This study was conducted to evaluate the methods used in nursing procedures for infection control. Skilled nursing procedures are related to prevention of hospital infection. The sample consisted of 301 nurses’ response to the questionnaire. Data were collected from Feb. to March, 1993 from 35 hospitals located in five major cities. The findings of the study are as follows : 1) Rotatively unsafe nursing procedures were reported for preperation of IV therapy, change of IV site and aseptic dressing on IV site, hand wash-ing and use of paper towels, use of sterile urine bottle with indwelling urinary catheter, management of dressing cart, disinfection of transfer forcep, ambu respirator, laryngoscope, humidifier and handling of incubator. 2) Relatively safe nursing procedures were reported for management of suction tube, marking for contaminated materials and waste collection. The rate of participation in education programs for infection control by the nurses was higher than in other research results. 3) Further study on procedures for IV site in-fusion and care of dressing cart are recommended.
The assessment of sodium intake is complex because of the variety and nature of dietary sodium. This study intended to develop a dish frequency questionnaire (DFQ) for estimating the habitual sodium intake and a short DFQ for screening subjects with high or low sodium intake. For DFQ112, one hundred and twelve dish items were selected based on the information of sodium content of the one serving size and consumption frequency. Frequency of consumption was determined through nine categories ranging from more than 3 times a day to almost never to indicate how often the specified amount of each food item was consumed during the past 6 months. One hundred seventy one adults (male: 78, female: 93) who visited hypertension or health examination clinic participated in the validation study. DFQ55 was developed from DFQ112 by omitting the food items not frequently consumed, selecting the dish items that showed higher sodium content per one portion size and higher consumption frequency. To develop a short DFQs for classifying subjects with low or high sodium intakes, the weighed score according to the sodium content of one protion size was given to each dish item of DFQ25 or DFQ14 and multiplied with the consumption frequency score. A sum index of all the dish items was formed and called sodium index (Na index). For validation study the DFQ112, 2-day diet record and one 24-hour urine collection were analyzed to estimate sodium intakes. The sodium intakes estimated with DFQ112 and 24-h urine analysis showed $65\%$ agreement to be classified into the same quartile and showed significant correlation (r=0.563 p<0.05). However, the actual amount of sodium intake estimated with DFQ112 (male: 6221.9mg, female: 6127.6mg) showed substantial difference with that of 24-h urine analysis (male: 4556.9mg, female: 5107.4mg). The sodium intake estimated with DFQ55 (male: 4848.5mg, female: 4884.3mg) showed small difference from that estimated with 24-h urine analysis, higher proportion to be classfied into the same quartile and higher correlation with the sodium intakes estimated with 24-h urine analysis and systolic blood pressure. It seems DFQ55 can be used as a tool for quantitative estimation of sodium intake. Na index25 or Na index14 showed $39\~50\%$ agreement to be classified into the same quartile, substantial correlations with the sodium intake estimated with DFQ55 and significant correlations with the sodium intake estimated with 24-h urine analysis. When point 119 for Na index25 was used as a criterion of low sodium intake, sensitivity, specificity and positive predictive value was $62.5\%,\;81.8\%\;and\;53.2\%$, respectively. When point 102 for Na index14 was used as a criterion of high sodium intake, sensitivity, specificity and positive predictive value were $73.8\%,\;84.0\%,\;62.0\%$, respectively. It seems the short DFQs using Na index 14 or Na index25 are simple, easy and proper instruments to classify the low or high sodium intake group.
서 론 : 방광요관역류로 인한 신조직의 손상은 소아에서 말기신부전 및 신성 고혈압의 중요한 원인중 하나이며 방광요관역류에 의한 신장변화는 조기에 발견될 경우 그 예방이 가능하다. 현재까지는 주로 반복적이며 항생제 치료에 잘 조절되지 않는 요로감염에서 방광요관역류를 의심하고 진단을 위한 방사선학적 진단법을 시행하였으나, 단발성의 요로감염 환아에서도 역류가 진행되어있는 경우를 경험하게 되므로, 요로감염이 있는 환아에서 방광요관역류를 조기에 의심할 수 있는 진단방법이 요구되고 있다. 본 연구에서는 요로감염이 있는 환아를 대상으로하여 방광요관역류가 있는 군과 방광요관역류가 없는 군에서 뇨중 ${\beta}_2$-microglobulin(${\beta}_2$-MG)을 측정하여 의미있는 차이가 있는지를 알아보고 그 차이가 요로감염이 진행중인 환아에서 방광요관역류의 존재의 지표로 이용될 수 있는지 여부를 확인하고자 하였다. 대상 및 방법 : 요로감염으로 처음 내원하여 치료를 시작하였던 57명의 환아를 대상으로 하였으며, 전례에서 방사선학적인 검사(복부초음파, DMSA scan 및 VCUG)를 시행하여, 단순 요로감염환아군과 요로감염이 있으면서 방광요관역류가 발견되었던 환아군으로 구분하였고, 각 군간의 무작위 추출 뇨중 ${\beta}_2$-MG치와 뇨중 creatinine치의 비율로 그 배설율을 비교하였다. 대상환아중 신독성 약물을 사용하였던 환아 및 ${\beta}_2$-MG의 생성 및 배출에 영향을 미칠 수 있는 질환을 앓고있는 환아는 본 연구에서 제외하였다. 결 과 : 전체 대상환아 57명중 남녀비는 28:29였고 평균연령은 $4.27{\pm}4.24$세 였다 요로감염이 배양검사상 확진된 례가 44례 였고, 임상적으로 요로감염이 의심되었던 례가 13례 였으며 배양된 균주는 E.coli($43.9\%$)와 Enterococcus($15.8\%$)가 가장 많았다. 방광요관역류는 12례($21.1\%$)에서 발견되었으며 평균연령은 $2.97{\pm}2.82$세로 단순 요로감염 환아군의 평균연령 $4.62{\pm}4.51$세에 비하여 낮은 경향을 보였고, 남녀비는 3:1로 남아가 많았다. 무작위 추출뇨중 평균 ${\beta}_2$-MG치는 단순 요로감염군 $0.19{\pm}0.16mg/dL$, 방광요관역류군 $2.2{\pm}5/91,g/dL$로 두군간에 의미있는 차이를 보였으며(P=0.03), ${\beta}_2$-MG/creatinine비는 단순 요로감염군은 $3.93{\pm}3.44$, 방광요관역류동반군은 $32.41{\pm}25.7$로 두군간에 의미있는 차이를 나타내었다(P=0.007). 두군간의 배양된 균주 및 균배양 양성율과 혈청 크레아틴치의 의미있는 차이는 없었다. 결 론 : 요로감염환자에서 무작위 추출뇨중 ${\beta}_2$-MG치와 배설율은 방광요관역류의 존재를 조기에 의심할 수 있게하는 검사법이며, 기존의 24시간뇨를 이용하여 측정하는 방법보다 간편하고 비용이 저렴하여 방광요관역류의 검색진단법으로서 유용하리라 생각되며 소아 고혈압과 말기신부전의 원인 질환중 많은 부분을 차지하고 있는 방광뇨관역류의 조기진단 및 조기치료로 그 이행율을 감소시키는데 많은 기여를 할 수 있을 것으로 기대된다.
Objective : The purpose of this study is to develop a personal health record(PHR) application of atopic dermatitis in Korean Medicine(KM). Methods : We have identified the items necessary to provide an PHR application that helps to record and manage the symptoms of an atopic dermatitis in KM. We also derived the symptom collection process and method and applied it to the application. Results : In this study, the types of symptoms collected for atopic dermatitis were derived. Symptoms include daily check, stool/urine/sleep, daily emotion, meal management, symptom photographs, SCORAD, quality of life, progress check, original symptom, pediatric health check, weakness check, and subjective symptoms. The recording cycle can be divided into the first, daily, weekly, specific day, and subjective. We developed the PHR application of atopic dermatitis in KM by deriving the type of symptoms and symptom recording process. The app organized menus into dashboards, checklists, daily checks, and health records. Conclusions : We developed a PHR application for atopic dermatitis by deriving symptom collection items of atopic dermatitis and developing symptom collection process and collection technique. The app does not make an accurate diagnosis of atopic dermatitis symptoms, but it helps facilitate symptom collection and helps to identify or predict a person's health condition. It can also be used for medical treatment through sharing symptoms with Korean medicine. Patients are able to communicate in both directions on a daily, weekly, self-aware basis, at the request of a doctor, to record their symptoms and use them for medical purposes. If the doctor asks for a progress check to refer to the next examination, it can be written through the system. This manages atopic dermatitis in daily life and can be used in the clinical field.
The purpose of this study was to assess the nutrition education program for older adults at rural area. Eighty three adults (mean age ;55.6$\pm$11.9) participated in nutrition education program. Data collection includes serum and urine collections for health state. The impact of nutrition KAP (knowledge attitude and practice) and retention was examined among participants who were assessed at program entry 3 times of education and 1 month of follow-up. The results were as follows : The participants were composed 52 persons of under 60-year-old and 31 persons over 60 years. Of the 83 participants, 60 were females and 23 were males. Serum albumin and calcium content of females (3.5$\pm$1.2g/dl, 7.5$\pm$3.9mg/dl) were significantly lower than males(4.7$\pm$0.9g/dl, 11.2$\pm$4.6mg/dl). Serum TG and cholesterol contents of females (143$\pm$117mg/d1, 205$\pm$31mg/d1) were higher than males (101$\pm$86mg/dl, 192$\pm$19mg/dl) but not significantly different. The calcium (13.4$\pm$9.0mg/dl) and urea (743$\pm$440mg/dl) contents of urine excretion were also significantly lower in female than in males (19.7$\pm$11mg/dl, 1041 $\pm$535mg/dl). Via the question of nutrition attitude, the participants improved significantly between pre and post education (the score of pre, post and after 1 month, 6.7$\pm$1.8, 7.2$\pm$1.5, 7.2$\pm$1.8), and were also attributed to better personal feeling health by modified CMI test but food habit was not significantly improved. As the differences of age and gender groups were compared, under 60-year-old group had better nutrition practice score than over 60 years group.
A simultaneous detection and quantification method for determining the Phenylalkylamine derivatives, such as methamphetamine (MA), amphetamine (AM), 3,4-methylenedioxymethamphetamine (MDMA), 3,4-methylenedioxyamphetamine (MDA), ketamine (KT), norketamine (NKT), phentermine (PT), fenfluramine (FFA) and phenmetrazine (PM), in oral fluid was developed and validated according to international guidelines. The validated method was applied to actual oral fluid samples collected from drug abuse suspects. The recovery of phenylalkylamines from oral fluid collection devices was also assessed. Oral fluid specimens from 20 drug abuse suspects submitted by the police were collected using Salivette$^{TM}$, Quantisal$^{TM}$ or direct expectoration. The samples were screened using a biochip array analyzer. For confirmation, the samples were analyzed by GC-MS in selected-ion monitoring (SIM) mode after extraction using automated SPE with a mixed-mode cation exchange cartridge and derivatization with trifluoroacetic anhydride (TFAA). The results from the immunoassay were consistent with those from GC-MS. All the oral fluid samples gave positive results for MA, AM, PT and/or PM. The detection of phenylalkylamines in oral fluid can provide a better indication of recent use than urine or hair. Therefore, the oral fluid specimen was useful for demonstrating phenylalkylamines abuse in the driving under the influence of drug (DUID) as an alternative specimen for urine.
The changes of acid-base status in vitro of the venous blood for 24 hours in ten Korean native goat were investigated. The acid-base parameters were measured within ten minutes after collection of the blood, and every hour during the first six hours and finally after twenty four hours of storage. Blood samples were stored at two different temperatures ($0-4^{\circ}C$ and $21-24^{\circ}C$). Twelve goats were induced acute acid-base disturbances by intravenous infusion of either hydrochloric acid or sodium bicarbonate and inhalated with $CO_2$ gas mixture (20% $CO_2$, 80% $O_2$) or hyperventilation were performed by means of respirator. The results were as follows; 1. Blood samples could be stored during the first two hours in ice water ($0-4^{\circ}C$) and one hour at room temperature without significant changes in pH. The magnitudes of changes were similar to those of cow, and lower than those of men and dogs. 2. The mean values of acid-base parameters in normal goat were arterial pH, 7.40; $P_{CO_2}$, 35.4mmHg; $HCO_3{^-}$, 21.8mEq/L. 3. Both the base excess and the bicarbonate showed high correlation (r=0.99) during the metabolic disturbance and were represented as $B.E.=1.38\;HCO^-{_3}-29.7$. 4. The slope of blood buffer curve obtained from the in vivo experiment was 16.3mEq/L/pH. 5. The magnitudes of changes in hydrogen ion concentration per unit change of $P_{CO_2}$ were 0.8nM/mmHg in hypercapnia and 1.0nM/mmHg in hypocapnia. 6. The ranges of acid-base parameters in normal goat urine were pH, 6.0-8.1; $P_{CO_2}$, 42-61mmHg; $HCO_3{^-}$, 2-110mEq/L. The concentration of potassium was higher (60-200mEq/L), and that of sodium was lower (8-70mEq/L) than those of human urine.
This study was started for the purpose of providing the basic data for continous managment of kidney transplantation patients after discharge. This study was conducted on 180 patients who received renal transplants at three hospital( B, M, P) pusan, korea. The data collection was done for june 1, to August 31, 2002. General characteristics, renal transplantaton characteristics, physical characteristics, the level of compliance and the degree of educational demand were done by the number and percentage, the mean, standard deviation. The level of compliance and educational demand followed by the characteristics of general and kidney transplantation were analyzed by t-test and ANOVA. The result were as fallows; 1. Man was higher than woman such as 60.0%, Mean age was 42.5 years old, Average total duration of after operation was 5.5 years. 2. Cases of systolic blood pressure over 140mmHg were 10.0%, cases of diastolic blood pressure over 90mmHg were 22.8% and obesity factor in BMI was 15.6%. The person who daily water intake amount is 5000cc was 0.6%, the case that daily urine output is below 1000cc was 8.9%, and the case that urine output is zero was 0.6%. 3. The mean score of compliance was 77.47 point, The score in medication part was highest such as 4.67 point, that in stress situation was lowest such as 3.50 point. 4. The average score of educational demand was 154.02 point, and physical state part was 4.36 points highest, activation part was 3.48 points lowest. As a role of nurse Confirmation of compliance is very important encourage to make good through regular hospital visitation, point out the noncompliance part and then increase compliance of renal transplantation patient As well there will be maintain the normal kidney function to satisfy educational demand through continous education.
Korean safflower seed has been known to have healing effects on both bone fracture and osteoporosis. On the base of such a notice, this experiment was carried out to explore the effects of safflower seed on bone formation and bone repair. The toxicity test and the effect of Korean safflower seed were evaluated with 60 rats, 3-month old. Forty Sprague-Dawley rats composed of 20 male and 20 female were underwent unilateral tibial defect and then fastened with unilateral fixators. The operated rats were divided into two groups depending on the composition of diet, such as positive control group fed normal diet(C-OP group) and safflower seed group fed 30% of safflower seed diet and 70% of normal diet(S-OP group). Another 20 rats without operation were maintained, each 10 rats were fed either normal diet or 30% of safflower seed diet and 70% of normal diet, and observed the toxicity of safflower seed by measuring weight and urine parameters. Postoperative radiography were taken once in 2 weeks to evaluate callus formation for operated groups and blood collection via heart puncture were carried out once in 3 weeks for 3 groups. The concentration of Ca and Pi in serum were measured using both auto Kit and $^{31}$ P Nuclear Magnetic Resonance(NMR). At present study, no toxic effect was observed from both weight increment and urine index after feeding the safflower seed diet. The comparison of the radiography between C-OP and S-OP group were showed that the safflower seed diet appeared to stimulate the formation of callus in the rat. The ratio of Ca/P in serum was low in S-OP group compared to C-OP group with the auto Kit, but there were no significant differences between two groups (p < 0.05). In addition, the variations of Pi values in NMR examination were also confirmed based on the result of auto Kit. Purpose: Although the 24-hours urinary copper excretion is useful for the diagnosis of Wilson disease (WD), there are practical difficulties in the accurate and timed collection of urine samples. The purpose of this study was to verify if the spot morning urinary Copper/Zinc (Cu/Zn) ratio could be used as a replacement parameter of 24-hours urinary copper excretion in the diagnosis of WD. Methods: A cross-sectional study was conducted at the Department of Pediatric Gastroenterology and Nutrition, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh, from June 2019 to May 2021 on 67 children over three years of age who presented with liver disease. Twenty-seven children who fulfilled the inclusion criteria for WD were categorized into the test group, and the remaining forty children were considered to have non-Wilsonian liver disease and were categorized into the control group. Along with other laboratory investigations, spot morning urinary samples were estimated for the urinary Cu/Zn ratio in all patients and were compared to the 24-hour urinary copper excretion. The diagnostic value of the Cu/Zn ratio was then analyzed. Results: Correlation of spot morning urinary Cu/Zn ratio with 24-hours urinary copper excretion was found to be significant (r=0.60). The area under ROC curve with 95% confidence interval of morning urinary Cu/Zn ratio measured using 24-hours urine sample was 0.84 (standard error, 0.05; p<0.001). Conclusion: Spot morning urinary Cu/Zn ratio seems to be a promising parameter for the replacement of 24-hours urinary copper excretion in the diagnosis of WD.

일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
