동작침법의 국내 임상 연구 동향: 주제범위 문헌고찰 (Domestic Clinical Research Trends of Motion-Style Acupuncture Treatment: A Scoping Review)

  • Objectives This scoping review aimed to investigate the domestic clinical research trends of motion-style acupuncture treatment (MSAT), identify diseases and symptoms for which MSAT is used, summarize specific methods of MSAT, and suggest the direction of future studies. Methods The study was conducted in accordance with a previously specified methodology, using the preferred reporting items for systematic reviews and meta-analyses extension for scoping reviews (PRISMA-ScR) checklist. We searched nine electronic databases for studies on MSAT reported till March 21, 2022. The search terms were 'kinematic acupuncture,' 'MSAT,' 'motion style acupuncture,' and 'motion style treatment.' Results A total of 29 studies were included in our analyses; of them, 23 (79.3%) were before-after studies. Lumbosacral disease was the most common for which MSAT was applied (n=16). The frequency and duration of treatments differed depending on the researchers, and local acupoints (including ashi points) were used in 22 (75.9%) studies. In most cases, the method of mobilizing the joint or stretching the muscle in the disease area was used after inserting the acupuncture; however, in 7 studies, gait exercise was used. Most studies used MSAT in combination with other treatments. Conclusions This study supports the direction of future research by presenting the methodological applications of MSAT. To increase its clinical applicability, studies with a high level of evidence investigating the application to various body part, standardization and safety of MSAT are necessary.

  • 1. Objectives : Millions of people throuout the world are affected by some form of dry eye disorder. I made researches for more effective treatments for dry eye. 2. Methodes : I refered to occidental and oriental medical records. 3. The results were as follows : Common symptoms of dry eye are dryness, burning, irritation, grittiness, itching, fatigue of eye, photophobia, congestion, mattering and tear. Treatments of dry eye are to clear away heat and fire, remove dampness, replenish Um and promote production of body fluid, tonify the blood and replenish Ki. Acupunctre and moxibustion therapy of dry eye are as follows: Very busy points of traditional regular acupuncture are Sangsong, Chanjuk, Sajukkong, Chongmyong, Sabaek, Tongjaryo, Taeyang, Pungji, Tuimup, Paekoe, Un-gyo, Chonjong, Hapkok, Yangbaek, Kansu, Yanggye, Imup, Chok-samni, Taechung, Kwangmyong, Yang-gok, Uihui, Chohae, Haenggan. 시상 및 시상하부 뇌경색 후에 수면과다와 기억력 장애가 발생한 환자의 증례보고 (A Case Report of Thalamic and Hypothalamic Infarction with Hypersomnolence and Memory Impairment)

  • Introduction: We report a rare case in which the symptoms of hypersomnia were caused by cerebral infarction that occurred in the hypothalamus and hypothalamus. Case Presentation: The patient underwent Korean medicine treatments using Gami-seonghyangjeonggi-san, acupuncture, electroacupuncture, and physical therapy for 37 days. We evaluated the degree of improvement in hypersomnolence based on the amount of sleeping time and the degree of memory impairment with the patient's suggestive conditions as observed by the patient's guardian, and one-sided facial palsy using the Yanagihara Scale. As treatment was applied, sleeping time was decreased from 22 to 9 hours a day. Memory impairment did not show clear improvements as the patient could not recognize others when they visited during the period in which the patient underwent treatment. One-sided facial palsy showed no improvements and there were no changes in the Yanagihara Scale results. Conclusion: This case showed that Korean medicine treatment could be effective to relieve some of the symptoms of thalamus and hypothalamus infarction. Further research is needed to confirm the effectiveness of Korean medicine treatment.

한방병원에 내원한 특발성 폐섬유화증 환자 82명에 대한 임상적 특징 분석; 후향적 차트 리뷰 (Effects of Korean Medicine in Idiopathic Pulmonary Fibrosis Patients - A Retrospective Chart Review)

  • Objective: This study was designed to analyze the clinical features of idiopathic pulmonary fibrosis patients who attended a Korean medicine hospital and the treatment effects through retrospective chart reviews. Methods: The medical records of 82 outpatients who had been diagnosed with idiopathic pulmonary fibrosis and visited the Allergy, Immune, and Respiratory System Department of Kyung Hee Korean Medicine Hospital from 8 January 2015 to 8 January 2021 were retrospectively reviewed. To assess the treatment outcomes, we used the FVC (Forced Vital Capacity), DLCO (Diffusing capacity of the Lung for CO), 6-minute walk test, and HRCT (High Resolution Computed Tomography). Results: The study group consisted of 28 females and 54 males. The median age of the patients was 67.98±11.44 years. The chief complaints were cough (n=51) and dyspnea (n=49). Other frequent symptoms were general weakness (n=8), weight loss (n=4), and a fever (n=2). A total of 77 (93.90%) patients were prescribed Korean herbal medicine, and 52 (63.41%) patients were treated with acupuncture, moxibustion, cupping therapy, ICT, or pharmacopuncture. After treatments, FVC, DLCO, the 6-minute walk test, and HRCT were maintained or worsened slightly. Conclusions: This study presented the characteristics of idiopathic pulmonary fibrosis patients treated by Korean medical therapies, and further studies of Korean medical treatments for idiopathic pulmonary fibrosis patients would be valuable.

유방암 환자의 항암 치료 부작용 및 한의학적 보완치료 경험에 관한 포커스 그룹 연구 (Experiences of Treatment-Related Side Effects and Supportive Care with Korean Medicine in Women with Breast Cancer - A Focus Group Study)

  • Objectives: To explore experiences of treatment-related side effects and supportive care among Korean breast cancer survivors (BCS). Methods: Focus group interview was conducted with six Korean women with breast cancer. Participants were recruited through snow-balling. Interview was audio-recorded and transcribed verbatim. NVivo-11 was used to code the data into themes. Results: Two major themes were identified: (1) experiences of Western medicine, including treatment, side effects, needs and costs; (2) experiences of supportive care with Korean medicine, including the same as above. All participants experienced Western medicine in treatment phase and reported impairment of physical, emotional, and social functioning during and after Western medicine treatment. Only three participants used Korean medicine after treatments end. The negative responses from Western medicine doctors were the most important factor keeping participants from accessing Korean medicine when treatment-related side effects occurred. For this reason, some participants used Korean medicine without disclosure. Participants usually acquired information about Korean medicine from online community or other BCS, which was another important factor because it raised concerns about side effects and credibility of Korean medicine. High cost was also reported as barrier in using Korean medicine. During the cancer treatment, participants tended to endure their treatment-related side effects. Conclusions: Korean BCS may be at high risk of physical or emotional distress during treatment period. Findings suggest that there is a high need for supportive care to relieve treatment-related side effects and improve patients' quality-of-life. Furthermore, developing a systematic guidance or credible information sources should be warranted to help patients find the best supportive care options including Korean medicine.

  • Parkinson's disease (PD) is a chronic degenerative disorder of the central nervous system. It has no cure, but current treatments can relieve symptoms and maintain quality of life. As PD progresses, controlling its symptoms becomes difficult. Here, we present the treatment of two patients with idiopathic PD using traditional Korean medicine (TKM) and temporomandibular joint balancing therapy (TBT). We measured the progress of the two patients using the unified PD rating scale (UPDRS) and the Hoehn and Yahr (HY) scale. Combined treatment of TKM and TBT decreased the total UPDRS score from 52 to 26 after 21 days in case 1, while it decreased the total UPDRS score from 91 to 65 after 20 days in case 2. In both patients, the HY scale score was maintained at 3, and no adverse events were observed. Thus, the combined treatment of TKM and TBT can produce a treatment response in PD patients.

사상인의 운동요법 적용에 관한 연구 (A Study of Application of Exercise Treatments according to Sasangin)

  • Sasang Constitutional Medicine (SCM) is based on the theory of Sasang (Taeyang, Soyang, Taeum, Soeum) in Traditional Korean Medicine (TKM). It is hypothesized that all of the things in the material world can be divided into the four categories of SCM. We made a new attempt to combine nutritive components, and patterns in physical activities with SCM. At first, we hypothetically divided the nutrients into 4 compartments according to SCM theory, and human body composition as well. These factors could be basically related to Sasangin Exercise Treatment (SET). Therefore, we applied sasangin theory to exercise prescription. As a research method, we hypothesized that human beings' types can be distinguished by mind and body. Mind was focused on exercise psychological aspects, and body also handled by nutrients, body compositions, body shapes, and exercise physiological aspects. For application of sasang theory to the exercise prescriptions, Taeumin as a sample sasangin was selected in this study. We concluded that in perspective of general traits, Taeumin have to reduce their carbohydrate intake volume, but greatly increase their intake of vitamins, minerals, and water in daily diet. They are also recommended exercises that can increase cardiorespiratory function and make them sweat, like endurance running, hiking, swimming, cycling, etc.

전신성 홍반성 낭창(SLE)환자 1예(例)에 대한 증례 보고 (One Case of Systemic Lupus Erythematosus treated with traditional Korean Medicine)

  • After observing a patient diagnosed with Systemic Lupus Erythematosus from the September 21, 2000 to the January 13, 2001, the following results are obtained. Method and Result: We treated one case of Systemic Lupus Erythematosus with the Korean Bee Venom Therapy and the other Oriental Medical treatments. In acupuncture therapy, Korean Bee-venom Therapy is believed to be outstanding way to treat Systemic Lupus Erythematosus. Korean Bee Venom Therapy is treated on the following acupuncture points: ST36(B23(Shinsu:腎兪), BL26(Guanyuanshu:關元兪), ST36(Chok-Samni:足三里), LI4(Hapkok:合谷), LV3(Taechung), SP10(Hyolhae:血海) SP6 (Samumgyo;三陰交). In addition, CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;) Herbal-Acupuncture was applied on the acupucture points of GB20(Pungji: 風池), GB21(Kyonjong:), as well as BUM(It was made of Boviscalculus(牛黃), Moschus and Fel ursi Herbal-Acupuncture was given on CV17(Chonjung). In herbal medication, based on the Sasang Constitution, Taeyumin Chungsimyunja-Tang was given to the patient. As the results of these treatments, general conditions of this patient improved drastically. Conclusion: Based on the clinical results, traditional Korean Medical treatment is believed to be effective for treating Systemic Lupus Erythematosus, and futher studies should be carried out to provide more valuable information.

A Clinical Study of Collaboration between Western and Korean Medicine for the Treatment of Peripheral Facial Palsy in a Korean Medicine Hospital

  • Background: The purpose of this study was to investigate the characteristics and direction of treatment for peripheral facial palsy (PFP) based on medical collaboration between Western and Korean medicine departments. Methods: There were 195 outpatients with PFP identified retrospectively by examining electronic medical records. These patients were treated with Korean and Western medicine from January 1st, 2018 to December 31st, 2018 at the Kyung Hee University hospital. Records were analyzed according to the patients' demographic characteristics and clinical features of the collaborative combined treatment. Results: According to the collaborative treatment pathway, the number of patients consulted from Western medicine departments was more than consulted from Korean department for the first time. The time taken by the Western medicine departments to consult with the patients at the Korean center for the first time was 14.9 days from the onset of symptoms. Acupuncture was the most frequently used Korean medicine treatment. The total treatment period for Korean medicine sessions and intervals were 91.9 days, 23 times and 3.6 days, respectively. When the Korean medicine center consulted with Western medicine departments, the time taken until the first consultation was 8.5 days from the onset of symptoms. Medication was the most used treatment, prescribed after 3.1 days. The most frequently used clinical test was an electromyogram, and this was performed after 20.5 days. The total treatment period, sessions and intervals were 21.2 days, 2.8 times and 5 days, respectively. Conclusion: The clinical status of collaborative treatment for PFP was determined including the timing and interval of consultation according to treatments.

원발성 월경통의 추나 치료에 대한 체계적 문헌 고찰 (Chuna Manual Therapy for Primary Dysmenorrhea: A Systematic Review)

  • Objectives The purpose of this study is to evaluate the effectiveness of Chuna therapy for primary dysmenorrhea. Methods Researchers searched on 7 electronic databases (Pubmed, National Digital Science Library, China National Knowledge Infrastructure, Wangfang med online, Korean Studies Information Service System, Research Information Sharing Service and Oriental medicine Advanced Searching Integrated System). The search included Korean, English, Chinese reports and there was no limit on the search period. All of randomized controlled clinical trials (RCTs) that used Chuna manual therapy for primary dysmenorrhea were selected. Results 27 RCTs met required condition. Meta-analysis showed positive results for Chuna manual therapy for primary dysmenorrhea in terms of therapeutic effects and reduction of symptom scores compared to west medicine, herbal medicine, acupucture and other treatments. Conclusions Above results showed that performing chuna is effective in treating dysmenorrhea. However, in some studies, there was no statistic significance between the experimental group and the control group. Also, according to Cochrane Risk of Bias (RoB) evaluation method, quality of the studies were not high enough. Since most of the materials were in Chinese, more high-quality clinical trials about Chuna therapy for primary dysmenorrhea are needed in Korea.
