TRAIL Suppresses Human Breast Cancer Cell Migration via MADD/CXCR7

  • Background: Tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) can specifically induce apoptosis limited to various cancer cells, so this reagent is considered a promising medicine for cancer therapy. TRAIL also exerts effects on non-apoptotic signals, relevant to processes such as metastasis, autophagy and proliferation in cancer cells. However, the mechanisms of TRAIL-regulated non-apoptotic signals are unclear. The purpose of this study was to investigate MADD/CXCR7 effects in TRAIL-mediated breast cancer cell migration. Materials and Methods: The ability of MADD/CXCR7 to regulate MVP signaling in TRAIL-mediated breast cancer cells migration was evaluated by transwell migration assay, quantitative RT-PCR, Western blotting and knock down experiments. Results: In this study, we found that treatment with TRAIL resulted in induced expression levels of MADD and CXCR7 in breast cancer cells. Knock down of MADD followed by treatment with TRAIL resulted in increased cell migration compared to either treatment alone. Similarly, through overexpression and knockdown experiments, we demonstrated that CXCR7 also positively regulated TRAIL-inhibited migration. Surprisingly, knock down of MADD lead to inhibition of TRAIL-induced CXCR7 mRNA and protein expression and overexpression of CXCR7 lead to the reduction of MADD expression, indicating that MADD is an upstream regulatory factor of TRAIL-triggered CXCR7 production and a negative feedback mechanism between MADD and CXCR7. Furthermore, we showed that CXCR7 is involved in MADD-inhibited migration in breast cancer cells. Conclusions: Our work defined a novel signaling pathway implicated in the control of breast cancer migration.

Thoracoscopic Sympathectomy for Essential Hyperhidrosis (흉강내시경을 이용한 교감신경절 절제수술)

  • Essential hyperhidrosis is a condition with excessive sweating, which may be localized in any part of the body Excessive sweating has a strong negative impact on the qual ty of life for many persons. From June 1992 to May 1996, 211 cases of thoracoscopic thoracic sympathectomy were performed in the Department of Thoracic Surgery, Yongdong Severance Hospital, Seoul, Korea. Among the 211 cases, 192 patients had palmar hyperhidrosis, and 19 cases had facial hyperhidrosis. There were 121 males and 90 (tamales, and the ages ranged from 10 to 67 years(average: 24.82 years old). The average operation time and the average postoperative hospital stay were 91.94 minutes and 4.31 days, respectively. Perioperative courses were uneventful, and all the patients had immediate and complete relief of symptoms with mild compensatory sweating on the chest wall and the back. Even though a thoracoscopy has the possibility of emergency conversion to a thoracotomy and technical difficulties still exist, especially in patients with facial hyperhidrosis, our experience indicates that video-assisted thoracoscopic thoracic sympathectomy is a very safe and useful procedure for h perhidrosis.

  • Heating of injured tissue has been used for centuries for pain relief and reduction of muscle spasm. In physical therapy locally applied heating gents are used not only to promote relaxation and provide pain relief, but they are also used to increase blood flow, to facilitate tissue healing, and to prepare stiff joints and tight muscles for exercise. Superficial heating agents primarily cause in increases in skin and superficial cutaneous tissue temperature. Superficial heating agents such as hot packs, paraffin wax, Deep heating agents, including shortwave diathermy and continuous-wave ultrasound, can increase tissue temperature at depths ranging from 3to 5cm without overheating the skim and subcutaneous tissue.

Analgesia Effect of Intraarticular Morphine or Ketorolac after Arthroscopic Knee Surgery (관절경을 이용한 슬관절 수술후 관절강내로 투여한 Morphine과 Ketorolac의 진통효과)

  • Background : Analgesic effect of intra-articular morphine or ketoronac treatment alone, or a combination of both drugs, on postoperative pain were evaluated in 40 healthy male patients undergoing arthroscopic knee surgery. Method : Upon completion of surgery under spinal anesthesia, each patients knee joint was injected with 30 ml of 0.25% bupivacaine. Then, via parenteral or intra-articular route, one study group received morphine and other group received ketorolac. Results : Groups who received either intra-articular ketorolac, or morphine, experienced decreased postoperative pain reducing need for additional analgesics. The combination treatment of intra-articular morphine and ketorolac did not improved results. Conclusions : Singular use of either intra-articular morphine, or ketorolac, improves postoperative analgesia in patients undergoing arthroscopic sugery: Combination of these drugs offered no further advantage over its single prescription.

  • Bisphosphonate inhibits the function of osteoclast, so they are widely used for multiple myeloma, Paget's disease, metastatic malignant bone disease, and severe osteoporosis. This drug is very effective for preventing severe complication of osteoporosis, some unpredictable complication occurred such as esophageal malignancy, atypical fracture of femur, and osteonecrosis of the jaw. Bisphosphonate related osteonecrosis of the jaw (BRONJ) is closely related with invasive, open bone surgery like tooth extraction. BRONJ associated with dental implant is rare, however, as the use of bisphosphonate increase, BRONJ cases with dental implant are increasing. In this article, we will describe the considerations during dental implant treatment for patient under bisphosphonate therapy.

Imprint Cytology of a Desmoplastic Small Round Cell Tumor -A Case Report- (결합조직형성소원형세포종양의 압착도말 세포학적 소견 -1예 보고-)

  • Desmoplastic small round cell tumor (DSRCT) is a rare malignant mesenchymal neoplasm. It mainly involves the abdominal or pelvic peritoneum of male adolescents. We report here the imprint cytologic features of a case of DSRCT occurring in the intraabdominal cavity of a 21-year-old man. A microscopic examination showed moderate cellularity. The tumor cells were singly arranged and arranged in clusters. The cells had round to oval nuclei with finely granular chromatin, inconspicuous nucleoli and scanty cytoplasm. Some tumor cells showed nuclear molding, and some cells had an epitheloid appearance with a large amount of lightly eosinophilic cytoplasm. A rosette-like pattern was present. Spindle-shaped, fibroblastic stromal cells were occasionally found. The tumor cells were immunoreactive for the markers cytokeratin (AE1/AE3), epithelial membrane antigen (EMA), desmin, vimentin and neuron specific enolase (NSE).

  • To analyze the outpatient of the Dental Infirmary Yonsei Medical Center, anthor ruled out the charts of 1,622 patients which were sampled from 18.921 patients among the selected years 1973.1977 and 1981. The results are as follows : 1. By the age-group the twenties and thirties were about 50% of the total patients. 2. According to the analysis by the district, the distance from residence was most influencial to select the hospital. 3. The frequency of the patient by the service departments, many patients wanted oral sugery tretment, and operative & periodontic tretments followed, And there were some necessities of dental health education for the early care and the protective tretment. 4. Of the season, there were more patients in Summer and Winter, especially in July and August than Spring and Autumn. It might be related with the vacations. 5. The ratio of professor's and general patients, were decreasing compared with the data of three selected years, and the fact might be studied to improve for the better quality of service, even the application of the medical and dental insurance is getting wider.

Ceruminous Gland Tumor in a Dog (귀지선 종양 증례 1)

  • A 9-year-old male Yorkshire terrior with recurrent chronic otitis externa and otitis interna was referred to the Veterinary Medical Teaching Hospital of Seoul National University. Radiographic findings indicated mineralization around the right ear canal. A tan nodular mats, 1 cm in diameter. was noted near the tympanum after total ear canal ablation. Cytologically. the imprint smear of the cut surface of the mass produced clusters of cells with abundant and weakly basophilic cytoplasm, one small round nucleus and one or two nucleoli, mixed with both degenerate and non degenerate neutrophils. The location of the nuclei was eccentric, and some cells contained somewhat coarse black granules in their cytoplasms, on which the mass was diagnoted as ceruminous gland adenoma or hyperplasia associated with inflammation. Histopathologic examination of the tumor was compatible with a diagnosit of ceruminous gland adenoma. The patient is responding well to sugery and antimicrobial treatment.

The Free Jejunal Autograft for the Hypopharynx and Cervical Esophagus Reconstruction (유리공장을 이용한 인두 및 경부식도 재건술)

  • Reconstruction of the pharynx and cervical esophagus presents a tremendous surgical challenge to the Head and Neck Surgeon. Because life expectancy of patients with advanced carcinoma of the hypopharynx, and cervical esophagus is limited, treatment must be aimed at palliation. A variety of techiques have been proposed over the years with none proving entirely satisfactory. These techiques include prosthesis; skin graft; cervical flaps; tubed cutaneous and myocutaneous chest flaps; visceral reconstruction with stomach, colon. and jejunum; and jejunal free autografts. Many factors dictate the best method of reconstruction in any given clinical situation. The goal of the surgery is a one-stage reconstruction of swallowing function with minimal morbidity to allow as short a hospital stay as posible. Nine patients underwent the free jejunal autograft reconstruction of the pharyngoesophagus after the ablative surgery for the advanced hypopharyngeal cancer. Postoperative complications included one perioperative death, two abdominal wound dehiscences, two neck hematomas, one carotid rupture, one funtional dysphagia, one late strictures. There were no graft failure, no immediate stenosis and no fistula. An oral diet was started between days 8 and 16, with an average of 9 days and median of 8 days. Patients left the hospital between days 9 and days 38, with an average of 23.4 days and median of 23 days. This method of reconstruction is advocated as reliable palliative procedure with short-term follow-up. In conclusion, we at Korea Cancer Center Hospital are of the opinion that the free jejunal autograft offers an excellent, safe and relative easy method of the pharyngeal and cervical esophageal reconstruction with significant advantages over other techiques.
