Spike protein D614G and RdRp P323L: the SARS-CoV-2 mutations associated with severity of COVID-19

  • The severity of coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), greatly varies from patient to patient. In the present study, we explored and compared mutation profiles of SARS-CoV-2 isolated from mildly affected and severely affected COVID-19 patients in order to explore any relationship between mutation profile and disease severity. Genomic sequences of SARS-CoV-2 were downloaded from Global Initiative on Sharing Avian Influenza Data (GISAID) database. With the help of Genome Detective Coronavirus Typing Tool, genomic sequences were aligned with the Wuhan seafood market pneumonia virus reference sequence and all the mutations were identified. Distribution of mutant variants was then compared between mildly and severely affected groups. Among the numerous mutations detected, 14408C>T and 23403A>G mutations resulting in RNA-dependent RNA polymerase (RdRp) P323L and spike protein D614G mutations, respectively, were found predominantly in severely affected group (>82%) compared with mildly affected group (<46%, p < 0.001). The 241C>T mutation in the non-coding region of the genome was also found predominantly in severely affected group (p < 0.001). The 3037C>T, a silent mutation, also appeared in relatively high frequency in severely affected group compared with mildly affected group, but the difference was not statistically significant (p = 0.06). We concluded that spike protein D614G and RdRp P323L mutations in SARS-CoV-2 are associated with severity of COVID-19. Further studies will be required to explore whether these mutations have any impact on the severity of disease.

Introduction of bacterial and viral pathogens from imported ornamental finfish in South Korea

  • Background: Live fish import may lead to the unintended introduction of pathogens. We examined the monthly distribution of microbial pathogens in ornamental finfish imported into South Korea over a 6-month period. Results: Vibrio alginolyticus was detected in one lemon damsel in June and July; V. vulnificus was detected in one lemon damsel, one caerulean damsel, and one pearl-spot chromis and one ocellaris clownfish in July, April, and May, respectively; Photobacterium damselae was detected in one ocellaris clownfish and one caerulean damsel in June and July, respectively; V. anguillarum was detected in one pearl-spot chromis in February; V. harveyi was detected in one ocellaris clownfish and two mandarin fish in February and April, respectively; Yersinia ruckeri was detected in a pearlscale goldfish group in June and July and in two colored carp groups in July; and Lactococcus garvieae was detected in a lemon damsel group and a sutchi catfish group in July and May, respectively. European catfish virus, the only viral pathogen detected, was found in two sutchi catfish groups in May. Conclusion: This study is the first to identify pathogenic species and the presence or absence of pathogens (non-quarantine diseases) in imported ornamental finfish. These results demonstrate that various pathogens with the potential to harm indigenous fish populations can accompany ornamental finfish imported into South Korea.

Distribution and genetic diversity of Feline calicivirus in Moscow metropolitan area

  • Background: Feline calicivirus (FCV) is widespread throughout the world. An FCV infection is associated with conjunctivitis, rhinitis, and mouth ulcers that can lead to the animal's death. Because vaccination is not always effective, it is necessary to monitor the infection regularly. Objectives: This study examined the FCV epizootic situation in the Moscow metropolitan area by conducting a molecular phylogenetic analysis of the virus isolates. Methods: Samples from 6213 animals were examined by a reverse transcription polymerase chain reaction. For phylogenetic analysis, 12 nucleotide sequences obtained from animal samples were selected. Sequencing was performed using the Sanger method. Phylogenetic analysis was conducted using the Maximum Likelihood method. Results: The FCV genome was detected in 1,596 (25.7%) samples out of 6,213. In 2018, calicivirus was detected in 18.9% of samples, 27.8% in 2019, 21.4% in 2020, and 32.6% in 2021. Phylogenetic analysis of the F ORF2 region and the ORF3 start region led to division into two FCV genogroups. Most of the isolates (8 out of 12) were close to the Chinese strains. On the other hand, there were isolates closely related to European and American strains. The isolates circulating in Moscow were not included in clusters with vaccine strains; their nucleotide similarity varied from 77% to 83%. Conclusions: This study revealed a high prevalence and genetic diversity of the FCV in Moscow. The epizootic situation remains stably tense because 24 viruses were detected in 25% of animals annually.

사상체질에 따른 COVID-19 감염증상에 대한 연구 (A Study on COVID-19 Symptoms according to Sasang Constitution)

  • Objectives This study aims to identify differences in clinical manifestations of COVID-19 between different Sasang constitution. Methods Subjects were recruited from August 29, 2022, to July 11, 2023. COVID-19 clinical symptoms were self-reported via questionnaires. Sasang constitutional diagnosis was performed using the K-PRISM, and Sasang constitutional specialist. Results A total of 66 subjects were recruited for the study, and the Sasang constitutional distribution of the subjects was 19 soyangin, 25 taeeumin, and 22 soeumin. For most of the COVID-19 clinical manifestations, the study found that soyangin experienced symptoms at a higher rate than other constitutions. Among the symptoms observed in the study, sore throat, pantalgia, and cough were severe in all subjects regardless of constitution. Soeumin was more likely to experience abdominal pain, chest pain, and diarrhea, while soyangin was more likely to experience chest pain, nausea/vomiting, diarrhea, and cutaneous symptoms compared to other constitutions. Taeeumin had more sputm, nasal congestion, and skin symptoms, but fewer digestive symptoms. There were differences in the onset and duration of symptoms by constitution. Conclusions This study is an important contribution to our understanding of the differences in response to the COVID-19 virus among different Sasang constitutions. Symptomatic differences between constitutions may have important implications for prevention and treatment strategies for infectious diseases, and personalized treatment and management based on these differences may be needed in the future.

Impact of the COVID-19 vaccine booster strategy on vaccine protection: a pilot study of a military hospital in Taiwan

  • Purpose: The global fight against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has led to widespread vaccination efforts, yet the optimal dosing schedule for SARS-CoV-2 vaccines remains a subject of ongoing research. This study aims to investigate the effectiveness of administering two booster doses as the third and fourth doses at different intervals to enhance vaccine protection. Materials and Methods: This study was conducted at a military regional hospital operated by the Ministry of National Defense in Taiwan. A cohort of vaccinated individuals was selected, and their vaccine potency was assessed at various time intervals following their initial vaccine administration. The study participants received booster doses as the third and fourth doses, with differing time intervals between them. The study monitored neutralizing antibody titers and other relevant parameters to assess vaccine efficacy. Results: Our findings revealed that the potency of the SARS-CoV-2 vaccine exhibited a significant decline 80 days after the initial vaccine administration. However, a longer interval of 175 days between booster injections resulted in significantly higher neutralizing antibody titers. The individuals who received the extended interval boosters exhibited a more robust immune response, suggesting that a vaccine schedule with a 175-day interval between injections may provide superior protection against SARS-CoV-2. Conclusion: This study underscores the importance of optimizing vaccine booster dosing schedules to maximize protection against SARS-CoV-2. The results indicate that a longer interval of 175 days between the third and fourth doses of the vaccine can significantly enhance the neutralizing antibody response, potentially offering improved protection against the virus. These findings have important implications for vaccine distribution and administration strategies in the ongoing battle against the SARS-CoV-2 pandemic. Further research and largescale trials are needed to confirm and extend these findings for broader public health implications.

  • Metallothionein (MT)는 세포내에 존재하는 cystein이 풍부한 적은 단백질로서 핵의학적 영상이나 치료에 이응이 가능할 여러 금속성 방사성 동위원소와 결합한다. 본 연구는 sarcoma를 주사한 Balb/C mice에서 MT와 결합하는 방사성동위원소인 cadmium이 종양에 축적되는가를 확인하고, 그것이 핵의학적인 영상과 치료에 이용될 수 있는지와 Ga-67을 대신할 수 있는지를 보기 위해 종양과 정상조직에서 Cd-109과 Ga-67의 섭취율을 비교하고 종양과 정상조직의 비를 비교하였다. Balb/3T3 세포를 Molony murine sarcoma virus (MMSV)로 변형시킨 세포를 Balb/C mice의 왼쪽 서혜부에 피하로 주사한 후 종양이 $0.6{\sim}l.0cm^2$로 자랐을 때 $25{\mu}Ci$ Cd-109 chloride와 $40{\mu}Ci$ Ga-67 citrate을 피하주사 한 후 18 또는 72시간에 쥐를 희생하여 종양과 여러 정상조직을 제거하여 무게를 잰 후 방사능을 측정하였다. Cd-109의 종양섭취는 Ga-67과 비슷하나 대부분의 정상조직에 비해 (MT가 풍부한 간과 신장을 제외) Ga-67보다 높았다. 종양대 정상조직의 섭취비는 Cd-109이 Ga-67에 비해 배후방사능에 중요한 영향을 미치는 장기인 뼈, 장, 지방조직, 근육, 혈액에서 월등히 높아서 (P<0.001) 같은 종양에서 Ga-67보다 좋은 영상을 얻을 수 있으리라고 생각되며, MT에 결합하는 cadmium은 암의 종양영상과 치료에 이용될 수 있을 것으로 생각된다.

급성 위장관염으로 입원한 소아에서 분자 유전학적 방법에 의한 Human Astrovirus 감염의 유병률 (Molecular Epidemiology of Human Astrovirus Infection in Hospitalized Children with Acute Gastroenteritis)

  • 목 적: Human astrovirus (HAstV)는 전세계적으로 영아 연령에서 중요한 위장관염의 중요 병원체로 알려지고 있지만 국내에서 HAtV 감염의 유병률에 대한 연구는 드문 편이다. 이에 저자들은 분자 유전학적 방법을 이용하여 위장관염으로 입원한 5세 이하의 소아에서 HAstV의 유병률과 유전형을 알아보기 위하여 본 연구를 시행하였다. 방 법: 2004년 2월부터 2005년 1월까지 12개월간 인제의대 상계백병원 소아과에 급성 위장관염으로 입원한 5세 이하의 소아에게서 수집된 대변 검체 812건을 대상으로 하였다. 대변 검체에 대해서 enzymeimmunoassay(EIA) 또는 RT-PCR을 이용하여 rotavirus, enteric adenovirus와 norovirus 감염여부를 진단하였다. HAstV 특이적인 시발체를 이용하여 RTPCR을 시행한 다음 염기서열 분석을 시행하였다. 결 과: 총 812건의 검체에서 rotavirus 양성률은 16.9% (138/812), norovirus 양성률은 11.5% (94/812), adenovirus 양성률은 4.0% (33/812)였다. HAstV의 유병률은 RT-PCR 방법에 의해 4.0% (33/812)였다. HAstV 양성인 환아의 연령은 생후 12개월 미만 82.0%, 1~2세 미만 6.0%, 2~3세 미만 6.0%, 3~5세 이하가 6.0%였다. HAstV의 월별 분포는 각각 4월 3건, 5월 5건, 6월 4건, 8월 12건, 9월 4건, 10월 2건, 11월 2건, 12월 1건으로 8월에 가장 높은 발생률을 보였다. 염기서열 분석이 가능하였던 HAstV 양성 검체 22건 중에서 유전형-1이 20건, 유전형-8이 2건이었다. 결 론: 2004년 2월부터 2005년 1월까지 12개월간 급성 위장관염으로 입원한 소아에서 HAstV 감염 유병률은 4.0%였으며, 영아기 연령에서 주로 발생하였다. 국내 HAstV 감염의 정확한 역학적 특성을 파악하기 위해서는 장기간에 걸친 추가 연구가 필요할 것으로 생각된다.

남.서해안과 동중국해 자연산 어류에서 Red Sea Bream Iridovirus (RSIV)의 검출 (Detection of Red Sea Bream Iridovirus (RSIV) from marine fish in the Southern Coastal Area and East China Sea)

  • Red sea bream iridovius (RSIV)는 우리나라와 일본의 참돔 (Pagrus major), 돌돔(Oplegnathus fasciatus) 등 주로 돔류에 질병을 유발하는 어류 병원바이러스로 알려져 왔으나, 돔류 이외에도 감수성을 나타내는 어종이 매우 다양하며 아시아를 중심으로 고수온기에 대량 폐사를 유발하는 중요한 원인체로 보고되고 있다. 본 연구는 RSIV에 대한 자연산어류에서 검출동향의 모니터링을 통해 금후 red sea bream iridoviral disease (RSIVD)에 대한 예방학적 접근의 일환으로서 자연산 어류로부터의 RSIV 검출 및 지리적 분포, 보균 어종에 대한 유전학적인 조사를 실시하였다. 조사는 2003년과, 2005년도에 동중국해역 3지점, 서해안 5지점 그리고 남해안 5지점에서 어류를 채집하여 PCR을 이용한 검출결과, 자연산 해산어류 160마리 중 39마리에서 RSIV가 검출되어 24.3%의 높은 검출율을 보였으며 특히 조사된 어류 중 상어목에서 가장 높은 검출율을 보였다. 자연산 어류에서 분리한 RSIV 분리주는 양식산어류에서 분리된 RSIV-K strain과 염기서열 분석에서 97.2%~100%, 아미노산 분석에서 94.7~100% 유사성을 나타내었으며 megalocytivirus Subgroup Ⅲ에 속함을 확인하였다.

Pandemic Influenza A/H1N1 Viral Pneumonia without Co-Infection in Korea: Chest CT Findings

  • Background: To evaluate chest CT findings of pandemic influenza A/H1N1 pneumonia without co-infection. Methods: Among 56 patients diagnosed with pandemic influenza A/H1N1 pneumonia, chest CT was obtained in 22 between October 2009 and Februrary 2010. Since two patients were co-infected with bacteria, the other twenty were evaluated. Predominant parenchymal patterns were categorized into consolidation, ground glass opacity (GGO), and mixed patterns. Distribution of parenchymal abnormalities was assessed. Results: Median age was 46.5 years. The CURB-65 score, which is the scoring system for severity of community acquired pneumonia, had a median of 1. Median duration of symptoms was 3 days. All had abnormal chest x-ray findings. The median number of days after the hospital visit that Chest CT was performed was 1. The reasons for chest CT performance were radiographic findings unusual for pneumonia (n=13) and unexplained dyspnea (n=7). GGO was the most predominant pattern on CT (n=13, 65.0%). Parenchymal abnormalities were observed in both lungs in 13 cases and were more extensive in the lower lung zone than the upper. Central and peripheral distributions were identified in ten and nine cases, respectively. One showed diffuse distribution. Peribronchial wall thickening was found in 16 cases. Centrilobular branching nodules (n=7), interlobular septal thickening (n=4), atelectasis (n=1), pleural effusion (n=5), enlarged hilar and mediastinal lymph nodes (n=6 and n=7) were also noted. Conclusion: Patchy and bilateral GGO along bronchi with predominant involvement of lower lungs are the most common chest CT findings of pandemic influenza A/H1N1 pneumonia.

토끼의 바이러스성 간염에 관한 연구 : 면역조직화학적 관찰 (Studies on the rabbit viral hepatitis : Immunohistochemical observations)

  • 토끼의 바이러스성 간염, 소위 토끼 출혈병의 원인체에 대한 각종 세포주들에 바이러스증식을 유도하고 한편 실험적 감염예에 대한 면역형광항체법과 immunoperoxidase 방법에 의한 원인체의 조직내에 분포상황을 조사하기 위하여 감염된 토끼를 폐사직후 부검하여 간장, 비장, 신장, 폐 및 뇌조직을 절제하여 동결절편하거나 또는 포르말린고정 파라핀 포매 절편을 $5{\sim}7{\mu}m$로 작제하여 면역반응에 공시하였다. 공시된 각종 세포주에 대한 원인체의 세포배양성은 인정되지 않았으며 면역조직화학적방법을 이용한 면역반응에서는 간장에서 강한 양성반응을 보였으며, 비장과 신장에서는 소수예에서 백색수주변 대식세포와 신사구체에서 양성반응을 각각 나타내었다. 그러나 기타 장기에서는 특이한 양성반응이 인정되지 않았다. ABC immunoperoxidase 방법을 이용한 간장의 포르말린고정 파라핀포매 절편에서 portal triad를 중심으로한 소엽주변부 간세포에서 강한 양성반응을 나타내었으며, 이들 양성반응은 간세포 및 동양혈관세포의 세포질내에 미세과립상으로 미만성 및 세포질주변성으로 관찰되었다. 그리고 감염세포와 비감염세포와의 구별이 명확히 인정되었고 양성반응을 보이는 부위는 H-E 염색상 변성괴사된 간세포에 일치되었다. 이상의 결과에서 본 질병의 표적기관은 간장이며 포르말린고정 파라핀포매 간조직의 immunoperoxidase 방법에 의한 면역조직화학적방법이 본병 진단에 크게 활용되리라 본다.
