Effect of Foot Eversion on Knee and Ankle of Trans-tibial Amputees (인공의족의 외반 특성이 하퇴절단자의 무릎과 발목에 미치는 영향)

  • One of the important functions of prosthetic foot is the foot inversion-eversion which is so important when walking on uneven surfaces. The aim of our study was to evaluate the effect of foot eversion angle especially on knee and ankle joint for transtibial amputees by motion analysis. The experimental data were collected from three transtibial amputees and then ten healthy individuals. To simulate walking on side sloping ground, we used custom-made slope (5, 10, 15 degrees). Motion analysis was performed by 3-dimensional motion analyzer for 6 dynamic prosthetic feet. The results showed that knee abduction moments of amputated leg were decreased but those of sound leg were mainly increased as foot eversion angle increased. And ankle abduction moments of sound leg were inconsistent in magnitude and tendency between control and experimental group. Therefore foot eversioncharacteristics should be considered to develop advanced prosthetic foot.

  • In this study, ground reaction force(GRF) and energy consumption of fixed. single-axis and multi-axis Prosthetic ankle assemblies were investigated to show the biomechanical evaluation for trans-tibial amputees. In the experiments. two male and two female trans-tibial amputees were tested with fixed, sin91e-axis and multi-axis Prosthetic ankle assembly. A three-dimensional gait analysis was carried out to derive the ratio of GRF to weight as the percentage of total stance Phase for nine Points Energy consumption of each Prosthetic ankle assembly was measured while subjects walked at 2km/h. 3km/h and the most comfortable walking speed on the treadmill The results showed that multi-axis ankle was superior to the other two ankle assemblies for the characteristic of forwarding and breaking forces. Fixed ankle was relatively superior to the other two ankle assemblies for gait balancing and movement of the center fur mass Compared to the other ankle assembly. sing1e-axis type showed lower energy consumption over 2.3km/h walking speed .

A Comparative Study of Gait Characteristics between Single Axis Foot and Energy Storing Foot for Sports in Trans-tibial Amputee (하퇴절단자용 단축식 발과 스포츠용 에너지 저장형 발 보행 특성 비교연구)

  • This study examined the differences in spatio-temporal parameters, joint angle, ground reaction force (GRF), and joint power according to the changes of gait speed for trans-tibial amputees to investigate the features of the energy-storing foot for sports. The subjects walked at normal speed and at fast speed, wearing a single-axis type foot (Korec) and an energy-storing foot for sports (Renegade) respectively. The results showed that Renegade yielded faster gait speed as well as more symmetric gait pattern, compared to Korec. However, as gait speed was increased, there was no significant difference in kinematics, ground reaction force, and joint power between two artificial foots. This was similar to the results from previous studies regarding the energy-storing foot, where the walking velocity and gait symmetry have been improved. Nevertheless, the result of this study differed from the previous ones which reported that joint angle, joint power, and GRF increased as the gait speed increased except spatio-temporal parameters.

Pressure Distribution in Stump/Socket Interface in Response to Socket Flexion Angle Changes in Trans-Tibial Prostheses With Silicone Liner

  • This study examined the effects of socket flexion angle in trans-tibial prosthesis on stump/socket interface pressure. Ten trans-tibial amputees voluntarily participated in this study. F-socket system was used to measure static and dynamic pressure in stump/socket interface. The pressure was measured at anterior area (proximal, middle, and distal) and posterior area (proximal, middle, and distal) in different socket flexion angles ($5^{\circ}$, $0^{\circ}$, and $10^{\circ}$). Paired t-test was used to compare pressure differences in conventional socket flexion angle of $5^{\circ}$ with pressures in socket flexion angles of $0^{\circ}$ and $10^{\circ}$ (${\alpha}$=.05). Mean pressure during standing in socket flexion angle of $10^{\circ}$ decreased significantly in anterior middle area (19.7%), posterior proximal area (10.4%), and posterior distal area (16.3%) compared with socket flexion angle of $5^{\circ}$. Mean pressure during stance phase in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (19.3%) and decreased significantly in anterior distal area (19.7%) compared with socket flexion angle of $5^{\circ}$. Mean pressure during stance phase in socket flexion angle of $10^{\circ}$ decreased significantly in anterior proximal area (19.6%) and increased significantly in anterior distal area (8.2%) compared with socket flexion angle of $5^{\circ}$. Peak pressure during gait in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (23.0%) compared with socket flexion angle of $5^{\circ}$ and peak pressure during gait in socket flexion angle of $10^{\circ}$ decreased significantly in anterior proximal area (22.7%) compared with socket flexion angle of $5^{\circ}$. Mean pressure over 80% of peak pressure ($MP_{80+}$) during gait in socket flexion angle of $0^{\circ}$ increased significantly in anterior proximal area (23.9%) and decreased significantly in anterior distal area (22.5%) compared with socket flexion angle of $5^{\circ}$. $MP_{80+}$ during gait in socket flexion angle of $10^{\circ}$ decreased significantly in anterior distal area (34.1%) compared with socket flexion angle of $5^{\circ}$. Asymmetrical pressure change patterns in socket flexion angle of $0^{\circ}$ and $10^{\circ}$ were revealed in anterior proximal and distal region compared with socket flexion angle of $5^{\circ}$. To provide comfortable and safe socket for trans-tibial amputee, socket flexion angle must be considered.

  • The alignment of the prosthetics is very important in an amputee's gait. In the present study. a static prosthesis-alignment device was developed. It consisted of a force plate with four load cells, a laser beam controlled by a step motor, and a control part programmed by PCBASIC. Using the static prosthesis-alignment device, we measured the distance between the load line and various joints of 24 normal volunteers in three standing postures. such as neutral, forward leaning, and backward leaning. Only neutral postures were evaluated on four trans-tibial amputees. The load line for the normal person's neutral position located anterior to the ankle, the knee, and the greater trochanter, but posterior to the shoulder joint. Forward and backward leaning of the normal person resulted in a significant anterior and posterior movements of the load line, respectively. The load line for the amputated side of the trans-tibial amputee also located anterior to the ankle, the extremity prostheses, providing a good relative locations of the load line with respect to various joints.

  • 본 연구는 하퇴의지 착용자와 정상 성인간의 시간-거리, 운동형상학 변수를 조사하고 비교하기 위하여 실시하였다. 연구 대상자는 외상으로 인한 하퇴 절단자로서 내골격식 하퇴의지를 착용하고 독립적으로 보행이 가능한 20명과 연령, 신장으로 짝짓기한 대조군(matched control group) 20명이 참여하였다. 보행분석은 Vicon Clinical Manager Software (VCM)를 내장한 PC에 5개의 카메라가 연결되어 있는 Vicon 512 Motion Analysis System (MAS, Oxford Metrics Inc.)을 이용하였다. 하퇴의지 착용군의 단하지 지지시간이 정상 성인군에 비해 유의하게 짧았으며(p<.05), 하퇴의지 착용군에서 슬관절의 선전이 증가되었으며 족관절의 저측굴곡이 감소되었다(p<.05). 하퇴절단자들의 보행개선을 위해서는 하퇴절단자 개인의 보행능력에 알맞은 부품의 선택, 체계적인 보행훈련 및 평가, 보행능력 향상을 위한 근력강화 프로그램 등 체계적인 재활훈련 프로그램이 필요한 것으로 사료된다. 본 연구는 연구대상자의 수가 제한되어 있으므로 연구의 결과를 일반화하기에는 제한점이 있으나, 향후 편측 하퇴절단자의 보행연구에 대한 기초 자료로 사용될 수 있을 것이라고 생각된다.
