Immunohistochemical Expression of B Cell Lymphoma-2 with Clinicopathological Correlation in Triple Negative Breast Cancers in Northern Pakistan

  • Background: Triple negative breast cancers (TNBCs) are high grade aggressive tumors generally with a poor prognosis, not responding to hormonal and anti Her2 Neu therapy. Expression of the antiapoptotic B cell lymphoma 2 gene (Bcl-2) is associated with low grade, slowly proliferating hormone receptor positive tumors with improved survival. Anti Bcl2 agents can be used as alternative targeted therapy in triple negative cancers. Materials and Methods: The objective of this study was to determine the immunohistochemical expression of Bcl2 in triple negative breast cancers and any correlation with clinicopathological variables in Northern Pakistan. Results: All 52 patients were females, aged between 28 and 80 years(average $48.0{\pm}12.1$). 28 cases (53.8%) were positive for Bcl2, this being associated with low grade invasive ductal carcinomas, lymph node metastasis and lymphovascular invasion. Conclusions: Bcl-2 may be an important prognostic factor and its expression might be used for targeted therapy using Anti Bcl2 drugs.

Rapid and Direct Detection of Vibrio vulnificus in Small Octopus (Octopus variabilis) Using Polymerase Chain Reaction

  • The cells of Vibrio vulnificus can be induced to the viable but nonculturable (VBNC) state by natural environmental parameters. The V. vulnificus cells in the VBNC state can not be recovered by ordinary laboratory techniques. This nonculturability could often hamper development of effective processing strategies to minimize the number of V. vulnificus in seafoods. Even with V. vulnificus cells in a culturable state, the length of time required to identify the bacteria in contaminated food by phenotyphic characterization may prevent appropriate in-time responses by public health agencies to infections of the bacteria. In the present study, we used polymerase chain reaction (PCR) to develop a rapid and direct detection method for V. vulnificus in small octopus (Octopus variabilis) which is consumed as a raw food in Korea. The region targeted was a 704-base pair (bp) portion of the hemolysin gene, vvhA, of V. vulnificus. The primers designed for PCR amplification were specific for all V. vulnificus sp. tested. Several methods were examined to extract total DNA directly from V. vulnificus seeded into the octopus homogenate and the guanidine isothiocyanate (CITC) method appeared to be most effective. From the octopus homogenate seeded by V. vulnificus at an initial level of $10^2$ CFU/ml of the homogenate and then incubated for 12 h, the targeted sequence was successfully amplified by PCR and the 704-bp DNA fragment was observed by gel electrophoresis. The total completion of this assay requires less than one day.

보완의학 연구동향 - 제3차 국제 보완의학연구학회 발표연구들을 중심으로 - (Research Trends in Complementary Medicine - Analysis of Studies Presented in International Congress of Complementary Medicine Research -)

  • Objective: This study was aimed to review the recent trends of complementary medicine research in ICCMR 2008. Methods: Analyzed data include all researches of both oral and poster presentations in ICCMR. We classified presented researches according to their treatment method, nations, and targeted diseases. Results: In total, 338 researches were presented. In terms of methods, 68 basic researches, 98 clinical trials, 55 commentaries, 46 reviews, 71 trend analysis papers were classified. Herbal medicine was the most common treatment for research in ICCMR. And in terms of nation. Australia's announcement was the most in ICCMR. Lastly, cancer was the most focused theme in the conference. Conclusion: In the recent CAM research, cancer, depression and stress are the most targeted diseases since they are difficult to cure in modern medicine and since patients have high hopes to overcome their state by CAM. This review will help Korean doctors or scientists to systematically understand traditional medicine and to develop their researches.

  • Paper is on defense for so-called internet crisis, the attack of DDoS (Distributed Denial of Service) which was targeted to the central government ministries, financial sector, and portal sites of chief counties including Korea on June 7th, 2009 as its start. By conducting attack with various DDoS attacking methods in the lab environment and dividing networks targeted by the attack by layers, this paper records and analyzes the chief information for attack, destination information of packets, defense policy setting, and the flow of packet attack with the subjects of the networks separated. This study suggests CFC system using multiple firewalls applying defense policy corresponding to the target layer for ultimate attack and tests it according to the result of analyzing the attack packet information and its amount, log analysis, access recording port, and MAC and IT information, etc. by layers. This article is meaningful in that it analyzes the attack by layers, establishes firewall policy for protecting each layer, and secures accurate mechanism for detect and defense.

Early-onset epileptic encephalopathies and the diagnostic approach to underlying causes

  • Early-onset epileptic encephalopathies are one of the most severe early onset epilepsies that can lead to progressive psychomotor impairment. These syndromes result from identifiable primary causes, such as structural, neurodegenerative, metabolic, or genetic defects, and an increasing number of novel genetic causes continue to be uncovered. A typical diagnostic approach includes documentation of anamnesis, determination of seizure semiology, electroencephalography, and neuroimaging. If primary biochemical investigations exclude precipitating conditions, a trial with the administration of a vitaminic compound (pyridoxine, pyridoxal-5-phosphate, or folinic acid) can then be initiated regardless of presumptive seizure causes. Patients with unclear etiologies should be considered for a further workup, which should include an evaluation for inherited metabolic defects and genetic analyses. Targeted next-generation sequencing panels showed a high diagnostic yield in patients with epileptic encephalopathy. Mutations associated with the emergence of epileptic encephalopathies can be identified in a targeted fashion by sequencing the most likely candidate genes. Next-generation sequencing technologies offer hope to a large number of patients with cryptogenic encephalopathies and will eventually lead to new therapeutic strategies and more favorable long-term outcomes.

Ribozyme-Mediated Replacement of p53 RNA by Targeted Trans-Splicing

  • In more than half of human tumors, the p53 tumor suppressor gene is mutated. Thus, restoration of wild-type p53 activity by repair of mutant RNA could be a potentially promissing approach to cancer treatment. To explore the potential use of RNA repair for cancer therapy, trans-splicing group I ribozymes were developed that could replace mutant p53 RNA with RNA sequence attached to the 3'end of ribozymes. By employing a mapping library of ribozymes, we first determined which regions of the p53 RNA are accessible to ribozymes, and found that the leader sequences upstream of the AUG start codon appeared to be particularly accessible. Next, trans-splicing ribozymes were generated that specifically recognized the sequences around these accessible regions. Subsequently, the ribozymes reacted with and altered the p53 transcripts by transferring a 3'exon tag sequence onto the targeted p53 RNA with high fidelity. Thus, these ribozymes could be utilized to repair mutant p53 in tumors, which would revert the neoplastic phenotype.

  • Purpose: The purpose of this study was to evaluate the effect according to the NEX, VENC, targeted cardiac phases on the velocity measurement of 4D phase-contrast MRI. Materials and Methods: The abdominal aortic phantom was made to experiment. The working fluid was mixed with water and glycerin to mimic the density and viscosity of human blood. The inlet velocity was Reynolds number 2000. The experimental conditions were NEX 1 and 4, VENC 102 cm/s and 200 cm/s, and 10 and 15 targeted cardiac phases, respectively. The average flow rate, average velocity, maximum velocity, and cross-section area were measured. Results: As a result of the case-by-case comparison, the error rate was less than 5%. There was no significant difference (p > 0.05). Conclusion: It is expected that this result will be useful for acquiring blood flow information in clinical practice.

The Power of Being Small: Nanosized Products for Agriculture

  • Certain agrochemicals may be tuned for increased effectiveness when downsized to nanoparticles (NPs), where one dimension is less than 100 nm. The NPs may function as fertilizers, pesticides and products to improve plant health through seed priming, growth promotion, and induction of systemic tolerance to stress. Formulations will allow targeted applications with timed release, reducing waste and pollution when compared to treatments with bulk-size products. The NPs may be a single component, such as nano-ZnO as a fertilizer, or be composites of compatible materials, for example where N, P, and K plus micronutrients are available. The active materials could be loaded into porous carriers or tethered to base nanostructures. Coatings could include such natural products alginate, chitosan, zein, or silica. Certain NPs are taken up and transported in the plant's phloem and xylem so systemic effects are feasible. Timed and targeted release of the active product could be achieved in response to changes in pH or availability of ligands within the plant or the rhizosphere. Global research has revealed the many potentials offered by NP formulations to aid sustainability in agriculture. Current work will provide information needed by regulatory agencies to assess their safety in the agricultural setting.

Reirradiation of head and neck cancer in the era of intensity-modulated radiotherapy: patient selection, practical aspects, and current evidence

  • Locoregional failure is the most frequent pattern of failure in locally advanced head and neck cancer patients and it leads to death in most of the patients. Second primary tumors occurring in the other head and neck region reach up to almost 40% of long-term survivors. Recommended and preferred retreatment option in operable patients is salvage surgical resection, reporting a 5-year overall survival of up to 40%. However, because of tumor location, extent, and underlying comorbidities, salvage surgery is often limited and compromised by incomplete resection. Reirradiation with or without combined chemotherapy is an appropriate option for unresectable recurrence. Reirradiation is carefully considered with a case-by-case basis. Reirradiation protocol enrollment is highly encouraged prior to committing patient to an aggressive therapy. Radiation doses greater than 60 Gy are usually recommended for successful salvage. Despite recent technical improvement in intensity-modulated radiotherapy (IMRT), the use of concurrent chemotherapy, and the emergence of molecularly targeted agents, careful patient selection remain as the most paramount factor in reirradiation. Tumors that recur or persist despite aggressive prior chemoradiation therapy imply the presence of chemoradio-resistant clonogens. Treatment protocols that combine novel targeted radiosensitizing agents with conformal high precision radiation are required to overcome the resistance while minimizing toxicity. Recent large number of data showed that IMRT may provide better locoregional control with acceptable acute or chronic morbidities. However, additional prospective studies are required before a definitive conclusion can be drawn on safety and effectiveness of IMRT.

나노의학: 나노물질을 이용한 약물전달시스템과 나노입자의 표적화 (Nanomedicine: Drug Delivery Systems and Nanoparticle Targeting)

  • Applications of nanotechnology in the medical field have provided the fundamentals of tremendous improvement in precise diagnosis and customized therapy. Recent advances in nanomedicine have led to establish a new concept of theragnosis, which utilizes nanomedicines as a therapeutic and diagnostic tool at the same time. The development of high affinity nanoparticles with large surface area and functional groups multiplies diagnostic and therapeutic capacities. Considering the specific conditions related to the disease of individual patient, customized therapy requires the identification of disease target at the cellular and molecular level for reducing side effects and enhancing therapeutic efficiency. Well-designed nanoparticles can minimize unnecessary exposure of cytotoxic drugs and maximize targeted localization of administrated drugs. This review will focus on major pharmaceutical nanomaterials and nanoparticles as key components of designing and surface engineering for targeted theragnostic drug development.
