Determinant factors of Exercise behaviors in Patients with Arthritis (관절염 환자의 운동행위 결정요인)

  • The aims of this study were to understand and to predict the determinant factors affecting the exercise behaviors and physical fitness by testing the Ponder's health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continuous exercise program, and to help them maximize the physical effect such as muscle strength. endurance, and fuctional status and mental effects including self efficacy and quality of life, and improve the physical and mental wellbeing, and to provide a basis for the nursing intervention strategies. We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul between October 5, 1999 and October 24, 1999. Data were composed of self reported questionnaire and good of fitness score which were obtained by pedalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. 24 Of 54 hypothetical paths were supported in modified model, which was considered as a proper model with improved fit index. The physical fitness was directly influenced by exercise participation behavior and education level, and indirectly by physical fitness, while fatigue, physical disability, pastexercise behavior, life-style, self-efficacy, which explained 20% of physical fitness. The exercise participation were directly influenced by perceived benefits and self-efficacy, and indirectly influenced by life-style, fatigue and physical disability, and directly and indirectly by past exercise behavior, which explained 53% of exercise participation. Exercise score were directly affected by perceived health status, perceived benefits, self efficacy, and past exercise behavior, and were indirectly affected by fatigue, physical disability, and life-style, which explained 50%. Perceived health status were directly influeced by level of education, depression, sleep disorder, and physical disability, which explained 34% of perceived health status. Perceived benefit was directly influenced by fatigue, sleep disorder, physical disability, and life-style, which explained 45%. Perceived barriers was directly influenced by fatigue, sleep disorder, and lifestyle, which explained 9%. Self- efficacy was directly influenced by fatigue, physical disability, past exercise behavior, and level of education, which explained 61%. In conclusion, important variables for physical fitness were exercise participation and level of education, and variables affecting exercise participation were perceived self-efficacy, benefits, and past exercise behavior. Perceived self-efficacy of exercise was a significant predictor of exercise participation. Life-style, fatigue, and physical disability showed direct effects on perceived benefit, perceived barriers, and self-efficacy, and indirect effects on exercise behavior. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be soaked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved.

  • ZITO/Ag/ZITO multilayer transparent electrodes at room temperature on glass substrates were prepared using RF/DC magnetron sputtering. Transparent conductive films with a sheet resistance of 9.4 Ω/㎡ and a transmittance of 83.2% at 550 nm were obtained for the multilayer structure comprising ZITO/Ag/ZITO (100/8/42 nm). The sheet resistance and transmittance of ZITO/Ag/ZITO multilayer films meant that they would be highly applicable for use in polymer-dispersed liquid crystal (PDLC)-based smart windows due to the ability to effectively block infrared rays (heat rays) and thereby act as an energy-saving smart glass. Effects of the thickness of the PDLC layer and the intensity of ultraviolet light (UV) on electro-optical properties, photopolymerization kinetics, and morphologies of difunctional urethane acrylate-based PDLC systems were investigated using new transparent conducting electrodes. A PDLC cell photo-cured using UV at an intensity of 2.0 mW/c㎡ with a 15 ㎛-thick PDLC layer showed outstanding off-state opacity, good on-state transmittance, and favorable driving voltage. Also, the PDLC-based smart window optimized in this study formed liquid crystal droplets with a favorable microstructure, having an average size range of 2~5 ㎛ for scattering light efficiently, which could contribute to its superior final performance.

P Wave Detection Algorithm through Adaptive Threshold and QRS Peak Variability (적응형 문턱치와 QRS피크 변화에 따른 P파 검출 알고리즘)

  • P wave is cardiac parameters that represent the electrical and physiological characteristics, it is very important to diagnose atrial arrhythmia. However, It is very difficult to detect because of the small size compared to R wave and the various morphology. Several methods for detecting P wave has been proposed, such as frequency analysis and non-linear approach. However, in the case of conduction abnormality such as AV block or atrial arrhythmia, detection accuracy is at the lower level. We propose P wave detection algorithm through adaptive threshold and QRS peak variability. For this purpose, we detected Q, R, S wave from noise-free ECG signal through the preprocessing method. And then we classified three pattern of P wave by peak variability and detected adaptive window and threshold. The performance of P wave detection is evaluated by using 48 record of MIT-BIH arrhythmia database. The achieved scores indicate the average detection rate of 92.60%.

  • Purpose : The aim of this study is to develope the Magnetic Resonance Spectroscopy(MRS) data processing S/W which plays an important role as a diagnostic tool in clinical field. Materials and methods : Post-processing software of MRS based on graphical user interface(GUI) under windows operating system of personal computer(PC) was developed using MATLAB(Mathwork, U.S.A.). This tool contains many functions to increase the quality of spectrum data such as DC correction, zero filling, line broadening, Gauss-Lorentzian filtering, phase correction, etc. And we obtained the normal human brain $^1H$ MRS data from parietal white matter, basal ganglia and occipital grey matter region using 1.5T Gyroscan ACS-NT R6 (philips, Amsterdam, Netherland) MRS package. The analysis of the MRS peaks were performed by obtaining the ratio of peak area. Results : The peak ratios of NAA/Cr, Cho/Cr, MI/Cr for the different MRS machines have a little different values. But these peak ratios were not significantly different between different echo time MRS peak ratios in the same machine (p<0.05). Conclusion : MRS post-processing S/W based on GUI using PC was developed and applied to the analysis of normal human brain $^1H$ MRS. This independent MRS processing job increases the performance and throughput of patient scan of main console. Finally, we suggest that the database for normal in-yivo human MRS data should be obtained before clinical applications.

An Efficient P2P Based Proxy Patching Scheme for Large Scale VOD Systems (대규모 VOD 시스템을 위한 효율적인 P2P 기반의 프록시 패칭 기법)

  • The main bottleneck for large scale VOD systems is bandwidth of storage or network I/O due to the large number of client requests simultaneously, and then efficient techniques are required to solve the bottleneck problem of the VOD system. Patching is one of the most efficient techniques to overcome the bottleneck of the VOD system through the use of multicast scheme. In this paper, we propose a new patching scheme, called P2P proxy patching, for improving the typical patching technique by jointly using the prefix caching and P2P proxy. In our proposed scheme, each client plays a role in a proxy to multicast a regular stream to other clients that request the same video stream. Due to the use of the P2P proxy and the prefix caching, the client requests that ive out of the patching window range can receive the regular stream from other clients in the previous patching group without allocating the new regular channels from the VOD server to the clients. In the performance study, we show that our patching scheme can reduce the server bandwidth requirement about $33\%$ less than that of the existing patching technique with respect to prefix size and request interval.

An Efficient Cache Maintenance Scheme for Long Disconnected Mobile Clients (장기간 접속 단절된 이동 클라이언트를 위한 효과적 캐시 유지 기법)

  • In the mobile computing environment, caching data at the client's side is a useful technique for improving the performance. For caching to be effective, the cache contents must be consistent with those stored in the server. That is, unfortunately, difficult to enforce due to the frequent disconnection and mobility of clients. In the literature, the basic approach adopted is for the sever to periodically broadcast invalidation reports(IRs) that contain information about objects that have been updated recently. However IR-based cache maintenance scheme has a main drawback. Any client who has been disconnected longer than w seconds has to discard all cached items even some of them may still be valid. In this paper, we propose two main schemes which can adapt clients' losing IR messages by long disconnection. First, the client sends its disconnection time when connection is re-established, and the server uses this information to decide the size of broadcast window w dynamically. Second, the server maintains IR messages for a given period of time and a client who has been disconnected longer than w seconds can verify its cache consistency individually. Compared to previous IR-based schemes, our schemes can significantly reduce cache missing and uplink request and improve the throughput by reducing response time.

Study on Application of Ultrasonic Propagation Imager for Non-destructive Evaluation of Composite Lattice Structure (복합재 격자 구조 비파괴평가를 위한 초음파전파 영상화 시스템 활용 연구)

  • Composite lattice structures are tried to be used in various fields because of its benefit in physical properties. With increase of demand of the composite lattice structure, nondestructive testing technology is also required to certificate the quality of the manufactured structures. Recently, research on the development of the composite lattice structure in Republic of Korea was started and accordingly, fast and accurate non-destructive evaluation technology was needed to finalize the manufacturing process. This paper studied non-destructive testing methods for composite lattice structure using laser ultrasonic propagation imaging systems. Pulse-echo ultrasonic propagation imaging system was able to inspect a rib structure wrapped with a skin structure. To reduce the time of inspection, a band divider, which can get signal in different frequency bands at once, was developed. Its performance was proved in an aluminum sandwich panel. In addition, to increase a quality of results, curvature compensating algorithm was developed. On the other hand, guided wave ultrasonic propagation imaging system was applied to inspect delamination in a rib structure. To increase an area of inspection, multi-source ultrasonic wave propagation image was applied, and defects were successfully highlighted with variable time window amplitude mapping algorithm. These imply that ultrasonic propagation imaging systems provides fast and accurate non-destructive testing results for composite lattice structure in a stage of the manufacturing process.

Development of a Voice-activated Map Information Retrieval System based on MFC (MFC 기반 음성구동 수치지도정보 검색시스템의 구현)

  • In retrieving and analyzing digital map information using mouse or key strokes, it needs several times of repeated mouse operation for designating the range of study area. In this study, we proposed a voice activated map information retrieval system for eliminating such repetitions and we realized the system on the personal computer. The system was constructed in two ways - traditional OLE(object linking embedding) method and MFC(Microsoft fundamental class) method in controlling of window display for practical use. In the system performance evaluation, the retrieval data for digital map were consisted of 68 words uttered by 3 male persons which include attribute words and control words for Susung-gu area of Taegu city in a 1:5,000 map. As the results, we obtained the average 98.02% of recognition rate through on-line tests in the office environment and the operating speed of 5.39 seconds by OLE, 10.38 seconds by MFC. These results showed the possibility for practical use of information retrieval system using speech recognition in digital map.

The Effect of Hand and Finger Exercise on Grip Strength and Pinch Pressure in Elderly Women (손과 손가락 근관절운동이 노년기 여성의 악력과 잡기력에 미치는 영향)

  • Introduction : Exercise has been suggested as an important nursing strategy in which to help elderly maintain functional performance and to enhanced quality of life. Most of exercise study has been reported on fitness exercise such as walking, swimming, dance etc for health of elderly. There have been few reports about exercise on the promotion of small and fine movement of elderly. The purpose of this pilot study was to determine an effect of 6 weeks hand and finger exercise in home to improve hand muscle strength such as grip strength and finger pinch pressure. Materials and Methods Design: This pilot study was used one group pre and post-test design. Sample: Twelve elderly women above 60 years of age or older living in community were selected by convenient sampling. Procedure: Signed informed consent was obtained prior to participate in this study. The authors met elderly and taught hand and finger exercise, daily a week for 6 weeks, within 30 minutes per session. But exercise frequency and strength were not same. Instruments: Left and right grip strength were measured by Bulb Dynamometer(made in USA) and left and right pinch pressure were measured by Baseline Hydraulic Pinch Gauge(made in USA). Data analysis: Discriptive data analyses were performed on all variables. Wilcoxon matched-pairs Signed-Ranks test were used to find difference of grip strength and pinch pressure between pre and post exercise using SPSS 10.0 for Window. Results: Samples age ranged from 60 to 73, Mean age was 65.3. All were women. Ten elderly were diagnosed osteoarthritis and one had DM. After six weeks hand and finger exercise, Left and right grip strength were higher than those of before exercise(Z=-2.667, P<0.01 ; Z=-3.065, p<0.01). And left pinch pressure after hand and linger exercise were higher than that of before pinch pressure (Z=-2.315, p<0.05). But Right pinch pressure was not shown significant change(Z=-1.099, p>0.05). Conclusions: Although this study was limited by the sample size and design, the findings provide some important implications for community based exercise nursing intervention. Short term (six weeks) exercise of hand and linger is shown to be useful as nursing intervention to maintain routine daily activities such as eating, writing, grip something for elderly.

DEM_Comp Software for Effective Compression of Large DEM Data Sets (대용량 DEM 데이터의 효율적 압축을 위한 DEM_Comp 소프트웨어 개발)

  • This paper discusses a new software package, DEM_Comp, developed for effectively compressing large digital elevation model (DEM) data sets based on Lempel-Ziv-Welch (LZW) compression and Huffman coding. DEM_Comp was developed using the $C^{++}$ language running on a Windows-series operating system. DEM_Comp was also tested on various test sites with different territorial attributes, and the results were evaluated. Recently, a high-resolution version of the DEM has been obtained using new equipment and the related technologies of LiDAR (LIght Detection And Radar) and SAR (Synthetic Aperture Radar). DEM compression is useful because it helps reduce the disk space or transmission bandwidth. Generally, data compression is divided into two processes: i) analyzing the relationships in the data and ii) deciding on the compression and storage methods. DEM_Comp was developed using a three-step compression algorithm applying a DEM with a regular grid, Lempel-Ziv compression, and Huffman coding. When pre-processing alone was used on high- and low-relief terrain, the efficiency was approximately 83%, but after completing all three steps of the algorithm, this increased to 97%. Compared with general commercial compression software, these results show approximately 14% better performance. DEM_Comp as developed in this research features a more efficient way of distributing, storing, and managing large high-resolution DEMs.
