In cleft palate patient, characteristic of speech disorder is the resonance disorder result from velopharyngeal incompetence. Clinically VPI caused by congenital factor as congenital palatal incompetence, submucosal cleft palate, and caused by acquired factor as CNS damage, tumor, palatal palsy. The clinicians more concerned about the speech disorders after cleft palate surgery rather than language pathologist. The resonance disorder devided for hypernasality, hyponasality and nasal emission, but as a rule, hypernasality is typical phenomenon of the resonance disorder. Traditionally clinicians and language pathologists evaluated four-stage or five-stage of hypernasality by subjective assessment. Although language pathologist is well-trained, results of the language level should be different. In late 1980s, Kay Elemetrics Corp. developed nasometer that objective nasalance identified with well-trained language pathologist and originate from nasometer Tonar I and II were developed by Fletcher. Therefore objective nasalance test was possible, the nasometer used in hospital, collage and speech clinic both and home and abroad. Standardization of the cleft palate speech assessment must be settled without delay because of different character result in different language and different assessment results by dialect in same language. In our study, we provide the data base for the standardization of cleft palate speech assessment which through report of objective assessment method, speech therapy effects and problems result in interdisciplinary teamwork by nasometer use in treatment of cleft palate patient.
피발산 비닐의 라디칼 텔로머화반응을 통하여 양 말단에 관능기를 포함한 폴리(피발산 비닐) 텔로머를 합성하였다. 합성된 텔로머의 수평균분자량 ($\bar{M}$n)은 GPC, $^1$H-NMR, 점도법 세가지로 각각 고찰하였고, 그 값은 2400~13000 g/mol 이었다. 또한, 텔로머의 분자량을 정밀제어하기 위하여 VPi에 대한 텔로겐($CCl_4$)의 연쇄이동상수값($C_s$)을 Mayo식 및 시뮬레이션에 의해 결정하였는데, $C_s$은 40, 50, 60 $^{\circ}C$에서 각각 1.15, 1.16, 1.18 이었다. 합성된 텔로머는 전환율 18~72%에서 $\bar{M}$n은 5100~5400 g/mol 사이의 값을 보였고, 이는 시뮬레이션 결과와 일치하였다.
The purpose of this in vitro study was to evaluate the sealing ability of three sealers(Sealapex, Pulp canal sealer, AH26) used with continuous wave method using an anaerobic bacterial leakage model. 53 extracted human teeth with straight and single canals were prepared with crown-down pressureless technique using .04, .06 taper Profile(Maillefer, Swiss). Master apical file was maintained as #35 K-file. All canals of the experimental teeth were obturated with continuous wave method using System B(Analytic technology, U.S.A.) The teeth were randomly divided into three experimental groups of 15 and two control groups of 4. Experimental group 1 was obturated with Sealapex and group 2 with Pulp canal sealer, and group 3 with AH26. A dual chamber anaerobic bacterial leakage model was assembled. Brain heart infusion with yeast extract, hemin, menadion, and the chromogenic indicator bromocresol purple was used as the culture broth for Fusobacterium nucleatum(VPI 10197), The specimens were incubated in anaerobic chamber at $37^{\circ}C$ and were observed every 2 to 3 clays, The coronal leakage was evaluated through the color change of culture broth in lower chamber for 60 days. The results were as follows: 1. The incidence of bacterial leakage in group 1 (Sealapex group was 80%, 53% in group 2 (Pulp canal sealer), 27% in group 3 (AH26). 2. There were statistically significant differences in leakage scores between group 1 and group 2, and between group 1 and group 3, respectively. (P<0.05) 3. There was no significantly difference in leakage score between group 2 and group 3. (P>0 05)
Purpose: It is accepted universally that correction of the cleft lip nasal deformity requires multiple stages of surgery. Following primary lip repair in infancy or early childhood, secondary surgery to improve the deformity of the lip and nose is frequently necessary. A suitable surgical procedure to correct the accompanying deformity, such as cleft palate and alveolus, must be carried out at an appropriate age. In developing countries, it is common for patients with cleft lip nasal deformity to present severe secondary deformities in adolescence, because of poor follow-up and inappropriate surgery. Methods: The first patient was a 12 year old Mongolian boy. He presented prominent lip scar, short lip, wide columella, asymmetric nostril, palatal fistula, cleft alveolus, and velopharyngeal incompetence. He underwent cheilorhinoplasty, transpositional flap, alveoloplasty by iliac bone graft, and sphincter pharyngoplasty. On follow-up, a bilateral maxillary hypoplasia and a class III malocclusion developed. He underwent LeFort I osteotomy and maxillary advancement at the age of 16 years. The second patient was an 18 year old Eastern Russian girl. She presented with a deviated nose, right alar base depression, short lip, protrusion on vermilion, large palatal fistula, and severe VPI due to short palate. She underwent the combined procedure of cheilorhinoplasty, corrective rhinoplasty, tongue flap for palatal fistula, and superiorly based pharyngeal flap. And the tongue flap was detached at postoperative 3 weeks. Results: The overall results have been extremely pleasing and satisfactory to patients. There were no postoperative complications. Conclusion: We discovered the one stage operation for radical correction was sufficient procedure to provide excellent clinical outcomes in patients with severe cleft lip nose deformity.

Background Educational resources on the internet are extensively used to obtainmedical information. YouTube is the most accessed video platform containing information to enhance the learning experience of medical professionals. This study systematically analyzed the educational value of microsurgery-related videos on this platform. Methods A systematic review was conducted on YouTube from April 18 to May 18, 2020, using the following terms: "microsurgery," "microsurgical," "microsurgical anastomosis," "free flap," and "free tissue transfer." The search was limited to the first 100 videos, and two independent reviewers screened for eligible entries and analyzed their educational value using validated scales, including a modified version of the DISCERN score (M-DISCERN), Journal of the American Medical Association (JAMAS) benchmark criteria, and the Global Quality Score (GQS). Evaluation of video popularity was also assessed with the video power index (VPI). Results Of 356 retrieved videos, 75 (21%) were considered eligible. The educational quality of videos was highly variable, and the mean global scores for the M-DISCERN, JAMAS, and GQS for our sample were consistent with medium to low quality. Conclusions A limited number of videos on YouTube for microsurgical education have high-educational quality. The majority scored low on the utilized criteria. Peer-reviewed resources seem to be a more reliable resource. Although the potential of YouTube should not be disregarded, videos should be carefully appraised before being usedas an educational resource.
Poly(vinyl alcohol) (PVA) is a common hydrophilic polymer that is synthesized through the saponification reaction of poly(vinyl ester)-based polymers, mostly using poly(vinyl acetate) (PVAc) as a precursor. The heterogeneous saponification reaction of poly(vinyl ester)-based films leads to PVA films with new surface properties. Cellulose acetate (CA), in which the hydroxyl group of cellulose is replaced by an acetyl group, is a typical cellulose derivative capable of overcoming the low processability of cellulose due to strong hydrogen bonding. In this study, P(VAc/VPi)/CA blended films were prepared by the solvent casting, and then PVA/Cellulose blended films with improved surface properties were prepared by heterogeneous saponification. The structural changes caused by heterogeneous saponification were confirmed by FT-IR analysis, where both saponification and deacetylation reactions occurred in the saponification solution. In addition, the surface property changes were analyzed by FE-SEM and contact angle analyses, and the transmittance changes of the modified films were also assessed.
의료 영상 처리는 특수한 전문 분야로서 새롭게 개발되는 기술을 임상에 적용하고 그 결과를 전문 의사들이 분석하여 기술을 보완하면서 발전시켜 나간다. 그러므로, 새로운 의료 영상 처리 알고리즘을 임상에 쉽게 적용하고 그 결과를 분석할 수 있는 도구의 필요성은 매우 크다고 볼 수 있다. 본 논문에서는 시각 프로그래밍 기법을 도입하여 프로그래밍 전문 기술이 없는 일반 의사들도 의료 영상 처리 프로그램을 쉽게 작성할 수 있는 환경을 설계하고 구현하여 의료 영상 알고리즘의 적용을 편리하게 하려고 한다. 개발된 시각 프로그래밍 환경에는 의료영상처리에 필요한 다양한 함수들을 코딩하여 아이콘(Icon)으로 정의한 다음, 함수 라이브러리로 만들어 놓았다. 사용자는 영상처리 응용 프로그램을 개발할 때에, 명령어나 변수를 타이핑하여 프로그램을 작성하는 대신, 아이콘으로 정의되어 제공되는 함수들 중에서 필요한 함수를 선택하고, 이들 사이의 자료흐름에 따라 아이콘들을 연결시켜서 프로그램의 논리를 자료흐름도(Dataflow Diagram)로 표현하여 작성하면 된다. “VPI(Visual Programming Interface)”라고 불리는 그래픽 편집기는 자료 흐름도를 그려내는 창과 준비된 함수 라이브러리를 시각화하여 보여주는 창으로 구성되어 있다. 함수 라이브러리에 포함된 함수들을 용도에 따라서 분류하고, 사용자가 필요한 함수를 찾기 쉽고 사용하기 편리하도록 시각화하여 하이퍼볼릭 트리(Hyperbolic Tree)라는 새로운 형태로 표현하였다. 하이퍼볼릭 트리를 사용하면 함수들의 개별적 특성과 함수들을 분류한 전체 구조를 한꺼번에 잘 볼 수 있다. 개발된 프로그램의 사용자 인터페이스를 쉽게 구현할 수 있는 “GUI Builder”라는 도구를 설계하고 구현하였다. 개발된 프로그래밍 환경을 사용하면 프로그래밍 전문 지식이 없는 의사도 쉽게 영상 처리 응용 프로그램을 작성하여 최신 의료영상 처리 기법을 쉽게 임상에 적용하고 실험할 수 있는 장점이 있다.

목적: 본 연구의 목적은 미숙아에게 조기 중재 프로그램을 시행할 때 운동 발달과 성장에 미치는 효과와 발달 평가 도구(NBAS, HNNE)의 유용성을 조사하고자 하였다. 방법: 부산성모병원에서 태어난 42명의 미숙아와 부산성모병원과 좋은문화병원에서 태어난 20명의 만삭아를 대상으로 하였다. 조기 중재 프로그램과 발달 평가는 신생아 중환자실 물리치료사에 의해 시행되었다. 자료는 전향적으로 수집되었다. 결과: 재태주령 34주 미만의 미숙아 중재 그룹에서 인공호흡기와 폐표면활성제 사용, 심한 기관지 폐 이형성증, 뇌실 내 출혈, 수술을 시행한 동맥관 개존증이 재태주령 34주 이상 37주 미만 미숙아 중재 그룹보다 많았다. 미숙아 중재 그룹은 1번째, 2번째, 3번째 평가에서 NBAS와 HNNE 평균 점수의 향상을 보였다 (P=0.000). 미숙아 중재 그룹은 재태주령 40주 평가에서 만삭아 대조 그룹에 비해 더 높은 NBAS와 HNNE 평균 점수를 보였다(P=0.000). 재태주령 34주 이상 37주 미만 미숙아 중재 그룹은 1번째와 3번째 평가 사이에 체중과 두위의 더 많은 성장을 보였다(P<0.05). 미숙아 중재 그룹은 중재와 평가 중 무호흡, 서맥, 후기 패혈증이 없었다. 결론: 조기 중재 프로그램은 미숙아의 운동 발달과 성장에 양호한 영향을 미쳤다. 발달 평가 도구(NBAS, HNNE)는 미숙아의 운동 발달을 평가하는 안전하고 유용한 도구로 사용될 수 있다고 생각한다.
Oronasal fistula are a well-known complication of surgical treatment of cleft palate, occurring most frequently in the alveolus and hard palate. Previous reports have demonstrated that oronasal fistulas, particularly if greater than l cm in diameter, had an adverse effect on speech. The aim of this study was to demonstrate the relationship between the size of the fistula and the influence on velopharyngeal function. The site and size of the fistula were indicated on graph paper with calipers and measured in $mm^2$. Speech assessment was carried out using a Nasometer, VPI articulation differential test, spectrography. Patient whose fistulas affected their speech had significantly larger fistulas than those whose fistulas did not. The study shows that the larger the fistula, the greater the risk of hypernasality and nasal emission, but even small fistulas can cause speech problems. If obstruction of the nasal passage is eliminated in a patient with a previously asymptomatic fistula, it may result in a fistula becoming symptomatic, resulting in hypernasality and nasal emission. In conclusion, even small fistulas can influence speech production and should be considered before any treatment is planned. The study lends support to early closure of oronasal fistulas, particularly before pharyngeal flap surgery is contemplated.
본 연구의 목적은 소아 청소년기 교정환자에 있어서 음파 칫솔과 일반 칫솔의 치태 제거 효과를 비교하는 것이다. 삼성서울병원 소아치과에서 고정식 교정 장치 치료를 받는 건강한 소아 청소년 환자 21명 (여자: 10명, 평균나이 $14.57{\pm}2.28$년; 남자: 11명, 평균나이 $13.04{\pm}1.55$년)을 대상으로 음파 칫솔(Sonicare$^{(R)}$, Optiva Corp., 미국)과 교정용 일반 칫솔(Orthocare$^{(R)}$, Tomy International Inc., 일본)을 사용하여 총 4회의 내원 시 각각 2회씩 회당 2분간 칫솔질을 하도록 하였다. 칫솔질 전과 후의 치태지수를 Ainamo와 Bay의 Visible Plaque Index를 변형한 방법으로 측정하여 그 전후 차이를 비교하였다. 음파 칫솔이 61.79%(${\pm}7.95%$), 일반 칫솔은 69.19%(${\pm}10.08%$)의 치태 감소율을 나타내어 통계학적으로 유의한 차이를 보였다(p<0.05). 음파 칫솔의 경우 남자에서 63.07%(${\pm}8.64%$), 여자에서 60.39%(${\pm}7.30%$)의 치태 감소율을 나타냈고(p>0.05), 일반 칫솔의 경우 남자에서 69.33%(${\pm}10.14%$), 여자에서 69.03%(${\pm}10.55%$)의 치태 감소율을 나타내어 통계학적으로 유의한 차이는 보이지 않았다(p>0.05). 연령에 따른 치태 제거 효과는 실험 대상자들의 연령범위에서 통계학적으로 유의할만한 연관성이 없었으며 (p>0.05), 음파 칫솔의 경우와 일반 칫솔의 경우에도 통계학적으로 유의할 만한 연관성이 없었다(p>0.05). 본 연구에서, 고정식 교정 장치를 부착한 소아 청소년기 환자에서 일반 칫솔의 치태 제거 효과가 음파 칫솔의 치태 제거효과보다 컸다(p<0.05).
