A Study on the Effect of Authentic Leadership of Hospital Organization on Organizational Commitment and Organizational Citizenship Behavior: Focusing on Mediating Effect of LMX (병원 조직의 진성리더십 유형이 조직몰입과 조직시민행동에 미치는 영향에 관한 연구 : LMX의 매개효과를 중심으로)

  • Purpose - This study is to clarify the structural relationship between the direct supervisor and organizational commitment and organizational citizenship behavior through mediation of the Leader-member exchange (LMX) in the hospital organization, and to confirm the mediating effect of the LMX. Through this study, we will provide basic data on the aspects of HR to enhance organizational immersion and organizational civic behavior in hospital organizations, and present suggestions on academic contribution and effective organizational operation measures. Research design, data, and methodology - This study is to investigate the relationship between authentic leadership and OC and OCB of the organization member, which is proposed to secure more active and high competitiveness in the recent changing medical environment, with multi-regression and B&K(1986) method. For this purpose, 653 employees of senior hospitals in Seoul and Kyeong-In area were surveyed using structured questionnaires. Each questionnaire was composed of 7-point scale, and each hypothesis was testified by multiple leaner regression and by BK method for testifying mediator effect. Result - As a result of the analysis, it was found that the male and non-medical staff had higher recognition level than the medical staff in the LMX, organizational commitment and organizational citizenship behavior. In the senior management or more than 5 years' experience group, organizational commitment and organizational citizenship behavior were recognized higher than the group of subordinate managers. Second, authentic leadership has a positive influence on LMX, organizational commitment and organizational citizenship behavior, and LMX has a significant positive impact on organizational commitment and organizational citizenship behavior. In addition, as a result of analyzing LMX mediating effect in the relationship of authentic leadership and organizational effectiveness by BK method, LMX had partial mediating effects between authentic leadership and organizational citizenship behavior, and between authentic leadership and organizational commitment. Conclusion - Results of the study confirmed that all the members of the hospital organization that were lacking served as positive factors for the relationships of authentic leadership and LMX, and authentic leadership and organizational effectiveness. It is expected that the hospital organization will be able to suggest such as the organizational approach to expect high organizational effectiveness centering on authentic leadership ability as psychological capital.

Thematic Analysis of the Therapeutic Song Writing Experience of Music Therapy Interns: A Focus Group (음악치료 인턴들의 치료적 노래만들기 경험에 대한 주제분석: 포커스 그룹을 중심으로)

  • The internship is essential for the music therapy curriculum and affords interns the opportunity to apply their classroom-based knowledge and skills to real-world clinical settings. However, challenges associated with the internship can result in interns undergoing trial-and-error learning, interpersonal conflicts, and intrapersonal difficulties. An experiential music therapy group may be useful in helping interns process these incidents and develop their personal and professional skills. We explored the experiences of music therapy interns participating in therapeutic song writing. In this study, five music interns completed two 4-hour sessions of therapeutic song writing. Following the second session, a group interview was conducted with participants to gather data on their experiences. The interview was recorded, transcribed, and analyzed. Six themes and 18 sub-themes were derived from the data. The six themes were preconceptions of therapeutic song writing, meaningful lyric creation, challenges in song composition, structured experiences during song writing process, development of self-awareness through music, and relational experiences resulting from the group process. Participants were able to incorporate their individual internship experiences into a single song by communicating with group members during the step-by-step process. Participation in therapeutic song writing was found to help music therapy interns identify and process challenges encountered during their internship and further their personal and professional development.

Analysis of the Related Factors on Job Satisfaction in Nursing Officers Using the Structural Equation Model (구조방정식모형을 이용한 간호장교의 직무만족도 관련요인 분석)

  • This study was performed to determine the job satisfaction and its association with sociodemographic and health related characteristics, job-related factors, perception on socioeconomic status, professional job perception, job stress and social support among nursing officers. The survey was administered to 542 nursing officers working at military hospitals of the ROK Armed Forces, from Oct. 1 to Nov. 30. The structured self-administered questionnaires were delivered and they were collected without respondents' personal information. As a results, The job satisfaction of respondents turned out to be significantly higher in the following groups: an elder, married, subjective sleep quality is good, feels they are healthy, higher positional status, a longer job career, satisfied with their work, without consider quitting the job, their physical burden of work is adequate. The survey results showed that respondents' job satisfaction is positively correlated with the professional job perception and social support. while it is negatively correlated with job stress. When analyzed by Covariance Structure, it turned out that the professional job perception is more influential on job satisfaction than job stress and social support. The study found the professional job perception, high social support, and low job stress tends to improve job satisfaction. Meanwhile, the study also found that the professional job perception and high social support reduces job stress.

Relationship between Music Cognitive Skills and Academic Skills (음악의 인지기술과 학습 기술과의 관계)

  • Melody is defined as adding spatial dimension to the rhythm which is temporal concept. Being able to understand melodic pattern and to reproduce the pattern also requires cognitive skills. Since 1980, there has been much research on the relationship between academic skills and music cognitive skills, and how to transfer the skills learned in music work to the academic learning. The study purported to examine various research outcomes dealing with the correlational and causal relationships between musical and academic skills. The two dominating theories explaining the connection between two skills ares are "neural theory" and "near transfer theory." The theories focus mainly on the transference of spatial and temporal reasoning which are reinforced in the musical learning. The study reviewed the existing meta-analysis studies, which provided evidence for positive correlation between academic and musical skills, and significance of musical learning in academic skills. The study further examined specific skills area that musical learning is correlated, such as mathematics and reading. The research stated that among many mathematical concepts, proportional topics have the strongest correlation with musical skills. Also with reading, temporal processing also has strong relationship with auditory skills and motor skills, and further affect language and literacy ability. The study suggest that skills learned in the musical work can be transferred to other areas of learning and structured music activities may be every efficient for children for facilitating academic concepts.

Assessment of Respiratory Problems in Workers Associated with Intensive Poultry Facilities in Pakistan

  • Background: The poultry industry in Pakistan has flourished since the 1960s; however, there are scarce data regarding the impact of occupational exposure on the pulmonary health of farm workers in terms of years working in the industry. The objective of the present study was to assess the effect of poultry environment on the health of occupationally exposed poultry farmers in countries of warm climatic regions, such as Pakistan. This study will also show the effect of exposure to poultry facilities on the health of poultry farmers in the context of low-income countries with a relatively inadequate occupational exposure risk management. Materials and methods: The lung function capacity of 79 poultry workers was measured using a spirometer. Along with spirometry, a structured questionnaire was also administrated to obtain information about age, height, weight, smokers/nonsmokers, years of working experience, and pulmonary health of farm workers. The workers who were directly involved in the care and handling of birds in these intensive facilities were considered and divided into four groups based on their years of working experience: Group I (3-10 months), Group II (1-5 years), Group III (6-10 years), and Group IV (more than 11 years). The forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and the FEV1/FVC ratio were considered to identify lung function abnormalities. Statistical analysis was carried out using independent sample t test, Chi-square test, Pearson's correlation, and linear regression. Results: Based on the performed spirometry, 68 (86 %) of workers were found normal and healthy, whereas 11 (14 %) had a mild obstruction. Of the 11 workers with mild obstruction, the highest number with respect to the total was in Group IV (more than 11 years of working experience) followed by Group III and Group II. Most of the workers were found healthy, which seems to be because of the healthy survivor effect. For the independent sample t test, a significant difference was noticed between healthy and nonhealthy farmers, whereas Chi-square test showed a significant association with height, drugs, and working experience. Linear regression that was stratified by respiratory symptoms showed for workers with symptoms, regression models for all spirometric parameters (FVC, FEV1, and FEV1/FVC) have better predictive power or R square value than those of workers without symptoms. Conclusion: These findings suggest that lung function capacity was directly related to years of working experience. With increasing number of working years, symptoms of various respiratory problems enhanced in the poultry workers. It should be noted that most of the poultry workers were healthy and young, the rationale being that there is a high turnover rate in this profession. The mobility in this job and our finding of 86% of the healthy workers in the present study also proposed healthy worker survivor effect.

The Effect of Depression and Self-efficacy on the Quality of Life (요양병원 입원노인의 우울과 자아효능감이 삶의 질에 미치는 영향)

  • Objective : Purpose of this study was to investigate the effect of depression and self - efficacy on the quality of life of the elderly hospitalized in a nursing hospital and to suggest a new direction for improving the quality of life of the elderly. Methods : The target was 212 senior citizens who were hospitalized at a nursing hospital located in K city in North Gyeongsang Province. The questionnaire was structured by adding the characteristics of the subjects, the Korean Senior Depression Index (GDS-K), the Self-effective Index (WHQOL-BRFA), and the World Health Organization's Life-Enhanced scale. coded using SPSS ver.18, the depression, self-efficiency, and quality of life differences according to the general characteristics (t-test, ANOVA), and Self-efficacy, depression, and the correlation coefficient between quality of life and A multI-sentence analysis to see the impact of depression and self-efficacy on the quality of life. Results : Self - efficacy and depression according to subject characteristics were different according to length of stay, and quality of life was different in religion. The correlation between depression, self - efficacy, and quality of life of admitted elderly showed statistically significant negative correlation (p <.01) with depression in both the quality of life and self - efficacy sub - variables. The depression and the self - efficacy of the elderly had the greatest effect on the quality of life. The variables were depression (${\beta}=-.328$), social efficacy (${\beta}=.248$), and physical efficacy (${\beta}=.193$). Conclusion : In order to improve the quality of life of the elderly, it will be necessary to provide and develop medical care services that reduce depression and improve self - efficacy (physical and social).

Factors Related to Motor Subtypes of Delirium Patients in a Surgical Intensive Care Unit (일 종합병원 외과 중환자실 환자의 섬망 아형별 관련 요인)

  • Purpose: The purpose of this study was to investigate the motor subtypes of delirium in patients in a Surgical Intensive Care Unit (SICU), and identify the factors related to the characteristics of patients according to the motor subtypes of delirium. Methods: Data were collected in the SICU of a tertiary hospital in city from October 2018 to June 2019. Delirium was detected using the Confusion Assessment Method for the ICU (CAM-ICU) and motor subtypes of delirium were measured with the Delirium Motor Subtype Scale (DMSS)-4. Patients' characteristics were obtained by using the electronic medical records. Descriptive statistics were used to analyze the data. Results: Among 1,112 patients, 172 patients showed delirium (15.5%). After excluding dementia patients and patients refusing to participate in the study, 126 patients included in the final analysis. Delirium patients were classified as hyperactive delirium (32.5%), hypoactive delirium (42.9%), mixed delirium (11.9%), and non-motor subtype delirium (12.7%). Conclusion: The study results suggest that hypoactive delirium is the most prevalent motor subtype of delirium in SICU. More application of ventilators, more administration of sedatives, more use of catheters, and higher nursing severity were reported for hypoactive delirium cases than hyperactive ones. Therefore, it is necessary to assess early the motor subtypes of delirium using structured tools and develop appropriate nursing interventions suitable for each subtype of delirium.

Utilization Behaviour and Influencing Factors of Complementary and Alternative Medicine Therapies Among the Elderly (일부 노인의 보완대체요법 이용행태와 영향요인)

  • Objectives: The aim of this study is to analyze the utilization behaviour and influencing factors of complementary and alternative medicine (CAM) therapies among elderly people with chronic diseases. Methods: The study population was selected among 9 welfare facilities for the aged in 2 cities among Daegu Metropolitan City & Gyeongsangnam Province by two-stage cluster sampling. 250 senior citizens participated in a face-to-face interview using a structured questionnaire. Logistic regression analysis was performed to examine the association between the utilization of CAM therapies and various characteristics of the elderly. All statistics were analyzed using the PASW (ver 18.0). Results: Among 233 elderly peoples, 70.4% (164 persons) had used various kinds of CAM therapies (including nutritional methods, pharmacologic and biologic treatments, etc.) more than once during the last year. 48.8% (64 persons) ~ 60.7% (88 persons) of the elderly used CAM therapies without health and medical experts' counsel. The elderly with perceptions of CAM therapy used it 2 times more than those without knowledge of CAM therapies. The number of chronic diseases was more likely to increase the usage of nutritional methods (OR=2.92, 95% CI: 1.07-7.97), manipulative and body-based practices (OR=5.85, 95% CI: 1.97-17.34), pharmacologic and biologic treatments (OR=2.92, 95% CI: 1.49-8.17). Elderly with diabetes used nutritional methods 3.76 (95% CI: 1.49-9.47) more than elderly without diabetes. Conclusions: CAM therapies use in the aged with chronic diseases appears common. The findings suggest that the clinical efficacy and safety of CAM therapies on medical management of chronic disease may be investigated and that patient-physician communication need to be strengthened.

Relationship Between Death Orientation of First Care Giver and Quality of Life of Hospice Patients (호스피스환자 주 수발자의 죽음에 대한 인식과 호스피스 환자의 삶과 질과의 관계)

  • In Korea, there are constantly increasing number of cancer patients with reaching 65,000 deaths and it was 26.3% of the total number of death in 2004. Many cancer patients suffer from surgery, chemotherapy, and radiotherapy after being diagnosed as cancer. And many of them are facing fear of death because they can't be perfectly cured. Due to patients' physical, psychological, and spiritual pain, quality of life drops dramatically. Patients' families also suffer from huge medical expenses while they have to take care of patients's suffering from pain. At the same time, family's attitude can influence on the quality of patients' life. The purpose of this study is to investigate the relationship between the death orientation of first care giver and the quality of life of hospice patient. The subjects of the study were 80 hospice patients registered at ten hospice institutions with hospice team and medical practitioners in six cities including Seoul as well as their first care givers. This study used 13 questions for the hospice patients and nine questions for the first care givers to recognize general characteristic. To measure death orientation of the first care giver the tool developed by Noh, Soon-hee (2003) was used. And to measure quality of life of the hospice patients Yoo, Seung-yeon's structured tool was used. The data were collected for a month through interview method. SPSS win 12.0 was used to analyze the data by using frequency, percentage, t-test, Pearson correlation. The study result is as follows. In relationship between general characteristic of hospice patient and quality of life, the highest suffering was pain (60%) and the second suffering was anorexia (23.8%). There was no significant relationship between physical pain and general characteristics of hospice patient. In psychological aspects, religion (p=.044) showed significant difference (p<.05). In existential aspects, age (p=.035) showed significant difference (p<.05). There was no significant difference variable in support aspects. And religion (p=.000) was statistically significant variable in spiritual aspects (p<.001). Age (p=0.025) and religion (p=.050) were the variable showed significant difference according to general characteristics of first care giver's death orientation. Although the relation between death orientation of first care giver and quality of life of hospice patient was not statistically significant correlation. In conclusion, while death orientation of first care giver and hospice patient's quality of life are not statistically significant in correlation analysis.

Convergent Influence of Subjective Happiness, Job Burnout and Psychosocial Stress on State Anxiety among Hospital Women Administrative Staff (병원여성행정직의 주관적행복감, 직무소진 및 사회심리적 스트레스가 상태불안에 미치는 융복합적인 영향)

  • This study ascertained convergent influence on state anxiety and its association with subjective happiness, job burnout and psychosocial stress among Hospital Women Administrative Staff(HWAS). The survey period was from July 4, 2018 to July 25, 2018. The subjects were 226 HWAS randomly extracted from 24 hospitals in J area. The data were collected using a structured self-administered questionaries. State anxiety was negatively correlated with subjective happiness, and it was positively correlated with job burnout and psychosocial stress. The covariance structure analysis showed that the lower subjective happiness, and the higher job burnout and psychosocial stress tend to increase state anxiety. In conclusion, in order to reduce the state anxiety of HWAS, it is necessary to increase subjective happiness, to reduce job burnout and psychosocial stress. These results are expected to be used for industrial health education and hospital internal customer management to reduce the state anxiety of HWAS. In the future study, it is necessary to investigate the convergent additional factors that affect the state anxiety of HWAS.
