  • In this article, the aim is at evaluating the knowledge level of the foodbome disease by developing a reasonable survey questionnaire model. Each questions of the questionnaire is made up to check the knowledge covering the several fields of materials related to the foodbome disease. The pilot survey is implemented to evaluate the validity of questionnaire. Each question in questionnaire is scored to get the quantitative measure of the foodbome disease knowledge by converting the total score into 100 points.

  • The main purpose of this paper is to develop a questionnaire creation system by expert co-operation on the web. The most difficult problem in a survey is to make questionnaires. To solve this problem, we suggest the questionnaire creation system which is to create questionnaire for survey by expert collaboration. We can create the best questionnaire because it can gather and exchange to questionnaire information from experts.

  • The internet survey is more time and cost efficient than the off-line survey. We design and implement a web-based questionnaire generating system for the internet survey. The system guides users to make up a questionnaire step by step. It also generates data code for the statistical packages such as SPSS and SAS when the internet survey has been done.

The Reliability of Satisfaction Questionnaire According to Item Arrangement (환자만족도 조사설문지의 설문문항 배열에 따른 신뢰도 분석)

  • Objectives : The purpose of this study was to evaluate the reliability of questionnaire according to item arrangement on patient satisfaction questionnaire. Methods : We developed the two types of questionnaire with different item arrangement. In the first type (A), questions were arranged according to medical service dimensions. Questions in the second type (B) were arranged according to medical process. Both questionnaires were composed of six dimensions: physical environments, process, competence, courtesy, information giving, understanding patients. Measurements were performed on a 5-score Likert scale. In an outpatients and inpatient survey, total 777 patients answered the type A (outpatients: 257, inpatients: 128) or Type B (outpatients: 257, inpatients: 135). In order to compare the internal consistency of two types. Cronbach's ${\alpha}$ were calculated. Multiple regression analysis was also performed to know which type of questionnaire explain more of the overall satisfaction. Results : In outpatient survey, type A questionnaire showed higher internal consistency than B except physical environments dimension. Also in inpatient survey, type A had higer internal consistency than B in four dimensions(process, competence, courtesy, understanding patients). In the results of multiple regression analysis, type A questionnaire ($R^2$=0.53) explained more of the variation in overall satisfaction then B questionnaire ($R^2$=0.43) in outpatient survey. In inpatient survey, type B questionnaire ($R^2$=0.40) explained, more of the variation in overall satisfaction than type A questionnaire ($R^2$=0.33).But the difference of R was not significant in inpatient survey. Conclusion : The results of this study support that type A questionnaire has higer reliability in assessment of consumer satisfaction than type B.

  • Today's database system needs to collect huge amount of questionnaire that results from development of the information technology by the internet, so it has to be administrable. However, there are many difficulties concerned with finding analytic data or useful information in the high capacity-database. Data mining can solve these problems and utilize the database. Questionnaire analysis that uses data mining has drawn relevant patterns that did not look or was tended to overlook before. These patterns can be applied by a new business rule. The purpose of this research is to analyze the questionnaire results and to present the result that can help to make decision easily with data mining. Recognition and analysis about these techniques of data mining show suitable type of questionnaire survey. This research focus on the form of present composition and the model of suitable questionnaire to analyze the type of it. Also, the comparison between the actual questionnaire result and the conventional statistical analysis is examined.

Development of a Questionnaire for Dietary Habit Survey of Korean Adults (델파이 기법에 의한 한국 성인의 식습관 조사용 설문지 개발)

  • Objectives: The purposes of the study were to develop a questionnaire for dietary habit survey for Korean adults, and unify the terms related to dietary habits. Methods: The Delphi method by 43 professionals on dietary habit research was applied to unify the terms. Results: With regard to results on terms related to dietary habit, the respondents recorded the highest percentage (90.7%) of selecting the term "dietary habits" and also the highest percentage (76.7%) of choosing "dietary habits" for English. The biggest percentage of the respondents chose "individual dietary behaviors repeatedly formed and habitualized under the social, cultural, and psychological influence in the group" as the concept of dietary habits. The Delphi survey for the development of a questionnaire resulted in the first questionnaire of 31 items, the second one of 27 items, and the third one of 25 items. The validity of questionnaire items was tested with content validity ratios (CVR). The items whose CVR value was 0.29 or lower were eliminated or revised, because the minimum CVR value needed to test validity was 0.29. To test the reliability of questionnaire items, test-retest method was performed in 163 adults. According to the Kappa coefficient in the range of 0.314-0.716, all of the 25 items were in the reliability scope. A survey was taken with 702 adults to finally revise and supplement the third questionnaire whose validity and reliability were tested. Conclusions: Through those processes, a questionnaire for adults' dietary habit survey was finally completed. The significance of the study lies in the development of the first questionnaire on dietary habits equipped with both validity and reliability in South Korea.

  • This study aims to improve the Korean Farmers' Occupational Disease and Injury Survey (KFODIS), using the questionnaire improvement model, and systematic appraisal tools. The questionnaire improvement model comprises three stages: pretesting, redesign, and field-testing. The survey was evaluated by 13 expert reviewers, using a modified version of the Questionnaire Appraisal System (QAS-99). Based on the results of pretesting, survey questions were modified at a group meeting of experts. To identify potential issues with the improved survey questions, data were collected from 20 interviews and 31 respondents before behavior coding analysis was conducted. The questionnaire improvement model and systematic appraisal tools were useful in identifying, evaluating, and correcting miscommunication and other similar issues in the KFODIS.

A Comparative Study of Two Survey Methods for Health Services Research Modified Self-administered Questionnaire vs. Interview Survey (조사방법에 따른 의료이용 비교 -보완된 자기기입조사와 면접조사 방법의 차이-)

  • The purpose of this study was to compare the difference of two survey methods for health services research. Data were collected by means of two types of household survey conducted from March 11 to September 19, 1985. A probability sample of 30,613 persons was taken from 180 Enumerated Districts designated by the Economic Planning Board. The sample was divided into two groups systematically. One group was surveyed by the self-administered questionnaire and the other group was interviewed. Response rates were 81.4% and 90.6% respectively. The data were analysed by the ratio of the results of the self-administered survey to those of the interview survey. No difference was observed in sex, age, residence, or occupation between the two groups. However the respondents' characteristics were statistically different between the two groups. The major findings of this study are as follows : 1. The morbidity rate was 142.5 per 1,000 persons during the two week period by the self-administered questionnaire survey and 74.3 per 1,000 persons with the interview survey method. The ratio of the morbidity rate by the self-administered questionnaire to that by the interview was 1.92, and the difference between the two rates were due to the personal characteristics. 2. The out-patient utilization rate was 10.2 visits per person per year by the self-administered questionnaire survey and 5.4 by the interview survey, and the ratio was 1.89; the admission rate was 3.2 times per 100 persons per year by the self-administered questionnaire survey and 1.9 times by the interview survey, and the ratio was 1.68. Differences due to the sociodemographic characteristics were greater in the out-patient utilization rates than in the adimssion rates. 3. Percentages of effective medical care demand were 90.2% in the self-administered survey and 92.3% in the interview survey; the ratio was 0.98 which was less than that of the morbidity rate and medical care utilization. But, differences of effective medical care demand occurred in persons with no occupation, and aged or low educated respondents. 4. Respiratory illness had the highest frequency in the two survey methods. But there was a slight difference between the two survey methods in morbidity composition. 5. It was concluded that data collected by the interview survey were inclined to be underestimated and this problem can be corrected by a modified self-administered survey.

  • This study aims to examine the perceptual features of street buildings, which affect the urban scape, and establish design guidelines by types of buildings. According to this purpose, this study conducted document research, field survey through site visits and questionnaire survey for each subject. The field survey was carried out to study the exterior form characteristics of street buildings, and images of the streetscape in Gyeongju. For the questionnaire survey, the preference of 33 architecture-related people on the formal constituents of street buildings was investigated. The results of the document research, field survey and questionnaire survey were put together to elaborate the design guideline for urban street facade in historical city.
