Robot-Assisted Transoral Odontoidectomy : Experiment in New Minimally Invasive Technology, a Cadaveric Study

  • Objective : In the field of spinal surgery, a few laboratory results or clinical cases about robotic spinal surgery have been reported. In vivo trials and development of related surgical instruments for spinal surgery are required before its clinical application. We investigated the use of the da $Vinci^{(R)}$ Surgical System in spinal surgery at the craniovertebral junction in a human cadaver to demonstrate the efficacy and pitfalls of robotic surgery. Methods : Dissection of pharyngeal wall to the exposure of C1 and odontoid process was performed with full robotic procedure. Although assistance of another surgeon was necessary for drilling and removal of odontoid process due to the lack of appropriate end-effectors, successful robotic procedures for dural sutures and exposing spinal cord proved its safety and dexterity. Results : Robot-assisted odontoidectomy was successfully performed in a human cadaver using the da $Vinci^{(R)}$ Surgical System with few robotic arm collisions and minimal soft tissue damages. Da $Vinci^{(R)}$ Surgical System manifested more dexterous movement than human hands in the deep and narrow oral cavity. Furthermore, sutures with robotic procedure in the oral cavity demonstrated the advantage over conventional procedure. Conclusion : Presenting cadaveric study proved the probability of robot-assisted transoral approach. However, the development of robotic instruments specific to spinal surgery must first precede its clinical application.

Long-Term Oncological Outcomes of Reduced Three-Port Laparoscopic Gastrectomy for Early-Stage Gastric Carcinoma: a Retrospective Large-Scale Multi-Institutional Study

  • Purpose: With advances in surgical techniques, reduced-port laparoscopic surgery is increasingly being performed for the treatment of gastric carcinoma. Many studies have reported satisfactory short-term outcomes after reduced 3-port laparoscopic gastrectomy (LG). The aim of this study was to investigate the long-term oncological outcomes of 3-port LG in patients with gastric carcinoma. Materials and Methods: We reviewed the medical records of 1,117 patients who underwent LG for gastric carcinoma in three major institutions between 2012 and 2015. The data showed that 460 patients underwent 3-port LG without assistance, and 657 underwent conventional 5-port LG. We compared the overall and disease-free survival rates between the 2 groups. Results: There were 642 male and 475 female patients with a mean age of 56.1 years. Among them, 1,028 (92.0%) underwent distal gastrectomy and 89 (8.0%) underwent total gastrectomy. In the final pathologic examination, 1,027 patients (91.9%) were stage I, 73 (6.5%) were stage II, and 17 (1.5%) were stage III, and there were no significant difference in the pathologic stage between groups. The 3- and 5-port LG groups showed no significant differences in the 5-year overall survival (94.3% vs. 96.7%, P=0.138) or disease-free survival (94.3% vs. 95.9%, P=0.231). Stratified analyses according to pT and pN stages also showed no significant differences in overall or disease-free survival between the two groups. Conclusions: Long-term survival after 3- and 5-port LG was comparable in patients with early-stage gastric carcinoma. The 3-port technique requiring limited surgical assistance may be an appropriate surgical option for this patient population.

Intracranial Pial Arteriovenous Fistulas

  • Intracranial pial arteriovenous fistula (AVF) is a rare cerebrovascular lesion that has only recently been recognized as a distinct pathological entity. A 41-year-old woman (Patient 1) presented with the sudden development of an altered mental state. Brain CT showed an acute subdural hematoma. A red sylvian vein was found intraoperatively. A pial AVF was revealed on postoperative angiography, and surgical disconnection of the AVF was performed. A 10-year-old boy (Patient 2) presented with a 10-day history of paraparesis and urinary incontinence. Brain, spinal MRI and angiography revealed an intracranial pial AVF and a spinal perimedullary AVF. Endovascular embolization was performed for both lesions. The AVFs were completely obliterated in both patients. On follow-up, patient 1 reported having no difficulty in performing activities of daily living. Patient 2 is currently able to walk without assistance and voids into a diaper. Intracranial pial AVF is a rare disease entity that can be treated with surgical disconnection or endovascular embolization. It is important for the appropriate treatment strategy to be selected on the basis of patient-specific and lesion-specific factors in order to achieve good outcomes.

치과 수술용 LED 광원모듈의 최적화 (The Optimization of an Operating Dental LED Light Source Module)

  • The internal temperature of an operating room had to keep within $20^{\circ}C$. However, the doctor who is wearing operating gown and mask caused to rise temperature because of the thermal occurrence of dental LED light source. At first, the surgery environment is getting worse. And then last, it would increase bleeding rate by the expansion of patient's exposured blood vessel. A surgical operator can distribute the patient's tissue through such surgery environment, exactly. It can do accurate surgery. So, it gave to effect that surgical operator's eye condition is getting better and it could keep a mutual assistance system. For this research, we develop the LED dental light source module of high color rendition. It performed simulation for replacing established the method of Halogen lamp and Plazma lamp of light source. We analyzed intensity of illumination and the change of viability by changing the height of light source module.

기관삽관후 발생한 기관협착증의 외과적 치료 (Surgical Treatment of Postintubation Tracheal Stenosis)

  • A total of 55 patients underwent surgical managements for postintubation tracheal stenosis from July 1975 through March 1997. All but 8 had received ventilatory assistance. The patients had S cuff lesions, 17 stoma lesions, 7 at both levels, 5 at subglottic lesions. Thirty two patients underwent the sleeve tracheal resection and end-to-end anastomosis. Five patients performed a wedge resection and end-to-end anastomosis. Twenty two patients received the Montgomery T-tube for relief of airway obstruction. Simple excision of granulation tissue was done in 7 patients. Rethi procedures(anterior division of cricoid cartilage, partial wedge resection of lower thyroid cartilage and T-tube molding) were performed in 2 subglottic stenosis patients. And the other subglottic patient was received permanent tracheal fenestration at 1975. The tracheoesophageal fistula patient was done sleeve tracheal resection and end-to-end anastomosis with interrupted double layer closure of esophageal fistula site. Cervical approach was used in 49 cases, cervicomediastinal in 13 cases and median stemotomy In 6 cases. Techniques for obtaining tension-free anastomosis included a cervical neck flexion(15-30$^{\circ}$) in all sleeve resection patients and laryngeal release in one. The length of resection was 1.5 to 5.0 on A total of 41 patients(74.5%) had good(24 patients) or satisfactory(17 patients) results. But in ten cases, the restenosis of anastomosis site which is the most common complication was developed Two of them underwent a second reconstruction and 8 patients required T-tube insertion for airway maintenance. Three patients(5.4%) died. The causes of death were tracheo-innominate artery fistula(2) and sudden obstruction of airway(1).

Robotic harvest of a latissimus dorsi flap using a single-port surgical robotic system in breast reconstruction

  • Robot-assisted surgery is evolving to incorporate a higher number of minimally invasive techniques. There is a growing interest in robotic breast reconstruction that uses autologous tissue. Since a traditional latissimus dorsi (LD) flap leads to a long donor scar, which can be an unpleasant burden to patients, there have been many attempts to decrease the scar length using minimally invasive approaches. This study presents the case of a patient who underwent a robot-assisted nipple-sparing mastectomy followed by immediate breast reconstruction with an LD flap using a single-port robotic surgery system. With the assistance of a single-port robot, a simple docking process using a short and less visible incision is possible. Compared to multiport surgery systems, single-port robots can reduce the possibility of collision between robotic arms and provide a clear view of the medial border of the LD where the curvature of the back restricts the visual field. We recommend the use of single-port robots as a minimally invasive approach for harvesting LD flaps.

NIC간호중재분류체계를 이용한 수술실 핵심간호중재분석 (Analysis of Core Interventions of Operating Room using Nursing Intervention Classification)

  • Purpose : The purpose of study was to identify to analysis of core nursing interventions performed by Operating Room nurses. Method : The subjects of the study were arbitrarily selected nurses(n=104) working in Operating Room. The period for data collection was 15 days from July, 15, 2002 to July, 30. 2002. The instrument for study was 486 Nursing Interventions Classification developed by McClosky & Bulechek(2000) and was translated into Korean. In 486 nursing interventions, 57 nursing interventions were selected by more than half of 47 professional nurses group of Operating Room. 57 nursing interventions were used as a secondary questionnaire. In the secondary questionnaire, labels and definitions of all 57 interventions were listed. The collected data were self reported by Operating Room nurses. The data were analysed with SPSS program. Result : In 57 nursing interventions, the 'Behavior' domain was the most frequently used. Core interventions of Operating Room were performed several times a day by more than 50% of Operating Room nurses. Core interventions of Operating Room were 16 Core interventions, 7 classes, 5 domains. In the core interventions, the 'Physiological:Complex' domain was the most frequently used. Core interventions of Operating Room were Surgical Preperation, Infection Control:Intraoperative, Surgical Precautions, Fall Prevention, Documentation, Surgical Assistance, Environmental Management:Safety, Skin Surveillance, Physical Restraint, Pressure Ulcer Prevention, Environmental Management:Comfort, Infection Protection, Presence, Emotional Support, Specimen Management, Shift Report. Conclusion : Core interventions of Operating Room have implications for nursing care practice, nursing education, nursing research, and nursing information system in Operating Room.

치과의사 및 치과위생사의 치과팀 구성 및 업무 분장에 대한 견해 (A Survey of Dentists and Dental Hygienists on Dental Team Composition and Work Assignments)

  • The purpose of this study was to investigate the status and views on the dental team composition and works, the experience of a dental manpower shortage and opinions on the solution. The subjects were 313 dentists and dental hygienists. The mobile questionnaires were conducted on the status of dental team composition and works, the experience of a dental manpower shortage and opinions on the solution. There was no big difference between the current dental team composition and ideal composition ratio, and dentists answered that it would be more ideal to reduce dental hygiene and increase nurse assistant when the working dentist was 3~5. 85.6% of the dentists and 56.4% of the dental hygienists had difficulty in employing the dental auxiliary personnel. The difficulty level was 8 out of 10 points among dentists and 5.7 points among dental hygienists. 91.1% of the dentists and 95.5% of the dental hygienists agreed on the necessity of expanding the dental hygienist's work. The dental works were temporary crown making and setting, surgical operation assistance, and overall dental assistance. Based on the results of this study, it is necessary to continuously make efforts to communicate with the dental personnel in Korea to solve a lot of problems such as the job assignment, the dental personnel policy and system, and the work environment.

Posterior Microscopic Lesionectomy for Lumbar Disc Herniation with Tubular Retraction Using $METRx^{TM}$ System

  • Objective : The authors have developed a procedure, termed posterior microscopic lesionectomy, that creates a minimal laminotomy site according to the location of the shifted disc using the $METRx^{TM}$ system in the lumbar spine. This study compared the usefulness and surgical outcomes of this procedure with those of traditional standard lumbar discectomy. Methods : From June 2003 to June 2004, Twenty-two patients with one-level radiculopathy due to lumbar disc herniation underwent posterior microscopic lesionectomy with the assistance of an operating microscope and the $METRx^{TM}$ tubular retractor. Surgical results of the new procedure were compared to those of 39 patients who underwent traditional lumbar discectomy from April 2003 to September 2004. All patients were evaluated for pain score, clinical assessment according to the VAS, and Roland-Morris scores pre-operatively and at 1, 3, 6, and 12 months post-operatively. Results : Mean blood loss, operation time, and admission date showed significant improvements for microscopic lesionectomy compared to traditional lumbar discectomy [P < 0.001]. Also, both measures of short-term functional improvement, the Visual Analogue Scale[VAS] and Roland-Morris[RM] scores, were statistically better for microscopic lesionectomy than for traditional discectomy [P < 0.001]. Conclusion : Posterior microscopic lesionectomy can be performed more safely and provide greater benefit than traditional discectomy. The procedure is associated with less post-operative pain, shorter hospital stays, and quicker rehabilitation.

Retromandibular reduction of medially dislocated condylar process fractures

  • Background Condylar process fractures account for one-third of all mandibular fractures, and the distal fragment is prone to dislocate to the medial side due to the pulling of the lateral pterygoid muscle. Retromandibular approaches are commonly used, but the intraoperative view becomes limited in medially dislocated fractures. This study summarized a series of cases of retromandibular reduction for medially dislocated condylar process fractures and described our supplementary procedure to realign the dislocated condylar process. Methods Nine patients with medially dislocated condylar process fractures underwent surgical correction from January 2012 to December 2016. In 6 of them, it was possible to realign the fractures with a conventional retromandibular approach, but for 3 cases of severe dislocation to the middle cranial fossa, a supplementary transoral procedure was carried out. The angle difference between the ramus and condyle, ramus height, and maximal mouth opening (MMO) were evaluated. Results All 9 cases were restored to the proper anatomical alignment without any major complications, and postoperative images revealed successful union. The angle difference was $8.94^{\circ}{\pm}4.11^{\circ}$ preoperatively, and $0.99{\pm}0.49^{\circ}$ at the 6-month follow-up. The pretreatment ramus height difference was $6.12{\pm}6.09mm$, and the postoperative difference was $0.18{\pm}0.10mm$. These changes after surgery were statistically significant. The MMO before surgery was $11.44{\pm}3.0mm$, and the postoperative MMO was $37.2{\pm}2.9mm$, reflecting a significant increase after reduction. Conclusions Retromandibular reduction is a useful method in medially dislocated condylar process fractures, and additional transoral assistance should be considered to realign condylar processes that severely dislocate to the middle cranial fossa.
