계단 보행 시 불안정성 신발 착용에 따른 슬관절 부하에 대한 연구 (A Study on Changes in Knee Joint Loading during Stair Gait with Unstable Shoes)

  • Purpose: The purpose of this study is to compare kinematics and kinetics on the knee joint between stair gait with unstable shoes and barefoot in healthy adult women. Methods: Seventeen healthy adult women were recruited for this study. The subjects performed stair ascent and descent with unstable shoes and barefoot. The experiment was repeated three times for each stair gait with unstable shoes and barefoot. Measurement and analysis of the movements of the knee joint were performed using a three-dimensional analysis system. Results: Statistically significant differences in the knee muscle force of semimembranosus, biceps femoris-long head, biceps femoris-short head and sartorius, patellar ligament, medial gastrocnemius, and lateral gastrocnemius were observed between unstable shoes and barefoot gait during stair ascent. Statistically significant differences in the knee muscle force of sartorius, rectus femoris, medial gastrocnemius, and lateral gastrocnemius were observed between unstable shoes and barefoot gait during stair descent. Statistically significant differences in the knee flexor moment of semitendinosus, biceps femoris-long head, biceps femoris-short head, sartorius, rectus femoris, vastus intermedialis, medial gastrocnemius, and lateral gastrocnemius were observed between unstable shoes and barefoot gait during stair ascent. Conclusion: Therefore, wearing unstable shoes during stair gait in daily life is considered to influence knee joint kinematics and kinetics due to the unstable shoes, and thus suggest the possibility that reducing the risks of pain, and knee osteoarthritis, stabilizing the knee joint caused by changes in the loading of the knee joint.

신발의 안정성에 따른 여성노인과 여대생의 균형능력 변화 (Changes of Balance Ability according to the Stability of Shoes in Elderly Woman and Female University Student)

  • Purpose: The purpose of this study was to investigate the changes of static and dynamic balance control ability according to the stability of shoes in elderly woman and female university student. Design: Cross-sectional study. Methods: Six elderly women and seven female university students were recruited for this study. The subject's static and dynamic balance were evaluated while wearing two different types of shoes (comfortable running shoe and masai walking shoe). The BT4 system was used to measure the static (postural sway area and velocity) and dynamic balance (limit of stability on forward, backward and left and right side). The measurement of static and dynamic balance control ability was performed in standing posture wearing comfortable running shoes and masai walking shoes. Results: In the static balance control ability, both female university students and elderly women showed significant increase in postural sway area and velocity when wearing unstable shoes (p<0.05) In addition, in the dynamic balance control ability, both female university students and elderly women showed significant decrease in limit of stability on forward and backward when wearing unstable shoes (p<0.05). Conclusion: In selecting shoes for the elderly, the stability of shoe should be considered for prevention of falls.

불안정 판을 이용한 훈련이 높은 굽 신발을 신은 여자 대학생의 발목관절 변위와 동적균형능력에 미치는 영향 (The effect of unstable plate on the ankle joint displacement and dynamic balance ability of female college students wearing high-heeled shoes)

  • 본 연구는 불안정 판을 이용한 훈련이 높은 굽 신발을 신은 여자대학생의 발목관절의 변위와 동적균형에 어떤 영향을 미치는지 알아보기 위해 실시하였다. 여자 대학생 18명을 무작위로 실험군과 대조군으로 나누고, 굽 높이가 9cm인 신발을 착용 한 후 3D 동작분석기를 이용하여 5m 보행 시의 발뒤꿈치와 발목의 변위를 검사하고, 동적 균형능력 평가를 실시하였다. 중재 프로그램은 실험군에게 4주 동안 주 3회, 불안정 판을 이용한 균형 훈련을 실시하고, 대조군에게는 중재를 실시하지 않았다. 연구 결과, 중재 후 실험군의 발목관절의 변위는 전반적으로 감소하였고, 동적균형능력은 유의하게 상승하였다. 대조군은 발목관절의 변위, 동적균형능력의 변화가 거의 없었다. 불안정 판을 이용한 훈련이 대상자들의 발목관절을 안정화 시켜주고 동적균형능력을 향상시키는 것으로 나타났다.

12주간의 불안정성 신발 착용이 직립 자세 및 보행역학에 미치는 영향 (Effects of 12-week Wearing of the Unstable Shoes on the Standing Posture and Gait Mechanics)

  • The purpose of this study was to determine effects of 12-week wearing of unstable shoe on the standing posture and gait mechanics. Nine healthy men were asked to wear the unstable shoes for 12-week and walk for 30 minute everyday. Their standing posture and gait mechanics were measured before and after treatment. Standing posture was measured for each side(anterior, posterior, lateral) for standing position. And gait analysis was measured joint angle of a right lower limb between first right heel contact and second right heel contact. Kinematic data were collected using video camera at 30 frame per seconds. Statistical analysis was paired t-test(p<.05) to compare before training with after that. A head tilt angle was significantly decreased for posterior side(p<.05). The angle of between center of line and surface was significantly decreased at midstance and take off during walking(p<.05). Ankle dorsiflexion significantly increased at heel contact2(p<.05) and ankle plantarflexion significantly increased at midstance and midswing(p<.05). The increase of ankle dorsiflexion showed that our results consisted with previous study. In conclusion, there was not large significant difference in static standing posture but joint angle of lower limb represented many changes with increasing of ankle motion during walking. These were of benefit to body by increasing leg muscle activity but it was necessary for man having a ankle problem to consider. Further studies concerning optimum outsole angle of unstable shoes are necessary.

불안정 지지면에서 정적 서기 동안 구두 굽 높이의 변화에 따른 자세 조절 전략 (Postural Strategy by the Difference of Shoe Heel Height During Quiet Standing on an Unstable Surface)

  • The purpose of this study was to evaluate the changes in the electromyographic (EMG) activity of the trunk and the lower limb muscles during quiet standing on an unstable surface while wearing low-heeled shoes (3 cm), high-heeled shoes (7 cm) and without footwear (0 cm) in 20 young healthy women. The subjects stood on an unstable surface for 30 seconds. We examined the differences in the EMG data of the erector spinae, rectus abdominis, biceps femoris, rectus femoris, tibialis anterior, and the gastrocnemius medialis muscle. A one-way repeated analysis of variance was used to compare the effects of shoe heel height on the EMG activity with the level of significance set at ${\alpha}=.05$. The EMG activity of the erector spinae and the rectus femoris were significantly increased (p<.05) in the subjects who wore elevated heel height, while the tibialis anterior and the gastrocnemius medialis were significantly decreased (p<.05). However, the rectus abdominis and the biceps femoris exhibited no significant difference among the three conditions. The above results indicate that wearing high-heeled shoes may change the postural strategy. The findings of this study suggest that excessive heel height could contribute to an increased fall risk during quiet standing.

The Influence of Unstable Shoes on Kinematics and Kinetics of the Lower limb Joints during Sit-to-stand task

  • Purpose: The purpose of this study was to investigate examine how the kinematics and kinetics of lower limb joints were changed depending on the unstable shoes (US) during sit-to-stand task (SitTS). Methods: Nineteen healthy females were participated in this study. The subjects performed sit-to-stand task with US and barefoot. The experiment was repeated three times for each tasks with conditions. The kinematics and kinetics of lower limb joint were measured and analyzed using a 3-D motion analysis system. A paired t-test was utilised performed for to identificationy of changes in mean of angle, force, and moment between both the two conditions. Results: The results of this study showed kinematic differences in lower limb joints during SitTS based on the US. The hip, knee, and ankle angle showed statistically significant differences during SitTS. At the initial of SitTS, Tthe force and moment of the hip flexor, hip extensor, knee flexor, knee extensor, ankle flexor, and ankle extensor showed statistically significant differences. At the terminal of SitTS, Tthe force and moment of the hip flexor, hip extensor, knee flexor, knee extensor, ankle flexor, and ankle extensor showed statistically significant differences. At the maximum of SitTS, Tthe moment of the hip extensor showed statistically significant differences. The force and moment of the ankle flexor, extensor moment showed statistically significant differences. Conclusion: Therefore, Wwearing US is considered to influence on the lower limb joints kinematics and kinetics during SitTS movements, and thus suggests the possibility that of reducing the risks of pain, and osteoarthritis caused by changes in the loading of lower limb joints.

The Influence of Flip-flops Gait on the Muscle Activity of Tibilalis anterior and Gastrocnemius

  • The fact that flip-flops, one of many different types of unstable shoes, are light and relatively easy to put on, accounts for their popularity among people. But because flip-flops rely heavily on the support of a single thong between your first and second toes, they impose a huge amount of pressure onto lower leg. Thus in the following experiment we tried to examine the different effects of flip-flops and running shoes in terms of their effect on muscle activity and fatigue of tibialis anterior and gastrocnemius during walking. In order to measure an electromyogram we used Free EMG system. 10 men and 10 women in running shoes ran on treadmills for 15 minutes at 4.8km/h, 2 days later the same experiment was carried out, but this time, in flip-flops. p value turned out to be greater than .05 and thus there was no considerable difference between the effects of flip-flops and running shoes on muscle activity and fatigue during walking. Therefore we conclude that despite the fact that flip-flops are considered unstable, their effects on muscle activity and fatigue of tibialis anterior and gastrocnemius are negligible.

Changes in Oxygen Saturation and Walk in Relation to Smoking and Types of Shoes

  • Purpose: The purpose of this study is to examine the effects of types of shoes and distance travelled on oxygen saturation and walk in relation to smoking. Methods: To achieve this goal, the researcher divided 20 normal male adults into two groups: a smoking group (10) and a non-smoking group (10) and used a treadmill to have them walk in intervals of 3 days, a total of 4 times. Results: By analyzing their walk, the following results were obtained: oxygen saturation showed statistically significant results in both the smoking group and the non-smoking group. The smoking group showed significant results for left pace and significant results were achieved for setting foot and shaking in both the smoking group and the non-smoking group. In comparison of oxygen saturation between groups, there were significant results for 3km sneakers walk and 3 km slippers walk. There were significant results for width in 3 km sneakers walk, left and right pace in 3 km slippers walk, and setting foot in 1 km sneakers walk. Conclusion: In conclusion, in the smoking group changes in walk occur if the shoes are more unstable and the distance travelled becomes longer.

롤러 신발과 조깅 신발 착용 후 보행 시 하지 분절의 운동학적 특성 비교 분석 (The Comparative Analysis of Wearing Roller Shoes and Jogging Shoes on Kinematic Characteristics in the Lower Extremity during Walking)

  • 본 연구의 목적은 롤러 신발과 조깅 신발 착용 후 보행 시 운동학적 변인에 대하여 비교 분석 하는데 있다. 이를 위해 하지 근골격계에 이상이 없는 중학생 8명을 피험자로 선정하여 3차원 동작 분석을 실시하였다. 분석 결과, 활보장, 인체무게 중심변위 및 선속도, 관절각 및 각속도에서 집단 간 통계적으로 유의한 차이를 보였다. 특히 롤러 집단의 경우 지지 시 발목 관절각이 증가하고 무릎각의 각속도가 감소하는 결과를 나타내었다. 이는 롤러 신발의 경우 장착된 휠에 의해 지지 시 적절한 배측굴곡이 이루지지 못하고, 불안정성을 극복하기 위해 무릎의 과도한 굴곡이 나타난 것으로 사료되어 진다. 이러한 운동학적 변인의 차이는 롤러 신발 보행 시 정상 패턴과는 다른 불안정한 보행 동작을 유발시키고 이러한 동작이 지속적으로 이루어진다면 하지 근골격계에 변화를 유발시켜 부상 유발의 가능성이 있을 것으로 생각된다. 따라서 본 연구 결과를 토대로 향후 보다 안정성 높은 롤러 신발을 개발할 수 있을 것으로 기대된다.

일어서기 동작 시 발 뒷굽의 형태가 하지 관절 움직임에 미치는 영향 (The Influence of Shoes with Different Heels on Movement of Lower Limb Joints during Sit-to-stand)

  • Purpose: The purpose of this study was to investigate changes in movement strategies of lower limb joints depending on the type of heel during sit-to-stand. Methods: Twenty healthy females participated in this study. All subjects performed sit-to-stand three times each with three different types of heels - bare feet, 9 cm high-heeled shoes, and unstable shoes. Trails were conducted in random order. Three-dimensional motion analysis systems were used for collection and analysis of the kinematic data of lower limb movements. Results: Results of this study showed kinematic differences in pelvis, hip joints, knee joints, and ankle joints during sit-to-stand based on the type of heels. At the initial sit-to-stand, hip joint flexion, knee joint flexion, ankle joint flexion, and ankle joint inversion showed significant differences. The maximal angles of hip joint flexion, hip joint adduction, knee joint flexion, ankle joint flexion, and ankle joint inversion were significantly different, while hip joint adduction, pelvic forward tilt, hip joint rotation, knee joint flexion, ankle joint flexion, and ankle joint inversion differed significantly during the terminal of sit-to-stand. Conclusion: Therefore, the type of heel played an important role in selection of lower limb movements during sit-to-stand which were essential parts of daily life movements.
