A Case of Pyungwijiyutang-gamibang Diagnosed as Constipation due to Stagnation of Eum with Ulcerative Colitis (음결(陰結)로 변증한 궤양성 대장염 환자의 평위지유탕가미방(平胃地楡湯加味方) 치험 1례)

  • Objectives : To see the effect of herbal medicine on ulcerative colitis by diagnosing ulcerative colitis as constipation due to stagnation of eum. Methods : We report here on a case of a patient who had abdominal pain, rectal urgency, diarrhea and hemafecia as chief complaints in December 2004 and was diagnosed with ulcerative colitis on January 20, 2007. The patient was treated with Pyungwijiyu-tang till Hwanggi was added on March 23rd of the same year. No change was made to the prescription since then. Results : After 14 weeks of herbal treatment the chief complaints decreased favorably and had no relapse. Conclusions : In this case, we recognized that the herbal medication could be effective for the clinical symptoms of ulcerative colitis. It was effective in decreasing or maintaining the symptoms of patients and in improving quality of life.

  • Pulmonary complications of ulcerative colitis are relatively uncommon and may present as a variety of disorders. Ulcerative colitis-related interstitial lung disease is extremely rare. There are a few case reports of nonspecific interstitial pneumonia in ulcerative colitis worldwide but none in Korea. We report a patient with ulcerative colitis related biopsy-proven nonspecific interstitial pneumonia, who responded to prednisolone (1 mg/kg) and mesalazine therapy.

Effects of Paeoniae Radix, Glycyrrhiza Uralensis and Jakyakgamcho-tang Treatment on Ulcerative Colitis Animal Model: Including Changes in Metabolites (작약, 감초 및 작약감초탕의 투여가 궤양성 대장염 동물 모델에 미치는 영향: 대사산물 변화를 포함하여)

  • Objectives To investigate the effect of Jakyakgamcho-tang (JGT) on Dextran sulfate sodium (DSS) induced ulcerative colitis. Methods Experimental animals were divided into six groups; group 1, normal group; group 2, DSS-induced colitis group; group 3, 5-aminosalicylic acid (5-ASA)-treated group; group 4, Glycyrrhiza Uralensis (Gamcho, GC)-treated group; group 5, Paeoniae Radix (Jakyak, JY)-treated group; group 6, JGT-treated group. Inflammatory cytokines and their metabolites were detected. Result In the JGT group, the levels of tumor necrosis factor alpha (TNF-α), prostaglandin E2 (PGE2) were significantly decreased, whereas that of Interleukin-10 (IL-10) were significantly increased. In addition, the metabolite profile changed in the JGT group with most metabolites increasing. Conclusion This study demonstrates that the therapeutic potential of JGT in ulcerative colitis. Further studies should be conducted to confirm our findings.

Therapeutic Effects of Gaejigayonggolmoryo-tang on Dextran Sodium Sulfate-induced Ulcerative Colitis in Mice (궤양성 대장염 유발 생쥐에 투여한 계지가용골모려탕의 치료효과)

  • Objectives: The aim of this study was to investigate the effects of Gaejigayonggolmoryo-tang (GYT) on ulcerative colitis induced by dextran sodium sulfate (DSS) in mice. Methods: Colitis was induced by free drinking of 5% DSS in six-week-old male ICR mice. The experimental groups were the sample group, the control group, and the normal group. The sample group was treated with GYT for three days after being was given 5% DSS for five days. The control group was given water, instead of GYT, for three days after the five days of 5% DSS. The normal group was untreated (not given 5% DSS), for comparison purposes. Results: Cellular experiments showed that GYT inhibits the expression of the inflammatory enzymes COX-2 and iNOS, and the production of NO. Based on the primary cellular experiments, the effects of GYT on ulcerative colitis induced by DSS of mouse tissues were investigated. GYT reduced tissue damage and apoptosis by inhibiting the expression of the inflammatory enzymes $NF-{\kappa}B$ p65, COX-2, and iNOS. In the cellular experiment, GYT was more effective in inhibiting the expression of COX-2 than in inhibiting the expression of iNOS. GYT was evidently effective in tissues in inhibiting the expression of COX-2. Conclusions: Based on the results here, GYT may have therapeutic effects on ulcerative colitis induced by DSS. GYT is worthy of research and development as a COX-2 inhibitor and a potential drug for inflammatory bowel diseases from natural products. Further investigations for exact mechanisms will be needed.

Current Status and Trends in Inflammatory Bowel Disease Surgery in Korea: Analysis of Data in a Nationwide Registry

  • Purpose: Inflammatory bowel disease (IBD) in Korea has been increasing in recent years, but accurate statistics about operations for IBD are lacking. The purpose of this study was to investigate the trends and current status of IBD surgeries in Korea. Methods: Using a national database from the Korea Health Insurance Review and Assessment Service, we analyzed data from patients who underwent surgery for Crohn disease and ulcerative colitis from January 2009 to October 2016. Results: The mean number of patients who underwent surgery for Crohn disease was 791.8 per year. Colorectal surgery, small bowel surgery, and anal surgery were performed fairly often (31.2%, 29.4%, 39.4%, respectively), and laparoscopic surgery continued to increase, recently exceeding 30%. About 50% of Crohn patients used biologics before and after surgery, and those patients also underwent a relatively high rate of anal surgeries (44.2%). The mean number of patients who underwent surgery for ulcerative colitis was 247.6 per year. Colorectal surgery accounted for more than half of all operations, and laparoscopic surgery has been increasing rapidly, having been performed in about 60% of patients in recent years. The incidence of colorectal cancer in patients with ulcerative colitis was very high and increased rapidly during the study period, reaching about 80%. Conclusion: The number of patients undergoing laparoscopic surgery for IBD in Korea has increased significantly. Biologics are actively used by patients with Crohn disease, with a high proportion of anal surgeries required. Many of the surgical indications for ulcerative colitis have shifted into colorectal cancer.

Protaetia brevitarsis extract ameliorates the severity of DSS-Induced Ulcerative Colitis in vivo (굼벵이 추출물의 DSS유도 궤양성 대장염 조절효능 탐색)

  • Protaetia brevitarsis (PB) extracts has been traditionally used as medicinal stuff to treat blood stasis, occlusion of menstruation, tetanus and liver cancer in Asian countries (Korea, Japan, China, Taiwan, India and Myanmar). Especially, Donguibogam, which is traditional korean medicinal book, described the PB extracts as traditional medicine to treat hepatic diseases and vascular disorders. The PB extracts has been considered as highly nutritional food. The major constituents of PB extracts are rich in protein, healthy fats, iron, calcium. Recent studies announced that PB extracts has hepatoprotective effect and anti-microbacterial effect. However, the effect of PB on ulcerative colitis has not been uncovered yet. The aim of this study was to examine the anti-inflammatory effect of PB extracts in dextran sulfate sodium (DSS)-induced colitis mice model. Cytotoxicity of PB was determined by MTT assay and the antiinflammatory effect of PB extract was investigated by measuring nitric oxide (NO) production. PB extracts did not show any cytotoxicity. AIso, PB extracts supressed NO production in LPS-stimulated mice peritoneal macrophages. To determine whether PB could be an effective treatment on ulcerative colitis, DSS was administered in BALB/c mice for 10 days. PB extract significantly improved the clinical signs of DSS-induced UC, including body weight loss, colon length shortening, and disease activity index increase, with histological markers of colon injury. These findings indicated the possibility of PB as a therapeutic agent on ulcerative colitis.

A Case Report of Chengsim Yeunja-Tang for Ankylosing Spondylitis associated with Severe Ulcerative Colitis (청심연자탕(淸心蓮子湯)으로 치료하여 호전된 강직성 척추염을 동반한 중증 궤양성 대장염 1례)

  • A 33 year-old male who had been diagnosed with severe ulcerative and ankylosing spondylitis was treated and observed. Even though he had been treated with western medicine, he still complained of severe diarrhia, hemafecia, abdominal pain, general weakness, mild fever($37.8^{\circ}C$) and lower back pain. On radiologic examination, the lumbar view revealed the sacroliac joint space are irregular with adjacent bony sclerosis on both sides. He was treated with Chengsim Yeunja-Tang. Having been treated for 99 days, severity of ulcerative colitis changed from severe to mild ade, lower back pain reduced from nine to three, and all other symptoms improved.

Fecal Microbiota Transplantation to Patients with Refractory Very Early Onset Ulcerative Colitis

  • Recently, fecal microbiota transplantation (FMT) has been attracting attention as a possible medical treatment of ulcerative colitis (UC). A randomized controlled trial of FMT for children with UC is currently underway. Therapeutic effects of FMT for adults with UC remain controversial. We report two cases of early-onset UC in children. A patient was diagnosed with UC at age 1-year 9-month and underwent FMT at age 2-year 3-month. He attained clinical remission for three weeks after FMT, but then relapsed at four weeks, ultimately undergoing a total colectomy. Another child was diagnosed with UC at 2-year 10-month and she underwent FMT at age 5 years. She has remained in clinical remission following FMT for 24 months and her UC has been maintained without complications with tacrolimus and azathioprine. We report that FMT for early-onset UC appears to be safe and potentially effective.

The Anti-Inflammatory Effect of Duchi extract on Ulcerative Colitis (궤양성 대장염 유발 생쥐에서 두시추출물의 항염증 효과)

  • Objective The purpose of this study is to learn the anti-inflammatory effect of Douchi on ulcerative colitis (UC). Methods Three-month-old mice were divided into 4 groups as follows: control group (Ctrl), UC induced group (UCEG), Pentasa treated group after inducing UC (OPTG), and Douchi treated group after inducing UC (FGTG). NF-κB, p-IκB, iNOS, COX-2 were observed by immunohistochemistry and Masson trichrome, PAS, and Phloxine-tartrazine staining were used to observe histochemical changes. Results Inflammation indicators of the large intestine were significantly lower in FGTG than in the UCEG and OPTG. Also, indicators involved in pulmonary alveolar formation were significantly higher in the FGTG than in the UCEG and OPTG. Conclusions The result of this study suggests that Douchi extract was effective in ulcerative colitis and helped in the formation of alveolar. This result suggests that the lungs and colon are correlated.

The Effects of Soyumjungjang-tang on DSS-Induced Ulcerative Colitis in Mouse (소염정장탕(消炎整腸湯)이 DSS로 유발(誘發)된 생쥐의 궤양성 대장염에 미치는 영향(影響))

  • Objectives : This study was carried out to investigate the effects of Soyumjungjang-tang(SJT) on the experimental ulcerative colitis induced by dextran sulfate sodium(DSS) in mice. Methods : Ulcerative colitis was induced through supplying 4% DSS solution as the drinking water for 7 days in 6-week-old male ICR mice. The colitic mice were divided into three groups: the sample groups were orally administered SJT in doses of 25mg/kg(S25 group) or 100mg/kg(S100 group) once a day for 10 days, from 3 days before starting drinking the DSS solution, and the control(C) group was administered normal saline instead of SJT. The DSS solution or SJT was not administered to the normal(N) group. The length of colon, histologic finding, the activities of myeloperoxidase(MPO) and alkaline phosphatase(AP), and the expressions of $IL-1{\beta}$, IL-6, COX-2, $NF-{\kappa}B$, and $I{\kappa}B$ in colonic mucosa was checked using immunoblot, ELISA, etc. The activities of chondroitinase, tryptophanase, ${\beta}-glucuronidase$ and ${\beta}-glucosidase$ in stool were also measured. Results : The length of colon shortened, histologic finding deteriorated, the activities of MPO, AP, chondroitinase, tryptophanase, ${\beta}-glucuronidase$ and ${\beta}-glucosidase$, and the expressions of $IL-1{\beta}$, IL-6, COX-2, $NF-{\kappa}B$ increased, and the expression of $I{\kappa}B$ decreased in the C group. All measures, except $NF-{\kappa}B$, were restored in S25 group, but some measures deteriorated more in the S100 group than in the C group. Conclusions : According to the above results, it is supposed that SJT has a potential therapeutic effect on ulcerative colitis.
