응급실 간호사와 응급구조사의 외상성 사건의 경험, 외상후 스트레스장애에 대한 지식, 태도 및 회복탄력성 (Experiences of traumatic events, knowledge and attitudes concerning post-traumatic stress disorder, and resilience among nurses and paramedics working in emergency department)

  • Purpose: This study aimed to assess exposure to traumatic events, knowledge and attitudes concerning post-traumatic stress disorder (PTSD) and the level of resilience among nurses and paramedics working in emergency departments. Methods: Data were collected from May 22 to June 12, 2022, using a self-administered survey questionnaire. The participants comprised 135 nurses and 80 paramedics working in emergency departments. The collected data were analyzed with descriptive statistics, a t-test, and an analysis of variance with Scheffé's test. Results: Compared with emergency room nurses, paramedics were more positive about the government's spending on job opportunities for people with PTSD. There were no significant differences in attitude regarding government strategies and people with PTSD between nurses and paramedics. Paramedics had higher scores on the effective treatment for PTSD, while emergency room nurses showed higher scores on effective psychotherapy. General knowledge of PTSD differed according to sex (t=-2.33, p=.021) and education level (F=3.21, p=.042). Resilience scores differed significantly according to sex (t=2.02, p=.045), education level (F=4.10, p=.018), self-reported economic state (F=10.34, p<.001), and self-reported health (F=11.57, p<.001). Conclusion: The findings support that emergency department nurses and paramedics are in need of self-care programs to support their mental health and indicate that intervention programs should be developed to enhance resilience in emergency department professionals.

한국형 이차적 외상스트레스척도(Secondary Traumatic Stress Scale)의 구성타당화 연구 (Psychometric Properties of the Korean Version of the Secondary Traumatic Stress Scale(K-STSS))

  • 본 연구는 이차적 외상스트레스척도(STSS)를 한국의 외상관련 원조실무자를 대상으로 적용하고 심리측정적 속성을 평가하여 한국형 이차적 외상스트레스척도(K-STSS)를 실증적으로 검증하는데 목적을 두었으며, 이를 위해 문항분석, 탐색적 요인분석, 수렴 및 개념구성 타당도 분석이 이루어졌다. 그 결과, 총 17문항 중 4문항을 제외한 13문항으로 구성된 두 가지 차원의 한국형 이차적 외상스트레스척도(K-STSS)가 수정 제시되었다. 제시된 척도에서 신뢰도상의 문제는 없는 것으로 나타났고, 우울, 불안척도와의 상관관계를 통해 수렴타당도가 성립되었다. 소진척도와의 상관관계 수준이 매우 높아 개념 구성타당도 성립에는 한계가 있었다. 이외 조사대상자의 특성(연령, 성별, 개인외상경험 여부)에 따른 차이를 보여줌으로써 한국형 이차적 외상스트레스척도(K-STSS)가 척도로서의 변별력을 갖춘 것으로 확인되었다.

트라우마 센터에서 근무한 사회복지사의 소진과 이차적 외상 스트레스 경험에 관한 질적 연구 (A Qualitative Study on the Burnout and Secondary Traumatic Stress Experience of Social Workers who Worked in Trauma Centers)

  • 본 연구는 트라우마 센터에 근무한 사회복지사들의 소진과 이차적 외상 스트레스 경험을 이해하기 위한 실체 이론 개발을 목적으로 한다. 근거이론을 통해 자료를 분석한 결과, 초기코딩을 통해서 159개의 의미단위, 47개의 하위범주, 그리고 12개의 범주를 구성하였다. 초점 코딩에서 중심현상을 '상처받은 존재성'으로 보고 '사회복지사로서의 자리 찾기'를 핵심범주로 정하였으며 이를 중심으로 연관된 범주들을 연결하였다. 이론적 코딩 결과 트라우마 센터에서 근무한 사회복지사의 소진과 이차적 외상 스트레스 경험 과정은 사명기, 갈등기, 침체기, 해체기로 나타났다. 이러한 연구 결과를 바탕으로 재난복지실천에 대한 준비, 트라우마 치유 관점에 대한 논의 및 합의, 그리고 소진과 이차적 외상 스트레스 예방과 대처를 위한 프로그램의 필요성을 제시하였다.

임상간호사의 인문학적 소양과 직무 스트레스가 전문직 삶의 질에 미치는 영향 (Influence of Humanistic Knowledge and Occupational Stress on Professional Quailty of Life of Clinical Nurses)

  • 본 연구는 임상간호사의 전문직 삶의 질에 영향을 미치는 요인을 확인하여 전문직 삶의 질을 높이기 위한 기초자료 제공을 위해 수행되었다. 연구대상자는 일 광역시 3개 종합병원에 근무하는 간호사 217명으로, 2019년 6월 3일부터 6월 14일까지 구조화된 설문지를 사용하였고, 수집된 자료는 SPSS Statistics 23.0 program을 이용하여 기술통계, t-test, ANOVA, Scheffé test, Pearson's Correlation Coefficient, Stepwise multiple regression으로 분석하였다. 연구결과, 공감만족은 인문학적 소양과는 양의 상관관계, 소진과는 음의 상관관계가 있었고, 소진은 인문학적 소양과는 음의 상관관계, 직무 스트레스와 이차 외상성 스트레스와는 양의 상관관계가 있었으며, 이차 외상성 스트레스는 직무스트레스와 양의 상관관계가 있었다. 공감만족에 영향을 미치는 요인으로는 소진, 간호만족, 임상경력으로 나타났으며 48.5%의 설명력을 보였고, 소진에 영향을 미치는 요인에는 공감만족, 이차 외상성 스트레스, 간호만족, 기혼, 직무 스트레스로 나타났고 68.8% 설명력을 보였으며, 이차 외상성 스트레스에 영향을 미치는 요인은 소진과 급여로 나타났으며 21.9%의 설명력을 보였다. 이상의 연구 결과 소진은 공감만족과 이차 외상성 스트레스 모두에 영향을 미치므로 소진을 예방하는 노력이 요구된다.

경찰관의 PTSD 수준 및 대처방안 (Levels and Countermeasures of PTSD among Police Officers)

  • 경찰관을 대상으로 한 연구들에 따르면 최근에 위급상황에 노출되었던 경찰관 중 상당수가 심각한 심리적인 스트레스 상태에 있으며, 사기, 장기 결근, 직업적 건강, 조기 퇴직, 가족 관계에 상당한 영향을 미치는 외상후 스트레스 장애(Post Traumatic Stress Disorder : PTSD) 증후를 보이는 것으로 나타났다. 그러나 우리나라에서 그 동안 경찰관의 PTSD를 실증적으로 살펴본 연구는 거의 없었다. 이 연구에서는 경찰관의 PTSD 수준을 파악하고 그에 따른 대처방안을 모색하고자 하였다. 경찰관의 PTSD 수준은 연구에 따라 다소 차이는 있으나 비교적 높은 수준이고, 소방관과 비교해도 상대적으로 높은 수준이다. 이에 대한 대처방안으로 고위험 경찰관의 발견 및 관리, 위기상황 스트레스 관리 기법의 보급, 직원 지원 프로그램의 도입, 동료 및 가족의 적극적 개입, 스트레스 관리 교육 등을 제시하였다.

Treadmill exercise ameliorates post-traumatic stress disorder-induced memory impairment in Sprague-Dawley rats

  • Post-traumatic stress disorder (PTSD) is a stress-related mental disorder induced by severe external stressors such as assault, disaster or severe accident. We investigated the effects of treadmill exercise on short-term memory in relation to apoptosis and cell proliferation in the hippocampus following PTSD. Stress to the pregnant rats was induced by exposure of maternal rats to the hunting dog in an enclosed room. Exposure time was 10 min, repeated three times per day, with 1 hour interval. Exposure of maternal rats to the hunting dog was continued 7 days after pregnancy until delivery. The pregnant rats in the exercise groups were forced to run on a treadmill for 30 min once a day for the same duration of stress exposure. Step-down avoidance task for short-term memory, western blot for Bcl-2, Bax, and immunohistochemistry for caspase-3, 5-bromo-2'-deoxyuridine (BrdU), and Ki-67 were conducted. Maternal rats exposed to stress during pregnancy showed short-term memory impairment. Expressions of Bax, Bcl-2, ratio of Bax to Bcl-2, and caspase-3 in the hippocampus were increased in the PTSD rats. Cell proliferation in the hippocampal dentate gyrus was decreased in the PTSD rats. Treadmill exercise alleviated short-term memory impairment and suppressed expressions of Bax, the ratio of Bax to Bcl-2, and caspase-3. Treadmill exercise also increased cell proliferation. The present results indicate that treadmill exercise alleviated PTSD-induced short-term memory impairment by suppressing apoptotic cell death and enhancing cell proliferation in the hippocampus.

두부외상 환자에서 전두엽 손상과 신경인지기능 변화에 따른 주관적인 증상 연구 (The Study of the Subjective Symptoms according to Frontal Lobe Damage and Change in Neurocognitive Function in Traumatic Head Injury Patients)

  • Objective : The purpose of this study was to analyze the correlation between symptom severity and neurocognitive factors in traumatic head injury patients. In addition, the effect of frontal lobe damage on these parameters was examined. Methods : We selected 18 patients who had brain damage for the moderate to severe traumatic brain injury (MSTBI) group, and 17 patients who met the diagnostic criteria for post-traumatic stress disorder (PTSD) without the finding of brain damage for the comparison group. For the evaluation of neurocognitive function, K-WAIS, Rey-Kim Memory Test, K-FENT, WCST, and MMPI-2 were used. Results : The results of the comparison (using the malingering scale) revealed that the values of PDS and PK, which express the severity of symptoms, and the values of the validity scale F, F (B), and F (P) were significantly higher in the overly-expressed group. F (B) in overly-expressed group and PK, Pt, and Sc in the properly-expressed group had significant correlation with the severity of symptoms. F (B), S, and Stroop error inhibition in PTSD, and PK, Pt, Sc, and MQ in MSTBI had significant correlation with the severity of symptoms. The results of the comparison based on the finding of frontal lobe damage revealed that PDS, EIQ, and MQ ware significantly higher in the group without brain damage. Conclusions : It was revealed that each neurocognitive factor was correlated with the severity of symptoms. There was a decrease in complaints or symptoms reported by the frontal lobe injury group, and this is believed to be due to degenerative change in the personality and emotional functioning of these patients following frontal lobe damage.

외상후 스트레스장애에 대한 하타 스타일 요가 치료의 해외 임상연구 동향 (A Review on Clinical Research Trends in the Treatment of Hatha-Style Yoga for Post Traumatic Stress Disorder)

  • Objectives: The purpose of this study was to review the research trends in the treatment of hatha-style yoga on Post Traumatic Stress Disorder (PTSD). Methods: We searched articles in Pubmed and the China National Knowledge Infrastructure (CNKI) January 2010-December 2019, for studies to treat PTSD using hatha-style yoga. Selected studies were evaluated by the CLEAR-NPT (A Checklist to Evaluate a Report of a Non-pharmacological Trial). Results: Seven randomized controlled clinical trials were selected. PSS-I (PTSD Symptom Scale-Interview) was the most frequently used as diagnostic criteria. The PCL (PTSD Checklist) was also the most commonly used outcome measurement. Of the seven articles, most studies reported that hatha-style yoga was effective to reduce symptoms of PTSD. Conclusions: Hatha-style yoga practice intervention can be used to relieve symptoms of PTSD. More studies should be conducted to make hatha-style yoga as protocol (complementary therapy) for PTSD patients.

  • Recent breakthroughs in functional neuroimaging techniques have launched the quest of mapping the connections of the human brain, otherwise known as the human connectome. Imaging connectomics is an umbrella term that refers to the neuroimaging techniques used to generate these maps, which recently has enabled comprehensive brain mapping of network connectivity combined with graph theoretic methods. In this review, we present an overview of the key concepts in functional connectomics. Furthermore, we discuss articles that applied task-based and/or resting-state functional magnetic resonance imaging to examine network deficits in post-traumatic stress disorder (PTSD). These studies have provided important insights regarding the etiology of PTSD, as well as the overall organization of the brain network. Advances in functional connectomics are expected to provide insight into the pathophysiology and the development of biomarkers for diagnosis and treatment of PTSD.

충격적 사고를 목격한 아동에 대한 미술치료 사례 (A Clinical Study of Art Therapy for Children Who Witnessed a Tragic Accident)

  • Purpose: This study aims to examine what can be seen in children's paintings, their common characteristics, and what role art therapy plays in diagnosing the post-traumatic stress disorder in children who experienced trauma, through an art therapy program among elementary school students that witnessed the death of parents from a fire drill accident. Methods: A program of three times of art therapy was progressed among 34 children who witnessed a fire accident. Revised Children's Manifest Anxiety Scale (RCMAS) was used for comparative analysis of art therapy results. Results and Conclusion: First, children who had the possibility to develop post-traumatic stress disorder were found through art therapy. Second, an opportunity to express themselves were given to children who refused psychological tests or treatment through art therapy.
