Job Analysis of the Nurses Who Work in Customized Visiting Health Care Services using DACUM Technique (맞춤형 방문건강관리사업 내 방문간호사의 직무분석)

  • Purpose: This study was performed to analyze the visiting nurses' job using Developing a curriculum (DACUM). Methods: The DACUM committee with 6 visiting nurses participated in the workshop. In this workshop, the visiting nurses role was defined and their duties and tasks were identified. After content validation via the experts, the pilot test was conducted to 78 nurses. Results: A total of 13 duties and 82 tasks were identified on the DACUM chart which represented the importance, difficulty, and frequency of tasks with alphabet A, B, and C as its higher degree. Regarding duties, 'visiting nursing for high risk group' is the highest, while the lowest was 'public welfare events'. Regarding to tasks, 'discovering a new patient' was the highest, while 'selecting patients and offering medical supplies (nutritional supplement, patch)' was the lowest one. Conclusion: The results showed that visiting nurses working in the visiting health care service center were doing more various duties and tasks than those working according to 'Act on Long-term Care Insurance for the Aged'. The results can be used to develop training programs for visiting nurses and evaluation-scale of their job performance.

  • Purpose: The purpose of this study is to find and suggest ways to invigorate operation of visiting nursing centers. Methods: SWOT analysis was performed based on the status of use of visiting nursing centers and opinion survey on the centers' nursing experts. Results: SO strategy was drawn to improve service satisfaction, develop standardized manuals, and improve the areas of visiting nursing services, and WO strategy was drawn to establish reliability, strengthen promotion or marketing strategy, strengthen management ability, and reinforce the governmental support of visiting nursing services. ST strategy was drawn to recover the functionality of health management, secure competitive advantage, and simplify the issuance of visiting nursing directions, and WT strategy was drawn to provide customized service, establish the cooperative system of related agencies, and adjust fees. Conclusion: For invigoration of visiting nursing centers, people must recognize the importance of the visiting nursing service and institutional standards should be established so that visiting nursing service, which is currently provided as an option according to Standard Long-Term Care Plan, can be provided on a mandatory basis.

A Study on Job Satisfaction and Relevant Variables in Employees of Multi-Cultural Family Support Center: Focused Home-Visiting Educator (다문화가족지원센터 종사자의 직무만족도 및 관련변인에 관한 연구: 방문교육지도사를 중심으로)

  • This study examined job satisfaction and relevant variables for multi-cultural family home-visiting educators. The selected study subjects were 192 home-visiting educators who work at Multi-Cultural Family Support Centers in the Seoul, Gyeonggi-do, and Chungcheongnam-do areas. Data analysis was conducted using SPSS ver. 18.0. The main results were as follows. First, the job satisfaction level in multi-cultural family visiting educators stood at 3.29 points, subjects' age, averagely monthly income, marital status, and activity career were shown to influence generally upon job satisfaction, self-efficacy, job stress, and cultural competency. Job satisfaction was high in groups with: a high age, small career, low averagely monthly income, and married. Second, self-efficacy and job stress were big in the job satisfaction of multi-cultural family visiting educators. The same outcome as the existing prior research was indicated. Cultural competency was not significant. Third, an examination of relative influence upon job satisfaction in home visiting educators indicated that the explanation power was 29.0%. Significant influential variables included academic background, marital status, self-regulation, and role ambiguity. Self-regulation was greater for those with lower academic backgrounds and unmarried. Job satisfaction was higher in the lower role ambiguity. Thus, the same outcome as the existing prior research was indicated. Cultural competency wasn't indicated to be significant. This has significance in having shown characteristics of job satisfaction in multi-cultural family visiting educators.

Benefit analysis of visiting healthcare in public health centers (보건소 방문건강관리사업의 편익 분석)

  • Objectives: This study is aimed to analyze service user's benefit and perceived-outcomes of visiting healthcare. Methods: Using a questionnaire survey we analyzed the subjective service quality and satisfaction of survey respondents. The sampling was designed with socioeconomic characteristics(age, sex, user group et al.), and each respondent (N=1,000) was presented with double-bounded dichotomous choice questions. To measure the value of visiting healthcare, we employed a contingent valuation method. Results: The respondents were satisfied with service and quality on visiting healthcare. And the acceptance-to-pay of respondent's benefit was 50,458 won for each visit and in totality, service user's benefit was 185.9 billion won. Conclusions: It is necessary to invest in visiting healthcare in public health centers.

Development of the Key Performance Indicators of Long-term Care Visiting Nursing Centers Using Balanced Score Cards (균형성과표를 이용한 노인장기요양 방문간호센터의 핵심성과지표 개발)

  • Purpose: The purpose of this study was to develop effective management indicators for improving efficiencies of visiting nursing centers. Method: This was a methodological research study to develop the key performance indicators based on balanced score cards for long-term care visiting nursing centers. The main methods used in this study were literature review, focus group interview, and content validity index. The data analysis was used frequency, percentage, mean, and standard deviation. Results: The common vision of the long-term care visiting nursing centers was identified as "The healthy visiting nursing center to serve high quality cares." Eight action strategies and 15 key performance indicators to achieve this vision were developed. Conclusion: Based on the results of this study, we suggest that the developed balanced score cards will be used as an effective managerial guideline to improve performances of long-term care visiting nursing centers.

Family Characteristics and Self-care Ability in Visiting Nursing Service based on Urban Public Health Center (일 도시지역 방문간호 대상 가족의 문제유형 및 자가관리능력)

  • Purpose: The study aim was to provide basic data needed for formulating systematic visiting nursing strategies by comprehending the characteristics and self-care ability of the object families of public health centers in Korea. Method: The research examined 252 families and 339 family members of the vulnerable class that were registered in a visiting nursing program of an urban public health center. The data of 220 families were analyzed using descriptive analysis, t-test, and ANOVA, after excluding any incomplete data. Result: 1. The most frequent characteristics of families were solitary families (52.8%) and financially vulnerable families (87.3%). The most frequent way of family detection was request of the community office. 2. The most frequent type of family problems were vulnerable families (93.2%), followed by patient families (91.0%). 3. The mean score was 11.67 for family self-care ability. 4. The variables of the number of family members, disease type of the patient family members, and the type of vulnerable family showed a significant difference of family self-care ability. Conclusion: This study suggests that vulnerable families demand specific nursing interventions focused on their own problems and that visiting nurses need to obtain and use supportive resources.

  • Purpose: This study aimed to present the management strategies necessary to improve the operational efficiency of visiting nurse centers and evaluate their effectiveness. Methods: The subjects of this study were visiting nurse centers registered as long-term care centers. Based on value chain analysis, cost information analysis, and data envelope analysis, the study was carried out according to the Magerison's management consulting procedure, for six months. This procedure comprised eight sub-steps of approach and application. Results: The following management strategies were agreed upon: establishment of a cooperative network with other visiting care centers, creation of high satisfaction of external customers by providing practical training to care workers, and making rehabilitation and exercise services as the core nursing activities to be focused on. Conclusion: The management consulting process and analysis method applied in this study can referred to as a useful methodological framework for revitalizing visiting nursing centers in the future.

  • Purpose : The this study was performed to purpose the data basis of medical law revision for home visiting physical therapy established as a legal system in hone visiting health care by researching the need, satisfaction, intention to use, management, expected effects. Methods : We were surveyed for 105 patients who was participated business from home visiting health care in community health center. We choose and modified the questionnaire that was used in previous study about home visiting physical therapy and home visiting program for nurses. The questionnaire was consisted of 4 questions of general characteristics of subject, 6 questions of satisfaction, 4 questions of satisfaction of service status, 2 questions of management. And frequency analysis, ANOVA were used for statically analysis. Results : In general satisfaction, satisfaction of service status, the result were very optimistic, but there were no difference by age. In management, participations of 64.8% was answered to within three years of clinical experence and trained in home visiting physical therapy ask on qualification criteria of physical therapists, and participations of 93.3% was answered to once a week ask on desired number of therapy. Conclusion : In our conclusion, home visiting physical therapy is very useful and effective health care system for satisfaction and need, so home visiting physical therapy expected to have great potential in home visiting health care. In addition, by train to home visiting physical therapist for provide professional physical therapy service provide an institutional base which can be expanded and invigorated home visiting physical therapy.

Development of a Blue Ocean Strategy Enhancing Management Efficiencies of Long-term Care Visiting Nursing Centers (방문간호센터의 경영 효율성 제고를 위한 블루오션 전략 개발)

  • Purpose: This study aimed to present Blue Ocean strategies by deriving the strategic elements of a visiting nursing center and conducting a survey on the importance and satisfaction of care clients. Methods: First, a FGI was conducted targeting the head of the visiting nursing center to derive its strategic elements. Subsequently, importance and satisfaction surveys on the derived strategy elements were analyzed, an IPA matrix was derived, and an as-is ERRC Blue Ocean strategy was established. Kano's Three-Factor Theory was used to derive a competitive position matrix and establish a to-be ERRC Blue Ocean strategy. The Blue Ocean Strategy for Visiting Nursing Center Management is presented in this study. Results: Four as raise factors were derived from combining the results of the as-is, to-be ERRC strategy element analysis: retention of competent nurses, education in medication management, maintenance of high customer satisfaction, and prompt handling of customer complaints. Additionally, the customer's health condition evaluation was derived. Conclusion: Blue Ocean Strategies can be used to analyze, derive, and establish management strategies in various nursing-related entrepreneurship fields.

Past, Present, and Future of Home Visiting Healthcare Services based on Public Health Centers in Korea (우리나라 방문건강관리사업의 과거, 현재와 미래)

  • Purpose: We present improvements to the Korean home visiting healthcare service based on analysis of Korean home visiting healthcare services considering recent sociodemographic changes and demands for healthcare services. Methods: This is a review study in which the results are derived through a literature review and data analysis. We collected data through a search of electronic databases, Google Scholar, and governmental websites. Results: Changes in Korean home visiting healthcare services are classified into four stages: 'introduction (1990-2000)', 'pilot project (2003-2006)', 'nationwide expansion (2007-2012)', 'various types (2013-2018)'. Korean home visiting healthcare service based on public health centers has achieved outcomes such as improved health behavior and health management, increased health management ability, and establishment of comprehensive healthcare infrastructure. Conclusion: In the future, the demand for home visiting healthcare service will increase steadily because of deepening social polarization, rapid aging of the population, and increases in chronic diseases. To improve health management and health equity, we suggest that Korean home visiting healthcare service will expand to all the people as a core public health service. It is necessary to establish a management team for various types of home visiting healthcare service in the public health center.
