Objectives: Chronic diseases, including back pain, result in significant patient morbidity and societal burden. Overall improvement in physical fitness is recommended for prevention and treatment. Walking is a convenient modality for achieving initial gains. Our objective was to determine whether neighbourhood walkability, acting as a surrogate measure of physical fitness, was associated with the presence of chronic disease. Methods: We conducted a cross-sectional study of prospectively collected data from a prior randomized cohort study of 227 patients referred for tertiary assessment of chronic back pain in Ottawa, ON, Canada. The Charlson Comorbidity Index (CCI) was calculated from patient-completed questionnaires and medical record review. Using patients' postal codes, neighbourhood walkability was determined using the Walk Score, which awards points based on the distance to the closest amenities, yielding a score from 0 to 100 (0-50: car-dependent; 50-100: walkable). Results: Based on the Walk Score, 134 patients lived in car-dependent neighborhoods and 93 lived in walkable neighborhoods. A multivariate logistic regression model, adjusted for age, gender, rural postal code, body mass index, smoking, median household income, percent employment, pain, and disability, demonstrated an adjusted odds ratio of 2.75 (95% confidence interval, 1.16 to 6.53) times higher prevalence for having a chronic disease for patients living in a car-dependent neighborhood. There was also a significant dose-related association (p=0.01; Mantel-Haenszel chi-square=6.4) between living in car-dependent neighbourhoods and more severe CCI scores. Conclusions: Our findings suggest that advocating for improved neighbourhood planning to permit greater walkability may help offset the burden of chronic disease.
This study aimed to investigate factors affecting frailty by urinary incontinence groups among the vulnerable elderly woman in Korea. In this secondary analysis, data were collected from records for 3,251 elders registered in the Visiting Health Management program of Public Health Centers in 2012. body mass index, waist circumference, timed up & go, depression, self rated helath, walking exercise, flexibility exercise, strengh exercise and frailty were assessed. Data were analyzed using $x^2$-test, t-test, ANOVA, Pearson's correlation and stepwise regression to determine the associated factors of frailty by urinary incontinence. Depression, walking exercise, timed up & go and age were found to be factors significantly associated with frailty among the elders with incontinence(F=38.321, p<.001). Age, depression, walking exercise, self rated health and tined up & go were found to be factors associated with frailty in the elders without incontinence(F=265.666 p<.001). The findings show that frailty of elders and associated factors were different by urinary incontinence, and common factors affecting frailty were depression, walking exercise, timed up & go, age. Thus, these factors should be considered in the development of intervention program for care and prevention of frailty and program should be modified according to urinary incontinence.
This study examined the effects of a brown rice vegetarian diet and outdoor walking exercise program on body composition and blood lipid parameters in collegians. The mean age of respondents was 21.8 yrs (males) and 21.7 yrs (females). During the ten-day program, the respondents lived in a dormitory and had three meals. The respondents exercised one hour in the morning (6:20~7:20 am) and attended one and a half hour evening lecture (7:00~8:30 pm) everyday. The brown rice vegetarian diet consisted of brown rice, whole grain bread, beans, fresh vegetables, and fresh fruits contained $2043.2{\pm}112.7$ kcal (97.3% of RNI), 66.7 g protein (133.3% of RNI), 33.6 g dietary fiber (168.2% of RNI), vitamin A (194.2% of RNI), vitamin $B_1$(245.5% of RNI), vitamin $B_2$(225.1% of RNI), niacin (233.7% of RNI), vitamin $B_6$(277.1% of RNI), folic acid (128.4% of RNI), vitamin C (334.6% of RNI), iron (131.9% of RNI), zinc (112.4% of RNI) and calcium (60.3% of RNI). The results showed that there were significant increases in body weight (P<0.05) and BMI (P<0.05) in males and body weight (P<0.05) and lean body mass (P<0.01) in females. In addition, there were significant decreases in total cholesterol (P<0.001), LDL cholesterol (P<0.001), TG (P<0.05), and HDL-cholesterol (P<0.001) in males and total cholesterol (P<0.01) and LDL-cholesterol (P<0.01) in female. The ten day brown rice vegetarian diet rich in fiber and outdoor walking exercise program significantly increased body weight and decreased total cholesterol and LDL-cholesterol in collegians.

Purpose: The purpose of the study was to identify the effects of problem solving nursing counseling and intensified walking exercise on diabetic self-care, coping strategies, and glycemic control among older adults with DM type II. Method: Ninety nine DM patients who were older than 50 were recruited from DM clinics or public health centers and conveniently assigned into three groups: the Polar(n=41), counseling(n=30) and control groups (n=28). Participants in both Polar and counseling groups attended weekly problem solving nursing counseling for 12 weeks. Polar heart rate monitors were used in the Polar group to intensify walking exercise. Data was collected from November 2003 to August 2004 and analyzed by ANOVA or ANCOVA using the SPSS WIN program. Result: After a 12 week intervention, participants in both the Polar and counseling groups reported increased diabetic self care behaviors and decreased blood glucose levels, which is significantly different from those in the control group. There were no distinctively different program effects between the Polar and counseling groups. Conclusion: Based on the findings, we concluded that problem solving counseling alone could have positive effects on diabetic self care and glycemic controls for older adults with DM. Future research is needed to identify long-term effects of the program.
Purpose: The purpose of this study was to identify the effect of regular Nordic walking exercise on the physical indicators and subjective happiness of middle-aged rural residents. Methods: A pretest and posttest design with a non-equivalent control group was conducted. The participants of experimental and control group were each 21 subjects from 40 to 64 years old. The Nordic walking exercise program consisted of 24 sessions which were focused on group exercise, and was conducted three times a week for eight weeks. Results: The experimental group showed significant decreases in body mass index (p=.042), body fat (p<.001), and abdominal fat (p=.026). The experimental group showed significant increases in muscle mass of right lower limb (p=.009) and left lower limb (p=.016) but was not statistically significant in right, left upper limbs and body. On the other hand, the control group showed significant decreases in muscle mass of right upper limb (p=.034), left upper limb (p=.038), and body (p=.034). There were no significant differences in systolic blood pressure, diastolic blood pressure, and subjective happiness between the experimental and the control group. Conclusion: The Nordic walking exercise should be actively encouraged to improve health of the middle-aged in rural area.
Purpose: The present study analyzed the contents of exercise programs for the elderly in Korean and foreign articles. Method: This study analyzed 27 Korean and 20 foreign exercise-related articles selected from Web DBs using keywords 'exercise', 'exercise program' and 'exercise effect'. The collected data were analyzed using SPSS 11.5 through descriptive statistics, $x^2$ test and Fisher's exact test. Results: In the description of population, random sampling and informed consents, foreign articles were higher than Korean articles. In research design, foreign articles randomized controlled trials (RCTs) higher than Korean articles. The contents of exercise programs in the Korean and foreign articles were aerobic, walking and stretching. On the effects of exercise programs in the Korean articles, walking showed the highest frequency in the physical strength as well as muscle strength, balance and flexibility. In the foreign articles, walking and stretching showed high frequency in physical strength. Conclusion: Contents significant in exercise programs for the elderly were walking and stretching. In addition, regular exercise had a positive effect on muscle strength, flexibility and balance in the elderly.

A biomechanical approach was carried out to analyze foot-floor reaction forces acting on five male subjects performing a walking task. The task analyzed was walking over an obstacle with the right bare foot. The foot-floor reaction data were measured from a force plate, and then processed using a software developed. The source program was coded in the C language for easier on-line data acquisition and graphic displays. High repeatability was found in the reaction data acquired from three trials by each subject. For obstacle height from 0 to 25 cm, the maximum in reaction force reached up to 1.048 BW in the vertical, 0.174 BW in the anterior-posterior, and 0.054 BW in the medio-lateral components, respectively. A quantity was defined by the difference between two vertical reaction values, and this quantity was found to be proportional to the obstacle height. It was also shown that the whole body motion could be predicted the measured foot-floor reaction data.
The purpose of this study would like to analyze statically significant difference for low back-pain frequency of mother after development-disability children. Seven nursery children with disability conducted survey from 122 mothers cared children with disability. Survey data was obtained from April 14. 2006 to May 23. 2006. The results were as follows: According to walking-existence, assistance walking, and disability-degree, low back pain incidence frequency of mothers were statically significant difference, (p<0.05). Low back pain incidence frequency of walker-ability population was 51.4%, but low back pain incidence frequency of walker-disability population was 80.0%. then low back pain incidence frequency of mothers to walking-existence was differed amount. Disabled not statically significant difference to encephalopathy and disability-type1 and disability -type2 (p>0.05). children with disability-degree and assistance walking benchmarked low back pain disability-measure. Low back pain degree not relevancy statically significant. Physical load was statically significant difference between Oswestry's low back pain score and reach effect to child-cared(p<0.05). As development-children with disability of disable degree, Mother appeared to highly low back pain frequency rate and appeared to large reach effect child-cared owing to physical load of low back pain. So hereafter, location and person request to approach with more clinical and objectively. As approach result, it will help to stress solution of children with disability owing to develop to low back pain class and family capable strengthening program and so on.
The purpose of this study is identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing care service has been expanding on a national scale from 2016 but most of public hospitals are currently unable to operate due to nursing shortage. In this study 8 wards of public hospital have been chosen and analyzed. The measure of space program and area distribution(patient area percentage, staff area percentage, circulation area percentage), nurses's walking distance(distance from ns to patient room, distance from ns to core) were calculated by depthmapX and autocad programs. The result of this study is as follows. Nurse's walking distance is more than 24m so the efficiency of nurse's patient care is reduced. The percentage of patient area in double-corridor is higher and the patient feels more comfortable but the Nurse's walking distance is longer and the accessibility is lowered. NS should be located in the center of the ward and close to the core but some wards are not composed of proper space-separation and flow of human trrafic is overlapped. This study may serve as basic research for the architectural plan for future integrated nursing care ward.
In the current study, the design methodology has been developed to produce shoe mold for a suitable walking shoes of the general Korean using ANN (Artificial Neural Network). To design the suitable and comfortable shoes for the Korean, the shapes of foots were measured for 513 people. In this research, the foot length, breadth and ankle were considered as design parameters. In order to find the optimal foot shape for the average value of design parameters, the average value of design parameters and the other measurements were used as input and output to the ANN. After training, the various foot measurements were predicted by ANN. Base on the ANN results, the walking shoes were manufactured by considering these measurements and designing a shoe mold. From the results, the proposed method could give a more systematic and feasible means for manufacturing walking shoes with greater usefulness and better generality.
