  • The causes of liver disease in children and abnormal liver function test are more diverse than those in adult. Therefore, the first step to evaluate the liver disease is the understanding the liver disease and interpreting the appropriate liver function test in children. The characteristics of each test and various conditions which we encounter in outpatients settings will be discussed in this article, especially focused on elevation of aminotransferase level without identifiable causes.

A Study on a Mutual Relation Between the Ultrasonic Diagnosis on the Fatty Infiltration of the Liver and Serum Biochemical Test (간(肝) 지방(脂肪) 침윤(浸潤)에 대한 초음파(超音波) 소견(所見)과 혈액(血液) 생화학적(生化學的) 검사(檢査)의 상관관계(相關關係)에 관한 연구(硏究))

  • In medical selection of Daehan Education Insurance, We doubted that 140 cases(male 117 cases, female 23 cases) might have the fatty infiltration of the liver by ultrasonography. We examined the weight and the hypertension of 140 patients by biochemically analyzing the blood extracted under fasting according to the age-distribution. The results are as follows: 1. 90% out of them had body weight than normal subject. 34.29% out of them were hypertension patients and most of them were $30{\sim}39's$ and $40{\sim}49's$. 2. As compared with the normal subjects in serum lipid value, HDL cholesterol value was low, but total cholesterol, Triglyceride, phospholipid and total lipid value were significantly high. Especially the statistical value of Triglyceride was very significantly high. 3. Total abnormal rate of sGOT value in Liver function test is 34.43% and the mean value of it is 66 unit and the abnormal rate of sGPT value is all 3.14% and the value of it is 70 unit. 4. The abnormal rate of Alkaline phosphatase value is 9.29% and the abnormal rate of total Bilirubine value is 14.29%. As HBsAg positive rate is 2.14% and HBs Ab positive rate is 31.43%, the positive rate of HBsAg represents Lower positive rate than Korean mean value. 5. The abnormal rate of the fasting blood glucose is 15.00% and the abnormal rate of Blood sugar pc 2hrs is 30.71%. 6. The abnormal case of protein, Albumin, Globulin, BUN, craetinine and Hemoglobin value was not found to any of them and in the above liver function test except serum lipid value, the rate not showing the abnormal value represents 45.3%(male) and 52.17%(female).

An Epidemiological Study on the Peasant's syndrome in Rural Korea (한국일부 농촌주민의 건강관리에 대한 역학적 고찰 (농부증을 중심으로))

  • Five hundred seventeen farmers(214 males and 303 females), over age 30 and living in Suh San County, Unsan township, were examined to determine the prevalence of peasants syndrome. Using the health interview questionnaire, the farmers were asked whether they had any of eight subjective symptoms during the past one month before the survey, October 1979. Those symptoms reported were scored based on a pre-determined point system. The farmers with high symptom scores(7 points or more) were further examined by the liver function test. In addition, all the sample(517 cases) were subjected to the examination of urinalysis and blood pressure. The liver function test is also undertaken for those farmers with high symptome scores(7 points or more) and those with abnormal findings in urinalysis and blood pressure. The results are summarized below: 1) 156 out of 517 farmers(30.2%) have had seven points or more of the symptom scores. Among the eight major symptoms, 72% of the farmers complained of lumbago. 2) The prevalence of peasant's syndrome were higher among females and the older age group. The symptom scores of this syndrome were slightly higher among women who have had many children (e. g. 4 or more). 3) The prevalence of peasant's syndrome was also related to the level of one's education-the lower education groups were found to have higher symptom scores of this syndrome. This may be due to the fact that the lower education groups are more exposed to manual labor. 4) The points of the peasants syndrome were higher in hypertensive farmers and those with abnormal findings of urinaly sis. 5) It was reported, in Japan, that those with high scores of the syndrome are more likely to have an abnormal findings in the liver function test. However, in this study it was observed that there was no significant difference between those with high scores of the syndrome (7 points or more) and those with low score of the syndrome(6 points or less) in the prevalence of abnormal liver function. As this study indicates that the point system given for the peasant's syndrome is closely related to the high bleod pressure and abnormal urinalysis(and to some extent to the abnormal liver function), there is a need to study peasant's syndrome more extensively. Such an effort will serve to detect varonic disease conditions among farmer's towards better management of farmer's health, which is vitally important for rural development in Korea.

Effects of Herbal and Western Medicines on Liver Functions (한.양약 복용이 간기능이상에 미치는 영향에 대한 연구)

  • Objectives : The aim of this study was to evaluate effects of herbal medicine and other associated factors for abnormal liver function tests(especially total bilirubin, AST, ALT, and ${\gamma}GT$) levels in outpatients at an oriental medical clinic. Methods : A cross-sectional study based on clinical records was conducted on 1,871 patients at one Oriental medical clinic in Seoul, Korea. 504 patients received a liver function tests for screening and 497 patients ranging from the age of 4 to 74 were chosen for the study. Patients with basal liver disease or DM were excluded during the screening process. Patients were classified into case(abnormal) and control(normal) groups by normal liver function test references. Results and Conclusions : In this study, 33.0% of the patients were females, and 67.0% were males. The mean age was $34.7{\pm}11.9$ years old. The mean total protein value was $6.78{\pm}0.66g/dL$(male $6.79{\pm}0.61$, female $6.76{\pm}0.76$), albumin $3.89{\pm}0.50g/dL$(male $3.94{\pm}0.47$, female $3.81{\pm}0.54$), total bilirubin $0.51{\pm}0.35mg/dL$(male $0.54{\pm}0.39$, female $0.45{\pm}0.23$), AST $23.31{\pm}18.22U/L$(male $26.37{\pm}20.73$, female $17.09{\pm}8.72$), ALT $33.49{\pm}36.36U/L$(male $40.56{\pm}41.77$, female $19.13{\pm}12.64$), LDH $258.07{\pm}74.84mg/dL$(male $263.68{\pm}73.77$, female $246.70{\pm}75.92$), and ${\gamma}GT$ $39.64{\pm}59.16U/L$(male $50.15{\pm}69.43$, female $17.83{\pm}8.36$). The percentage of abnormal total bilirubin levels(>1.0) in these patients was 2.6%, abnormal AST levels(>39m, >29f) 8.5%, ALT levels(>47m, >32f) 18.8%, and ${\gamma}GT$ (>50m, >40f) 19.9%.

Retrospective Observation of Liver Function Test in 147 Patients Admitted to Korean Medicine Hospital (한방병원에 입원한 환자 147명의 간기능 검사에 대한 후향적 관찰)

  • The purpose of this study was to evaluate the changes of liver function test before and after treatment in patients admitted to Korean medicine hospital. We checked liver function test level (aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, total bilirubin, direct bilirubin) of 147 patient who admitted in Korean medicine hospital from July 1, 2015 to June 30, 2018. The subjects were selected those who took herbal medicine continuously during the admission period and who performed liver function test on admission and before discharge. And the subjects were excluded those who had a history of liver and biliary disease at the time of admission or who took hepatoprotectants. Aspartate aminotransferase, alanine aminotransferase and alkaline phosphatase significantly decreased compared with the values of admission(p<0.05), but total bilirubin and direct bilirubin were not significantly changed(p>0.05). On admission 31 patients(21.1%) had abnormal liver function and 6 patients(4.1%) had liver injury while 19 patients(12.9%) had abnormal liver function and 6 patients(4.1%) showed liver injury before discharge. This study suggests that herbal medicine may not injure liver function.

Retrospective Observation of Liver Function Parameters for 101 Patients Using Herbal Drugs for One Month (1개월 이상 한약을 복용한 101명의 간기능 검사에 대한 후향적 관찰)

  • Objective: The purpose of this study was to investigate the level of safety on liver functions when Korean herbal medicine was taken internally. Method: 101 inpatients who took Korean herbal medicine were enrolled and liver function test (aspartic aminotransferase, alanine aminotransferase, alkaline phosphatase, total bilirubin, gamma glutamyltranspeptidase, albumin, lactate dehydrogenase) was performed on admission and 1 month later. Results: In 101 patients, alkaline phosphatase and gamma glutamyltranspeptidase decreased significantly compared with the value taken on admission (p<0.05) but aspartic aminotransferase, alanine aminotransferase, total bilirubin, albumin, and lactate dehydrogenase were not significantly changed (p>0.05). In the patients who took Scutellaria baicalensis (n=34), alkaline phosphatase decreased and albumin increased significantly (p<0.05). Among the patients who took Atractylodes macrocephala (n=29), alkaline phosphatase decreased significantly (p<0.05). In the patients who took Glycyrrhiza uralensis or Paeonia lactiflora, liver function parameters were not significantly changed (p>0.05). On admission 11 patients had abnormal liver function and 2 patients had liver injury while 7 patients had abnormal liver function and 2 patients showed liver injury 1 month later. Conclusions: This study suggests that prescribed Korean herbal medicine does not injure liver function.

Prevalence of HBsAg Positive and Abnormal Hepatic Enzyme among College Students by Health Examination in a Korean Medicine Hospital (한방병원의 건강검진 분석을 통한 대학생들의 B형간염 유병률과 간효소 이상의 유병률 특성)

  • Objectives: Given current vaccinations against the hepatitis B virus and changes in dietary lifestyles, the status of liver disorders, especially in the younger population, is different than it was in the past. This study aimed to discover the prevalence of the HBsAg positive and abnormal hepatic enzyme among college students.Methods: The study analyzed the medical examinations of 2,646 college students that were performed at Semyung University from March to September 2013.Results: A total of 11 out of 2,646 students (0.4%) showed as positive for the hepatitis B antigen, while 23.8% of students had the serum HBs antibody. A total of 212 students (8.0%) showed abnormally elevated serum levels of ALT, with significant differences between men and women. The prevalence of the abnormal serum ALT and its average value significantly increased according to an increase of BMI score, especially in male students.Conclusions: This study showed the importance of obesity as a causative factor in liver disorders, especially in the young generation. This finding will be useful for the development of therapeutics using Korean medicine in the future.

An Association between Liver Markers and Physiological Variables: Comparison between Normal and Fatty Liver Subjects

  • We evaluated whether liver markers such as aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma-glutamyltranspeptidase (GGT), and bilirubin have a relationship with other physiological factors in the normal (n=115) and fatty liver subjects (n=122) and there are differences between the two populations. Body indices were higher in the fatty liver group than in the normal group. Liver markers and blood pressure (BP) were greater in the fatty liver group than in the normal group. AST and ALT levels were positively correlated with body indices in the fatty liver group, but not in the normal group. AST, ALT and GGT levels in the fatty liver group had positive relationship with cardiovascular indices (CI). ALP and bilirubin levels were negatively associated with some of CI. Liver markers were negatively or positively correlated with inflammatory markers, thyroid hormones, or several biochemical markers levels. These findings suggest that abnormal changes in liver markers may be useful tool for diagnosis or prognosis of development of cardiovascular and/or inflammatory diseases as well as metabolic syndrome.

Safety of Prescribed Korean Herbal Medicine on Liver Function : Prospective Multi-center Pilot Study (한약 복용이 간기능에 미치는 영향 : 전향적 다기관 예비연구)

  • The aim of this study was to investigate the safety of short-term use of Korean herbal medicine (KHM; prescribed herbal medicine by doctors of traditional Korean medicine) on liver function. Three hundred eighty four outpatients who took KHM for various conditions were enrolled for multi-center, prospective observational study. Of them 237 patients completed questionnaire and were checked liver function (aspartic aminotransferase;AST, alanine aminotransferase;ALT, alkaline phosphatase;ALP, total bilirubin;t-Bil, direct bilirubin;d-Bil, gamma glutamyltranspeptidase;GGT, protein and albumin) before and after taking KHM (duration; 21.4${\pm}$10.0 days), and data were analysed statistically. Of the 213 patients showing normal liver function test (LFT) at baseline, 209 (98.1%) remained within the normal range at the second test while 3 (1.4%) revealed slight increase of LFT. Only one subject had raised LFT regarding level of liver injury without perceived symptoms. Twenty-four of 237 patients were abnormal at baseline, and 16 at the second testing. Of the patients taking KHM, only 4 changed from normal to abnormal while 12 from abnormal to normal in their LFT. There were no significant increase in LFT level between the first and second test, except in the t-Bil level, however, the change of t-Bil was small and within normal range. The current study showed that the use of KHM did not increase the frequency of abnormal LFTs, at least in the short term.

Risk Factors Analysis of Alcoholic Liver Diseases by Ultrasonography (초음파검사에 의한 알코올성 간질환의 위험요인 분석)

  • This research attempted to find risk factors of alcoholic liver diseases by ultrasonography at the K image medicine clinic center located in Kwangju city, Kyunggi-Do from March to May, 2007. Six risk factors were selected for this study, age, sex, frequency of alcohol drinking, body mass index(BMI), cholesterol and GPT. The data collected from 353 patients of aged between 20 and 69. This study found the relationships between liver diseases and alcohol drinking style by liver ultrasonography. The results of the analyses showed that the male were 2.12 times more likely to have liver diseases than the female. The persons drinking alcohol more than 3 times per week had 2.37 times higher likelihood of showing liver diseases than below 2 times per week or non drinking at all.. The persons with normal body mass index have 0.52 times lower probability of liver diseases than the persons with abnormal BMI. The persons with abnormal cholesterol level have 9.13 times higher probability of liver diseases. The persons with abnormal GPT have 4.66 times higher probability of liver diseases. The results of this study suggested applying ultrasonography in health promotion programs for diagnosis of liver diseases.
