Atrial Septal Defect Closure: Comparison of Vertical Axillary Minithoracotomy and Median Sternotomy

  • Background: This study aims to evaluate whether or not the method of right vertical axillary minithoracotomy (RVAM) is preferable to and as reliable as conventional sternotomy surgery, and also assesses its cosmetic results. Methods: Thirty-three patients (7 males, 26 females) with atrial septal defect were admitted to the Cardiovascular Surgery Clinic of Cukurova University from December 2005 until January 2010. The patients' ages ranged from 3 to 22. Patients who underwent vertical axillary minithracotomy were assigned to group I, and those undergoing conventional sternotomy, to group II. Group I and group II were compared with regard to the preoperative, perioperative and postoperative variables. Group I included 12 females and 4 males with an average age of $16.5{\pm}9.7$. Group II comprised 14 female and 3 male patients with an average age of $18.5{\pm}9.8$ showing similar features and pathologies. The cases were in Class I-II according to the New York Heart Association (NYHA) Classification, and patients with other cardiac and systemic problems were not included in the study. The ratio of the systemic blood flow to the pulmonary blood flow (Qp/Qs) was $1.8{\pm}0.2$. The average pulmonary artery pressure was $35{\pm}10$ mmHg. Following the diagnosis, performing elective surgery was planned. Results: No significant difference was detected in the average time of the patients' extraportal circulation, cross-clamp and surgery (p>0.05). In the early postoperative period of the cases, the duration of mechanical ventilator support, the drainage volume in the first 24 hours, and the hospitalization time in the intensive care unit were similar (p>0.05). Postoperative pains were evaluated together with narcotic analgesics taken intravenously or orally. While 7 cases (43.7%) in group I needed postoperative analgesics, 12 cases (70.6%) in group II needed them. No mortality or major morbidity has occurred in the patients. The incision style and sizes in all of the patients undergoing RVAM were preserved as they were at the beginning. Furthermore, the patients of group I were mobilized more quickly than the patients of group II. The patients of group I were quite pleased with the psychological and cosmetic results. No residual defects have been found in the early postoperative period and after the end of the follow-up periods. All of the patients achieved functional capacity per NYHA. No deformation of breast growth has been detected during 18 months of follow-up for the group I patients, who underwent RVAM. Conclusion: To conclude, the repair of atrial septal defect by RVAM, apart from the limited working zone for the surgeon in these pathologies as compared to sternotomymay be considered in terms of the outcomes, and early and late complications. And this has accounted for less need of analgesics and better cosmetic results in recent years.

한국의 낙농시설 개선에 관한 연구 (A Study on the Improvement of Dairy Rousing Systems in Kore)

  • Engineering phase of dairy housing systems has close connection with the milk produc- tivity of dairy cattle, the quality of milk, extension of dairy production systems, labor- saving in management of dairy cattle and the like. Moreover, the rate of investment of dairy housing facilities is of relatively high level, However, there has been almost no research effort for the improvement of engineering aspects of dairy housing systems in Korea. The purpose of this study is to find out general engineering problems and to recomm- end the improvement in dairy housing systems in Korea. Field survey by means of questionaire, direct measurements, taking pictures and sketching was conducted to get necessary information for the study. Kyung-ki Do region was firstly chosen for sampling area since it has included more than half of the number of dairy farms of the whole country. The results obtained are summarized as follows: 1. In overall dairy farm layout, the dwellings of workers were ignored in the light of sanitary environment 2. The layout of stalls in a dairy barn belongs mostly to the type of double-row face-out, which is compatible with the emphasis of manure disposal activities. 3. While the width and length of stalls were sufficiently close to the standard dimension, the width of mangers was much less than the standard dimension. 4. The width and depth of manure gutters and the width of working alleys were much. less than the standard dimension. 5. The mooring equipment was mostly in the classes of chain or rope. The watering equipment was not facilitated independantly except the one cese of using watercup. 6. The bucket milkers with one or two bucket milkers with the capacity of two cattles. each were used as milking equipment in most dairy farms. 7. There were only few milk rooms independently spaced from other space, in which the arrangement of milking equipment was much less than the standard condition. 8. The lounging ground area was averaged to be sufficient for the activity of dairy herd. 9. Silos for silage used during winter consisted of mostly bunker silos, trench silos and underground vertical silos. Ordinary vertical silos were considered for the farmers to be inconvenient for the labor saving. 10. From the view point of heat conservation and moisture removal within the dairy barns, windows were not flexible for the easy ventilation and ceiling part was not adequate for temperatur maintenance. 11. Waste treatment and disposal systems were not provided with most dairy farms, therefore the livestock waste pollution problems would be serious in the near future.

Development of an Automated Measurement System for Dilution Process and Spraying Amount of Disinfectant

  • Purpose: The objectives of this study were to develop an automated disinfectant dilution system, and an automated data management system for spraying amount for resolving uncertainty problem. Methods: Proper diluting rate was made by a controlled volume pump for liquid disinfectant and a screw conveyer pump for solid disinfectant. The water capacity of disinfecting system of 400 L was controlled by two water level sensors. The water quantity of water tank was controlled by the signals which were produced by the water level sensors. Signals were processed by Labview Programming, and ON/OFF of solenoid valve that was used for controlling water supplying to water tank, was controlled by SSR. The operating time of pumps for disinfectant was controlled quantitatively. A turbine flowmeter was used for development of automated measurement system for spraying amount of disinfectant. In order to save the flowmeter data and to control the spraying system, a multi-function data logger was used, and it was processed and saved in Excel file by a program developed in this study. Results: Labview 2010 was used for programming to control the automated measurement system for spraying amount of disinfectant. Results showed that the relationship between flowmeter value and time had a significant linear relationship such as 0.99 of $R^2$. Generally, 6.74 L/s of diluted disinfectant is sprayed for a vehicle passing through the disinfection system (about 15 seconds). Test results showed that average error between the measured spraying amount and the flowmeter data was 50 mL, and the range of error was 1.3%. Since the amount and time of spraying could be saved in real-time by using the spreadsheet files which could not be modified arbitrarily, it made possible to judge objectively whether the disinfection spraying was performed or not. Test results of spraying liquid and solid disinfectant showed that the errors between the measured discharge rate and the theoretical one were ranged within 3-4% for various dilution rates. Conclusions: The disinfection system developed would be working accurately. The automated spraying data base management system satisfied the purpose of this study. The automated dilution process system developed in this study could discharge liquid and solid disinfectant with accurate dilution rate, relatively.

치과위생사 전문직 정체성 측정도구의 타당도와 신뢰도 (Validity and Reliability of Professional Identity of Dental Hygienists)

  • 본 연구의 목적은 치과위생사 전문직 정체성의 정의와 구성 요인을 정립하고 이를 반영한 측정도구를 개발하여 평가한 후 이를 활용하여 국내 치과위생사 전문직 정체성의 수준과 특성을 분석하는 것이다. 2015년 7월 25일부터 2016년 10월 24일까지 3회에 걸쳐 편의추출한 880명의 치과위생사에게 설문조사를 실시하였다. 전문직 정체성의 정의를 정립하기 위해 체계적 문헌고찰을 실시하였고, 구성 요인을 정립하기 위해 내용타당도 분석, 탐색적 요인분석, 확인적 요인분석, 신뢰도 분석을 실시하였으며, 요인별 수준을 파악하기 위해 기술통계, 독립표본 t검정, 일원배치분산분석을 실시하였다. 치과위생사의 전문직 정체성 정의에 따른 구성 요인은 '직업적 소명의식', '학문적 역량강화 욕구', '위임된 권한 발휘도', '윤리강령 준수도', '전문직 단체 활용도' 등 5개로 나타났고, 하나의 구성 요인당 3개의 문항으로 총 15개 문항으로 개발되었으며 타당도와 신뢰도는 양호한 수준이었다. 본 연구 대상자의 전문직 정체성은 5점 기준에 3.72점으로 나타났다. 구성 요인 중 위임된 권한 발휘도가 4.50점으로 가장 높았으며, 전문직 단체 활용도가 2.96점으로 가장 낮았다. 일반적 특성에 따른 전문직 정체성 수준을 평가한 결과, 최종 학력이 높아질수록 전문직 정체성이 높아지는 것으로 나타났고, 근무기관종별에서는 종합/대학병원에 근무하는 치과위생사의 전문직 정체성이 가장 높았다. 치과위생사의 전문직 정체성에 영향을 미치는 선행요인과 전문직 정체성이 영향을 미치는 결과요인에 대한 후속연구가 이뤄진다면 전문직 정체성을 중심으로 한 모형을 확립할 수 있을 것이다. 치과위생사의 역할이 시대적 요구에 의해 변화하고 있는 시점에서 치과위생사의 전문직 정체성은 치과위생사에게 중요한 변수이다. 본 연구를 통해 개발된 측정도구는 향후 치과위생사 전문직 정체성에 대한 후속 연구에 기초자료로 활용할 수 있을 것이다.

노인의료 복지시설 영양사의 업무활동 및 요구도 분석 (Analysis of the Job Activities and Demand of Dietitian in the Elderly Health-care Facilities)

  • This study was objectively performed to identify dietitians' job in the elderly health-care facilities, to assess facilities and dietitian's demographic characteristics, and to determine performance and importance of dietitian's job including the demand of therapeutic diet development. Survey was conducted by mail and samples were the dietitians working in 376 facilities which capacity is over 50 members from nationwide 583 the elderly health-care facilities. Returned questionnaire was 102 and used for statistic analysis. The distributions of the elderly health-care facilities showed 39 the elderly nursing facilities(38.2%), 32 skilled nursing facilities (32.4%), 13 geriatrics hospital facilities(12.7%) and 9 the elderly cost nursing facilities(8.8%). 60.0 percent of the samples showed its menu price as 1,000 to 1,500 won. A cycle-menu program was in-use at the 79.0% facilities, but only 7.1% facilities have been introduced a selected menu system. 92.9% facilities employed only one dietitian. In the demographic characteristics of dietitian only 14.7% dietitian had a clinical dietitian license and 51.5% of respondents answered at least 1 to 3 months internship program is needed. Job activities of the dietitian in the elderly health-care foodservice were identified as 45 activities with 9 dimensions. Job performance score evaluated dietitian oneself was 4.71 of 7 points. The average importance score that the dietitian evaluated their own job was 5.66 points of 7. The job activities shown higher importance but lower performance were therapeutic diet development for in-patients, menu development suitable for taste of the elderly, and leadership. Job performance score by characteristics of dietitian and their elderly health-care facilities was significantly associated with experience of dietitian in elderly health-care (F=4.480, p<0.05), education of dietitian(F=2.659, p<0.01), number of dietitian(F=2.245, p<0.05), and number of employee in foodservice(F=2.607, p<0.05). Most common diseases of the aged was proved as hypertension(81.7%), diabetes mellitus(71.4%), and dementia(65.0%). The therapeutic diets frequently provided were diabetes mellitus diet, dysphagia diet, low sodium diet, high fiber diet, and high protein diet, in order. For those reasons, dietitian in the elderly health care emphasized that the information about therapeutic diet development such as diabetes mellitus diet, dysphagia diet, low sodium diet and hypertension diet must be continuously developed and provided. The result from this study can be applicable to enlarge and enrich job activities of dietitian in elderly health-care foodservice.

사회적기업은 괜찮은 여성일자리인가? (Does Social Enterprise Provide a Decent Work to Women?)

  • 사회서비스는 서구 자본주의 국가를 중심으로 높은 실업과 복지에 대한 국가지출 축소 등의 문제에 대응하고 성장과 분배의 선순환 구조를 이룩하기 위해 일자리창출정책과 연계되어 논의가 시작되었다. 특히 사회서비스는 여성노동력이 비교우위를 기대할 수 있는 분야이고 비교적 괜찮은 일자리라는 점에서 주목받고 있다. 양질의 사회서비스 제공과 지속가능한 일자리 창출을 목표로 하는 사회적기업은 여성친화성이 높아 여성일자리의 새로운 대안으로 기능할 수 있다. 이에 본 연구는 사회적기업이 실제 여성에게 괜찮은 일자리를 제공하고 있는지를 살펴보았다. 사회적기업 491개소 전체를 대상으로 한 조직수준 분석에서는 종단분석을 통해 사회적기업에서의 여성 참여율, 조직내 지위와 안정성, 임금수준 등에 대해 성별 비교분석을 하였다. 사회적기업 종사자 830명을 대상으로 한 개인수준 분석에서는 횡단분석을 통해 사회적기업 진입이전 상태를 비롯하여 일자리 만족도와 일자리 지속희망도에 대해 성별 비교분석을 하였다. 분석 결과, 사회적기업에서 여성의 비중은 70% 수준이며 특히 40대 이상 중장년층과 취약계층 여성의 참여가 높은 편이었다. 여성의 임금수준은 남성의 85~95% 수준으로 일반기업체와 비교할 때 임금격차가 거의 없는 일자리로 나타났다. 그러나 사회적기업에서 여성 일자리는 업종을 불문하고 비정규직과 비관리직에 집중되어 있었다. 이는 여성의 경력단절과 직업경험 부족 등에서 기인한 것으로 보인다. 사회적기업 여성근로자, 특히 취약계층 여성일수록 현재의 일자리를 지속하고자 욕구가 강한 것으로 나타났다. 조직수준 및 개인수준 분석 결과를 종합해 볼 때, 사회적기업은 괜찮은 여성일자리를 제공하고 있는 것으로 볼 수 있다. 그러나 향후 사회적기업 여성근로자의 고용안정성을 높여 여성의 경력이 유지 개발될 수 있도록 하고 여성 개인의 역량 향상이 사회적기업 경쟁력으로 연계되도록 방안을 마련할 필요가 있다.

입원노인과 일반노인의 삶의 질의 비교 (Comparison of the Quality of Life of the Elderly inpatient with that of the normal elderly people)

  • This research was designed to compare the health condition and the quality of life of the elderly inpatients with those of the normal elderly people. The subjects of study were 482 elderly inpatients and 304 normal elderly people. The research was conducted in July and August using the instrument WHOQOL. The results of the research are as follows: For the level of health, it was recognized that male was healthier than female, having higher the education, living in the city rather than in the rural, keeping the normal life than being in the hospital. There were little difference in the past medical histories of the elderly inpatients and the normal elderly people. As to the diseases currently under the treatment, there were some difference between the elderly inpatients and the normal elderly people but the tendency was similar in the kinds and the frequencies. As to the recognition for the quality of life between the elderly inpatients and the normal elderly people based on the records reflected on each of the domains of WHOQOL, the normal elderly people more positively recognized in the overall quality of life and the following domains: physical, psychological, level of independence, and spirituality/religion/personal beliefs. The normal elderly people more positively recognized especially in the level of independence domain (mobility, activities of daily living, dependence on medication or treatments, working capacity). In the social relationships domain only the sexual activity was significant and the normal elderly people more positively recognized. The elderly inpatients showed the correlation of over 0.5 in the overall quality of life and the following domains: environment, social relationships, physical, psychological, level of independence. It was over 0.6 in the physical domain and the domains of psychological and level of independence. and the psychological domains of level of independence and social relationships. For the points of overall quality of life, it had no correlation with hospitalization but the health condition, residential district, occupation, and taste exerted a significant effect. As a result of separate analyses of the elderly inpatients and the normal elderly people, the health condition and the age only were the common variable which would exercise a significant effect. Besides, the primary factors which would exercise the quality of life were the occupation and taste for the elderly inpatients, and the residential district and source of income for the normal elderly people. In conclusion, it is first and foremost important to improve the standard of health for the overall quality of life for the elderly people, regardless of hospitalization. Therefore, a plan must be urgently drawn up for revitalization of the health promoting projects for the elderly people and the public health projects for the elderly people, and the investment must be increased for settlement of health problems of the elderly people.

상세 모델링을 통한 RDX 연소 동특성 분석 (An Analysis of Dynamic Characteristics of RDX Combustion Using Rigorous Modeling)

  • 수명이 도래한 고에너지물질의 처리를 위해 환경오염 및 안전성, 처리용량 등을 고려해야 하며, 현재 가장 주목 받고 있는 처리방식은 소각처리공정이다. 그러나 처리대상 고에너지물질의 종류가 매우 다양하고, 특성 또한 다르기 때문에 범용적 기술개발이 힘든 실정이다. 본 연구는 상세 수학적모델링 및 동적모사를 통하여 가장 널리 사용되는 고에너지물질의 하나인 고폭약(research department explosive, RDX)을 플러그흐름반응기(plug flow reactor, PFR)에서 소각 시 반응기 내부의 물리-화학적 변화를 예측하였다. 본 연구에서 사용된 RDX반응은 263개의 상세한 기초반응식으로 이루어져 있으며 43개의 성분이 반응에 관여한다. 모사결과 반응기 내부온도를 제어하여 RDX의 민감성을 통제할 수 있었다. 반응기 내부온도를 1,200 K로 유지 할 때 RDX는 분해반응만 일어나 폭발과 같은 큰 에너지 방출을 막을 수 있었으나 공급되는 열원이 높아져 1,300 K이상 반응기 온도가 증가 시에는 3,000 K 이상의 온도상승을 수반하는 발화반응이 일어났다. 본 연구를 통하여 반응기의 운전온도변화에 따른 RDX반응 특성을 제시함으로써 효율적인 RDX소각로 공정설계 및 운전에 기초가 될 것으로 사료된다.

격벽식 분말활성탄 접촉조의 흐름해석을 위한 전산유체역학 프로그램의 적용 (Application of CFD Program for Analyzing the Hydrodynamic Characteristics of Baffled PAC Contactor)

  • 본 연구에서는 상수원수 내 조류 이취미(taste and odor)에 대응하기 위한 수단으로 도입되고 있는 격벽식 분말활성탄(PAC) 접촉조의 효과적인 설계를 위해 전산유체역학 프로그램인 FLOW-3D를 도입하였다. 일차적으로 FLOW-3D의 성능을 검증하기 위하여 용량 288 리터의 PCA 접촉조를 대상으로 수행된 추적자실헐 결과와 FLOW-3D의 모의결과를 비교하였다. 또한, 이미 설계된 P정수장 PAC 접촉조에 적용하여 흐름특성을 예측하였다. 다양한 격벽 조건에서 모의된 FLOW-3D 결과는 실제 추적자실험 결과와 아주 유사하였다. 한편, 수리학적 체류시간이 20분인 P정수장의 접촉조에 투입된 PAC 입자가 시간에 따라 어떻게 분포하는지를 FLOW-3D로 모의수행한 바에 의하면, 기존의 PAC 접촉조에서는 모든 격벽의 후면에 정체구역이 발생하였고, 이 정체구역에는 PAC 입자가 거의 존재하지 않았다. 이와 같은 격벽 후면부의 정체영역은 PAC 입자가 체류하는 시간을 감소시켜 흡착효율을 저하시킬 수 있다. 반면에, PAC 접촉조의 흐름특성을 개선하고자 격벽의 형상을 미로형(maze-type)으로 변경한 경우, PAC 입자가 접촉조 내에 상대적으로 균일하게 분포하였으며 정체영역도 크게 감소하였다. 결론적으로 FLOW-3D 모의는 정수장에서 사용되는 수리구조물의 흐름특성 해석에 유용한 수단으로 사용될 것으로 기대된다.

DEA/AHP 결합모형을 이용한 건축 설계사무소의 교육효율성 분석 (Education Efficiency Analysis of Architectural Design Firms Using a Combined AHP and DEA Model)

  • 현대사회는 산업경제사회에서 지식기반사회로 급격히 변화되어 현대인에게 요구하는 지식과 기술의 변화를 따라가기 위하여 지속적인 학습 또는 교육을 받지 않으면 현대사회를 살아갈 수 없게 되었다. 건축 설계사무소의 경우 내부역량 향상에 초점을 맞춰 자체적인 교육을 실시하여 실무자의 능력을 배양함으로서 기업의 생산성을 향상시키는데 주력하고 있다. 이와 관련하여 최근 다양한 산업분야에서 재무비율 지표들을 활용한 자료포락분석(Data Envelopment Analysis, DEA)을 실시하여 기업의 효율성을 분석하고 있으며, 건설산업에서 또한 DEA를 활용한 효율성 분석 연구가 증가하고 있다. 그러나 건설산업의 경우 경영 효율성에만 초점을 맞춰 연구가 진행되었을 뿐 각 기업의 교육효율성 분석과 같은 특정 분야에 대한 접근이 매우 부족한 실정이다. 이에 본 연구에서는 교육효율성과 관련된 선행연구와 전문가 인터뷰를 통한 변수 도출 및 계층적 분석방법(Analytic Hierarchy Process, AHP)을 활용하여 투입 및 산출 변수를 선정한 후 DEA 모형을 활용하여 건축 설계사무소의 교육효율성을 분석하였다.
