  • Although the inferior oblique (IO) muscle is positioned considerably deep in the orbit, transconjunctival lower lid blepharoplasty may affect it and transient or permanent IO muscle palsy might result. Therefore diplopia should be explained before cosmetic blepharoplasty performed with transconjunctival approach.

Numerical Investigation of Smoke Behavior in Rescue Station for Tunnel Fire (철도터널 화재 시 구난역 내의 연기거동에 대한 수치해석 연구)

  • The present study deals with numerical investigation for smoke behavior in rescue station by using the commercial CFD code (FLUENT Ver 6.3). With the use of the MVHS(Modify Volumetric Heat Source) model modified from the original VHS(Volumetric Heat Source) model, a 10 MW mode was adopted for simulation and the MVHS model can describe the generation of product and the oxygen consumption at the stoichiometric state. In addition, the present simulation includes the species conservation equations for the materialization of heat source and the estimation of smoke movement. From the results, the smoke flows are moving along the ceiling because of thermal buoyancy force and as time goes, the smoke gradually moves downward at the vicinity of the entrance. Moreover, without using ventilation, it is found that the smoke flows no longer spread across the cross-passages because the pressure in the non-accident tunnel is higher than that in the accident tunnel.

  • In this study, disease surveillance was performed to monitor the prevalence of viral haemorrhagic septicaemia virus (VHSV) and red seabream iridovirus (RSIV) in olive flounder, Paralichthys olivaceus in 2015. The fish samples were collected in March (60 farms), May (55 farms) and July (52 farms) from different farms in Jeju. Reverse transcription polymerase chain reaction (RT-PCR) (VHSV) or PCR (RSIV) results showed that VHSV detected in 2 farms, but RSIV has not been detected in any farms. The sequences of the nucleocapsid protein (N) and glycoprotein (G) gene of the 2 VHSV isolates were successfully sequenced. Phylogenetic analysis was included VHSV isolates reported here together with a representative VHSV isolates available in GenBank. Phylogenetic analysis indicated that most of Korea VHSV isolates were closely related to the Japan and China genotype IVa which is clearly distinct from the North American genotype IVb.

A Study on Ventilation Effects on Smoke Behavior in Rescue Station for Tunnel Fires (철도터널 화재시 구난역 내의 연기거동에 미치는 배연효과에 관한 연구)

  • The present study investigates the ventilation effects on smoke spreading with the rescue stations. Experiments for tunnel fires were carried out for n-heptane pool em at different fire locations, and the heat release rates (HRR) were obtained by addition, using the commercial code (FLUENT), the present article presents numerical results for smoke behavior in railway tunnels with rescue station, and it uses the MVHS (Modified Volumetric Heat Source) model for estimation of combustion products resulting from the fire source determined from the HRR measurement. As a result, it is found that smoke propagation is prevented successfully by the fire doors located inside the cross-passages and especially, the smoke behavior in the accident tunnel can be controlled through the ventilation system because of substantial change in smoke flow direction in the cross-passages.

Congenital Aortic Valvular Insufficiency Caused by Abnormal Valvular Structures in a Labrador Retriever Dog (래브라도 리트리버종 개의 비정상 판막 구조에 의한 선천성 대동맥 판막 부전)

  • A 10-month-old intact male Labrador Retriever dog was referred with the primary complaint of exercise intolerance, especially after vigorous exercise. Physical examination revealed split S1 and grade III/VI diastolic regurgitant murmur at the left apex and base, respectively. ECG finding was normal sinus rhythm at rest, but supraventricular tachycardia with bundle branch blocks after exercise. Thoracic radiography revealed dilated ascending aorta with normal range of cardiac silhouette (VHS 10.2). Echocardiography revealed abnormal valvular structures just above the aortic valvular cusps causing aortic regurgitation with a reduction of left ventricular ejection fraction (LVEF). Based on those findings, the case was diagnosed as congenital aortic regurgitation caused by abnormal valvular structures. The dog was managed with diltiazem and exercise restriction. This is a rare case of aortic deformity in dogs.

An Assessment of Vertebral Left Atrial Size in Relation to the Progress of Myxomatous Mitral Valve Disease in Dogs

  • Left atrial enlargement (LAE) is an important diagnostic factor in dogs with myxomatous mitral valve disease (MMVD). It is associated with the onset of congestive heart failure (CHF). Recently, a new radiographic left atrial measurement called vertebral left atrial size (VLAS) was introduced. This can be considered as a left atrial enlargement above 2.3. It appears to be related to the severity of MMVD. However, serial changes in VLAS in relation to disease progression and improvement in patients have yet to be studied. This study aims to assess the value of VLAS as a left atrial size monitoring indicator by examining correlations with VHS, LA/Ao ratio and LVIDDN, and comparing serial changes in dogs. A total of 126 dogs were studied with their owners' consent. The dogs were classified into four MMVD groups (Control, B1, B2, C-D) following the ACVIM Guideline by performing a physical examination, radiography and echocardiography. Besides, 24 and 17 dogs were reevaluated to compare values in relation to the progression and improvement of MMVD. VLAS showed significant increase according to the progress of the MMVD stage. This was the same in the Maltese breed group. A strong positive correlation was found between LVIDDN, VHS, LA/Ao ratio, and VLAS. The results of this study found VLAS to be significantly different according to left atrium size, and there was a correlation between disease progression and VLAS levels in each dog. Therefore, VLAS may be used to detect changes in left atrium size as an additional monitoring index of MMVD.

  • Fetal bovine serum (FBS) is an essential element of cell growth and can also affect the viral replication. In this study, we tried to find out whether FBS concentration affects the viral infectivity titer of IHNV and IPNV. EPC cells were suspended with MEM supplemented with various concentrations of FBS (MEM0, MEM2, MEM5 and MEM10) and cultured in 96-well plate. Each virus was 10-fold diluted virus and inoculated in 96-well plate. The highest infectivity titer of IHNV was $10^{7.88}\;TCID_{50}/mL$ in 96-well plate using MEM5 and the lowest one was $10^{7.30}\;TCID_{50}/mL$ in 96-well plate using MEM10. The highest infectivity titer of IPNV was $10^{7.47}\;TCID_{50}/mL$ in 96-well plate using MEM5 and the lowest one was $10^{6.97}\;TCID_{50}/mL$ in 96-well plate using MEM10. This study showed that not only 0% FBS but 10% FBS leads low infectivity titer of IHNV and IPNV. Therefore, it is considered that the desirable concentration of FBS is 2% or 5% for measurement of infectivity titer of IHNV and IPNV.

Assessment of radiographic left atrial dimension and C-reactive protein in dogs with myxomatous mitral valve disease

  • Radiographic left atrial dimension (RLAD) is a valuable metric for assessing left atrial enlargement in dogs. While there have been studies on the use of RLAD and the increase in C-reactive protein (CRP) levels based on heart disease stages, there has been no prior research on the correlation between RLAD and CRP. In this study, the objective was to investigate the relationship between the rise in RLAD as myxomatous mitral valve disease (MMVD) stages advance and the increase in CRP levels with MMVD stage progression. In this study, a total of 30 small-breed dogs were included as subjects. These dogs were diagnosed with MMVD at the American College of Veterinary Internal Medicine (ACVIM) stage B1 or B2, or stage C, based on a comprehensive assessment including physical examination, thoracic radiography, and echocardiography. Measurements of VHS and RLAD were compared to assess any significant differences. There were significant differences in RLAD between dogs with MMVD ACVIM stage B1 and those with stage C. The monocytes and CRP levels showed significant differences between ACVIM stage B1, B2 and ACVIM C. Additionally, a significant correlation was observed between the RLAD and VHS measurements. This underscores the notable association between MMVD stage advancement and elevated monocyte and CRP levels. The RLAD scores exhibited a significant difference among dogs with ACVIM stages B1, B2, and C, and significant variations were also observed in monocyte and CRP levels. These results suggest that monocyte and CRP levels may be a valuable diagnostic indicator for heart disease in dogs during the diagnostic evaluation.

Cementless Total Hip Arthroplasty Using Ceramic Femoral Head on Cross-Linked Ultra-High-Molecular Weight Polyethylene Liner in Patients Older than 65 Years: Minimum Five-Year Follow-Up Results (세라믹 대퇴 골두 및 교차결합 초고분자량 폴리에틸렌 라이너를 이용한 65세 이상 무시멘트형 인공 고관절 전치환술: 최소 5년 중기 추시 결과)

  • Purpose: To evaluate the utility of ceramic-on-polyethylene articular bearing surface when cementless total hip arthroplasty is performed in patients older than 65 years through an analysis of the minimum five-year follow-up results using the ceramic femoral head and cross-linked polyethylene liner. Materials and Methods: From March 2010 to September 2012, 51 patients (56 hips) who were older than 65 years were enrolled in this retrospective study. The mean age at surgery was $70.9{\pm}5.1years$ old. A clinical assessment was analyzed using the Harris hip score. For the radiographic assessment, the cup inclination and anteversion, stem alignment, and wear amount were measured. The postoperative complications were also determined. Results: The mean Harris hip score was improved from preoperative 48 points to postoperative 87 points (p<0.05). The mean cup inclination was $40.9^{\circ}{\pm}6.4^{\circ}$ and the mean cup anteversion was $20.3^{\circ}{\pm}8.1^{\circ}$. The mean cup anteversion of the elevated liner-used group (16 cases) was $14.3^{\circ}{\pm}7.9^{\circ}$ and the mean cup anteversion of the neutral liner used group (40 cases) was $22.4^{\circ}{\pm}9.1^{\circ}$ (p<0.05). The mean stem alignment angle was $0^{\circ}$ (range, varus $4^{\circ}$-valgus $4^{\circ}$). The mean linear wear amount was $0.458{\pm}0.041mm$ and the average annual linear wear rate was $0.079{\pm}0.032mm/yr$. Six cases (10.7%) of intraoperative periprosthetic femoral fractures were encountered. Conclusion: Based on these results, the use of a ceramic-on-polyethylene articular bearing surface in elderly patients with cementless total hip arthroplasty is beneficial. On the other hand, careful effort is needed to prevent intraoperative periprosthetic femoral fractures.

Multiple Spinal Revision Surgery in a Patient with Parkinson's Disease

  • Parkinson's disease (PD) patients frequently have several spinal deformities leading to postural instabilities including camptocormia, myopathy-induced postural deformity, Pisa syndrome, and progressive degeneration, all of which adversely affect daily life activities. To improve these postural deformities and relieve the related neurologic symptoms, patients often undergo spinal instrumentation surgery. Due to progressive degenerative changes related to PD itself and other complicating factors, patients and surgeons are faced with instrument failure-related complications, which can ultimately result in multiple revision surgeries yielding various postoperative complications and morbidities. Here, we report a representative case of a 70-year-old PD patient with flat back syndrome who had undergone several revision surgeries, including anterior and posterior decompression and fusion for a lumbosacral spinal deformity. The patient ultimately benefitted from a relatively short segment fixation and corrective fusion surgery.
