Medical Information Dynamic Access System in Smart Mobile Environments (스마트 모바일 환경에서 의료정보 동적접근 시스템)

  • Recently, the environment of a hospital information system is a trend to combine various SMART technologies. Accordingly, various smart devices, such as a smart phone, Tablet PC is utilized in the medical information system. Also, these environments consist of various applications executing on heterogeneous sensors, devices, systems and networks. In these hospital information system environment, applying a security service by traditional access control method cause a problems. Most of the existing security system uses the access control list structure. It is only permitted access defined by an access control matrix such as client name, service object method name. The major problem with the static approach cannot quickly adapt to changed situations. Hence, we needs to new security mechanisms which provides more flexible and can be easily adapted to various environments with very different security requirements. In addition, for addressing the changing of service medical treatment of the patient, the researching is needed. In this paper, we suggest a dynamic approach to medical information systems in smart mobile environments. We focus on how to access medical information systems according to dynamic access control methods based on the existence of the hospital's information system environments. The physical environments consist of a mobile x-ray imaging devices, dedicated mobile/general smart devices, PACS, EMR server and authorization server. The software environment was developed based on the .Net Framework for synchronization and monitoring services based on mobile X-ray imaging equipment Windows7 OS. And dedicated a smart device application, we implemented a dynamic access services through JSP and Java SDK is based on the Android OS. PACS and mobile X-ray image devices in hospital, medical information between the dedicated smart devices are based on the DICOM medical image standard information. In addition, EMR information is based on H7. In order to providing dynamic access control service, we classify the context of the patients according to conditions of bio-information such as oxygen saturation, heart rate, BP and body temperature etc. It shows event trace diagrams which divided into two parts like general situation, emergency situation. And, we designed the dynamic approach of the medical care information by authentication method. The authentication Information are contained ID/PWD, the roles, position and working hours, emergency certification codes for emergency patients. General situations of dynamic access control method may have access to medical information by the value of the authentication information. In the case of an emergency, was to have access to medical information by an emergency code, without the authentication information. And, we constructed the medical information integration database scheme that is consist medical information, patient, medical staff and medical image information according to medical information standards.y Finally, we show the usefulness of the dynamic access application service based on the smart devices for execution results of the proposed system according to patient contexts such as general and emergency situation. Especially, the proposed systems are providing effective medical information services with smart devices in emergency situation by dynamic access control methods. As results, we expect the proposed systems to be useful for u-hospital information systems and services.

An Application of Single Screen-Film System on General Radiography (일반(一般)X선촬영(線撮影)에 있어서 편면(片面)시스템의 응용(應用))

  • In clinical fields, single screen-film system will be generalized according to high speed and high image qualify of intensifying screen film system in future. In single screen-film system, for imaging the best image, we must choice good film according to speed and gredient. Double screen-film system will be replaced single screen film system in general radiography. Author has thought that single screen film system will be developed new diagnosis area according to X-Ray films.

  • The liver imaging reporting and data system (LI-RADS) has been developed with the support of the American College of Radiology to standardize the diagnosis and evaluation of treatment response of hepatocellular carcinoma (HCC). The CT/MRI LI-RADS version 2018 has been incorporated in the American Association for the Study of Liver Diseases guidance. This review examines the effect of CT/MRI LI-RADS on the standardized reporting of liver imaging, and the evidence in diagnosing HCC and evaluating treatment response after locoregional treatment using CT/MRI LI-RADS. The results are compared with other HCC diagnosis guidelines, and future directions are described.

  • Many system devices for fluoroscopic and general X-ray studies in diagnostic radiographic system have been being changed from analog mode to digital mode. In addition, among diagnostic imaging and radiologic examinations, fluoroscopic studies that requires functional diagnosis is being widely used. The video recording method of fluoroscopic studies has been useful in functional image diagnosis and dynamic image observation, but the utility of its image quality is being reduced because of limitation in setting play segments of the video player, inconvenience of play, difficulties in preserving reproduced images, the change of image quality, etc. In order to complement these shortages, it is necessary to facilitate access to patient diagnosis information such as storing, editing and sharing functional diagnosis images in response to the trend of the digitalization of digital radiographic & fluoroscopic system(DRF). Thus this study designed and implemented a device of storing functional dynamic images real time using a computer rather than existing video recording, aiming at contribution to functional image diagnosis.

Analysis of Patient Exposure dose with Glass Dosimeter (Glass Dosimeter를 이용한 환자피폭선량에 관한 분석)

  • Far reducing medical radiation exposure and managing patient doses, Entrance surface doses(ESDs) were measured at Diagnostic Radiology Department in ASAN medical center, also we determined and compared with the Diagnostic Reference Level(DRL) of some other countries. ESDs were measured far the most common types of X-ray procedures, such as chest PA, lumbar spine AP, lumbar spine lateral, Pelvis AP, Skull PA. ESDs were measured by Glass dosimeter and Unfors Xi meter. Those were applied collimation center of phantom's entrance skin surface. The results of ESDs were compared Glass dosimeter with Unfors Xi meter. Those were measured within 5% statistical difference. It seemed well agreement at two devices. In most cases ESDs measured far the different types of X ray procedures were found to be lower than the DRL of IAEA, but ESDs on chest PA, lumbar spine AP, lumbar spine lateral, Pelvis AP, Skull PA were proximity ar excesses at DRL of advanced country. Through this study, we need an investigation and improvement at present diagnostic radiology exam system. Also, radiologists make an effort to reduce patient dose and having a technical skill.

Fast Dimming Associated with a Coronal Jet Seen in Multi-Wavelength and Stereoscopic Observations

  • We have investigated a coronal jet observed near the limb on 2010 June 27 by the Hinode/X-Ray Telescope (XRT), EUV Imaging Spectrograph (EIS), and Solar Optical Telescope (SOT), and the SDO/Atmospheric Imaging Assembly (AIA), Helioseismic and Magnetic Imager (HMI), and on the disk by STEREO-A/EUVI. From EUV (AIA and EIS) and soft X-ray (XRT) images we have identified both cool and hot jets. There was a small loop eruption in Ca II images of the SOT before the jet eruption. Using high temporal and multi wavelength AIA images, we found that the hot jet preceded its associated cool jet by about 2 minutes. The cool jet showed helical-like structures during the rising period. According to the spectroscopic analysis, the jet's emission changed from blue to red shift with time, implying helical motions in the jet. The STEREO observation, which enabled us to observe the jet projected against the disk, showed that there was a dim loop associated with the jet. We have measured a propagation speed of ~800 km/s for the dimming front. This is comparable to the Alfven speed in the loop computed from a magnetic field extrapolation of the HMI photospheric field measured 5 days earlier and the loop densities obtained from EIS Fe XIV line ratios. We interpret the dimming as indicating the presence of Alfvenic waves initiated by reconnection in the upper chromosphere.

3-D Structure of a Coronal Jet Seen in Hinode, SDO, and STEREO

It is noted that there was a small loop eruption in Ca II images of the SOT before the jet eruption. Using high temporal and multi wavelength AIA images, we found that the hot jet preceded its associated cool jet. The jet also shows helical-like structures during the rising period. According to the spectroscopic analysis, the jet structure changes from blue shift to red one with time, implying the helical structure of the jet. The STEREO observation, which enables us to observe this jet on the disk, shows that there was a dim loop associated with the jet. Comparing the observations from the AIA and STEREO, the dim loop corresponds to the jet structure which implies the heated loop. Considering that the structure of its associated active region seen in STEREO is similar to that in AIA observed 5 days before, we compared the jet morphology on the limb with the magnetic fields extrapolated from a HMI vector magnetogram observed on the disk. Interestingly, the comparison shows that the open field corresponds to the jet which is seen as the dim loop in STEREO. Our observations (XRT, SDO, SOT, and STEREO) are well consistent with the numerical simulation of the emerging flux reconnection model.

  • Ultrasonic has been proposed as an attractive means of detecting bone loss. There have been several commercial ultrasound devices developed for measuring the heel to predict fracture at other bones. However, these devices select only single point of heel bone as measurement site. It causes poor assessment of bone quality due to the error of transducer positioning. In an effort to improve current ultrasound systems, we evaluated the linear scanning method which provides better prediction of bone quality and an accurate image of bone shape. The system used in this study biaxially scans a heel bone using automated linear scanning technique. The results demonstrated that the values of ultrasound parameters varied with different positions within bone specimen. It has been also found that the linear scanning method could better pre야ct bone quality, eliminating the error of transducer positioning.

Image Quality Evaluation of Digital X-ray Image and Film Image using Contrast Method (Contrast Method를 이용한 디지털 X선 영상과 필름 영상의 영상 질(Quality) 평가)

  • In this paper, an image quality evaluation of amorphous selenium based x-ray system was performed using modulation transfer function (MTF) measurement. MTF as a main factor of imaging quality evaluation was investigated by contrast method from acquired test pattern image. The MTF value was measured to 36.1% at 3.0 lp/mm which is twice as high as that in the film image. Compared with other studies, the results demonstrate that amorphous selenium based x-ray detector is clinically meaningful.

  • X-ray is currently most effective method in detecting small malignant breast tumors but has the several problems due to suppressing breast, ionizing radiation and not detecting small cancer. In this paper, a new method is proposed by using dielectric characteristics of pathological tissues and time delay of backscattered response. We have developed a detection algorithm and verified it by numerical simulation and measurement for a prototype system. For a prototype system, we have fabricated experimental model(artificial breast with a cancer) and UWB(ultra-wideband) antenna. The results of the measurement simulation show an excellent detection capability of a cancer tissue. It is found that a good UWB antenna is a key element of such detection system. Further study is ongoing to develop a commercial system.
