The effects of digital image processing for noise reduction on observer performance (노이즈 감소 필터 사용이 판독능에 미치는 효과)

  • Purpose : This study was performed to examine the effects of image filter on observer performance by counting the number of holes at each wedge step on a radiographic image. Materials and Methods : An aluminum step wedge with 11 steps ranged in thickness from 1.5 mm to 16.5 mm in 1.5 mm increments was fabricated for this study. Each step had 10 notched holes with 1.0 mm diameter on the bottom of the step wedge which were ranged in depths from 0.1 mm to 1.0 mm in 0.1 mm increments. Digital radiographic raw images of the aluminum step wedge were acquired by using CCD intraoral sensor. The images were processed using several types of noise reduction filters and kernel sizes. Three observers counted the number of holes which could be discriminated on each step. The data were analyzed by ANOVA. Results : The number of holes at each step was decreased as the thickness of step was increased. The number of holes at each step on the raw images was significantly higher than that on the processed images. The number of holes was different according to the types and kernel sizes of the image filters. Conclusions : The types and kernel sizes of image filters on observer performance were important, therefore, they should be standardized for commercial digital imaging systems.

  • In this paper, We developed a radiation therapy planning system using photon beams. Firstly We accomplished simulation on the regular field. Then it has been modified step by step to handle the irregular field, wedge filter, and inhomogeneous field. This system is equipped with useful user interface and database. This system is implemented in Visual C++ as a window-based application program.

Thickening and Dewatering of Municipal Wastewater Sludge : Separate and Combined Treatment of Primary and Secondary Sludge (도시하수슬러지의 농축과 탈수 : 1차와 2차슬러지의 분리 및 혼합처리특성비교)

  • Comparative thickening and dewatering characteristics of municipal wastewater sludge were investigated in terms of separated and combined treatment of primary and secondary BNR sludge. Also, various conditioning methods such as cation polymer addition, steam and ultrasonication treatment were examined to improve dewaterability of sludge. The dewaterability was measured by using specific resistant test, wedge zone simulator and centrifuge. The result of the sludge thickening test revealed that separated thickening was better in terms of solids recovery and supernatant quality. Particularly, the thickening of primary sludge with high solids (about 3.5% TS) showed very poor solid separation. The addition of cation polymer showed better conditioning characteristic for dewatering and the optimal polymer dosage was 0.26% for primary sludge, 0.43% for secondary sludge and 0.38% for combined sludge. Contrary to the result of the thickening, the dewatering test revealed that dewatering of the combined sludge is better than that of separated sludge, representing better solids separation and filtrate quality. The polymer addition was essential to improve dewaterability in filter (belt) press type dewatering but it was inefficient for the dewatering of secondary sludge only. The centrifuge type dewatering showed better performance and the dewaterability was slightly improved when the polymer was added. Based on the results of this research a sustainable sludge treatment process, particularly in terms of the recycle water quality and solids recovery, was proposed.

Homogeneous Dose Planning to Paranasal Sinus with the Partial Attenuation filters and Wedged Beams in 6 MV Photon Beam (6 MV 광자선의 투과성필터와 Wedge 선속을 이용한 부비강의 균등선량계획)

  • The homogeneous dose planning is one of the most important roles in radiation therapy. But, it is not easy to obtain a homogeneous dose to paranasal sinus region including the ethmoidal sinus with conventional irradiation techniques. In this experimental study, the authors tried to get a homogeneous dose at PNS region, but the nasal cartirage does not exceed the tolerance dose, with anterior-posterior beam and two both lateral wedged beams. Used three fields were shielded with full thickness of blocks to preserve the eye-balls and with blocks of one half value layer to create a homogeneous dose at the whole treatment volume. The dose computations are based on the three dimensonal structure with modified scatter contributions of partial shielders and attenuated beams in 6 MV photon beams. The dose distributions of mid-plane is examined with Kodak verification films and teflon-embedded TLD rod (1 mm diameter and 6 mm length) to confirm the computed dose. In our study, the whole PNS regions have shown within $85{\%}$ of the resultant isodose curves with relatively homogeneous dose distribution. The results of dose computation and measurements are agree well within $5{\%}$ uncertainties.

A Study of a Non-commercial 3D Planning System, Plunc for Clinical Applicability (비 상업용 3차원 치료계획시스템인 Plunc의 임상적용 가능성에 대한 연구)

  • Purpose : The objective of this study is to introduce our installation of a non-commercial 3D Planning system, Plunc and confirm it's clinical applicability in various treatment situations. Materials and Methods : We obtained source codes of Plunc, offered by University of North Carolina and installed them on a Pentium Pro 200MHz (128MB RAM, Millenium VGA) with Linux operating system. To examine accuracy of dose distributions calculated by Plunc, we input beam data of 6MV Photon of our linear accelerator(Siemens MXE 6740) including tissue-maximum ratio, scatter-maximum ratio, attenuation coefficients and shapes of wedge filters. After then, we compared values of dose distributions(Percent depth dose; PDD, dose profiles with and without wedge filters, oblique incident beam, and dose distributions under air-gap) calculated by Plunc with measured values. Results : Plunc operated in almost real time except spending about 10 seconds in full volume dose distribution and dose-volume histogram(DVH) on the PC described above. As compared with measurements for irradiations of 90-cm 550 and 10-cm depth isocenter, the PDD curves calculated by Plunc did not exceed $1\%$ of inaccuracies except buildup region. For dose profiles with and without wedge filter, the calculated ones are accurate within $2\%$ except low-dose region outside irradiations where Plunc showed $5\%$ of dose reduction. For the oblique incident beam, it showed a good agreement except low dose region below $30\%$ of isocenter dose. In the case of dose distribution under air-gap, there was $5\%$ errors of the central-axis dose. Conclusion : By comparing photon dose calculations using the Plunc with measurements, we confirmed that Plunc showed acceptable accuracies about $2-5\%$ in typical treatment situations which was comparable to commercial planning systems using correction-based a1gorithms. Plunc does not have a function for electron beam planning up to the present. However, it is possible to implement electron dose calculation modules or more accurate photon dose calculation into the Plunc system. Plunc is shown to be useful to clear many limitations of 2D planning systems in clinics where a commercial 3D planning system is not available.

  • 지금까지 해외의 여러 달 궤도선에서 달의 영상을 다양한 방법으로 관측한 것에서 알 수 있듯이 영상 관측은 달 탐사에서 중요한 부분이다. 특히 그 중에서 입체 영사기(Stereo Imager)는 달의 3차원적 영상을 관측하여 달 표면의 구조를 파악할 수 있고 분광 카메라(Hyperspectral Camera)는 달 표면을 이루고 있는 물질을 분광 정보를 통해서 알아낼 수 있다. 본 연구에서는 우리나라가 2020년경 독자적인 달 궤도선을 계획하고 있는 것에 발맞추어 이 두 관측 장비를 설계해 보았다. 본 연구에서 설계한 탑재체는 하나의 광학계를 이용해 가시광 영역에서 동시에 입체 영상과 분광 영상을 얻을 수 있는 장치이며, 달 표면에서의 궤도선의 속도와 저장 가능한 정보의 양을 고려하여 100 km의 고도에서 속도를 1.6 km/s로 가정할 때 interline CCD가 17.5 m의 공간 분해능을 갖기 위해 92 frame/s 이상의 frame rate로 관측을 수행할 수 있게 하였다. 특히 분광 카메라는 wedge filter를 사용하여 광학계의 부담을 줄였으며 검출기로는 interline CCD를 사용하여 channel 수를 조절할 수 있도록 하였다. 또한 달 표면을 구성하는 입자의 크기에 대한 정보를 얻을 수 있는 편광판도 설치 하였다. 시험 모델의 문제점을 분석하여 새롭게 개선된 탑재체를 설계하여 개발하였다. 렌즈를 수정해 vignetting과 왜곡 현상을 보정하였고 전체 무게를 1.5 kg으로 줄여서 시험 모델보다 30% 이상 줄일 수 있었다. 파장 분해능은 20nm로 시험모델보다 더 개선된 분해능을 얻을 수 있었다. 출력 효율의 증대를 위해 power board의 수정을 수행하였다.

A comparative study of dose distribution for whole brain with field-in-field technique (전뇌(Whole Brain)치료 시 Field-in-Field Technique 적용에 관한 고찰)

  • Purpose : Uniform dose distribution of the target volume is very important in the radiation treatment. We will evaluate the usefulness of Field-in-Field Technique use to get uniform dose distribution of the target volume and try to find Apply possibility out to a whole brain treatment patient of various thickness. Material and method : We compare the dose distribution when we applied Field-in-Field Technique and parallel opposed fields technique. establish the treatment plan to a phantom(acryl 16cm spheral phantom) and do the measurement, assessment use the TLD and Low sensitivity film. Also the assessment did Apply possibility of Field-in-Field Technique to 20 patient object of various thickness. Result : In the case to use the parallel opposed fields at the whole brain treatment $10-12\%$ high dose region appeared but reduce to $3-4\%$ lesses when we used the Field-in-Field technique. We could get similar numerical value the film and TLD measurement result also. The change of the dose distribution appeared to its ${\pm}1{\sim}2\%$ although it applied such Field-in-Field technique to various patient so that we were identical. Conclusion : We can get uniform dose distribution of in the treatment region if we apply the Field-in-Field technique at the whole brain treatment. Also alternate can play the role of the wedge filter and 3D compensator and We are thought by minimizing the obstacle to be happened due to the high dose region when radiation treatment.

  • Purpose: The purpose of this study is to evaluate the ovarian dose during radiation therapy for breast cancer in women of childbearing age through an experiment. The ovarian dose is evaluated by comparing and analyzing between the calculated dose in the treatment planning system according to the treatment technique and the measured dose using a thermoluminescence dosimeter (TLD). The clinical usefulness of lead (Pb) apron is investigated through dose analysis according to whether or not it is used. Materials and Methods: Rando humanoid phantom was used for measurement, and wedge filter radiation therapy, 3D conformal radiation therapy, and intensity modulated radiation therapy were used as treatment techniques. A treatment plan was established so that 95% of the prescribed dose could be delivered to the right breast of the Rando humanoid phantom 3D image obtained using the CT simulator. TLD was inserted into the surface and depth of the virtual ovary of the Rando hunmanoid phantom and irradiated with radiation. The measurement location was the center of treatment and the point moved 2 cm to the opposite breast from the center of the Rando hunmanoid phantom, 5cm, 10cm, 12.5cm, 15cm, 17.5cm, 20cm from the boundary of the right breast to the center of treatment and downward, and the surface and depth of the right ovary. Measurements were made at a total of 9 central points. In the dose comparison of treatment planning systems, two wedge filter treatment techniques, three-dimensional conformal radiotherapy, and intensity-modulated radiation therapy were established and compared. Treatments were compared, and dose measurements according to the use of lead apron were compared and analyzed in intensity-modulated radiation therapy. The measured value was calculated by averaging three TLD values for each point and converting using the TLD calibration value, which was calculated as the point dose mean value. In order to compare the treatment plan value with the actual measured value, the absolute dose value was measured and compared at each point (%Diff). Results: At Point A, the center of treatment, a maximum of 201.7cGy was obtained in the treatment planning system, and a maximum of 200.6cGy was obtained in the TLD. In all treatment planning systems, 0cGy was calculated from Point G, which is a point 17.5cm downward from the breast interface. As a result of TLD, a maximum of 2.6cGy was obtained at Point G, and a maximum of 0.9cGy was obtained at Point J, which is the ovarian dose, and the absolute dose was 0.3%~1.3%. The difference in dose according to the use of lead aprons was from a maximum of 2.1cGy to a minimum of 0.1cGy, and the %Diff value was 0.1%~1.1%. Conclusion: In the treatment planning system, the difference in dose according to the three treatment plans did not show a significant difference from 0.85% to 2.45%. In the ovary, the difference between the Rando humanoid phantom's treatment planning system and the actual measured dose was within 0.9%, and the actual measured dose was slightly higher. This did not accurately reflect the effect of scattered radiation in the treatment planning system, and it is thought that the dose of scattered radiation and the dose taken by CBCT with TLD inserted were reflected in the actual measurement. In dosimetry according to the with or without a lead apron, when a lead apron was used, the closer the distance from the treatment range, the more effective the shielding was. Although it is not clinically appropriate for pregnancy or artificial insemination during radiotherapy, the dose irradiated to the ovaries during treatment is not expected to significantly affect the reproductive function of women of childbearing age after radiotherapy. However, since women of childbearing age have constant anxiety, it is thought that psychological stability can be promoted by presenting the data from this study.

  • Purpose : The measurement of skin dose is very important that treatment of breast cancer. On account of the cold or hot dose as compared with prescription dose, it is necessary to analyse the skin dose occurring during the various plan of the breast cancer treatment. At our hospital, we want to apply various analyses using a diversity of dosimeters to the breast cancer treatment. Subjectss and Methods : In the study, the anthropomorphic phantom is used to find out the dose difference of the skin(draining site), scar and others occurring from the tangential treatment plan of breast cancer. We took computed tomography scan of the anthropomorphic phantom and made plans for the treatment planing using open and wedge, Field-in-Field, Dose fluence. Using these, we made a comparative analysis of the dose date points by using the Eclipse. For the dose comparison, we place the anthropomorphic phantom in the treatment room and compared the measurement results by using the TLD and MOSFET on the dose data points. Results : On the central point of treatment planing basis, the upward and downward skin dose measured by the MOSFET was the highest when the fluence was used. The skin dose of inner and outer was distinguished from the figure(5.7% ~ 10.3%) when the measurements were fulfilled by using TLD and MOSFET. The other side of breast dose was the lowest in the open beam, on the other hand, is highest in the Dose fluence plan. In the different kinds of treatment, the dose deviation of inner and outer was the highest, and so this was the same with the TLD and MOSFET measurement case. The outer deviation was highest in the TLD, and the Inner'was highest in the MOSFET. Conclusion : Skin dose in relation to the treatment plan was the highest in the planing using the fluence technique in general and it was supposed that the high dose had been caused by the movement of the MLC. There's some differences among the all the treatment planning, but the sites such as IM node occurring the lack of dose, scar, drain site are needed pay close attention. Using the treatment planning of dose fluence is good to compensate the lack of dose, but It increases the dose of the selective range rather than the overall dose. Therefore, choosing the radiotherapy technique is desirable in the lights of the age and performance of the patient.

A Comparison of Field-in-Field Intensity Modulated Radiation Therapy Planning and Conventional Radiation Therapy Planning with Tangential Beam for Breast Cancer (유방암의 접선조사 시 Field-in-Field Intensity Modulated Radiation Therapy와 Conventional Radiation Therapy의 전산화 치료계획에 관한 고찰)

  • Purpose: To analyze differences in the dose uniformity for the computed breast radiation therapy planning with tangential beam between conventional RT using wedge filter and FiF-IMRT using multileaf collimator based onsizes and volumes of breasts. Materials and Methods: Thirty breast cancer patients were classified according to the sizes and volumes of the breasts using Eclipse treatment planning system ($Varian^{TM}$, USA, V8.0). Conformity Index and Homogeneity Index were computed along with Dose Volume Histogram. Results: No differencein CI (${\pm}1.2%$) was observed. However, lower mean HI (1.67%) in FiF-IMRT was observed compared to that of the conventional RT. Statically significant (P<0.01) correlation was identified between the values of ${\Delta}HI$ (%) and physical parameters such as breast volumes and separations. Conclusion: Increase in breast volume and separation improves the dose uniformities in computed radiation therapy planning for FiF-IMRT. Physical dimension of the breast should be considered to optimize the compured radiation therapy planning.
