Effects of Thermotherapy Combined with Aromatherapy on Pain, Flexibility, Sleep, and Depression in Elderly Women with Osteoarthritis (라벤더향유를 이용한 온습포법이 무릎관절염 여성노인의 통증, 무릎유연성, 수면 및 우울에 미치는 효과)

  • Purpose: This study examined the effects of heat therapy combined with aromatherapy on pain, flexibility, sleep, and depression in elderly women with osteoarthritis. Methods: This study was designed as a nonequivalent control group quasi-experimental study based upon data acquired through a pre-post test. The experimental group (n=21) was treated using an aroma-permeated hot compress applied to the knee. The cotton towel was heated in a microwave to $45^{\circ}C$, and 2~3 drops of lavender were applied on it. The towel was wrapped with plastic bag to prevent aroma leakage and retain heat. The treatment was applied the towel for 20 minutes, three times a day for 3 weeks. The control group (n=24) was not given any treatment. Data were collected using a VAS(100mm) for pain, questionnaire for sleep and depression, and a goniometer for knee flexibility. The data were analyzed with ${\chi}^2$-test and t-test using SPSS/Win18.0. Results: There were significant differences in knee pain (p=.001) and flexibility (p<.001), subjective quality of sleep (p=.020) and depression (p=.003) between the groups. Conclusion: Heat therapy combined with aromatherapy may be an effective nursing intervention and self-care technique to ameliorate pain and depression and to improve flexibility and sleep in elderly suffering from osteoarthritis.

The Effectiveness of Self-exercise Program for Low Back Pain, Abdominal-back Muscle Strength, and Waist Flexibility in Hospital Nurses (요통자가운동프로그램이 간호사의 요통, 복.배근력, 허리유연성에 미치는 효과)

  • Purpose: The purpose of the study was to identify the effect of a self-exercise program on low back pain, abdominal-back muscle strength and waist flexibility in hospital nurses. Method: An experimental research design was employed. 43 subjects (20 in the experimental group and 23 in the control group) who worked in a general hospital in W city agreed to participate in this study. The intervention had 6 sessions with self-exercise program including flexion and extension demonstrated by a trained research assistant. A Visual Analog Scale was used to measure the subjective pain severity. Abdominal-back muscle strength was measured with a back strength dynamometer, while waist flexibility was gauged by the standing of the subject's upper body. Analysis included descriptive statistics, $x^2$ test, t-test and ANOVA. Result: The experimental group exhibited significantly decreased low back pain, an increase in abdominal-back muscle strength, and increased waist flexibility after 6 sessions. The experimental group were not significantly increased in abdominal-back muscle strength than the control group. Conclusion: These results suggest that further research is needed to replicate for verification of clinical validity with the methodological rigors, and to utilize the self-exercise program for care and prevention of low back pain.

Convergence Study on Effects of Passive Stretching on Blood Fatigue and VAS after Delayed Onset Muscle Soreness (지연성근육통 유발 후 수동적 스트레칭이 혈중피로물질 및 VAS에 미치는 융합연구)

  • The purpose of this study was to prove the effect of passive stretching after delayed muscular pain induction in university students and to propose a rehabilitation exercise program for effective pain relief and prevention in case of injury. Subjects were divided into passive stretching group and control group. Passive stretching group was performed passive stretching after delayed muscular pain induction and control group did not perform any treatment after delayed muscular pain induction. The delayed muscular pain induction method was induced by bench step motion. The height of the step box was 50cm. The data were analyzed by two-way RGRM ANOVA for comparison of passive stretching group and control group. In conclusion, passive stretching after delayed myalgia has a positive effect on blood fatigue (CRP, LDH) and subjective pain scale(VAS).

The Relationship among the Number of Teeth, Salivary Flow Rate, Oral Health-Related Quality of Life, and Depression in the Elderly Population in Korea

  • Purpose: Depression is a condition that weakens psychosocial functioning and reduces quality of life. In Korea, the prevalence of depression among the elderly is 29.2% and depression is a considerable public health concern for the elderly. Depression has a statistically significant relationship with oral conditions such as number of teeth and salivary flow rate. Oral health-related quality of life (OHRQoL) is the concept including subjective evaluations of psychological, physical and social aspects of oral health. Tooth loss and hyposalivation can affect mental health and quality of life. Our study was designed to identify the relationship among the number of teeth, salivation, OHRQoL, and depressive symptoms in the elderly in Korea. Methods: We recruited 100 participants, aged over 65 years, and assessed their number of teeth, unstimulated salivary flow rate. All participants filled out oral health impact profile 14 (OHIP-14) and Zung self-rating depression score (SDS) for checking OHRQoL and depressive symptoms. Statistical analysis was done by R program. Results: We found that the positive relationship between OHIP-14 and SDS after Mann-Whitney test (p=0.03). The lower SDS group had an OHIP-14 median score of 4. On the other hand, the higher SDS group had a median value of 7.5. The other factors (number of teeth and salivary flow rate) did not show correlations with OHIP-14 or SDS. Conclusions: OHRQoL and depressive symptoms have significant correlation.

Clinical Evaluation of Selected Naturopathic Therapeutic Procedures in Individuals with Low Back-pain

  • Background: The understanding of back-pain as one of the commonest clinical presentations, made the path to the present study. More than three-quarters of the world's population experience back-pain at some time in their lives Purpose: The present study has evaluated the selected Naturopathic therapeutic procedures on individuals with low back-pain. Methods: Thirteen patients were selected for the study and randomly divided into two groups, viz., Group A, Naturopathic group and Group B, Control group. One patient discontinued. Naturopathic group was treated with mud pack, aromatherapy massage with Eucalyptus oil, acupressure, steam, wet trunk pack, exercises, relaxation and breathing exercises. Advice for life style and diet was given for patients of both the groups. The effect of the therapy was assessed subjectively and objectively. Particular scores drawn for Naturopathic group and control group were individually analyzed before and after treatment and the values were compared using standard statistical protocols. Results: Naturopathic intervention revealed 100% relief in both subjective and objective parameters (i.e., 6 out of 14 parameters showed statistically highly significant P < 0.01 results, while 8 showed significant results P < 0.05). Conclusion: Statistically highly significant results of this study point out the successful management of the multi factorial low back-pain with a multi focused Naturopathic treatment approach. Such encouraging results may pave the way for a future study on a large sample in a longer duration incorporating clinical investigations.

A Systematic Review of Acupuncture Treatment Effect for Patellofemoral Pain Syndrome (슬개대퇴통증증후군의 침 치료 효과에 대한 체계적 문헌고찰)

  • Objectives The purpose of this study is to systematically review the degree of pain and function improvement in acupuncture treatment for patellofemoral pain syndrome (PFPS). Methods Randomized controlled trials (RCTs) were searched from Medline/PubMed, Cochrane Library, Embase, WanFang Data, China National Knowledge Infrastructure (CNKI), Citation Information by NII (CiNII), Koreanstudies Information Service System (KISS), ScienceON, KoreaMed, and Korean Medical Database (KMbase). The methodological quality of selected RCTs' were evaluated according to Cochrane RoB2 tool. Results Total of 158 studies were identified and 7 RCTs were selected for systematic review. Frequently used evaluation method were visual analogue scale and Kujala scores. All studies showed significant effect on pain reduction and knee function between before and after treatment, but showed partial significance between the intervention and control group. Conclusions Research showed that acupuncture treatment is an effective treatment for improving the pain and function of PFPS, but it is difficult to conclude that it has a significant effect compared to other treatments. Selected studies included subjective evaluation indicators and the total of selected studies were small, so the interpretation of the results is limited. Also most of the included studies are evaluated to have some concerns of bias. Therefore, subsequent research should continously be done in more objective and systematic method.

The Influence of Botulinum Toxin Type A Masticatory Efficiency (보툴리눔 A형 독소가 저작효율에 미치는 영향)

  • This study was aimed to evaluate the masticatory efficiency after botulinum toxin type A (BTX-A) injection during 12 weeks using objective and subjective test. Also, we compared the difference of masticatory efficiency between group that injected into the masseter muscle only (M-group) and group that injected into the masseter and temporalis muscle (M-T group). The mixing ability index (MAI) was used as the objective indicator, and visual analogue scale (VAS) and food intake ability (FIA) index were used as the subjective indicators. It was concluded that masticatory efficiency was significantly lowered after a BTX-A injection into the masticatory muscle, but it gradually recovered in a predictable pattern by the 12 weeks. The disturbance of subjective masticatory efficiency was lasted longer than objective masticatory efficiency. The masticatory efficiency was lower in M-T group than M group. It was statistically significant in the VAS and FIA at 4 weeks, but the MAI showed no significancy. After 4weeks, there was rapid recovery of muscle function in M-T group, and the difference between two groups was not significant. It could be concluded that there will be no serious disturbance of mastication compared to injection is done only into the masseter muscle, even if injection is done into the masseter and temporalis muscle in dose of this study. According to the food properties, it was confirmed that people feel more discomfort on taking hard and tough foods after BTX-A injection and not only hard foods, but also intake of soft and runny foods were influenced by botulinum toxin injection.

De Quervain's Disease after Delivery Treated with SBV Herbal-acupuncture : Clinical Study on 3 Cases (Sweet Bee Venom 약침을 이용한 산후 De Quervain's disease 치험 3례)

  • Purpose: To investigate effect of Sweet Bee Venom on postpartum's De Quervain's Disease. Methods: We investigated 3 patients diagnosed as De Quervain's Disease. Sweet Bee Venom Herbal-acupuncture was used for treatment of De Quervain's Disease, once every two days, 5 times. Pain, satisfaction at treatment and function of wrist were measured with VNS, Baumgaertner's nine point scale and dynamometer. Results: Pain reduction, functional improvement, increased grip power were found out in cases treated with Sweet Bee Venom Herbal-acupuncture. In every cases, they represent high subjective satisfaction with treatment. Conclusion: Clinical results show that Sweet Bee Venom Herbal-acupuncture has a good analgesic effect in De Quervain's Disease.

  • High frequency system has been used for the purpose of skin care and obesity treatment, by high-frequency energy is applied to the human body generates deep heat. Conventional high frequency system could not selection control by depending on the body load fluctuations. Such as burns and side effects have been reported by system instability and then therapeutic effect is insufficient. During treatment, objective information about the status of the patient was no. Because of treatment methods are subjective, and so tailored treatments were impossible. In this paper, high frequency medical system with sinusoidal frequency characteristics without distortion of the Push pull switching scheme for pain relief therapy was designed. And control circuit that was designed by feedback using the output changes according to the body-load fluctuation. Last, power circuit for efficient control the heat generated from the hardware was proposed.

The Change of the Digital Skin Temperature in Dental Outpatients by Biotrainer (Biotrainer를 이용한 치과외래환자의 수지피부온도변화에 관한 연구)

  • In order to evaluate objectively the anxiety level in dental treatment, the author used Biotrainer(BF-120R), one of the skin temperature biofeedback apparatus, to examine 68 dental outpatients on their digital skin temperature change due to routine consecutive dental procedures(oral examination, anesthetic injection, cavity preparation, fissure sealing, polishing). The subjective anxiety level change was also evaluated by visual analog scale. The obtained results were as follows : 1. The skin temperature decreased through consecutive procedures and the temperature in each procedure decreased until 60sec, and then increased on 120sec. 2. The temperature changes in Preparation and Injection were greater than those in other procedures. 3. Generally, male exhibited more change of skin temperature in all procedures than female. 4. Type II, continuously decreasing after procedure, occupied the most in all patients and yhad the lowest beseline temperature. 5. The anxiety level of before-procedure was higher than that of after-procedure and the levels in Injection and Preparation were higher than in other procedures.
