A Systematic Review of Clinical Studies of Tuina Treatment on Congenital Muscular Torticollis - Focused on Chinese Randomized Controlled Trials - (소아 선천성 근성 사경의 추나 치료 효과에 대한 임상연구 고찰 - 중의학 무작위 대조군 임상연구를 중심으로 -)

  • Objective This study aimed to analyze the efficacy of the tuina treatment for congenital muscular torticollis based on randomized controlled clinical studies in China. Methods The China National Knowledge Infrastructure was the main source of data. Using the keywords "tuina", "chuna", "推拿", and "斜颈", 12 randomized controlled clinical studies were identified and analyzed. Results Compared with the control group, the tuina treatment group showed statistically significant changes with respect to the therapeutic effect. Conclusions This study suggests that tuina treatment is effective for congenital muscular torticollis patients and provides evidence for further studies on oriental medicine treatments, including tuina for congenital muscular torticollis.

Effect of Kamicheonmagudeungeum on Hypertension (가미천마구등음이 고혈압 병태모델에 미치는 영향)

  • Kamicheonmagudeungeum(KCGE), a traditional herbal medicine, has been used as a therapeutic agent for the treatments of acute stage of hypertension. We evaluated the effects of KCGE on hypertension induced by DOCA-salt in rats. The systolic blood pressure and pulse rate significantly lowered by oral administration with KCGE. The levels of plasma hormones including dopamine, epinephrine and norepinephrine, and serum electrolyte were also reduced in KCGE treated rats. In addition, the production of reactive oxigen species (ROS) were greatly decreased by the treatment with KCGE in cultured bovine endothelial cells and angiotensine converting enzyme (ACE) activites were also inhibited by KCGE in a dose dependent manner. This study indicated that KCGE is a safe and effective treatment for hypertension.

Characteristics of Patients' Self-Perceived Health in Traditional Korean Medical Facilities - Based on the Ministry of Health and Welfares Report on Usage and Consumption of Korean Medicine in 2011 - (주관적 건강인식수준에 따른 한방의료기관 이용환자의 특성 비교 - 2011년 한방의료이용 및 한약소비실태조사(보건복지부)를 중심으로 -)

  • Objective : The purpose of this study was to identify characteristics of patients' self-perceived health in traditional Korean medical facilities. Method : This research was conducted based on the survey on patients whom have visited traditional Korean medical facilities in 2011 by the Ministry of Health and Welfares and Korean Institute for Health and Social Affairs. Using a sample of 3,931 (1,180 male and 2,751 female) outpatients' self-perceived health based on the data from usage and consumption of Korean Medicine. 'Healthy', 'Fair', and 'Poor Health' were used to measure patients' self-perceived health status. The data was analyzed by frequency, t-test, cross correlation analysis and multiple logistics regression analysis using the SPSS program package. Results : Sex(P<0.001), age(P<0.001), marital status(P<0.001), education(P<0.001), employment status(P<0.001), occupation(P<0.001), health insurance(P<0.001) and income level(P<0.0001) showed statistical significance. Main Treatment Facilities(P<0.001), experience of taking Korean medicine(P=0.032), experience of receiving acupuncture treatment(P<0.001), number of visits(P<0.001), medical expense (P=0.005), and subjective health status after the treatments showed statistical significance for Korean herbal medicine(P=0.038), acupuncture (P=0.001), cupping therapy(P=0.006), oriental physiotherapy(P=0.003), and treatment satisfaction(P<0.001). For subjective health status based on suffering disorders in the past three months, the response of poor health was higher in the group suffering recent illnesses. Statistical significance was seen in hypertension (P=0.002), arthritis(P<0.001), lumbar pain(P<0.001), diabetes mellitus(P=0.001), stroke(P<0.001), hwa-byung (P=0.001), gastric disorders(P=0.021), common cold(P<0.001), ankle sprain(P<0.001), muscular injury(P<0.001), lumbar sprain(p=0.009) and fracture(P=0.03). Also the number of diseases during the past three months showed statistical significance(P<0.001). Statistical significance was also seen in Level of knowledge(P<0.001), route of information(P<0.001), reliability of Korean medicine(P=0.003), insurance coverage(P=0.005), medical costs(P<0.001), and future willingness to use Korean medicine(P<0.001). As a result of the multiple logistics regression analysis, risks of subjective poor health statistically increased in female population, elderlies, medicaid beneficiaries, less educated, higher medical expense, and more disorders during the past three months. Conclusion : Patients' self-perceived health status has significant differences with each variables such as sex, age, marital status, education, health insurance, medical expense, number of diseases.

Systems Pharmacological Analysis of Dichroae Radix in Anti-Tumor Metastasis Activity (시스템 약리학적 분석에 의한 상산의 암전이 억제 효과)

  • Objectives : While treatments for cancer are advancing, the development of effective treatments for cancer metastasis, the main cause of cancer patient death, remains insufficient. Recent studies on Dichroae Radix have revealed that its active ingredients have the potential to inhibit cancer metastasis. This study aimed to investigate the cancer metastasis inhibitory effect of Dichroae Radix using network pharmacological analysis. Methods : The active compounds of Dichroae Radix have been identified using Traditional Chinese Medicine System Pharmacology Database and Analysis Platform. The UniProt database was used to collect each of information of all target proteins associated with the active compounds. To find the bio-metabolic processes associated with each target, the DAVID6.8 Gene Functional classifier tool was used. Compound-Target and Target-Pathway networks were analyzed via Cytoscape 3.40. Results : In total, 25 active compounds and their 62 non-redundant targets were selected through the TCMSP database and analysis platform. The target genes underwent gene ontology and pathway enrichment analysis. The gene list applied to the gene ontology analysis revealed associations with various biological processes, including signal transduction, chemical synaptic transmission, G-protein-coupled receptor signaling pathways, response to xenobiotic stimulus, and response to drugs, among others. A total of eleven genes, including HSP90AB1, CALM1, F2, AR, PAKACA, PTGS2, NOS2, RXRA, ESR1, ESR2, and NCOA1, were found to be associated with biological pathways related to cancer metastasis. Furthermore, nineteen of the active compounds from Dichroae Radix were confirmed to interact with these genes. Conclusions : The results provide valuable insights into the mechanism of action and molecular targets of Dichroae Radix. Notably, Berberine, the main active ingredient of Dichroae Radix, plays a significant role in degrading AR proteins in advanced prostate cancer. Further studies and validations can provide crucial data to advance cancer metastasis prevention and treatment strategies.

  • This study was performed on the basis of clinical consideration about patient who has hepatoma, cholangiocarcinoma and pancreatic cancer. Cancer has gradually increased morbidity and mortality all over the world including Korea. Western medicine has treated with surgical therapy, chemotherapy, radiotherapy and immunotherapy, but has not obtained outstanding results and has a difficulty due to side effects by those therapies. In this study, we recognized that symptom including pain by cancer are rapidly decreased with oriental medicine treatment. Oriental medicine treatment is applied throughly on the basis of the oriental medicine principle and the prescrptions were used Bulwhankumjunggisan(不換金正氣散), Sojukbaekchulsan(消積白朮散) Banhabackchulchunmatang(半夏白朮天麻湯). The communitial effect of those prescriptions are invigorating the spreen and benefiting vital energy(健脾益氣), supporting healthy energy to eliminate evils(扶正祛邪) and those treatments are corresponed with traditional oriental treatment principle. Also we can know this patient has a little improvement in the general test including biochemistry, blood test, urine analysis and etc. in spite of outstanding improvement of patient's symptom on administration of oriental medicine. According to the above results, though we has not got satisfaction in getting rid of cancer completely on administration of oriental medicine, it is helpful in decreasing symptoms including pain by cancer and in improving quality of life of patient and in improving quality of patient life.

A Review of the TCM Articles about Cauterization Treatment (낙법(烙法)에 관한 중의(中醫) 논문 고찰)

  • Objectives : The purpose of this study is to review the chinese published papers on cauterization of traditional chinese medicine. Methods : We searched chinese published papers from 1958 until May 2017 via CNKI(China National Knowledge Infrastructure) with the keyword "cauterization". We analyzed the studies covered cauterization medical treatment, and classified them by 5 categories including periods, type of study, treatment site, experiment target site, and cauterizing method. Results : We reviewed 112 chinese papers which include 10 Original articles, 43 Review articles, and 59 Case reports. Examining yearly distributions, we can see that research on cauterization is becoming more active than in the past. In classification of 93 clinical studies by treatment site, the number of research on Tonsillitis accounts for almost half(43 studies), followed by Sore throat(14 studies). Among 10 experimental studies, Eye is the most frequently targeted organ which was used to study high intra-ocular pressure(5 studies), and retinal ganglion cell(2 studies). Lastly, there are various methods of cauterization used in papers: Branding iron(54 studies), and Red-hot needle(24 studies) are two major heating methods. Conclusions : This analysis shows that studies on the application of cauterization have been actively conducted in China these days. Furthermore, up-to-date cauterizing methods have been developed such as electric type and microwave type beyond traditional ways. We expect this article will encourage further research on cauterization in order to apply it to a variety of diseases. Then, it could become one of new effective medical treatments in Korean medicine.

Application of Complex Korean Medicine Therapy to a Patient with Behavioral and Psychological Symptoms of Dementia from Suspected Alzheimer's Disease: Case Report (알츠하이머 치매 의심 환자의 행동심리증상에 대한 복합한의치료 1례: 증례 보고)

  • Objectives: The purpose of this study was to report the clinical effectiveness of the complex Korean medicine therapy on a patient with BPSD (Behavioral and Psychological Symptoms of Dementia) due to suspected Alzheimer's disease. Methods: We experienced an age 73 female who complained of a depressive mood, insomnia, and accompanying symptoms such as abnormal behavior. During treatment, the patient received acupuncture and herbal medicine. The clinical effects were evaluated through KGDS (Korean Form of Geriatric Depression Scale) and subjective statements of the patient. Results: After the complex Korean medicine therapy, the treatments ameliorated overall symptoms that the patient complained about. The patient was more positive and slept better after the treatment. No specificities or adverse effects were reported during treatment. Conclusions: This study suggests the possibility of treating patients with BPSD with complex Korean medicine therapy.

Comparison between Moxibustion and the Intense Pulsed Light (IPL) and the Clinical Application of Photo-moxa (구법(灸法)과 intense pulsed light(IPL)의 특성 비교 및 광선구(光線灸)의 한방임상(韓方臨床) 활용(活用))

  • Objectives : The purpose of this study is to compare the principles between moxibustion and the 'Intense pulsed light (IPL)' and to investigate the clinical application of the photo-moxa for the Korean medicine fields. Methods : The characteristics of moxibustion, IPL, parabolic reflector moxa and the dermatology surgical use of moxibustion were reviewed in the literature. Results : IPL is one type of light treatments that is employed by radiating the short-pulse wave, which is transformed from the light of high intensity. There has been used parabolic reflector in Asia for more than 2,000 years, and this hand-held device used for moxibustion in traditional medicine. Moxibustion is one of the treatment tools in Korean medicine using the heat energy. The fluence or energy density of IPL in clinical circumstances is similar to parabolic reflector moxa of moxibustion. IPL and parabolic reflector moxa shared same treatment principle employing the heat energy. Therefore IPL could apply for many indications in Korean medicine as a moxibustion tool and acupuncture. Conclusions: IPL may be plausible to be a fascinate method in phototherapy of Korean medicine.

Application of Health Care Big data and Necessity of Traditional Korean Medicine Data Registry (보건의료 빅데이터를 활용한 연구방법 및 한의학 레지스트리의 필요성)

  • Health care big data is thought to be a promising field of interest for disease prediction, providing the basis of medical treatment and comparing effectiveness of different treatments. Korean government has begun an effort on releasing public health big data to improve the quality and safety of medical care and to provide information to health care professionals. By studying population based big data, interesting outcomes are expected in many aspects. To initiate research using health care big data, it is crucial to understand the characteristics of the data. In this review, we analyzed cases from inside and outside the country using clinical data registry. Based on successful cases, we suggest research method for evidence-based Korean medicine. This will provide better understanding about health care big data and necessity of Korean medicine data registry network.

Traditional Chinese Medicine and its Application for Fashion Design

  • Therapeutic garments are an undeveloped area and yet they have the potential to generate considerable profit for the fashion industry. In traditional Chinese medicine, there are spots and pathways on the body through which energy flows and practitioners consider that disease results when this energy flow is hindered. Clothes which stimulate the spots can therefore potentially help to cure disease. In addition, clothes are worn every day and can therefore offer prolonged treatment certainly in comparison with other treatments. Obesity is a big problem nowadays and one which is caused by poor energy circulation (according to TCM). A garment which stimulates the spots can assist energy flow and therefore cause the wearer to lose weight. In the process, it can also alleviate side effects of obesity such as diabetes and high blood pressure. The was to stimulate spots include acupuncture, acupressure, moxibustion etc. There are lots of products to press spots which improve energy flow both in the Western and Chinese markets. The basic principle of circulation is in fact the same in both Western and Chinese medicine. However, most of these are products are footwear ones because they can easily provide the necessary stimulation to reduce tiredness and improve circulation. For garments, to press the spots effectively it is best to use tight clothes such as corsets and leggings. The important point is to tighten the body and to make energy flow and yet feel comfortable at the same time. Choice of fabric is therefore an important issue. Although the idea has been introduced, it will be necessary to develop a technique which will allow the necessary amount of pressure to be applied. It could be concluded that this area has a lot of possibility for the future but further research will need to be done before the idea becomes workable.
