Post-traumatic Stress Disorder and Post-traumatic Growth in Breast Cancer Patients - a Systematic Review

  • Breast cancer (BC) is potentially a traumatic stressor which may be associated with negative outcomes, such as post-traumatic stress disorder (PTSD) or positive changes, such as post-traumatic growth (PTG). This study aims to identify the core issues of BC related PTSD, PTG and psychological distress by interrogating the literature in BC survivors. We have also highlighted issues related to the assessment, diagnosis and clinical management of PTSD and PTG. The authors systematically reviewed studies published from 1985 to 2014 pertaining to PTSD, psychological distress and PTG in BC survivors with particular attention paid to incidence rates and causative factors. Multiple studies intimated that women with BC have evidence of PTSD at the initial stages of diagnosis, whereas PTG develops once patients undergo treatment. Early diagnosis and treatment of PTSD/PTG is paramount from literature review but the previously mentioned relationship between PTSD and PTG in BC patients could not be verified. It is evident from the literature that a small percentage of BC patients experience PTSD, while the majority experience PTG after BC diagnosis and treatment. Future research should include prospective studies focusing on high-risk patients, causative factors and the development of psychological interventions.

A study to identify an occupational hazards for hospital working health care providers from needle stick injury as an occupational hazards of health care providers in hospital and prevention (의료요원의 주사바늘 상해 실태와 예방대책을 위한 일 연구)

  • A study .to identify an occupational hazards for hospital working health care providers from needle stick injury as an occupational hazards of health care providers in hospital and prevention A survey of 2430 health professionals (2184 nurses, 182 doctors, and 64 Lab technicians) was conducted to describe and provide information about 1) the experience of needle stick. 2) the number of needle stick, 3) the treatment after needle stick. 3) the situation of needle stick, 4) the report of needle stick, 5) the cause of needle stick, 6) the discard method of used needles, and 7) how to worry about getting infection disease after needle stick. Data were collected using questionnarires constructed by the authors and tested by a pilot study. Results of the study showed that 96.7% of the sample had an experience of needle stick (96.8% of the nurses, 96.7% of the doctors, and 92.2% of Lab technicians). Seventy seven percent of the sample experienced less than 10 needle sticks, 19% of the sample experienced 11 to 20 needle sticks, and the rest of the sample experienced more than 20 needle sticks. The situations where needle sticks occurred include intrvenous injection (36.5%), intramuscular injection (21.6%), blood withdraw (17.8%), and preparation(11.8%). The study showed that needle sticks (67.5%) usually occurred after client treatment. Health professionals used recapping method (55%) after they used needles. Needle sticks were predominantly caused by the carelessness of health professionals (61%), After needle sticks, 88.2% of the sample subjects treated needle sticks using disinfection technique by themselves. Most of health professionals (92.6%) did not report the accident. and 95.6% of them did not receive any test or further treatment. After needle sticks, 87.8% of nurses, 83.6% of doctors, and 96.6% of lab technicians worried about hepatitis infection. 'Health professionals also worried about AIDS infection, tetanus, venereal infection, and skin injury. These findings suggest that health professionals are at high-risk of needle stick and fail to report needle stick accidents. They should pay more attention to needle stick in order to avoid unwanted infection.

  • Complex lesions of the thoracic aorta are traditionally treated in 2 surgical steps with the elephant trunk technique. A relatively new approach is the frozen elephant trunk (FET) technique, which potentially allows combined lesions of the thoracic aorta to be treated in a 1-stage procedure combining endovascular treatment with conventional surgery using a hybrid prosthesis. These are very complex and time-consuming operations, and good results can be obtained only if appropriate strategies for myocardial, cerebral, and visceral protection are adopted. However, the FET technique is associated with a non-negligible incidence of spinal cord injury, due to the extensive coverage of the descending aorta with the excessive sacrifice of intercostal arteries. The indications for the FET technique include chronic thoracic aortic dissection, acute or chronic type B dissection when endovascular treatment is contraindicated, chronic aneurysm of the thoracic aorta, and chronic aneurysm of the distal arch. The F ET technique is also indicated in acute type A aortic dissection, especially when the tear is localized in the aortic arch; in cases of distal malperfusion; and in young patients. In light of the great interest in the FET technique, the Vascular Domain of the European Association for cardio-thoracic Surgery published a position paper reporting the current knowledge and the state of the art of the FET technique. Herein, we describe the surgical techniques involved in the FET technique and we report our experience with the F ET technique for the treatment of complex aortic disease of the thoracic aorta.

  • Some female college students have to recognize and understand the actual conditions of the tooth bleaching, and effective consultation and training to provide basic data for the purpose of investigation. this study made a survey about recognize and understand the actual conditions of the tooth bleaching and oral health knowledge and generalization methods for 649 female college student in Daejeon and Jeonbuk areas during the period between March. 15 and April. 10,2008. The results were as follows: 1. Tooth Status was found low, self-discontent respondents Status 44.2% and average 31.1%, self-contentment were 14.8%. Tooth color was average respondents were 69.8%, yellow 29.0%, White 1.7% (p=0.001, p=0.030). 2. Tooth bleaching experience has not experienced the most the military was 86.4%, to the desired Tooth bleaching for the external beauty 44.2%, confidence of smile time 37.5%. self-discontent 10.7%(p=0.000, p=0.000). 3. Tooth health status satisfaction was dental hygiene students higher than non dental hygiene, and scaling knowledge of the Tooth bleaching effect was non dental hygiene higher than non dental hygiene(p=0.039, p=0.000). 4. General knowledge for Tooth bleaching was found high 96.1%, as for the recognition route, 55.6% were through broadcast medium(p=0.025, p=0.000). 5. Medical institution chosen for Tooth bleaching treatment method appears the most preferred by 79.9% to the dental hospital dental clinic. 6. Important to consider that the choice of Tooth bleaching was Tooth bleaching duration of 37.1% cost 33%, And when Tooth bleaching hoped to be long-lasting. In this research the high recognized of Tooth bleaching treatment, but very low Tooth bleaching experience female college students for the Tooth bleaching had the wrong information. Therefore, Tooth bleaching treatment and counseling that can be used to development and education were required to provide the correct information.

  • Neuroblastoma treatment remains challenging, but treatment has become more effective due to the establishment of clinical and biological variables that determine prognostic risks. Initially, stage and age were the prime determinants of survival used in clinical practice. Risk-based therapy currently is the hallmark of neuroblastoma treatment. This study reviews one center's experience with the management of neuroblastoma. Sixty-three patients with neuroblastoma were treated from 1989 to 2003. All patients were graded according to the International Neuroblastoma Staging System (INSS) at diagnosis. There were 37 boys and 26 girls. The median age was 2.14 years (range, 33 days-10.2years). The primary site was the adrenal gland in 47, dumbbell shape extending into spinal canal in 6, retroperitoneum in 5, mediastinum in 3, and other sites in 2. The probability of 5-year overall survival (OS) and event free survival (EFS) were 46.7 % and 44.2 % by Kaplan-Meier method. According to INSS, there were stage 1 in 2 cases, stage 2 in 5, stage 3 in 12, stage 4 in 42, and stage 4s in 2. There were statistically significant differences in the survival rates between patients with stage1, 2 and stage 3, 4(P<0.05). For the stage 3 and 4, the extents of surgical resection, determined from the operative records and pathologies, were complete resection in 17 cases, minimal residual in 15, and partial resection 11, and the 5-year OS rate was 57.8, 51.4, and 13.6 %, respectively. There is a trend toward higher OS with more complete resection (P<0.05). We conclude that age and stage at diagnosis are prognostic factors, and complete excision of the primary tumor can provide better prognosis for patients with stage 3 and 4 neuroblastoma.

The Analysis of Research in Arthritis (관절염에 관한 국내 간호학 연구 논문 분석)

  • The purpose of this article was to identify trend in Arthritis research and to provide ba an approach of Arthritis research. The results of analysis of 69 Arthritis articles public year of between 1970-1999 found that most articles(27.6%, n=19) were published since few(10.1%, n=7) were before 1994. Vast majority of articles(79.8%, n=55) were printed in The Journal of Rheumatology Health, and 79.8%(n=55) of those were quantitative research, 8.7%(n=6) were qualitative research, and 14%(n=1) was methodological research. 29.0%(n=20) of quantitative research were carried in experimental design and over half(50.8%, n=35) in nonexperimental design. Having concerned the types of treatment(or intervention) used in experimental study, the most common treatment was 'exercise' (45.0%, n=9), and other less common were 'self-help education(15.0%, n=3), home care(10.0%, n=2) supportive care(10.0%, n=2), flexibility exercise combined with local heat therapy(5.0%, n=1), relaxation training(5.0%, n=1). Of the types of exercise, aquatic exercise was 30.0%(n=6). The effect of exercise on outcome variable was found to be most strong among the other types of treatment. 18 articles of nonexperimental research were correlational research. From the analysis of correlational research, 29 concepts were identified, and the relationship between depression and other variable were strongly reported. In descriptive studies, most commonly physiological variables were examined in 5 articles(29.5%). Only one study(5.9%) explored patient's perception of the causal factors and those causality. In qualitative researches, two study were carried out by the means of grounded theory. Majority of those subjects were RA patients, and the largest number of sample were 23 and the smallest was 1 In two studies, patient's perceived causality of RA was identified as a research problems, and an experience of ADL, fatigue Phenomenon, an experience of having a total knee replacement with osteoarthritis were each found in one study. Methodological research was carried out to test validity and reliability of the RA Fatigue Scale.

Endovascular Treatment of Dural Arteriovenous Fistulas : Single Center Experience

  • Objective : Treatment of intracranial dural arteriovenous fistulas (dAVFs) remains a challenge. However, after introduction of Onyx, transarterial approach is the preferred treatment option in many centers. We report our experience of dAVFs embolization with special emphasis on transarterial approach. Methods : Seventeen embolization procedures were performed in 13 patients with dAVFs between Jan 2009 and Oct 2014. Clinical symptoms, location and type of fistulas, embolization methods, complications, radiological and clinical outcomes were evaluated using charts and PACS images. Results : All 13 patients had symptomatic lesions. The locations of fistulas were transverse-sigmoid sinus in 6, middle fossa dura in 4, cavernous sinus in 2, and superior sagittal sinus in 1 patient. Cognard types were as follows : I in 4, IIa in 2, IIa+IIb in 5, and IV in 2. Embolization procedures were performed ${\geq}2$ times in 3 patients. Nine patients were treated with transarterial Onyx embolization alone. One of these required direct surgical puncture of middle meningeal artery. Complete obliteration of fistulas was achieved in 11/13 (85%) patients. There were no complications except for 1 case of Onyx migration in cavernous dAVF. Modified Rankin scale score at post-operative 3 months were 0 in 11, and 3 in 2 patients. Conclusion : Transarterial Onyx embolization can be a first line therapeutic option in patients with dAVFs. However, transvenous approach should be tried first in cavernous sinus dAVF because of the risk of intracranial migration of liquid embolic materials. Furthermore, combined surgical endovascular approach can be considered as a useful option in inaccessible route.

A Clinical Analysis of 140 cases of Seborrheic Dermatitis Patients (지루성 피부염 환자 140례의 임상 특성 연구)

  • Objectives : This study was performed to investigate the characteristic of Seborrheic Dermatitis(SD) patients who visited Korean medicine clinics. Methods : The study was carried out with 140 SD outpatients who visited fourteen Gowoongyul Korean traditional medical clinics from January to December 2016 and was completed by reviewing patients' survey. Results : 1. Among 140 patients, there were 61 male patients(43.6%), 79 female patients(56.4%). The average age of the patients was $32.6{\pm}10.9years$ old, with many in their 20s and 30s in the distribution. 2. The average duration of disease was $4.2{\pm}4.11years$. 3. 80% of the patients had experience of treatment, 58.6% only took Western medicine, 17.1% had experience of both Western and Korean medicine treatment, and 4.3% received only Korean medicine treatment. 4. Analysis of seasonal effects of SD showed that 48.6% of the respondents reported that skin disease always appeared regardless of the season, with 33.6% in winter, with 29.3% in summer and 25.7% in seasonal change. 5. The rate of family history of seborrheic dermatitis was low(6.4%), and 75.7% of the patients answered that they had no family history of skin disease. 6. The major lesion of SD appeared on face(87.9%) and scalp(36.4%). 7. 70.7% answered they do some kind of self-managements. 70.0% used moisturizers. Bathing therapy was performed in 17.1%, and 55% were taking the health supplement food, and 35.7% restricted the avoidance diet such as instant and fatty foods. Conclusions : This study demonstrated the various clinical characteristics of Korean patients with seborrheic dermatitis.

Effect of Dialysis Nurses' Perception of Patient Safety Risk Factors and Patient Safety Competency on Safety Nursing Activities (혈액투석실 간호사의 환자안전에 대한 위험요인 인식과 역량이 안전간호활동에 미치는 영향)

  • Purpose: The study was conducted to investigate the correlation between perception of patient safety risk factors, patient safety competency, and safety nursing activities of nurses in hemodialysis units and identify factors affecting patient safety activities. Methods: The participants were 146 nurses from 16 hemodialysis units located in Gyeongsangnam-do. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis using the SPSS, version 24.0. Results: The mean safety nursing activity score was 3.47±0.38. safety nursing activities of the participants were significantly correlated with patient safety competency. The characteristics showing significant differences in safety nursing activities were educational level, hospital type, hospital work experience, number of hemodialysis treatment per day, number of hemodialysis treatment per nurse, educational experience of patient safety, presence of a patient safety incident report registration system, and direct registration of patient safety incident report. The multiple regression analysis revealed that the factors influencing safety nursing activities were patient safety incident report, patient safety competency, and number of daily hemodialysis treatment (<5~7 times/day) per nurse (R2=.34). Conclusion: The results of this study suggest that the safety nursing activities of hemodialysis unit nurses should be intensified. In addition, the registration system of patient safety incident report and nurses' competency on patient safety should be improved, and the number of hemodialysis per nurse should be fewer than 7 times per day.

Proposal of Collaborative Sharing Services and Applications for the Treatment of Children with Developmental Disabilities (발달장애 아동의 치료를 위한 협업 공유 서비스 및 애플리케이션 제안)

  • Recently, due to the temporary closure and isolation of facilities for developmental disabilities due to the prolonged COVID-19, the pain suffered by consumers is getting worse, and there is no clear solution due to the lack of information on inpatient services. Therefore, there is a need for a service system that can prevent institutional and psychological problems for consumers. The purpose of this study is to provide systematic inpatient treatment guides, post-discharge management, and correct education for children with developmental disabilities and their guardians. After deriving the needs and improvement factors of consumers through domestic service case analysis, we conducted co-creation with end-users using the double diamond methodology. A possible service concept was derived. Accordingly, a collaborative sharing app service was proposed, and the usefulness of the service was confirmed through the usability evaluation and verification of various stakeholders. It is expected that the results of this study will be utilized in the development of an assistance system for the developmental disability ward based on user experience.
