavitec.
Statement of problem. Implant abutment screw joints tend to loosen under clinical conditions. Abutment screw loosening results in loss of preload in function. Purpose. Anti-rotational inner post screw (ARIPS) systems were compared with conventional abutment screws to reduce screw loosening. Reverse torque values were evaluated. Material and methods. 32 implant assemblies (Warentec, Co, Ltd, Seoul, Korea) were organized as the 30-Ncm-torque conventional groups and 30-Ncm-torque ARIPS groups in external and internal system. The specimens were tested to 106 cycles at a load of 200N. Preload reverse torque, postload reverse torque, and the ratio of postload reverse torque to preload reverse torque were evaluated. The data were analyzed with unpaired t-test in external and internal systems. Results. In the ratio of postload reverse torque to preload reverse torque, the ARIPS groups showed significant differences than the conventional screw group in both external and internal system. Conclusion. Within the limitations of this study, abutment screw loosening was effectively reduced using ARIPS system.
Purpose : The purpose of this study was to evaluate the machining accuracy and consistency of implant/abutment/screw combination or internal connection type. Material and methods: In this study, each two randomly selected internal implant fixtures from ITI, 3i, Avana, Bicon, Friadent, Astra, and Paragon system were used. Each abutment was connected to the implant with 32Ncm torque value using a digital torque controller or tapping. All samples were cross-sectioned with grinder-polisher unit (Omnilap 2000 SBT Inc) after embeded in liquid unsaturated polyester (Epovia, Cray Valley Inc). Then optical microscopic and scanning electron microscopic(SEM) evaluations of the implant-abutment interfaces were conducted to assess quality of fit between the mating components. Results : 1) Generally, the geometry of the internal connection system provided for a precision fit of the implant/abutment into interface. 2) The most precision fit of the implant/abutment interface was provided in the case of Bicon System which has not screw. 3) The fit of the implant/abutment interface was usually good in the case of ITI, 3I and Avana system and the amount of fit of the implant/abutment interface was similar to each other. 4) The fit of the implant/abutment interface was usually good in the case of Friadent, Astra and Paragon system. The case of Astra system with the inclined contacting surface had the most Intimate contact among them. 5) Amount of intimate contact in the abutment screw thread to the mating fixture was larger in assembly with two-piece type which is separated screw from abutment such as Friadent, Astra and Paragon system than in that with one-piece type which is not seperated screw from abutment such as ITI, 3I and Avana system. 6) Amount of contact in the screw and the screw seat of abutment was larger in assembly of Friadent system than in asembly of Astra system of Paragon system. Conclusion: Although a little variation in machining accuracy and consistency was noted in the samples, important features of all internal connection systems were the deep, internal implant-abutment connections which provides intimate contact with the implant walls to resist micro-movement, resulting in a strong stable interface. From the results of this study, further research of the stress distribution according to the design of internal connection system will be required.
Statement of problom: In the internal connection system the loading transfer mechanism within the inner surface of the implant and also the stress distribution occuring to the mandible can be changed according to the abutment form. Therefore it is thought to be imperative to study the difference of the stress distribution occuring at the mandible according to the abutment form. Purpose: The purpose of this study was to assess the loading distributing characteristics of 3 implant systems with internal connection under vertical and inclined loading using finite element analysis. Material and method: Three finite element models were designed according to the type of internal connection of ITI(model 1), Friadent(model 2), and Bicon(model 3) respectively. This study simulated loads of 200N in a vertical direction (A), a $15^{\circ}$ inward inclined direction (B), and a $30^{\circ}$ outward inclined direction (C). Result: The following results have been made based on this numeric simulations. 1. The greatest stress showed in the loading condition C of the inclined load with outside point from the centric cusp tip. 2. Without regard to the loading condition, the magnitudes of the stresses taken at the supporting bone, the implant fixture, and the abutment were greater in the order of model 2, model 1, and model 3. 3. Without regard to the loading condition, greater stress was concentrated at the cortical bone contacting the upper part of the implant fixture, and lower stress was taken at the cancellous bone. 4. The stress of the implant fixture was usually widely distributed along the inner surface of the implant fixture contacting the abutment post. 5. The stress distribution pattern of the abutment showed that the great stress was usually concentrated at the neck of the abutment and the abutment post, and the stress was also distributed toward the lower part of the abutment post in case of the loading condition B, C of the inclined load. 6. In case of the loading condition B, C of the inclined load, the maximum von Misess stress at the whole was taken at the implant fixture both in the model 1 and model 2, and at the abutment in the model 3. 7. The stress was inclined to be distributed from abutment post to fixture in case of the internal connection system. Conclusion: The internal connection system of the implant and the abutment connection methods, the stress-induced pattern at the supporting bone, the implant fixture, and the abutment according to the abutment connection form had differenence among them, and the stress distribution pattern usually had a widely distributed tendency along the inner surface of the implant fixture contacting the a butment post.
PURPOSE. The purpose of this study was to achieve more retention and stability and to delay or prevent screw loosening. MATERIALS AND METHODS. Twenty implants (Implantium 3.4 mm, Dentium, Seoul, Korea) were divided into 2 groups (n = 20). In the first group, an adhesive material was applied around the screw of the abutments (test group). In the second group, the screws are soaked in saliva (control group). All the screws were torqued under 30 N/cm, Then, the samples were gone through a cyclic fatigue loading process. After cyclic loading, we detorqued screws and calculated detorque value. RESULTS. In comparison with the control group, all the implant screws in the test group were smeared with the adhesive material, showing significant higher detorque value. CONCLUSION. There are significantly higher detorque values in the group with adhesive. It is recommended to make biocompatible adhesive to reduce screw loosening.
Statement of problem. The performance and maintenance of implant-supported prostheses are primarily dependent upon load transmission both at the bone-to-implant interface and within the implant-abutment-prosthesis complex. The design of the interface between components has been shown to have a profound influence on the stability of screw joints. Purpose. The Purpose of this study was to compare the strength and the fatigue resistance of 1-piece and 2-piece abutment connected to oral implant, utilizing an internal conical interface. Material and methods. Twenty $Implatium^{(R)}$ tapered implants were embedded to the top of the fixture in acrylic resin blocks. Ten $Combi^{(R)}$(1-piece) and $Dual^{(R)}$(2-piece) abutments of the same dimension were assembled to the implant, respectively. The assembled units were mounted in a testing machine. A load was applied perpendicular to the long axis of the assemblies and the loading points was at the distance of 7mm from the block surface. Half of 1-piece and 2-piece abutment-implant units were tested for the evaluation of the bending strength, and the others were cyclically loaded for the evaluation of the fatigue resistance until plastic deformation occurred. Nonparametric statistical analysis was performed for the results. Results. Mean plastic and maximum bending moment were $1,900{\pm}18Nmm,\;3,609{\pm}106Nmm$ for the 1-piece abutment, and $1,250{\pm}31Nmm,\;2,688{\pm}166Nmm$ for the 2-piece abutment, respectively. Mean cycles and standard deviation when implant-abutment joint showed a first plastic deformation were $238,610{\pm}44,891$. cycles for the 1-piece abutment and $9,476{\pm}3,541$ cycles for the 2-piece abutment. A 1-piece abutment showed significantly higher value than a 2-piece abutment in the first plastic bending moment (p<.05), maximum bending moment (p<.05) and fatigue strength (p<.05). Conclusion. Both 1-piece and 2-piece conical abutment had high strength and fatigue resistance and this suggests long-term durability without mechanical complication. However, the 1-piece conical abutment was more stable than the 2-piece conical abutment in the strength and the fatigue resistance.

Statement of problem: Little is known about the effect of abutment preparation procedure on do-torque values in different implant platform and the relationship of final do-torque values with different implant platform size. Purpose: This study evaluated the effect of abutment preparation procedure on do-torque values in different implant platform and the relationship of final do-torque values with different implant platform size. Material and method: Six ITI implants (2 narrow-neck implants, 2 regular-neck implants, 2 wide-neck implants) and six Branemark implants (2 narrow platforms, 2 regular platforms, 2 wide platforms) were embedded in each acrylic resin block with epoxy resin. Eighteen $synOcta^(R)$ abutments (6 narrow-neck implant-abutments, 6 regular-neck implant-abutments, 6 wide-neck implant-abutments) and eighteen esthetic abutments (6 narrow platform-abutments, 6 regular platform-abutments, 6 wide platform-abutments) were tightened to each implant with digital torque gauge. Initial do-torque values were measured using digital torque gauge. After preparation of abutments, Final do-torque values were measured with digital torque gauge. Results and conclusion: 1. Screws loosening or abutments motion were not detected in all experimental group, but some scratches of implant-abutment joints were detected in all group 2. Reduction ratios of final do-torque values were greater than initial do-torque values in all measured group, except in narrow-neck implant-abutment group (p<0.05). 3. Reduction ratios of final do-torque values in wide-neck implant-abutment group were greater than regular-neck implant-abutment group (p<0.01). 4. The greatest standard deviation value was detected in wide platform group in both implant systems.
Statement of problem: Currently, there are some 20 different geometric variations in implant/abutment interface available. The geometry is important because it is one of the primary determinants of joint strength, joint stability, locational and rotational stability. Purpose: As the effects of the various implant-abutment connections and the prosthesis height variation on stress distribution are not yet examined this study is to focus on the different types of implant-abutment connection and the prosthesis height using three dimensional finite element analysis. Material and method. The models were constructed with ITI, 3i TG, Bicon, Frialit-2 fixtures and solid abutment, TG post, Bicon post, EstheticBase abutment respectively. And the super structures were constructed as mandibular second premolar shapes with 8.5 mm, 11 mm, 13.5 mm of crown height. In each model, 244 N of vertical load and 244 N of $30^{\circ}$ oblique load were placed on the central pit of an occlusal surface. von Mises stresses were recorded and compared in the crowns, abutments, fixtures. Results: 1. Under the oblique loading, von Mises stresses were larger in the crown, abutment, fixture compared to the vertical loading condition. 2. The stresses were increased proportionally to the crown height under oblique loading but showed little differences with three different crown heights under vertical loading. 3. In the crown, the highest stress areas were loading points under vertical loading, and the finish lines under oblique loading. 4. Under the oblique loading, the higher stresses were located in the fixture/abutment interface of the Bicon and Frialit-2 systems compared to the ITI and TG systems. Conclusions: The stress distribution patterns of each implant-abutment system had difference among them and adequate crown height/implant ratio was important to reduce the stresses around the implants.

Purpose: As the esthetic demands of dental implant patients are increased, the demands of zirconia as implant abutment material are also increased. It has non-metalic color, good biocompatibility, high strength and high toughness. Even thought the advatage of zirconia abutment, there are a few studies about mechanical properties of zirconia abutment. This study evaluated the mecanical strength with compressive bending strength and endurance limit of implant-zirconia abutment assembly. Materials and Methods: Static and cyclic loading of implant-Zirconia abutment assembly were simulated under worst case condition according to ISO. Test groups were implants of external butt joint with straight regular diameter and angled regular diameter zirconia abutment, implant of external butt joint with narrow straight diameter zirconia abutment and implant of internal conical joint with straight narrow diameter zirconia abutment. All test group were evaluated the mecanical strength with compressive bending strength and endurance limit. After fatique testing, fracture surface were examined by SEM. Results: The compressive bending strengths exceed 927N. Regular diameter zirconia abutment were stronger than narrow diameter zirconia abutment(P<.05). The endurance limits ranged from 503N to 868N. Conclusion: Within the limitation of this study, zirconia implant abutment exceeded the estabilished values for maximum incisal biting forces reported in the literature.
