Oreochromis spp. and Clarias Lazera as a Source of Transmitting Encysted Metacercariae to Man

  • A total of 135 freshwater fish samples (Oreochromis spp. <85> and Clarias lazera <50>) were collected from different localities (shops and fish markets) in Alexandria Province-Egypt and examined for the presence of parasitic larval stages. The obtained data found the overall mean of the infestation with encysted metacercariae was 71.1% in the examined fish. The rates of infestation with encysted metacercariae were 72.9% and 68.0% in the examined Oreochromis spp. and Clarias lazera, respectively. The highest prevalence of the encysted metacercariae was found in summer (81.8%) in case of Oreochromis spp. and in winter (92.3%) in case of Clarias lazera, all these seasonal differences are statistically significant. The rate of infestation of encysted metacercariae in the anterior third, middle third and posterior third of Oreochromis spp. was found to be 82.3%, 74.2% and 95.2%, respectively, while 88.2%, 88.2% and 91.2%, respectively, in Clarias lazera. Moreover, the rate of infestation of encysted metacercariae in eyes, gills, liver, kidney and branchial cavity of Oreochromis spp. was 91.9%, 70.9%, 20.9%, 46.8% and 32.2%, respectively. In Clarias lazera the infestation rate was found to be 47.1%, 70.6%, 8.8% and 38.2% in the gills, liver, spleen and kidney, respectively. The zoonotic and public health importance of the encysted metacercariae are causes severe visceral pain, abdominal discomfort, intermittent bloody diarrhoea and colic after consumption of inadequately cooked fish especially Tilapia spp. (Oreochromis spp.).

Sirolimus and Non-melanoma Skin Cancer Prevention after Kidney Transplantation: A Meta-analysis

  • Background: Whether sirolimus is useful in the prevention of non-melanoma skin cancer (NMSC) remains unclear and we therefore performed this meta-analysis of randomized controlled trials to test the hypothesis that Sirolimus-based immunosuppression is associated with a decrease in NMSC. Methods: The main outcomes were NMSC, squamous-cell carcinoma and basal-cell carcinoma. The pooled risk ratio (RR) with its 95% confidence interval (95%CI) were used to assess the effects. Results: 5 randomized trials involving a total of 1499 patients receiving kidney transplantation were included. Patients undergoing Sirolimus-based immunosuppression had much lower risk of NMSC (RR = 0.49, 95%CI 0.32-0.76, P = 0.001). Subgroup analyses by tumor type showed that Sirolimus-based immunosuppression significantly decreased risk of both squamous-cell carcinoma (RR = 0.58, 95%CI 0.43-0.78, P < 0.001) and basal-cell carcinoma (RR = 0.56, 95%CI 0.37-0.85, P = 0.006). The quality of evidence was high for NMSC, and moderate for squamous-cell carcinoma and basal-cell carcinoma. No evidence of publication bias was observed. Conclusion: High quality evidence suggests that Sirolimus-based immunosuppression decreases risk of non-melanoma skin cancer, and Sirolimus has an antitumoral effect among kidney-transplant recipients.

  • Penetrating chest trauma by stab injury may result in massive hemothorax from damage to single or multiple intrathoracic organs such as heart, aorta, internal mammary artery, intercostal artery or pulmonary parenchyme. Prognosis of massive hemothorax necessitating emergency thoracotomy is fatal especially so if there exists concomitant underlying compromise of cardiopulmonary function. A 56 year old man with destroyed left lung due to old pulmonary tuberculosis was stabbed in right parasternal lesion through third intercostal space. Intubation with cardiopulmonary resuscitation and closed thoracostomy were performed to resuscitate from cardiac asystole from hemorrhagic shock and acute respiratory distress. Midsternotomy was made to expose active bleeding foci in right mammary artery, subclavian vein, intercostal artery and anterior segment of right upper lung showing severe bullous change and pleural adhesion. Postoperative care included ventilator support, inotropic instillation and cautious, balance fluid therapy ; successful extubation was done on third postoperative day and patient was discharged on tenth postoperative day without any complication.

A Clinical Study of Mandibular Angle Fracture

  • Purpose: To establish management protocol for mandibular angle fracture, we describe pertinent factors including cause, impacted third molar and recent treatment tendency. Methods: We examined the records of 62 patients who had unilateral mandibular angle fracture. Sixty patients who had open reduction surgery were examined at postoperative weeks 1, 4, 8, 12, and 28. Results: Left mandibular angle fracture is frequent in younger males. Presence of the mandibular third molar can increase fracture risk. Because of attached muscle, favorable fractures occurred primarily in the mandibular angle area. Conclusion: Extracting the mandibular third molar can prevent angle fractures, and open reduction with only one plate adaptation is generally the proper treatment method for mandibular angle fracture.

  • This paper considers the problem of estimating paramaters of the bivariate exponential distribution with a loaction parameter for a two-component shared parallel system using component data from system-level life test terminated at the time of the prespecified number of system failure. In the system-level life testing, there are three patterns of failure types; 1) both component failed 2) both component censored 3) one is failed and the other is censored. In the third case, we assume that the failure time might be known or unknown. The maximum likelihood estimators are obtained for the case of known/unknown failure time when the other component is censored.

  • In this paper, the LQG/LTR design method is applied of the third order linear time invariant plant model which is the SISO turret-gun servo-mechanism. The dynamic characteristics and the performance of the LQG/LTR controller are analyzed by the computer simulation, and compared with those of PID controller which has been already applied to the turret servomechanism under the sane design specifications.

  • Adenoid cystic carcinoma of the esophagus has been relatively an uncommon, slow growing tumor. A 51 year-old man patient had a tumor in the lower third of the esophagus which was incidentally found during an examination for UPPER C-I series, and resected successfully without Thoracotomy. The tumor exhibited a polypoid appearance covered by normal esophageal epithelium, localized entirely in the submucosal layer of the esophagus and morphologically identical to adenoid cystic carcinoma in the salivary glands.

  • Choonwon, Lee Kwang-Soo was a literary man and activist for educational movement in modem age of Korea. This research was to review his arguments about improvement of house living. The major results of the research were as follows. First, he asked Korean people to establish solid identity of living. Second, he placed great emphasis on building hygiene facilities and bathroom. Third, he argued that the space within a household need to be divided in accordance with function. Finally he suggested Korean people to make home environment beautiful and convenient.

  • The extracted right mandibular third molar of a 37-year-old man was transplanted into the first molar area, and a bone graft procedure using autogenous tooth-bone graft material was performed for the space between the root and the alveolar socket. Reattachment was achieved after 10 months. Therefore, autogenous tooth-bone graft material is considered reasonable for bone induction and healing in the autotransplantation of teeth.
