Thyroid uptake measurements can be subject to measurement errors due to the scoping and positioning of the thyroid gland. To compensate for these limitations, the clinical utility of the thyroid simultaneous counting method as an alternative to thyroid uptake measurement was analyzed and evaluated experimentally through quantitative analysis of images acquired after thyroid scanning. Experimental data were obtained using a Gamma camera (GE infinia), a thyroid uptake system (KOROID 1), and a thyroid neck phantom. Based on the thyroid uptake rate of 1-5% according to the protocol of thyroid scan test (99mTcO4 - , 370 MBq) in normal results, 99mTcO4 - was set in the range of 3.7-18.5 MBq (Matrix: 256×256, Scan time: 1 min, collimator: pin hole, phantom-collimator distances: 7 cm). The acquired images were corrected for the attenuation of isotopes due to the set-up time and half-life by applying the Auto Region of interest (ROI) drawing system, and the significance of the experimental results was evaluated by Multiple linear regression analysis (SPSS, ver. 22, IBM). The thyroid uptake rate showed a significant correlation between the dose and the measured counts when using the thyroid uptake system equipment. Meanwhile, the quantitative analysis counts of phantom images using Gamma camera also showed a significant correlation. Thus confirmed that the correlation between these two experiments was statistically significant (P<0.05). The simultaneous counting protocol, which indirectly measures thyroid uptake from thyroid scans, is likely to be clinically relevant if complemented by additional studies with different variables in patients with thyroid disease.
Objectives: Any uptake of I-131 after total thyroidectomy means the remant thyroid tissue or distant metastasis of the thyroid cancer. However diffuse hepatic uptake of I-131 without abnormal uptake was showen in many cases on I-131 whole body scan. The aim of this study was to classify the liver uptake after I-131 scan and to evaluate the analysis of this finding. Materials and Methods: Between 1982 and 1998, 104 patients(l4 males, 90 females) with normal liver function underwent I-131 scan after total thyroidectomy. Prospectively we reviewed the films of the whole body scan and analysed the correlations between results of radioiodine uptake, pathologic diagnosis, prognostic factors, lymphatic metastasis, and thyroid function test. Result: Diffuse hepatic uptake was found in 44 of 104(42%) patients. 10 of 39(26%) patients on I-131 100mCi, and 34 of 63(54%) on I-131 150mCi showed hepatic uptake. 52 of 104(50%) patients was locally invasive thyroid cancer. The rate of the hepatic uptake was no significant differences with the thyroid hormone levels(T3, Free T4) and thyroglobulin between uptake group and non-uptake group. Conclusion: The rate of I-131 uptake was high in high-dose radioiodine treatment group. However, we can not find any correlation among the thyroid functions, the extent of metastasis or the extent of local invasion. We need further study to find out the causes of the hepatic uptake of I-131 after total thyroidectomy, besides liver metabolism of I-131 attached thyroid hormones.
The $^{99m}Tc$-pertechnetate thyroid uptake rates(20 min) were measured in 24 healthy normal subjects, 140 patients with nontoxic goiter and 98 patients with thyrotoxicosis who were treated at the Thyroid Clinic, Seoul National University Hospital, from August 1972 to August 1973. Diagnostic reliabilities and correlations between $^{99m}TcO_4$-thyroid uptake rate (20 min) and other thyroid function tests were evaluated. The observed results were as follows 1. The $^{99m}TcO_4$-thyroid uptake rates (20 min) in normal subjects, euthyroid group and hyperthyroid group were $4.1{\pm}0.9%,\;5.2{\pm}1.8%\;and\;29.7{\pm}10.6%$. There was a significant difference between the mean of the euthyroid group and the mean of the hyperthyroid group and so differentiation between them can be easy. 2. In the diagnosis of hyperthyroidism, the reliabilities of $^{99m}TcO_4$- thyroid uptake rate(20 min), $^{131}I$ thyroid uptake rate(24hrs), serum $T_3$ resin uptake rate, serum $T_4\;and\;T_7\;were\;87.9{\sim}97.9%,\;81.2{\sim}94.4%,\;87.9{\sim}97.9%,\;90.5{\sim}99.3%\;and\;93.7{\sim}100%$. $^{99m}TcO_4$-thyroid uptake rate(20 min) is more accurate than $^{131}I$ thyroid uptake rate (24 hrs) in the diagnosis of hyperthyroidism. 3. $^{99m}TcO_4$-thyroid uptake rate (20 min) was well correlated with $^{131}I$ thyroid uptake rate (24 hrs), serum $T_3$ resin uptake rate, serum $T_4\;and\;T_7$. Points in favor of $^{99m}Tc$ are that it gives a small radiation dose to the thyroid, that tests can be repeated at the short interval, the study can be completed at a single patient visit and it is particularly well suited for the assessment of thyroid function in patients being treated with an antithyroid drug.
갑상샘 섭취율 측정을 위해 과산화테크네슘을 사용하고 있다. 대퇴부의 배후방사능 측정 시 무릎에서 골반 쪽으로 10 cm 위의 위치에서 검출기와 대퇴부의 거리를 20 cm, 25 cm 위치에서 병원들마다 서로 다른 거리에서 측정하고 있다. 두 거리에서 갑상샘 섭취율 측정을 정량 분석하여 갑상샘 섭취율을 구함에 있어서 유효성의 차이를 연구 하고자 하였다. 결과로써 50명의 대퇴부 20 cm에서 측정한 평균 갑상샘 섭취율은 3.532±4.312%로 나타났으며 25 cm에서 측정한 평균 갑상샘 섭취율은 3.680±4.304%로 나타났다. 갑상샘 섭취율 검사 결과를 통하여 20 cm 거리에서 측정된 대퇴부의 배후방사능 값이 상대적으로 25 cm 거리에서 측정된 배후방사능 값 보다 높게 측정되어 결과적으로 갑상샘 섭취율이 0.148% 낮게 나왔으며 배후방사능 측정을 위한 일정한 거리에서의 측정이 중요하다고 판단된다.
The 20-minute $^{99m}Tc-pertechnetate$ uptake became readily available for routine use and it replaced $^{131}I$ for thyroid imaging. However measuring thyroid uptake during a 5-minute minimizes pertechnetate uptake by the salivary glands and presence of contaminated saliva from those glands in to the pharynx and esophagus. A study was carried out to determine the suitability of the utility of a S-minute and 20-minute interval from administration of $^{99m}Tc-pertechnetate$ to imaging and uptake measurement as a replacement for the 24 hour standard originally established with $^{131}I$, and to evaluate the relationship between 5-minute $^{99m}Tc-pertechnetate$ uptake and other thyroid functions. A 5-minute and 20-minute uptake of $^{99m}Tc-pertechnetate$ were measured in 70 patients with thyroid disease at Yeungnam University Hospital from March 1, 1991 to Feb. 29, 1992. The results were as follows. 1) The 5-minute $^{99m}Tc-pertechnetate$ uptake in Graves' disease, Hashimoto's thyroiditis, simple goiter, non toxic nodular goiter, subacute thyroiditis and euthyroid were 18.2%, 14.6%, 2.8%, 3.2%, 1.2% and 1.1%, respectively. There was a significant difference between the mean of the euthyroid group and the mean of the Graves' disease. So differenciation between them can be easily made. 2) The 5 minute $^{99m}Tc-pertechnetate$ thyroid uptake was well correlated with 24 hour $^{131}I$ thyroid uptake (r=0.75, p<0.001). These data provided an equation for estimating the 24 hour uptake of iodide given the 5 minute pertechnetate uptake: Estimated 24-hour $^{131}I$ thyroid Uptake= 7.188*ln (5 minute $^{99m}Tc-pertechnetate$ uptake)+16.94 3) The 20-minute $^{99m}Tc-pertechnetate$ thyroid uptake was well correlated with 24-hour $^{131}I$ uptake (r=0.72, p<0.001) and 5-minute $^{99m}Tc-pertechnetate$ thyroid uptake (r=0.96, p<0.001). 4) In the Graves' disease, The 5-minute $^{99m}Tc-pertechnetate$ thyroid uptake was well correlated with serum $T_3-resin$ uptake (r=0.46, p<0.01), serum total $T_3$ (r=0.55, p<0.05), serum total $T_4$ (r=0.46, p<0.05). These results suggest that 5-minute ${99m}Tc-pertechnetate$ thyroid uptake has been found at least as useful as 24-hour $^{131}I$ uptake for diagnostic confirmation at our hospital, the logistical advantages of completing the diagnosis. The exam in 5-minutes led us to abandon the 24-hour study in the majority of patients, but the 24-hour $^{131}I$ uptake is still obtained in patients with planned or potential radioiodine therapy.

Hyperthroidism in known to alter the activity of a number of enzymes involved in the catabolism of histidine to CO2. 10-Formyltetrahydrofolate dehydrogenase(EC 1.5, 1.6, 10-formyl-THE dehydrogenase) catalyzes the NADP-dependent conversion of 10-formyltetrahydrofolate to tetrahydrofolate and CO2. In previous studies, 10-formyl-THF dehydrogenase purified from rat and pig liver was coidentified with the cytosolic folate-binding protein. In this study, we investigated the effects of feeding thyroid powder (TP) and thiouracil(TU) on the folate-binding properties of 10-formyl-THE dehydrogenase, the uptake of an injected dose of [3H] folate, and the metabolism of labeled folate to pteroylopoly-${\gamma}$-glutamate in rat liver. The initial hepatic uptake(24hr) of the labeled folate dose was higher in TU-rats and slightly higher in TP-rats in controls. With longer time periods, decreased hepatic uptake of labeled folate was observed in TP-animals compared to euthroid animals, and high levels of hepatic uptake of labeled folate were maintained in TU-animals. This data shows that high levels of thyroid hormone decreased the retention of folate in rat liver. Folate polygutamate chain length was shorter in TU-rats than controls, which suggests that thyroid states do not affect the ability to synthesize pteroylpolyglutamates and that folate polyglutamate might be modulated by altered folate pool size. The ability of 10-formyl-THE dehydrogenase to bind folate in rat liver was similar in both TP-and TU-rats although dehydrogenase activity was changed by thyroid sates.
A 30 year-old female patient with papillary thyroid carcinoma received her fifth radioiodine ablation therapy after the subtotal thyroidectomy. The scan, which was peformed one week after the last therapy, revealed residual uptake in the thyroid bed and uptake in the anterior mediastinum suggesting metastasis. However, further evaluation of the thorax with chest CT and camera-based FDG PET confirmed normal thymus without metastatic focus. Occasionally thymus remains intact in adult and has avidity for I-131 and FDG. Therefore, normal thymus (instead of metastasis) should be considered in patients with well differentiated thyroid carcinoma and anterior mediastinal radioiodine uptake.
목적: 방사성옥소를 잘 섭취하는 갑상선 전이암들은 낮은 FDG 섭취를 보이나, 방사선옥소를 섭취하지 않는 경우는 FDG PET에서 잘 보인다고 알려져 있다. 본 연구에서는 원발성 갑상선 유두상암의 FDG 섭취 양상이 어떠한지 알아보고자 하였다. 대상 및 방법: 초음파유도하 세침흡입술로 갑상선 유두상암을 진단을 받고 갑상선 전절제술이 예정되었던 환자 40명들이 시행한 FDG PET/CT들을 대상으로 후향적 분석을 시행하였다. FDG 섭취의 정도는 병변이 주변 조직보다 높은 섭취를 보이는 것을 육안적으로 확인할 수 있는 경우 양성으로 하고 그렇지 않은 경우 음성으로 나누었으며, 병변의 최대 표준화섭취계수(peak SUV)를 구하여 그 크기와 비교하였다. 결과: FDG 섭취 양성소견을 보이는 27례의 갑상선 유두상암들의 평균 크기는 $1.9{\pm}1.4\;cm\;(0.5{\sim}5\;cm)$였고, FDG 섭취를 보이지 않는 13례의 경우는 $0.4{\pm}0.2cm\;(0.2{\sim}0.9\;cm)$이었다. 갑상선의 크기가 1 cm 이상인 19례들은 ($2.5{\pm}1.4\;cm,\;1{\sim}5\;cm$) 모두 FDG 섭취 양성소견을 보였다 (19/19, peak $SUV\;=\;6.4{\pm}5.7,\;1.7{\sim}22.7$). 1cm 미만의 21례의 미세갑상선암들 중 8례가 FDG 섭취 양성소견을 보인 반면 (8/21, peak $SUV\;=\;2.9{\pm}1.3,\;1.7{\sim}5.5$), 13례는 음성소견을 보였다(peak $SUV\;=\;1.2{\pm}0.2,\;1.1{\sim}1.7$). 미세갑상선암들 중 FDG 섭취 양성 소견을 보이는 경우의 크기는 $0.7{\pm}0.1\;cm$으로 음성소견을 보이는 경우의 크기 $0.4{\pm}0.2\;cm$와 비교하여 유의하게 컸다(P=0.01). 결론: 본 보고에서 40례의 원발성 갑상선 유두상암 가운데 1 cm 이상 크기의 갑상선 유두상암들은 모두 FDG 섭취 양성소견을 보였는데, 이는 방사성옥소를 섭취하지 않는 분화도가 나쁜 갑상선암들만 FDG 섭취를 보인다는 이전 문헌의 내용과 상충되며, 이러한 원발성 갑상선 유두상암의 FDG 섭취를 이해하기 위해서는 GLUT 및 NIS 발현과 연관한 후속 연구가 필요하겠다.
A 44 year-old male patient was undergoing diagnosis and therapy for acute myelogenous leukemia (AML, M2). On physical examination a thyroid mass was palpated in the left lower lobe. He had palpitation and intolerance to heat. Thyroid function tests revealed hyperthyroidism; T3: 150ng/dl (N:60-90), fT4: 2.26 ng/dl (N:0.70-1.80), TSH: 0.01 ulU/ml (N:0.25-5.00). Ultrasonography demonstrated a hypoechoic mass with scattered calcifications measuring 2.55 2.03 3.64 cm in size. F-18 FDG camera-based PET scan performed as a follow-up study of AML revealed a focal increased uptake in the left neck, where an autonomous nodule was detected on Tc-99m thyroid scan. After the diagnosis of toxic autonomous nodule, Goetz disease, he underwent surgical nodulectomy. Microscopically, the nodule contained follicular proliferation with degenerative change but without evidence of thyroid carcinoma. Focal uptake in autonomous thyroid nodules is due to increased glycolysis within the nodules.
