  • Objective: Case report of hypothyroidism improved by ortho-cellular nutrition therapy. Methods: A 55-year-old Korean woman diagnosed with hypothyroidism in the past, did not take any Synthyroid medication while undergoing OCNT for a year. Results: A blood test was conducted one year after the initiation of OCNT, and she was diagnosed with normal thyroid hormone level. Conclusion: Applying nutritional therapy to patients with hypothyroidism may assist in the recovery of thyroid hormone levels and the alleviation of symptoms.

A Case of Functionary Cystic Parathyroid Adenoma with Papillary Thyroid Carcinoma (유두상 갑상선암과 동반된 기능성 낭종성 부갑상선 선종 1예)

  • Cystic parathyroid adenoma is one of rare causes of hyperparathyroidism, and tends to cause increased serum level of parathyroid hormone, alkaline phosphate and serum calcium level similar to when compared to those of solid adenoma. The possibility of a coincidental appearance of primary hyperparathyrodism and nonmedullary thyroid cancer is rare, and often neglected. A 40-year-old female presented with constipation and weight loss for 3 months. The serum calcium, phosphate and alkaline phosphate were 16.6 mg/dl, 2.2 mg/dl and 505 IU/L respectively and serum parathyroid hormone level was 1556.2 pg/mL. Neck US showed mixed nodules at both thyroid lobes, and PET-CT showed a right thyroid nodule without FDG uptake. The patient was diagnosed of primary hyperparathyroidism and underwent parathyroidectomy and a total thyroidectomy. Histopathologic results revealed parathyroid adenoma with cystic change (2 cm greater diameter) in the right lower parathyroid gland and coexistent papillary carcinoma of the right lobe of thyroid gland (infiltrating, 0.3 cm in greater diameter). She remained well and serum calcium, phosphate, parathyroid hormone level were normalized within 24 hours.

THE EFFECT OF HYPERTHYROIDISM ON THE RATE OF ORTHODONTIC TOOTH MOVEMENT (갑상선 기능 항진증이 교정적 치아 이동 속도에 미치는 영향)

  • There are various local and systemic factors which alter the rate of tooth eruption movement. Thyroid hormone has been reported to have proportional relationship with the rate of tooth eruption. The main function of thyroid hormone is the regulation of basal metabolism, but it also affects the rate of tooth eruption. In this report, we will present a case of an 11-year-old girl, who showed sudden increase in orthodontic tooth movement of the impacted canine at certain points, which coincided with the hyperthyroid period. It shows possible relationship between the serum level of thyroid hormone and the rate of orthodontic tooth movement.

The Effects of Thyroid Hormone on the HMG-CoA Reductase Gene Expression

  • The effects of the thyroid hormone ($T_3$) on 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase activity were evaluated in a baby hamster kidney cell line, C100. The cells cultured in MEM were supplemented with 10% thyroid hormone-depleted fetal bovine serum (THDS-MEM) and had a 82.5% lower level of HMG-CoA reductase activity than the cells grown in a medium supplemented with fetal bovine serum (FBS-MEM). When $T_3$ was supplemented to THDS-MEM, the reduction of the reductase activity was blocked in a dose-dependent manner. In the cells grown in THDS-MEM containing $T_3$ at a concentration of $10^{-6}$ M, the level of HMG-CoA reductase activity was 91.8% relative to the cells grown in FBS-MEM. These changes in HMG-CoA reductase activity seemed to be at least partly due to the changes of HMG-CoA reductase mRNA levels. The level of HMG-CoA reductase mRNA in cells incubated in THDS-MEM decreased to 76.2% relative to the cells grown in FBS-MEM, while the level of reductase mRNA in cells incubated in THDS-MEM containing $T_3$ at a concentration of $10^{-6}$ M increased to 243.4% relative to the cells grown in FBS-MEM. The increase of HMG-CoA reductase mRNA level after $T_3$ treatment may have been due to the increased stability of reductase mRNA, because the transcriptional rate of the reductase gene did not change significantly in the presence or absence of $T_3$. These results indicate that $T_3$ stabilizes HMG-CoA reductase mRNA at the posttranscriptional level and regulates HMG-CoA reductase activity in a dose-dependent manner.

The relationship between shift work pattern and thyroid stimulating hormone in female workers

  • Background: Shift work is known to cause changes in the circadian rhythm of the human body and adversely affect not only physical health but also mental health. Some studies have demonstrated the correlation between shift work and thyroid stimulating hormone (TSH), a hormone that changes according to the diurnal rhythm, but few studies have reported the different TSH levels according to the shift work type. This study aimed to investigate changes in TSH according to the shift work type. Methods: This study included 1,318 female workers who had a medical checkup at a university hospital in Changwon from 2015 to 2019. Shift work types were classified as non-shift work, regular 2 shifts, and irregular three shifts, and a TSH ≥ 4.2 mIU/L was defined as abnormal. A general linear model (GLM) was used to compare the TSH levels and the risk of subclinical hypothyroidism in each year, and a binary logistic analysis was performed using a generalized estimation equation (GEE) to compare the risk of subclinical hypothyroidism over the 5-year period. Results: Of the 1,318 participants included in this study, 363, 711, and 244 were non-shift, two-shift, and irregular three-shift workers, respectively. In the GEE analysis, after adjusting for age, body mass index, smoking, and alcohol consumption, the odds ratios (ORs) were 1.81 (95% confidence interval [CI]: 1.15-2.86; p = 0.011) in 2 shifts and 2.02 (95% CI: 1.23-3.32; p = 0.006) in irregular three shifts, compared to non-shift. Conclusions: Our results showed that shift work had a higher risk of subclinical hypothyroidism than non-shift work and that there was a significant difference in the risk of subclinical hypothyroidism according to the shift work type. These findings suggest that the shift work type can be considered in future thyroid function tests and evaluations.

Leptin, adiponectin levels, and thyroid hormones in normal and obese dogs

  • The present study compared leptin, adiponectin, and thyroid hormone concentrations in normal and obese dogs, and evaluated the association between leptin and adiponectin concentrations and thyroid function. The serum leptin, adiponectin, thyroid-stimulating hormone (TSH), total thyroxine (tT4), free thyroxine (fT4), triiodothyronine (T3), and cortisol concentrations were measured in 18 normal dogs (body condition score [BCS]: 4-5/9) and 16 obese dogs (BCS: 8-9/9). Leptin and T3 concentrations were higher in the obese group than the normal weight group (p < 0.01 and p < 0.05, respectively). In both groups, the T3 and leptin concentrations were correlated (r = 0.370, p < 0.05), as were the TSH and fT4 and adiponectin concentrations (r = -0.373, p < 0.05 and r = 0.369, p < 0.05, respectively). In the normal weight group, the TSH and fT4 concentrations were correlated with the adiponectin concentrations (r = -0.528, p < 0.05 and r = 0.482, p < 0.05, respectively). The results of the present study suggest that leptin and T3 concentrations are significantly higher in obese dogs than normal weight dogs, and the serum T3 and leptin concentrations are positively correlated.

Relationship between Changes in Manic Symptoms and in Thyroid Hormone Level (조증 증상 변화와 갑상선 호르몬 변화의 관계)

  • This study was designed to investigate the relationship between changes in manic symptoms and in thyroid hormone level in manic patients. Thyroid hormones(T3, T4) and TSH were measured, and Young's Mania Rating Scale(YMRS) was scored in twenty manic inpatients, at the time of admission and 6 weeks later. Mean value of T3 was slightly increased(increase : 9, decrease : 11). Mean value of T4 was fairly decreased(increase 4, decrease 16), while that of TSH was increased(increase 14, decrease : 6). Mean total score of YMRS was fairly decreased and mean values of each eleven subscales were decreased. Subjects were classified as increased, decreased and unchanged group for each thyroid hormones. for T3, there was statistically significant differences in YMRS subscale of 6(speech : speed/amount), 7(language, thought disturbance) and 10(appereance) between 3 groups. With the above results, the changes of manic symptom during the course of illness could probably be indicated by the thyroid hormon change, and manic symptoms could be classified as subtypes in terms of thyroid hormone change.

Selective Permeate Transport Characteristics of Iodine ion at Cell Membrane Model of Thyroid which Irradiated by High Energy X-Ray (고에너지 엑스선을 조사한 갑상선의 세포막모델에서 요오드이온의 선택적 투과성 전달 특성)

  • The selective permeate transport characteristics of iodine ion at follicle cell membrane model in thyroid which irradiated by high energy x-ray(linac 6 MV) was investigated. The follicle cell membrane model used in this experiment was a polysulfonated copolymerized membrane of poly(4-vinylpyridine-co-acrylonitrile:VP-AN). The difference of membrane thickness [2 mole AN%(w/w)], fixed carrier concentration[VP-AN%(w/w)], OH- concentration were occurred at difference of I- concentration and quantity of thyroid hormone, respectively. The tensile strength in fixed carrier concentration[VP-AN% (w/w): 0-62 %] of irradiated membrane was found to be decreased about 1.2-1.8 times than non-irradiated membrane. The I- selective permeate initial flux with increase of membrane thickness [2mole AN%(w/w)], fixed carrier concentration[VP-AN%(w/ w)], OH- concentration in irradiated membrane were found to be decreased about 2.1-4.5 times, about 2.2-2.5 times, about 2.1-2.67 times than non-irradiated membrane, respectively. As a result, the quantity of thyroid hormone was decreased at irradiated membrane than non-irradiated membrane. The decrease of thyroid hormone was occurred at hypothyroidism and hyperthyroidism, thyroid cancer, and so on. As the thyroid hormone in cell membrane model were abnormal, cell damages were appeared at cell.

The Effects of Astragali Radix on Hypothyroidism Rat Model Induced by 6-Propyl-2-thiouracil(PTU) (황기가 6-Propyl-2-thiouracil(PTU)로 유발된 rat의 갑상선기능저하증에 미치는 영향)

  • Objectives : In present study, we investigated a therapeutic effect of Astragali Radix on hypothyroidism rat model induced by 6-Propyl-2-thiouracil (PTU). Methods : Six-week-old male Sprague-Dawley rats were divided into five groups : Group one included the normal mice. Group two was administrated PTU. Group three and four were administrated the aqueous extract of Astragali Radix 150 and 300 mg/kg before start of PTU treatment. Group five (Positive control) was administrated with levothyroxine 0.5 mg/kg. During this moment the body weight, liver $H_2O_2$ and catalase (CAT) amount, serum thyroid hormone, serum asparte aminotransferase (AST) and alanine aminotransferase (ALT), gland weights were measured with histopathological changes of thyroid glands. These results were compared with levothyroxine 0.5 mg/kg treated rats. Results : The PTU treatment lead to marked decreases of body weight, level of thyroid hormone in serum and liver CAT activation. Also, PTU treatment increased thyroid gland weight, thyroid gland hormone TSH, liver $H_2O_2$ amount and level of AST in serum. On the other hands, the administration of Astragali Radix extract increased body weight gains and ameliorated histopathological changes of thyroid such as hyperplasia of follicular cells with of follicular colloid contents and sizes. In addition, the administration of Astragali Radix extract increased level of $T_4$ in serum, CAT activation in liver. Moreover, the administration of Astragali Radix extract decreased levels of TSH and AST in serum and $H_2O_2$ amount in liver Conclusions : This study suggests that Astragali Radix extract has therapeutic effects on hypothyroidism via promoting thyroid hormone production.

Hormonal Analysis of Female Transgender Patients Performed Gender Reassignment Operation (여성 성전환수술을 받은 환자의 호르몬 정량분석)

  • Transgender is the severe type of gender identity disorder. The prevalence rate of transgender is reported to occur to about 1 out of 50,000 men, and about 1 out of 10,000 women. As for Korea, it is estimated to have about 1400 transgender patients. Lately, not only the numbers of them are increasing but also they are influencing our society increasingly. As for female transgender patients, they take female hormone for a long term before and even after the operation to maintain their physical identity of female. We have analyzed insulin like growth factor-1(IGF-1), insulin like growth factor protein binding-3(IGFBP-3), female hormone, male hormone and thyroid hormone in female transgender patients who have undergone the gender reassignment operation. We examined the changes of hormone level due to having female hormone steadily, and also examined how the steady use of the hormone could affect body organs. As for IGF-1, it showed significantly low in the female transgender group compared to control ($319.30{\pm}37.4$ vs $539{\pm}55.0$, p<0.05). As for IGFBP-3, there was no significant difference ($2859{\pm}200.3$ vs $2607{\pm}262.5$, p>0.05). As for female hormone, there was no significant differences in FSH($13.42{\pm}13.8$ vs $8.95{\pm}3.5$, p>0.05), estradiol($104.41{\pm}97.1$ vs $121.68{\pm}60.2$, p>0.05), and LH($7.62{\pm}5.6$ vs $7.4{\pm}3.3$, p>0.05). Even in comparison of testosterone, there was no significant differences($0.23{\pm}0.09$ vs $0.33{\pm}1.33$, p>0.05). As for thyroid hormone, there was no significant differences in TSH and free T4($1.34{\pm}0.94$ vs $1.71{\pm}0.12$, $1.4{\pm}0.37$ vs $1.46{\pm}0.17$, p>0.05). Therefore, this study concludes that apart from the decreased level of IGF-1, the possible endocrine side-effect problem due to female hormone seems to be low because there was no differences of female, male, and thyroid hormone level compared with normal female. Further study will be required in metabolic change including bone metabolism occurred by decrease level of IGF-I.
