실험적(實驗的) 뇌허혈(腦虛血) 및 저산소증(低酸素症)에 대한 Flunarizine의 약효(藥效) -뇌장해에 대한 Flunarizine 효능- (Effect of Flunarizine on Experimental Ischemia and Hypoxia in Rats and Mice)

  • Recent hypothesis suggested that intracellular accumulation of calcium is a common denominator of ischemic celullar damage. Flunarizine, a calcium entry blocker, posses vasodilating properties in cerebral vascular beds and clinically used in circulatory disorders. The present study was designed to evaluate the effect of flunarizine on ischemic and hypoxic brain damage. An ischemic model was made by bilateral carotid artery ligation (BCAL) in Wistar strain rat. Hypoxic model was made by intravenous injection(i.v.) of KCN to rats and mice. In mice, flunarizine not only reduced the mortality of KCN, but also delayed the onset time of convulsion. The contents of ATP, creatine phosphate and glucose, cerebral energy metabolite, decreased 30 minutes after BCAL and KCN i, v, while that of lactate increased. But these variations were suppressed by flunarizine. Furthermore, increase in the dosage of flunarizne generally promoted the recovery of cerebral energy metabolites in hypoxic animals. The results suggest that flunarizine had a protective effect against ischemic and hypoxic brain damage due to its ameliorating action on the cerebral energy metabolism.

치매의 FDG PET 영상 (FDG PET Imaging For Dementia)

  • Dementia is a major burden for many countries including South Korea, where life expectancy is continuously growing and the proportion of aged people is rapidly growing. Neurodegenerative disorders, such as, Alzheimer disease, dementia with Lewy bodies, frontotemporal dementia, Parkinson disease, progressive supranuclear palsy, corticobasal degeneration, Huntington disease, can cause dementia, and cerebrovascular disease also can cause dementia. Depression or hypothyroidism also can cause cognitive deficits, but they are reversible by management of underlying cause unlike the forementioned dementias. Therefore these are called pseudodementia. We are entering an era of dementia care that will be based upon the identification of potentially modifiable risk factors and early disease markers, and the application of new drugs postpone progression of dementias or target specific proteins that cause dementia. Efficient pharmacologic treatment of dementia needs not only to distinguish underlying causes of dementia but also to be installed as soon as possible. Therefore, differential diagnosis and early diagnosis of dementia are utmost importance. F-18 FDG PET is useful for clarifying dementing diseases and is also useful for early detection of the diseases. Purpose of this article is to review the current value of FDG PET for dementing diseases including differential diagnosis of dementia and prediction of evolving dementia.

Isolation and In Vitro Culture of Vascular Endothelial Cells from Mice

  • In cardiovascular disorders, understanding of endothelial cell (EC) function is essential to elucidate the disease mechanism. Although the mouse model has many advantages for in vivo and in vitro research, efficient procedures for the isolation and propagation of primary mouse EC have been problematic. We describe a high yield process for isolation and in vitro culture of primary EC from mouse arteries (aorta, braches of superior mesenteric artery, and cerebral arteries from the circle of Willis). Mouse arteries were carefully dissected without damage under a light microscope, and small pieces of the vessels were transferred on/in a Matrigel matrix enriched with endothelial growth supplement. Primary cells that proliferated in Matrigel were propagated in advanced DMEM with fetal calf serum or platelet-derived serum, EC growth supplement, and heparin. To improve the purity of the cell culture, we applied shearing stress and anti-fibroblast antibody. EC were characterized by a monolayer cobble stone appearance, positive staining with acetylated low density lipoprotein labeled with 1,1'-dioctadecyl-3,3,3',3'-tetramethyl-indocarbocyanine perchlorate, RT-PCR using primers for von-Willebrand factor, and determination of the protein level endothelial nitric oxide synthase. Our simple, efficient method would facilitate in vitro functional investigations of EC from mouse vessels.

Inhibitory Effect of Scopoletin on U46619-induced Platelet Aggregation through Regulation of Ca2+ Mobilization

  • Platelet aggregation is essential for hemostatic process in case of blood vessels damages. However, excessive platelet aggregation can cause cardiovascular disorders including atherosclerosis, thrombosis and myocardial infarction. Scopoletin is usually found in the roots of genus Scopolia or Artemisia, and is known to have anticoagulant and anti-malarial effects. This study investigated the effect of scopoletin on human platelet aggregation induced by U46619, an analogue of thromboxane $A_2(TXA_2)$. Scopoletin had anti-platelet effects by down-regulating $TXA_2$ and intracellular $Ca^{2+}$ mobilization ($[Ca^{2+}]_i$), the aggregation-inducing molecules generated in activated platelets. On the other hand, scopoletin increased the levels of cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP), which are known to be intracellular $Ca^{2+}$ antagonists. This resulted in inhibition of fibrinogen binding to ${\alpha}IIb/{\beta}_3$ in U46619-induced human platelet aggregation. In addition, scopoletin inhibited the release of adenosine trisphosphate (ATP) in dose-dependent manner. This result means that the aggregation amplification activity through the granule secretion in platelets was suppressed by scopoletin. Therefore, we demonstrated that scopoletin has a potent antiplatelet effect and is highly likely to prevent platelet-derived vascular disease.

Physical Recovery through Health Management Education for the Disabled or the Elderly

  • Purpose: The number of people with disabilities and the elderly over 65 years of age is gradually increasing due to physical disabilities and social aging. Their typical physical disorders or chronic diseases include low back pain, sciatic pain, arthritis, and musculoskeletal systems such as discs. The average prevalence of disease is 78%. These are various physical obstacles and hindrances in daily life. Research design, data and methodology: From August 6, 2019 to September 24, 2019, the Senior Welfare Center in Gyeyang-gu, Incheon, operated a healthy body exercise and health education program for living health management. Results: The vascular health index using U-Bio pulse wave was relatively good at the first average of +7.4, but the second average of -6.3. This can be seen as a result of the combination of diet and lifestyle education along with the effect of corrective exercise. As a result of body shape measurement analysis, the number of persons requiring management with 3 or more body imbalances was found to be from 75% before to 62.5% afterwards. Conclusions: Exercise effect appears when exercise lasts for at least 10 weeks. Some performances were good, but there were limitations due to the operation of a short training period.

Contribution of Microbleeds on Microvascular Magnetic Resonance Imaging Signal

  • Purpose: Cerebral microbleeds are more susceptible than surrounding tissues and have been associated with a variety of neurological and neurodegenerative disorders that are indicative of an underlying vascular pathology. We investigated relaxivity changes and microvascular indices in the presence of microbleeds in an imaging voxel by evaluating those before and after contrast agent injection. Methods: Monte Carlo simulations were run with a variety of conditions, including different magnetic field strengths (B0), different echo times, and different contrast agents. ΔR2* and ΔR2 and microvascular indices were calculated with varying microvascular vessel sizes and microbleed loads. Results: As B0 and the concentration of microbleeds increased, 𝜟R2* and 𝜟R2 increased. 𝜟R2* increased, but 𝜟R2 decreased slightly as the vessel radius increased. When the vessel radius was increased, the vessel size index (VSI) and mean vessel diameter (mVD) increased, and all other microvascular indices except mean vessel density (Q) increased when the concentration of microbleeds was increased. Conclusions: Because patients with neurodegenerative diseases often have microbleeds in their brains and VSI and mVD increase with increasing microbleeds, microbleeds can be altered microvascular signals in a voxel in the brain of a neurodegenerative disease at 3T magnetic resonance imaging.

  • 철은 중추신경계 및 인체에 필수적인 성분으로 노화 및 다양한 퇴행성 뇌질환에서 뇌의 철 침착이 증가된다. 철은 MRI에서 독특한 특성을 가지고 있어 인체의 철 침착과 분포를 비침 습적으로 평가 및 정량화가 가능하다. 이 종설에서는 철 영상을 위한 MRI 기법에 대하여 알아보고, 노화 및 알츠하이머병을 포함한 퇴행성 뇌질환에서 변화를 고찰해 보고자 한다. 또한 현재 접근법의 제한점과 앞으로 기대되는 새로운 접근도 확인해 보고자 한다.

  • Chunghyul-dan (CHD) is a combinatorial drug known to exert anti-inflammatory effects in endothelial cells. In this study, we employed global transcriptional profiling using cDNA microarrays to identify molecular mechanisms responsible for the anti-inflammatory activity of CHD in endothelial cells. An analysis of the microarray data revealed that transcript levels of monocyte chemotactic protein-1 (MCP-1), vascular cell-adhesion molecule-1 (VCAM-1) and activated leukocyte cell-adhesion molecule were dramatically altered in CHD-treated endothelial cells. These changes in gene expression were confirmed by RT-PCR, Western blotting and ELISA. Chronic CHD treatment also appeared to decrease MCP-1 secretion, probably as a result of decreased MCP-1 expression. In addition, we determined that chronic CHD treatment inhibited lipopolysaccharide-stimulated adhesion of THP-1 leukocytes to endothelial cells. The inhibitory effect of CHD on LPS-stimulated adhesion resulted from downregulation of VCAM-1 expression. Transmigration of THP-1 leukocytes through endothelial cells was also inhibited by chronic CHD treatment. In conclusion, CHD controls a variety of inflammatory activities by regulating MCP-1 and VCAM-1 gene expression.

Phosphodiesterase-5 Inhibitor Attenuates Anxious Phenotypes and Movement Disorder Induced by Mild Ischemic Stroke in Rats

  • Objective : Patients with mild ischemic stroke experience various sequela and residual symptoms, such as anxious behavior and deficits in movement. Few approaches have been proved to be effective and safe therapeutic approaches for patients with mild ischemic stroke by acute stroke. Sildenafil (SIL), a phosphodiesterase-5 inhibitor (PDE5i), is a known remedy for neurodegenerative disorders and vascular dementia through its angiogenesis and neurogenesis effects. In this study, we investigated the efficacy of PDE5i in the emotional and behavioral abnormalities in rats with mild ischemic stroke. Methods : We divided the rats into four groups as follows (n=20, respectively) : group 1, naïve; group 2, middle cerebral artery occlusion (MCAo30); group 3, MCAo30+SIL-pre; and group 4, MCAo30+SIL-post. In the case of drug administration groups, single dose of PDE5i (sildenafil citrate, 20 mg/kg) was given at 30-minute before and after reperfusion of MCAo in rats. After surgery, we investigated and confirmed the therapeutic effect of sildenafil on histology, immunofluorescence, behavioral assays and neural oscillations. Results : Sildenafil alleviated a neuronal loss and reduced the infarction volume. And results of behavior task and immunofluorescence shown possibility that anti-inflammation process and improve motor deficits sildenafil treatment after mild ischemic stroke. Furthermore, sildenafil treatment attenuated the alteration of theta-frequency rhythm in the CA1 region of the hippocampus, a known neural oscillatory marker for anxiety disorder in rodents, induced by mild ischemic stroke. Conclusion : PDE5i as effective therapeutic agents for anxiety and movement disorders and provide robust preclinical evidence to support the development and use of PDE5i for the treatment of mild ischemic stroke residual disorders.

폐쇄성 수면 무호흡이 Soluble Intercellular Adhesion Molecule-1과 Vascular Endothelial Growth Factor에 미치는 영향 (Influence of Obstructive Sleep Apnea on Soluble Intercellular Adhesion Molecule-1 and Vascular Endothelial Growth Factor)

  • 연구배경 : 폐쇄성 수면 무호흡 증후군은 고혈압과 심부정맥 및 허혈성 심질환 등의 심혈관질환을 일으키는 위험요소로 알려져 있으나 정확한 기전에 대해서는 아직 확실히 밝혀져 있지 않다. 최근에 soluble intercellular adhesion molecule-1(sICAM-1)과 vascular endothelial growth factor(VEGF)가 죽상경화증의 발생과 진행에 관여한다고 알려지면서, 일부 연구에서는 이러한 사이토카인이 폐쇄성 수면 무호흡 증후군에서도 증가한다고 보고하였다. 저자는 폐쇄성 수면 무호흡 증후군 환자에서 sICAM-1과 VEGF의 농도 변화를 관찰함으로써, 이들 환자에서 동반될 수 있는 심혈관질환 발생의 병태생리를 일부 이해하고자 하였다. 방 법 : 폐쇄성 수면 무호흡 증후군을 진단하기 위해 가톨릭대학교 성바오로병원 수면장애크리닉을 방문하여 수면다원검사를 시행한 37명을 대상으로 하였다. 철야 수면다원검사를 종료한 직후 혈액 채취를 한 후 혈청분리를 하여 $-70^{\circ}C$ 냉장고에 보관하였다. 사이토카인의 측정은 ELISA 방법으로 시행하였고 수면다원검사의 각 지표와 혈청 sICAM-1 농도 및 혈청 VEGF 농도와의 상관 관계를 비교 분석하였다. 결 과 : 무호흡-저호흡지수와 혈청 sICAM-1 농도 사이에는 유의한 상관관계가 없었지만(r=0.27, P>0.05), 무호흡-저호흡지수와 혈청 VEGF 농도는 유의한 상관관계가 있었다(r=0.50, P<0.01). 무호흡지수와 혈청 sICAM-1 농도는 다소 유의한 상관관계가 있었으며(r=0.31, P<0.01) 무호흡지수와 혈청 VEGF 농도 사이에는 유의한 상관관계가 있었다(r=0.45, P<0.01). 결 론 : 폐쇄성 수면 무호흡 혹은 저호흡은 혈청내 sICAM-1 및 VEGF 농도를 증가시키는 것을 알 수 있었으며, 이러한 사이토카인의 발현 증가는 폐쇄성 수면무호흡 증군 환자에서 심혈관질환의 발생에 관련이 있을 것으로 추측된다.
