노년 인구의 주거환경 평가도구에 관한 체계적 고찰 (Systematic Review of Assessment Tools for the Housing Environment of the Old Adults Population)

  • 목적 : 본 연구 목적은 노년 인구의 주거환경을 평가하기 위해 사용된 평가도구들을 체계적 고찰하고자 하였다. 연구방법 : 자료수집은 2015년 1월부터 2023년 8월 31일까지 해외 데이터베이스 Cochranelibrary, Pubmed, Proquest를 검색하여 주제와 관련된 자료를 추출하였다. 주거환경 평가도구를 사용한 267편에서 원도구를 기준으로 최종 9개의 평가도구를 추출하였다. 분석은 최종 9개 평가도구의 사용 동향 및 빈도, 특성, 평가 범위, 타당도 및 신뢰도 여부 등을 체계적으로 분석하였다. 결과 : 총 9개 평가도구의 사용빈도는 HOME FAST, IPAQ-E가 각 20%로 많이 사용되었다. 원도구의 평가 목적은 친화성(AFCCQ, AFEAT, NEWS), 물리적 장벽(CHIEF, PENBOM), 낙상 예방(HOME FAST), 치매노인의 가정환경(HEAC), 신체활동에 관한 환경(IPAQ-E), 접근성(Walk Score®)으로 평가하고 있었다. 평가 범위는 고령친화도시(Age-Friendly Cities: AFC)의 8개 영역을 기준으로 AFCCQ, AFEAT, CHIEF는 8개 영역 모두를 평가하고 있었으며, 이외 NEWS (Full)는 3개, HOME FAST, IPAQ-E, Walk Score®는 2개 그리고 HEAC, NEWS (Abb), PENBOM는 1개 영역을 포함하고 있었다. 결론 : 노년기에 살던 곳에서 살고자 하는 계속 거주를 실현하기 위해 고려되는 주거환경을 평가할 때, 평가 목적과 범위의 적절성 그리고 논리적 타당성 여부를 판단할 수 있는 기초자료를 제시하고 있음에 의의가 있다.

어성초 ethanol 추출물의 마우스 살모넬라 감염증에 대한 항균 및 치료효과 규명 (Antibacterial and Therapeutic Effects of Houttuynia cordata Ethanol extract for Murine Salmonellosis)

  • 항생제의 오남용으로 인한 항생제 내성을 가진 세균의 출현은 공중보건학적인 측면에서 많은 피해를 입히고 있다. 또한, 세포 내 기생세균의 경우 예방과 치료가 어려운 상황에 놓여져 있고 항생제 대체 물질개발에 많은 관심이 집중되고 있다. 어성초는 오랜 기간 동안 소독제, 이뇨제, 해열제, 항균제, 항바이러스 제제, 항염증 제제로서 사용되어왔으며 중요한 민간요법의 하나로서 인식되고 있는 바, 본 실험에서는 어성초의 ethanol 추출물이 세포내 기생 난치성 세균 감염증인 S. typhimurium의 감염증에 대한 치료효과를 검증하였다. 본 실험의 결과로서 HCEE 추출물이 탐식세포의 형태적 변화를 유도하였고, S. typhimurium에 대한 직접적인 살균작용과 탐식세포를 통한 항균작용은 미약한 것으로 나타났다. 또한 HCEE 추출물이 S. typhimurium의 탐식세포 감염 시 시간경과에 따라 감염능 및 세포내 증식능이 감소하는 것이 확인되었다. 탐식균에 의한 사멸을 유도하는 탐식세포의 NO의 산생량에 있어서는 HCEE 추출물의 처리가 탐식세포로부터 NO 산생이 감소되어 NO를 이용한 탐식세포의 균 사멸과는 직접적 연관이 없을 것으로 추정되며, 마우스 감염시험에서 HCEE를 투약이 상당한 치료효과를 나타내어 HCEE가 S. typhimurium에 의한 염증을 감소시키고, 또한 apoptosis를 유도함으로 인해 균을 제거함으로 이루어지는 것으로 추측된다. 본 연구를 통해 HCEE의 살모넬라증에 대한 치료효과를 확인 할 수 있었으며, 천연소재 약용자원을 활용한 난치성 세균 감염증에 대한 신약개발이 가능하고, 이의 활용은 항생제 오남용을 줄일 수 있고 국민보건 증진에 이바지 할 것이다.

펄스자기장이 위팔 두갈래근의 지연성 근육통에 미치는 영향 (The Influences of Pulsed Electromagnetic Field Treatment Following Experimentally Induced Delayed-Onset Muscle Soreness in Biceps Brachii)

  • Delayed onset muscle soreness (DOMS) is a painful condition that arises from exercise-induced muscle damage after unaccustomed physical activities. Various therapeutic interventions have been applied to reduce the intensity and duration of DOMS-related symptoms. Recently, pulsed electromagnetic field (PEMF) intervention has been introduced as an alternative noninvasive treatment for DOMS. This randomized, double-blind, placebo-controlled experiment was conducted to examine the effects of PEMF therapy on DOMS in elbow flexors at 24, 48, and 72 hours after the experimental DOMS induction. Thirty healthy volunteers ($23{\pm}2.4$ yrs, $175{\pm}5.7$ cm, and $74{\pm}7.8$ kg) participated in this study. Each was randomly assigned to a PEMF or placebo group. On the first day, DOMS was induced in the elbow flexors by repeated isokinetic motions at low ($60^{\circ}/s$) and fast ($120^{\circ}/s$) speeds in all subjects. Thereafter, the PEMF group received 15-min daily treatment with a PEMF device. The placebo group received sham treatment of the same duration. Overall, PEMF application was more effective than the sham treatment in reducing the physiological symptoms associated with the DOMS including perceived soreness, median frequency, and electromechanical delay of the surface electromyography. In addition, median frequency and isokinetic peak torque of the PEMF group recovered to the pre-DOMS induction level earlier than the placebo group. In conclusion, this study suggests that PEMF can be applied as a new recovery strategy in reducing DOMS symptoms. Further experiments are required to examine the effect of the PEMF treatment on different types of exercise conditions and to determine the optimal treatment dosage and duration in a real clinical setting.

국내 평가 가이드라인 제시를 위한 전기식 온구기의 열특성에 관한 연구 (Study on the Thermal Properties of Electric Moxibustion Apparatus for Presenting Assessment Guideline in Korea)

  • Objectives : To characterize the thermal properties of several Korean electric moxibustion apparatus and to suggest methods for improving their quality for related industry as well as the welfare of Korean. Methods : We reviewed previous literatures on traditional moxibustion to find necessary factors for electric moxibustion apparatus. We measured the thermal characteristics of electric moxibustion apparatus commercially available by using an automatic temperature acquisition system in a controlled environment. Uniformity and heat loss of the apparatus were also analysed. We followed the user's manuals of apparatus provided by manufacturers. Results : Temperature control of all apparatus could not be achieved by automatic manners. Most of them were dependent on user's discretion. Maximum temperatures obtained were barely in the therapeutic temperature of $40-45^{\circ}C$. Unnecessary heating of moxibustion parts was detected. Chemicals from moxa were not properly delivered to the human skin. Conclusions : Temperature control of all apparatus needs to be improved in terms of temperature setting, retention time and maximum temperature. Design should be altered to utilize pharmacological effects from moxa to obtain its maximum efficacy. User's manuals should be revised for its clarity.

The Effect of Prism Adaptation Following Traumatic Brain Injury: A case report

  • Background: The presence of visuospatial impairment can make patients slow functional recovery and impede the rehabilitation process in TBI patients. Objective: The aim of this study is to investigate effects of prism adaptation treatment for functional outcomes in patients following traumatic brain injury. Methods: The subject received prism adaptation treatment for 2 weeks additionally during traditional rehabilitation for 4 weeks. The Patient has prism adaptation treatment while wearing wedge prisms that shift the external environment about $12^{\circ}$ leftward. The patient received 10 sessions, 15-20min each session. Outcome measures were visuospatial deficit(line bisection, latter cancellation), Visual and spatial perception(LOTCA-visual perception and spatial perception), motor function of upper extremity(FMA U/E; Fugl-Meyer motor assessment upper extremity, ARAT; Action research arm test), balance(BBS; Berg Balance Scale), mobility(FAC; Functional ambulation classification) and functional level(FIM; Functional independent measure). All Assessments took place on study entry and post-treatment assessments were performed at discharge from the hospital. Results: After prism adaptation, the visuospatial impairment scores improved as indicated in the line bisection(-15.2 to -6.02), latter cancellation(2 to 0) and LOTCA- spatial perception scores(7 to 9). The upper motor function improved as indicated in the scores of affected FMA U/E(21 to 40) and ARAT(4 to 22). Ambulation and balance improved as indicated in the BBS scores(25 to 38) and FAC scores(0 to 4). ADL function improved as indicated in the FIM total scores 54 to 70(motor 34 to 61, cognition 20 to 29). Conclusion: Prism adaptation did improve functional level such as motor functions and ADL abilities in TBI patient. Further research is recommended.

조현병 환자의 입원스트레스 경험에 대한 융합연구 (Integrated Study on Hospitalization Stress Experience of Schizophrenic Patient)

  • 본 연구의 목적은 심층면담을 통해 조현병환자의 입원스트레스 경험을 규명하고 입원스트레스를 중재 할 수 있는 기초 자료를 마련하고자 한다. 연구방법은 두 지역의 3개 정신과 병원 폐쇄 병동에 입원중인 조현병환자 15명을 대상으로 심층 면담을 시행하여 면담 내용을 질적 내용분석하였다. 연구 대상자는 15명으로 평균 나이는 40세였다. 현재 입원 기간은 평균 2년이었다. 연구기간은 2015년 10월 21일부터 11월 10일까지이다. 연구결과 입원스트레스 내용은 5개의 상위범주와 17개의 하위범주 및 58개의 의미 있는 내용이 도출되었다. 5개의 상위범주는 '가족에 대한 그리움과 죄책감', '미흡한 치료적 환경으로 인한 불편함', '환자의 권리를 존중받지 못함', '퇴원 후 삶에 대한 불안', '사회적인 편견과 자기낙인감'이었다. 본 연구 결과는 조현병환자의 입원스트레스를 중재 할 수 있는 기초자료를 마련한 것에 의의가 있다. 후속연구에서는 같은 환경에 입원중인 조울증, 알코올 중독 등 다른 정신질환을 가진 대상자의 입원스트레스를 파악하여 비교 분석해보는 연구를 제언한다.

Long-Duration Three-Dimensional Spheroid Culture Promotes Angiogenic Activities of Adipose-Derived Mesenchymal Stem Cells

  • Mesenchymal stem cells (MSCs) offer significant therapeutic promise for various regenerative therapies. However, MSC-based therapy for injury exhibits low efficacy due to the pathological environment in target tissues and the differences between in vitro and in vivo conditions. To address this issue, we developed adipose-derived MSC spheroids as a novel delivery method to preserve the stem cell microenvironment. MSC spheroids were generated by suspension culture for 3 days, and their sizes increased in a time-dependent manner. After re-attachment of MSC spheroids to the plastic dish, their adhesion capacity and morphology were not altered. MSC spheroids showed enhanced production of hypoxia-induced angiogenic cytokines such as vascular endothelial growth factor (VEGF), stromal cell derived factor (SDF), and hepatocyte growth factor (HGF). In addition, spheroid culture promoted the preservation of extracellular matrix (ECM) components, such as laminin and fibronectin, in a culture time- and spheroid size-dependent manner. Furthermore, phosphorylation of AKT, a cell survival signal, was significantly higher and the expression of pro-apoptotic molecules, poly (ADP ribose) polymerase-1 (PARP-1) and cleaved caspase-3, was markedly lower in the spheroids than in MSCs in monolayers. In the murine hindlimb ischemia model, transplanted MSC spheroids showed better proliferation than MSCs in monolayer. These findings suggest that MSC spheroids promote MSC bioactivities via secretion of angiogenic cytokines, preservation of ECM components, and regulation of apoptotic signals. Therefore, MSC spheroid-based cell therapy may serve as a simple and effective strategy for regenerative medicine.

Pichia pastoris: A Recombinant Microfactory for Antibodies and Human Membrane Proteins

  • During the last few decades, it has become evident that the compatibility of the yeast biochemical environment with the ability to process and translate the RNA transcript, along with its capacity to modify a translated protein, are relevant requirements for selecting this host cell for protein expression in several pharmaceutical and clinical applications. In particular, Pichia pastoris is used as an industrial host for recombinant protein and metabolite production, showing a powerful capacity to meet required biomolecular target production levels in high-throughput assays for functional genomics and drug screening. In addition, there is a great advantage to using P. pastoris for protein secretion, even at high molecular weights, since the recovery and purification steps are simplified owing to relatively low levels of endogenous proteins in the extracellular medium. Clearly, no single microexpression system can provide all of the desired properties for human protein production. Moreover, chemical and physical bioprocess parameters, including culture medium formulation, temperature, pH, agitation, aeration rates, induction, and feeding strategies, can highly influence product yield and quality. In order to benefit from the currently available wide range of biosynthesis strategies using P. pastoris, this mini review focuses on the developments and technological fermentation achievements, providing both a comparative and an overall integration analysis. The main aim is to highlight the relevance and versatility of the P. pastoris biosystem to the design of more cost-effective microfactories to meet the increasing demands for recombinant membrane proteins and clinical antibodies for several therapeutic applications.

Role of Catheter's Position for Final Results in Intrathecal Drug Delivery. Analysis Based on CSF Dynamics and Specific Drugs Profiles

  • Intrathecal drug delivery is an effective and safe option for the treatment of chronic pathology refractory to conventional pain therapies. Typical intrathecal administered drugs are opioids, baclofen, local anesthetics and adjuvant medications. Although knowledge about mechanisms of action of intrathecal drugs are every day more clear many doubt remain respect the correct location of intrathecal catheter in order to achieve the best therapeutic result. We analyze the factors that can affect drug distribution within the cerebrospinal fluid. Three categories of variables were identified: drug features, cerebrospinal fluid (CSF) dynamics and patients features. First category includes physicochemical properties and pharmacological features of intrathecal administered drugs with special attention to drug lipophilicity. In the second category, the variables in CSF flow, are considered that can modify the drug distribution within the CSF with special attention to the new theories of liquoral circulation. Last category try to explain inter-individual difference in baclofen response with difference that are specific for each patients such as the anatomical area to treat, patient posture or reaction to inflammatory stimulus. We conclude that a comprehensive evaluation of the patients, including imaging techniques to study the anatomy and physiology of intrathecal environment and CSF dynamics, could become essential in the future to the purpose of optimize the clinical outcome of intrathecal therapy.

Antiviral Efficacy of a Short PNA Targeting microRNA-122 Using Galactosylated Cationic Liposome as a Carrier for the Delivery of the PNA-DNA Hybrid to Hepatocytes

  • Peptide nucleic acids (PNAs) that bind to complementary nucleic acid sequences with extraordinarily high affinity and sequence specificity can be used as antisense oligonucleotides against microRNAs, namely antagomir PNAs. However, methods for efficient cellular delivery must be developed for effective use of PNAs as therapeutic agents. Here, we demonstrate that antagomir PNAs can be delivered to hepatic cells by complementary DNA oligonucleotide and cationic liposomes containing galactosylated ceramide and a novel cationic lipid, DMKE (O,O'-dimyristyl-N-lysyl glutamate), through glycoprotein-mediated endocytosis. An antagomir PNA was designed to target miR-122, which is required for translation of the hepatitis C virus (HCV) genome in hepatocytes, and was hybridized to a DNA oligonucleotide for complexation with cationic liposome. The PNA-DNA hybrid molecules were efficiently internalized into hepatic cells by complexing with the galactosylated cationic liposome in vitro. Galactosylation of liposome significantly enhanced both lipoplex cell binding and PNA delivery to the hepatic cells. After 4-h incubation with galactosylated lipoplexes, PNAs were efficiently delivered into hepatic cells and HCV genome translation was suppressed more than 70% through sequestration of miR-122 in cytoplasm. PNAs were readily released from the PNA-DNA hybrid in the low pH environment of the endosome. The present study indicates that transfection of PNA-DNA hybrid molecules using galactosylated cationic liposomes can be used as an efficient non-viral carrier for antagomir PNAs targeted to hepatocytes.
